The value of parenteral nutrition support. Chemotherapy and radiation treatment.

Nutritional support in chemotherapy and radiation therapy patients is a very attractive concept. This group of patients frequently begins treatment malnourished, and the malnutrition becomes worse because of treatment-related nausea and vomiting, mucositis, fever and other side effects. Effective nutritional support would enhance wound healing and visceral function, improve cellular immunity and allow delivery of adequate drug and irradiation doses, thereby increasing response rates and prolonging survival. Unfortunately, standard techniques of parenteral nutritional support have had only limi

Mesenchymal R-spondin 3 promotes an immunogenic tumor microenvironment with adaptive immune resistance in human gastric adenocarcinoma.

BACKGROUND: The main risk factor for gastric cancer (GC) is inflammation driven by Helicobacter pylori (H. pylori) infection. Despite growing use of immunotherapies targeting the tumor inflammatory microenvironment, responses remain suboptimal, highlighting the need to define inflammatory drivers and improve patient stratification. METHODS: We assessed the role of R-spondin 3 (RSPO3), a stroma-secreted Wnt potentiator, in a Caucasian tissue microarray of 277 chemotherapy-naïve GC patients by associating RSPO3 expression with tumor inflammation, tumor stage, and survival. To mechanistically ex

Acid-enhanced drinkable hydrogel co-delivering riboflavin-loaded nanozymes for orchestrated inflammation-immune modulation and redox homeostasis in colitis therapy.

Conventional inflammatory bowel disease therapies are limited by inefficient colon-targeted delivery, systemic side effects, and a failure to rectify the underlying oxidative stress and immune dysregulation. Riboflavin (RAF) possesses ideal antioxidant and immunomodulatory properties for colitis therapy but suffers from rapid proximal absorption. While acid-degradable metal-organic framework (MOF) nanozymes can co-deliver RAF and catalytically eliminate reactive oxygen species (ROS), they lack colon-retentive capacity. To overcome these barriers, we engineered an acid-enhanced, drinkable hydro

A macrophage-targeted bivalent saRNA vaccine protects against ocular and cutaneous HSV-1 infection in mice.

Herpes simplex virus type 1 (HSV-1) infects over 1.5 million individuals annually, causing herpes simplex keratitis (HSK) with irreversible vision loss, as well as skin and mucosal infections. The virus establishes lifelong latency in the nervous system, rendering current therapies ineffective for eliminating latent reservoirs or preventing recurrence. To address this challenge, we developed a novel bivalent self-amplifying RNA (saRNA) vaccine, termed MTL-bivalent-IL12, which incorporates saRNAs encoding HSV-1 glycoproteins gD and gB, formulated within ganglioside GM3-engineered lipid nanopart

Integrated Foodomics Reveals Gut Microbiota-Metabolite-Gene Interactions Associated with the Immunoprotective Effects of Ganoderma lucidum Polysaccharide Peptide.

Ganoderma lucidum polysaccharide peptide (GLPP) is a food-derived macromolecule with immunomodulatory potential, but its gut-centered mechanisms under chemotherapy-associated immunosuppressive stress remain unclear. This study aimed to evaluate the protective effects of GLPP against cyclophosphamide (CTX)-induced immunosuppression and intestinal injury in mice and to explore the associated microbiota-metabolite-gene interaction network using integrated foodomics. BALB/c mice were treated with CTX and then administered GLPP at 50, 100, or 200 mg/kg/day for 42 days, with levamisole as a positive

Molecular mechanisms of allergic type 2 immunity in the pathogenesis of Crohn's disease: From epithelial alarmins to IgE signaling.

