A new CAD system for the evaluation of kidney diseases using DCE-MRI.

Acute rejection is the most common reason of graft failure after kidney transplantation, and early detection is crucial to survive the transplanted kidney function. In this paper, we introduce a new approach for the automatic classification of normal and acute rejection transplants from Dynamic Contrast Enhanced Magnetic Resonance Imaging (DCE-MRI). The proposed algorithm consists of three main steps; the first step isolates the kidney from the surrounding anatomical structures by evolving a deformable model based on two density functions; the first function describes the distribution of the g

Detecting patients with acute coronary syndrome in the chest pain center of the emergency department with cardiac magnetic resonance imaging.

Managing patients with chest pain in the Emergency Department (ED) remains a challenge where advanced imaging technology may play a role. Detecting acute coronary syndrome in the emergency department with cardiac magnetic resonance imaging (MRI) was studied in 161 consecutive chest pain patients in a prospective observational study. Patients with ST elevation myocardial infarction were excluded from study participation. All patients underwent MRI study when hemodynamically stable, free of ongoing chest pain, and within 12 hours of symptoms onset. The MRI study takes an average of 38 +/- 12 min

Functional brain imaging in the dementias: role in early detection, differential diagnosis, and longitudinal studies.

This review considers the role of functional brain imaging techniques in the dementias. The substantial assistance that especially single-photon emission tomography and positron emission tomography can play in the initial diagnosis of dementia and in the differential diagnosis of the specific dementing disorder is discussed. These techniques alone essentially match the sensitivity and specificity of clinical diagnoses in distinguishing Alzheimer's dementia (AD) from age-matched controls, from frontal lobe dementia and vascular dementia, and even from Lewy body dementia. Newer analytic techniqu

Ultrasound of the kidney: obstruction and medical diseases.

Ultrasound has emerged as the primary imaging modality in conditions where either renal obstruction or renal medical disease is suspected on the basis of clinical and laboratory findings. In urinary tract obstruction, pathophysiologic changes affecting the pressure in the collecting system and kidney perfusion are well understood and form the basis for the correct interpretation of real-time US and color Doppler duplex sonography (CDDS). Ultrasound is very sensitive for the detection of collecting system dilatation ("hydronephrosis"); however, obstruction is not synonymous with dilatation, as

Cerebral blood flow in frontal lesions of aneurysms of the anterior communicating artery.

BACKGROUND AND PURPOSE: The aim of this study was to investigate local and remote regional cerebral blood flow in patients with prefrontal lesions resulting from rupture (and operation) of aneurysms of the anterior communicating artery. METHODS: The localization and severity of the lesions were evaluated by magnetic resonance imaging on T2 sequences. Blood flow measurements were performed in 21 patients at least 3 weeks after surgery using single-photon emission computed tomography. Flow values were calculated in 10 regions of interest in each cerebral hemisphere and compared with those of 21

Multiparametric MRI for In Vivo Visualization of Renal Inflammation in Acute Kidney Injury.

Renal ischemia-reperfusion injury (IRI) is a major cause of acute kidney injury (AKI), yet early diagnosis remains challenging owing to the lack of sensitive and specific approaches for in vivo inflammation visualization, which severely limits accurate diagnosis and effective intervention. To address this limitation, this work reports an inflammation-targeting multiparametric MRI strategy based on a nanoprobe (NPs@PEG-PEP) for real-time visualization of AKI-associated inflammatory responses. NPs@PEG-PEP was modified with functional peptides targeting CD137, which is an activation-induced immun

Rupture of mycotic aneurysm complicating deceased donor renal transplantation: a dual case report.

BACKGROUND: Mycotic aneurysms of the transplant renal artery (TRA) are rare but life-threatening complications following renal transplantation, often resulting in graft loss and high mortality. Prompt recognition and timely management are essential to prevent catastrophic haemorrhage and improve patient outcomes. CASE PRESENTATION: We report two cases of ruptured mycotic aneurysms in cadaveric renal allografts. Both patients were on maintenance haemodialysis and underwent deceased donor kidney transplantation. In the first case, the patient presented two weeks postoperatively with haemodynamic

Cerebral arterial oxygen embolism as a complication of hyperbaric oxygen treatment: a case report.

Cerebral arterial gas embolism (CAGE) is a recognised complication of diving-related barotrauma and of medical procedures whereby gas enters the vascular system. CAGE is a principal indication for hyperbaric oxygen treatment (HBOT). In contrast, CAGE resulting from HBOT itself is exceedingly rare. We describe the case of a 73-year-old man undergoing his first session of HBOT for a chronic lower limb wound, who developed acute focal neurological deficits during decompression. He had no known respiratory disease or smoking history. Immediate assessment demonstrated stable physiology. Initial com

Multi-modal magnetic resonance imaging to assess cerebrovascular function in patients with atrial fibrillation.

Atrial fibrillation (AF) is associated with a high risk of stroke, as well as cognitive decline and dementia. Advanced imaging techniques were employed to test the hypothesis that AF worsens cerebrovascular function as evaluated using neurovascular coupling (NVC) and cerebrovascular carbon dioxide reactivity (CVRCO2) tests. Twenty healthy controls (HC; 63.4 ± 9.5 years) and 19 AF patients (66.6 ± 12.0 years) completed NVC (visual stimulation with a flickering checkerboard) and CVRCO2 (5%CO2) assessments during functional (blood oxygen level-dependent [BOLD]) and perfusion (arterial spin labell

Plasma Glial Fibrillary Acidic Protein and Neurofilament Light Chain Concentrations Are Inversely Associated with Retinal Microvascular Perfusion and Vessel Density in Cognitively Normal Individuals with Familial or Genetic Risk Factors for Alzheimer's Disease.

