Association between prehospital end-tidal carbon dioxide levels and mortality in patients with suspected severe traumatic brain injury.

PURPOSE: Severe traumatic brain injury is a leading cause of mortality and morbidity, and these patients are frequently intubated in the prehospital setting. Cerebral perfusion and intracranial pressure are influenced by the arterial partial pressure of CO2 and derangements might induce further brain damage. We investigated which lower and upper limits of prehospital end-tidal CO2 levels are associated with increased mortality in patients with severe traumatic brain injury. METHODS: The BRAIN-PROTECT study is an observational multicenter study. Patients with severe traumatic brain injury, tre

Prevention of Blast-induced Auditory Injury Using 3D Printed Helmet and Hearing Protection Device - A Preliminary Study on Biomechanical Modeling and Animal.

INTRODUCTION: Repeated blast exposures result in structural damage to the peripheral auditory system (PAS) and the central auditory system (CAS). However, it is difficult to differentiate injuries between two distinct pathways: the mechanical damage in the PAS caused by blast pressure waves transmitted through the ear and the damage in the CAS caused by blast wave impacts on the head or traumatic brain injury. This article reports a preliminary study using a 3D printed chinchilla "helmet" as a head protection device associated with the hearing protection devices (e.g., earplugs) to isolate the

The Dangers of Sneezing: A Review of Injuries.

BACKGROUND: Sneezing is an almost universal daily phenomenon as a reflex to evacuate irritants from the nasal cavities. An autonomic-controlled buildup of intrathoracic pressure against a closed glottis followed by sudden release results in a burst of 1 kPa of air through the upper airway. Active intervention to limit a sneeze can be deleterious. Closure of the airway during a sneeze can result in over 20 times the airway pressures resulting in a variety of untoward events. This review summarizes all reported injuries that occurred as the result of a sneeze. OBJECTIVE: The objective of this r

The Total Calvarial Transsutural Distraction Osteogenesis for 26 Children with Slit Ventricle, Craniosynostosis, or Microcephaly After Shunt Operation.

OBJECTIVE: Among shunt complications, the postshunt slit ventricle (PSSV) and the postshunt craniosynostosis (PSCS) may be managed by shunt valve upgrade and/or cranial expansion surgery. Here, we analyzed 26 children with PSSV, PSCS, or microcephaly who received simple generalized cranial expansion (ie, total calvarial transsutural distraction osteogenesis [TC-TSuDO]). METHODS: Among 254 children with shunt surgery, 26 children received TC-TSuDO. These 26 children included 14 with PSSV, 4 with PSCS, and 8 with both PSSV and PSCS. The mean age of patients who underwent shunt procedures was 8.

Pulsatility in CSF dynamics: pathophysiology of idiopathic normal pressure hydrocephalus.

BACKGROUND: It is suggested that disturbed CSF dynamics are involved in the pathophysiology of idiopathic normal pressure hydrocephalus (INPH). The pulsatility curve describes the relationship between intracranial pressure (ICP) and the amplitude of cardiac related ICP pulsations. The position of baseline ICP on the curve provides information about the physiological state of the CSF dynamic system. The objective of the study was to investigate if shunt surgery modifies the pulsatility curve and the baseline position on the curve, and how this relates to gait improvement in INPH. METHODS: 51 I

The arterial to end-tidal carbon dioxide difference in neurosurgical patients during craniotomy.

PETCO2 is often used as an estimate of PaCO2, with the understanding that PaCO2 usually exceeds PETCO2. During intraoperative craniotomies, because hyperventilation is used to therapeutically lower intracranial pressure, the difference between PaCO2 and PETCO2 (P(a-ET)CO2) has therapeutic implications. The P(a-ET)CO2 was hypothesized to be stable during craniotomies with relatively short-term monitoring and controlled cardiorespiratory variables. Thirty-five patients undergoing elective craniotomies were studied. Arterial blood gases (with PaCO2) were measured after induction of general anesth

Long-term effects of tissue expansion on cranial and skeletal bone development in neonatal miniature swine: clinical findings and histomorphometric correlates.