Crohn's disease (CD) is a chronic inflammatory bowel disorder driven by complex interactions between genetic susceptibility, gut microbiota, and immune dysregulation, typically associated with Th1 and Th17 responses. However, emerging evidence highlights that type 2 allergic mechanisms also significantly contribute to CD pathogenesis. This review examines the cellular and molecular basis of allergic pathways in CD, focusing on IgE-mediated hypersensitivity, mast cell and eosinophil effector functions, and Th2 cytokines (IL-4, IL-5, IL-13). Key epithelial-derived alarmins-namely IL-33, TSLP, an

Effects of Probiotics and Vitamin D Deficiency Correction on Clinical, Inflammatory, Metabolic, and Microbiota Profiles in Older Adults with Oral Lichen Planus: A Multi-Omics Study.

Oral lichen planus (OLP) is a chronic inflammatory oral disease associated with immune dysregulation and malignant transformation risk. Vitamin D and probiotics may modulate immune and microbial pathways involved in OLP. In this study, we evaluated their effects on clinical outcomes and multi-omics profiles in 25 adult OLP patients (median age: 68 years). Vitamin D-deficient patients received 2000 IU/day vitamin D3, and all participants received a probiotic blend (Limosilactobacillus reuteri LRE11, Lacticaseibacillus rhamnosus LR04, and Lacticaseibacillus casei LC04) for 16 weeks. A low-endotoxic Salmonella vector with dual bacterial-host promoter expression of Lawsonia intracellularis antigens elicits protective immunity in a murine model.

Lawsonia intracellularis, the etiological agent for proliferative enteropathy, remains a major cause of productivity loss and economic burden in the global swine industry. Currently available vaccines provide suboptimal protection and raise safety concerns, highlighting the need for improved immunization strategies. In this study, we developed Salmonella Typhimurium-based vaccine candidates against L. intracellularis in a murine model, incorporating immunoprotective epitopes from the surface autotransporter protein LatA, the flagellar protein FliC, and the heat shock protein 60 (HSP60). Emerging markers in celiac disease.

Celiac disease (CD) is a chronic autoimmune disorder triggered by gluten ingestion in genetically predisposed individuals. Current diagnostic methods rely on serological markers, histological examination of duodenal biopsies, and HLA genotyping. However, these approaches have limitations. Advancements in serology have introduced novel autoantibodies beyond tissue transglutaminase (tTG) and endomysial antibodies (EMA), such as neo-epitope tTG and transglutaminase isoforms. Genetic and epigenetic markers, including non-HLA risk alleles, DNA methylation patterns, and non-coding RNAs, provide deep

Coordinated kinetics of humoral, T-cell, and cytokine responses to SARS-COV-2 in West African healthcare workers: a multicenter longitudinal study in university hospitals of Abidjan, Côte d'Ivoire.

BACKGROUND: Healthcare workers in resource-limited settings are frequently exposed to SARS-CoV-2, often with incomplete vaccine coverage. Yet, their adaptive immune responses remain poorly characterized in sub-Saharan Africa. METHODS: We conducted a multicenter study involving 36 healthcare workers with RT-PCR-confirmed SARS-CoV-2 infection (83.3% vaccinated) and 40 strictly unvaccinated, COVID-19-naïve controls from three university hospitals in Abidjan. Blood samples were collected on Days 0, 7, 14, 21, and 28. IgM and IgG were measured by ELFA (Mini VIDAS®), neutralizing antibodies with CH

Alcohol, aging, and the gut microbiome: Intersections of immunity, barrier dysfunction, and disease.

Alcohol consumption exerts complex, dose- and context-dependent effects on human health, particularly by influencing the gut microbiome, intestinal barrier integrity, immune regulation, and aging processes. Genetic variation and advancing age are two major, and often interacting, factors that modify the risk of alcohol-related diseases. Among genetic factors, the prevalent aldehyde dehydrogenase 2 polymorphism (ALDH2∗2) compromises acetaldehyde clearance, driving toxic metabolite accumulation, oxidative stress, and increased intestinal permeability that disrupts gut microbial communities, even

RNA sequencing and proteomic profiling reveal alterations by MPTP in chronic stomach mucosal injury in tree shrew Chinese (Tupaia belangeri chinensis).