Background/Objectives: Evaluating noninvasive, accessible ocular and blood-based biomarkers could aid in early risk stratification and disease detection during the preclinical phase of Alzheimer's disease. This study investigates associations between plasma biomarkers of neurodegeneration and retinal structural and microvasculature parameters in cognitively normal adults with familial or genetic risk factors for Alzheimer's disease. Methods: Forty-one participants underwent plasma sampling for glial fibrillary acidic protein (GFAP), neurofilament light chain (NfL), amyloid-beta42 (β42), amyloi

An Adaptive, Time-Informed, Phenotype-Guided Pharmacological Framework for Severe Traumatic Brain Injury : A Hypothesis-Driven Narrative Review.

Severe traumatic brain injury (TBI) remains difficult to treat because successful control of mass lesions, intracranial pressure, cerebral perfusion, oxygenation, and systemic instability does not ensure that secondary cellular injury has stopped. PubMed/MEDLINE and ClinicalTrials.gov were searched through July 28, 2026, and representative candidates were organized across five overlapping pathophysiological nodes. Evidence was described by study source, purpose, and result direction rather than assigned a formal quality grade. Human evidence was heterogeneous. Glibenclamide/glyburide and hyper

Aberrant Scrotal Extension of an Incarcerated Incisional Hernia With Testicular Displacement.

Incisional hernias are common postoperative complications, whereas scrotal extension is usually associated with inguinal hernias traversing the inguinal canal. Scrotal involvement from a ventral incisional defect through a subcutaneous route superficial to the inguinal canal is uncommon and can substantially distort groin and testicular anatomy. We report a 62-year-old man with a remote laparoscopic right inguinal hernia repair who presented with acute pain, nausea, emesis, and a large non-reducible ventral and right inguinoscrotal swelling. Computed tomography demonstrated a midline/right par

Omental infarction as an unusual manifestation of decompensated chronic left heart failure: a rare case report.

INTRODUCTION AND IMPORTANCE: Omental infarction (OI) is a rare cause of acute abdominal pain, frequently misidentified as appendicitis or diverticulitis. It generally originates from vascular compromise, torsion, or thrombosis and is infrequently associated with left-sided heart failure. This appears to be the inaugural documented instance of OI linked to left-sided heart failure. CASE PRESENTATION: A 43-year-old man with a history of coronary artery disease and heart failure with reduced ejection fraction (35%) presented with right lower quadrant abdominal pain initially localized to the per

Aphasic Status Epilepticus Treated in the Emergency Room: A Report of Two Cases.

The clinical characteristics of patients with an aphasic status epilepticus (ASE) remain unclear. Here, we present two cases with ASE. Case 1 was a 61-year-old man who was admitted to our hospital with cortical deafness and severe aphasia. He was treated in our emergency department for status epilepticus, and his symptoms resolved. Electroencephalography (EEG) showed lateralized periodic discharges in the left temporo-parietal lobes. Single-photon emission computed tomography (SPECT) showed a high-uptake lesion in the left temporo-parietal lobes. Case 2 was an 80-year-old woman who was admitte

Neuroprotection by post-stroke administration of the slow-releasing hydrogen sulfide (H2S) donor AP39: Long-Term functional, MRI, and molecular evidence in a rodent stroke model.

Hydrogen sulfide (H2S) is an endogenous gaseous signalling molecule with potential cytoprotective properties in neurological disorders. AP39, a mitochondria-targeted H2S donor, has previously demonstrated neuroprotection in stroke models. Here, we investigated the therapeutic efficacy of AP39 administered after stroke in a rat model of transient middle cerebral artery occlusion (MCAO). AP39 (100 nmol/kg) was delivered intravenously 10 min after reperfusion. Animals underwent longitudinal assessments at 24 h and 7 days post-stroke, including neurological deficit assessment, grid-walking test, M

A ketogenic-promoting beverage acutely elevates cardiac function and myocardial blood flow compared to placebo in adults: A cardiac MRI investigation.

Increasing evidence suggests cardiac function improves in healthy and failing hearts alongside circulating ketones (1-4 mM). This study characterized cardiac function and blood flow responses to a ketogenic beverage compared to a volume/calorie matched placebo with repeated imaging over 120 min. This was a two-group, placebo-controlled, acute cardiac imaging study. Adults without cardiac abnormalities underwent baseline cardiac MRI including quantitative myocardial perfusion to measure myocardial blood flow (MBF). Subjects consumed 50 g of a ketogenic-promoting beverage [bis-hexanoyl R-1-3-but

Diagnostic Challenges in Inflammatory Choroidal Neovascularization.

Inflammation plays a key role in the induction of choroidal neovascularization (CNV). Inflammatory choroidal neovascularization (iCNV) is a severe but uncommon complication of both infectious and non-infectious uveitides. It is hypothesized that its pathogenesis is similar to that of wet age-related macular degeneration (AMD), and involves hypoxia as well as the release of vascular endothelial growth factor, stromal cell-derived factor 1-alpha, and other mediators. Inflammatory CNV develops when inflammation or infection directly involves the retinal pigment epithelium (RPE)-Bruch's membrane c