Progressive tissue expansion induces significant gross, histologic, and bony changes in skulls and long bones of neonatal miniature swine. These bony changes consist of erosion underlying tissue expanders, with bony lipping and bone deposition at the periphery of the expander. Cranial suture lines underneath expanders appear effaced and convoluted. Serial CT scans reveal decreased bone thickness and volume (p less than 0.02) but identical bone density (p = 0.60) beneath expanders. Increased bone volume and thickness occur at the periphery of expanders (p less than 0.02). Bone density (CT numbe

[A case of cryptococcal meningitis successfully treated with miconazole and CSF drainage].

Cryptococcal meningitis is the most frequent fungal infection of the central nervous system, known readily to complicate with immuno-compromised patients. There are only a few cases of primary infection in healthy non-immuno-compromised patients. Amphotericin-B (AMPH-B) and 5-Fluorocytosine (5-FC) are effective agents against Cryptococcal meningitis, although, their toxicity and drug resistance are limiting factors. However, in recent years Miconazole has been widely used against fungal infections and it's effectiveness has been reported. This is a 68 y.o. male who was admitted to Toyohashi Mu

Effects of prolonged supratentorial mass expansion on regional blood flow and cardiovascular parameters during the Cushing response.

To clarify the central nervous and the cardiovascular mechanisms mediating the Cushing response (CR) during intracranial hypertension, the effects of continuous and prolonged expansion of an epidural supratentorial balloon was studied in six dogs. Regional cerebral blood flow, cardiac output, extracerebral tissue blood flow and regional vascular resistance were measured with radioactive microspheres before and during the CR while heart rate, arterial blood pressure and ECG were continuously recorded. The results showed that balloon expansion led progressively to cerebrospinal ischemia beginnin

Ventriculoperitoneal Shunt Overdrainage Has Transient and Steady-State Components Strongly Dependent on Cerebroventricular Compliance: A Benchtop Experimental Study.

BACKGROUND AND OBJECTIVES: Using benchtop testing, we previously demonstrated that ventriculoperitoneal shunts are susceptible to chronic overdrainage, not only because of a siphoning effect but also because of intracranial pressure pulsations. These pulsations may be rhythmic (cardiac-related) or sporadic (Valsalva maneuvers) and can drive excess cerebrospinal fluid (CSF) drainage at both horizontal and vertical positions, an effect that is strongly dependent on system compliance. The objective was to characterize the temporal dynamics of CSF flow through a differential pressure valve across

Normal pressure hydrocephalus in patients with myelomeningocele.

Although the syndrome of normal pressure hydrocephalus (NPH) was described in the adult as early as 1964, it has only recently been recognized in the child. In this preliminary report, eight myelomeningocele patients with presumed NPH were evaluated before and after ventricular shunting procedures. Cranial computed tomography and serial psychological testing have proved to be particularly valuable both in the pre-operative and post-operative assessment of these patients and have the distinct advantage of being simple, non-invasive diagnostic measures. Continuous intra-ventricular pressure moni

Multiple intracranial lesions in head injury: clinical considerations, prognostic factors, management, and results in 95 patients.

BACKGROUND: The goal of this study was to identify clinical and radiological predictors of prognosis in patients with multiple post-traumatic intracranial lesions. METHODS: We reviewed 95 patients (75 male and 20 female) between the ages of 18 and 70 (average 38) admitted between 1993 and 2000 with multiple post-traumatic intracranial lesions. Intracranial pressure (ICP) monitoring was carried out in 67 patients (70%); 77 received intensive care unit (ICU) treatment. Since in all cases it was possible to identify a clearly predominant lesion, 3 groups of patients emerged from the data: the fi

Cysticercosis of the central nervous system. I. Surgical treatment of cerebral cysticercosis: a 23 years experience in the Hospital das Clínicas of Ribeirão Preto Medical School.

Cysticercosis is the most frequent parasitosis of the nervous system and nowadays it is widespread through the world. Despite the development of anticysticercal drugs (praziquantel and albendazole), their efficacy is more marked in cases with parenchymal active cysts and they do not prevent complications such as hydrocephalus. Thus, many patients with neurocysticercosis require surgical intervention, generally of palliative nature, but that may occasionally produce a cure. The clinical outcome of 180 patients with cerebral cysticercosis who underwent surgical treatment form 1970 to 1993 was an

Estimation of metabolic water production in human and rat brain and spinal cord.