1-Methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) is a neurotoxin that can cause gastrointestinal ulcers by affecting dopamine levels. Therefore, MPTP has been considered a toxic substance that causes gastric ulcer disease in experimental animals. In this study, tree shrews were used as the animal model of gastric mucosa injury, and MPTP was intraperitoneally injected at a lower MPTP dosage 2 mg/kg/day for 13 weeks, while tree shrews were not injected as the control group. Under the light microscope, local congestion or diffuse bleeding points of gastric mucosa and multiple redness and swell

Role of vitamin D in COVID-19 and other viral infections.

Vitamin D is a steroid hormone that is naturally produced in the body or obtained through dietary sources, primarily under the influence of UVB radiation. This essential nutrient has a vital role in numerous physiological processes, encompassing immune function, cell growth, differentiation, insulin regulation, and cardiovascular well-being, along with its pivotal role in sustaining the delicate equilibrium of calcium and phosphate concentrations in the body. Moreover, vitamin D reinforces mucosal defense and bolsters the immune system through immunomodulation, making it a critical component o

Activation Mechanism of CcGSDMEb-1/2 and Regulation for Bacterial Clearance in Common Carp (Cyprinus carpio).

Gasdermin E (GSDME), to date, is considered the only direct executor of the pyroptosis process in teleost and plays an important role in innate immunity. In common carp (Cyprinus carpio), there contains two pairs of GSDME (GSDMEa/a-like and GSDMEb-1/2), and the pyroptotic function and regulation mechanism of GSDME still remain unclear. In this study, we identified two GSDMEb genes of common carp (CcGSDMEb-1/2), which contain a conserved N-terminal pore-forming domain, C-terminal autoinhibitory domain, and a flexible and pliable hinge region. We investigated the function and mechanism of CcGSDM

[Diagnosis and Treatment of IgA Nephropathy-2023].

Immunoglobulin A nephropathy (IgAN) is the most common glomerulonephritis. It leads to end-stage kidney disease in about a third of the patients within 10 to 20 years. The pathogenesis of IgAN is incompletely understood. It is believed that a dysregulation of the mucosal immune system leads to undergalactosylation of IgA, followed by formation of IgG autoantibodies against undergalactosylated IgA, circulation of these IgG-IgA immune complexes, deposition of the immune complexes in the mesangium, ultimately resulting in glomerular inflammation. IgAN can occasionally be triggered by other diseas

Immune Response in Severe and Non-Severe Coronavirus Disease 2019 (COVID-19) Infection: A Mechanistic Landscape.

The mechanisms underlying the immune remodeling and severity response in coronavirus disease 2019 (COVID-19) are yet to be fully elucidated. Our comprehensive integrative analyses of single-cell RNA sequencing (scRNAseq) data from four published studies, in patients with mild/moderate and severe infections, indicate a robust expansion and mobilization of the innate immune response and highlight mechanisms by which low-density neutrophils and megakaryocytes play a crucial role in the cross talk between lymphoid and myeloid lineages. We also document a marked reduction of several lymphoid cell t

Malnutrition Decreases Antibody Secreting Cell Numbers Induced by an Oral Attenuated Human Rotavirus Vaccine in a Human Infant Fecal Microbiota Transplanted Gnotobiotic Pig Model.

Human rotavirus (HRV) is a leading cause of morbidity and mortality in children, especially in developing countries. Malnutrition is prevalent in these countries, which may contribute to the decreased oral vaccine efficacy, posing a concern for global health. Neonatal gnotobiotic (Gn) pigs closely resemble human infants in their anatomy, physiology, and outbred status and are a unique model to investigate malnutrition, oral live attenuated HRV (AttHRV) vaccination, and subsequent virulent HRV (VirHRV) challenge. We evaluated the impact of malnutrition on AttHRV vaccine efficacy and B cell immu