BACKGROUND: Cerebrospinal fluid (CSF) is vital to maintain brain homeostasis by facilitating waste removal, nutrient transport, and regulation of intracranial pressure. The choroid plexus is generally assigned as the primary source of CSF secretion, but it remains unresolved to what extent water produced during cellular metabolism in the central nervous system contributes to the CSF pool. METHODS: Brain and spinal cord parenchyma volumes in rats (n = 6) and humans (n = 6) were assessed with volumetric magnetic resonance imaging. Segmentation techniques were applied to obtain the mass of metabo

Mitigating failures and enhancing reliability of a canine ventricular shunt through robust multi-objective design method.

Hydrocephalus is a condition that can result in increased intracranial pressure due to the excessive accumulation of cerebrospinal fluid (CSF) or blockage of the outflow of CSF at the level of the third or fourth ventricle. The basic treatment for hydrocephalus is CSF drainage by inserting a surgical tube (shunt) into the ventricle. Despite many new shunt design solutions, shunt failures are a regular occurrence, further complicating treatment. To address this problem, we demonstrate the effectiveness of a robust multi-objective design method to optimize the design of a canine ventricular shun

Noninvasive MRI assessment of cerebrospinal fluid pressure in different phases of Ménière's disease: a prospective study.

OBJECTIVES: This study aimed to determine whether there are differences of cerebrospinal fluid pressure (CSF-P) on patients with Ménière's disease (MD) during different phases. METHODS: Noninvasive CSF-P measurement was performed using MRI with a fat-suppressed fast recovery fast spin echo T2-weighted sequence, with the optic nerve subarachnoid space width (ONSASW) posterior to the globe serving as an indicator. Endolymphatic hydrops (EH) grades and hearing thresholds were analysed to investigate the potential correlations with CSF-P. RESULTS: A total of 66 participants were included. At the

Effect of nitrous oxide on clinical outcome in patients undergoing cerebellopontine tumor surgery under sevoflurane anesthesia: a randomized controlled trial.

Nitrous oxide (N2O) is a unique anesthetic agent that has both advantages and disadvantages, especially in neurosurgical patients. Various studies evaluating the use of N2O in different surgical populations have been inconclusive so far. In this prospective, single-blinded, randomized study, 50 patients of either sex, aged 18-60 years, were enrolled and randomly allocated into N2O or N2O free group. Data including demographics, intraoperative vitals, blood gases, intravenous fluids, anesthetic drug consumption, brain condition, emergence and recovery time, duration of surgery and anesthesia, d

"Artificial intelligence-driven infection risk prediction in ventriculoperitoneal shunting: a novel approach for normal pressure hydrocephalus treatment".

Idiopathic normal pressure hydrocephalus (iNPH) affects approximately 1.5% of the population, with a higher prevalence in men than women. Ventriculoperitoneal shunting (VPS) is the standard treatment for iNPH, but it poses a notable risk of infection, occurring in 8-10% of cases. Recent advancements in non-invasive diagnostic techniques, such as superb microvascular ultrasound (SMI), have demonstrated potential in evaluating cerebrospinal fluid (CSF) flow within VPS systems. A single-center feasibility study involving 19 asymptomatic patients with VPS systems showed that SMI reliably detected

Posterior Reversible Encephalopathy Syndrome With Hemorrhagic Conversion in a Patient With Active Polysubstance Abuse: A Case Report and Review of Literature.

Posterior reversible encephalopathy syndrome (PRES) is a neurovascular sequence noted in patients with preeclampsia/eclampsia, solid-organ/bone marrow transplantation, and malignant hypertension. The mechanism in which PRES occurs has not yet been determined. It has been hypothesized that it may be related to endothelial cell dysfunction or injury leading to the compromise of the blood-brain barrier. The clinical presentations vary but are similar to symptoms of increased intracranial pressure, such as headache, visual changes, focal neurological deficits, seizures, and altered mental status.
