Cosmesis after pterygium extended removal followed by extended conjunctival transplant as assessed by a new, web-based grading system.

OBJECTIVE: To design a web-based system to grade the cosmetic results after pterygium surgery and to use this to assess the aesthetic results of pterygium extended removal followed by extended conjunctival transplant. DESIGN: A standardized grading system with gradings of normal, excellent, good, fair, poor, and ungradeable was designed and tested by the author (standard gradings). This was then installed on a website where ophthalmologists and lay people could undertake grading and anatomy tutorials, and subsequently grade randomly assigned images in a masked fashion, of eyes after pterygium

Wide topical negative pressure wound dressing treatment for patients undergoing abdominal dermolipectomy following massive weight loss.

Postbariatric plastic surgery is considered to be a high-risk procedure, which entails such frequent minor complications as postoperative seroma, bleeding and wound dehiscence. These occur with a high incidence, especially, following postbariatric abdominal dermolipectomy. In order to reduce these complication rates, a new type of dressing with wide abdominal topical negative pressure (TNP) application was applied. We performed abdominal dermolipectomy in 23 obese patients. The average body mass index was 32.8 kg/m(2), and the median age of the patients was 42.9 years. Ten patients received co

A new operative technique for a rare case of pyelic fusion.

This case report presents the first documented case of pyelic fusion with two sites of obstruction. The patient was diagnosed following antenatal presentation and the operative technique described in this case report is the only way by which the obstructions could be treated with a single procedure. The left pelvicalyceal system was grossly dilated and was draining into the common pelvis via an obstructed calyx in the isthmus of the horse shoe. The common ureter also had a pelviureteric junction stenosis. Incisions were made in the left dilated pelvis and in the obstructed part of the pelviure

[Oropharynx and hypopharynx reconstruction. What have we learnt?].

INTRODUCTION AND OBJECTIVES: Oropharyngeal and hypopharyngeal reconstructions require significant human and technical resources. This study analysed our reconstruction program over the last 13 years. MATERIAL AND METHODS: Retrospective study in a tertiary reference centre. RESULTS: Forty-three reconstruction procedures, of which 67.4 % (29/43) were microvascular (radial forearm 17, rectus abdominis 10, scapular 1, jejunum 1) and 42.6% (14/43) myocutaneous (pectoralis major 13, latissimus dorsi 1). Of these reconstructions, 83% (37/43) were for oropharyngeal defects and 17% (6/43) for hypopha

Histopathological differences in bony destruction of malleus and incus following mastoidectomy.

OBJECTIVES: Upon direct inspection of surgically removed ossicles from the ears of patients with long-term post-mastoidectomy cavity problems, the extent of malleus destruction often appears greater in patients with a longer duration of cavity problems, whereas the extent of incus destruction does not appear to correlate with the duration of cavity problems. This study aimed to investigate this impression. MATERIALS AND METHODS: As a result of total middle-ear reconstruction, 41 ossicles (21 malleus and 20 incus bones) were obtained from 31 patients with post-mastoidectomy cavity problems. Th

Treatment of flaring of the costal arch after the minimally invasive pectus excavatum repair (Nuss procedure) in children.

Flaring of the costal arch may be part of the pectus excavatum deformity. This aspect will in rare cases be even worsen after the Nuss repair. This remaining deformity can be treated with a minimal subperichondral partial resection of the costal arch cartilage. In 5 patients, this additional technique regained good to excellent results.

[Repair for anterior chest wall deformity by Nuss procedure performed in young adults and older adolescents].

Results of 86 minimally invasive anterior chest wall reconstructions for pectus excavatum by the method of Nuss are evaluated retrospectively. Complications, possible ways of avoiding those and technical details of the operation are discussed. Nuss method is also evaluated in terms of suitability patients' age. Minimally invasive anterior chest wall reconstructions were performed in 86 patients in the last seven years. The average age was 21.4 years. There were no severe intra-operative complications. Four dislocations of the metal bar (12 and 53 days after chest reconstruction) were observed

Long-term significance of injury to the supraorbital or supratrochlear nerves during frontoorbital advancement in infancy.

INTRODUCTION: In a series of 26 infants undergoing frontoorbital advancement in their first year of life, the supraorbital and supratrochlear nerves were not preserved during surgery in nearly all cases. Of these 26 patients, 25 could be followed up postoperatively. RESULTS: None of the children spontaneously complained of any sensory disturbances thereafter. In neurologic follow-up examinations or telephone interviews 11-81 (median, 39) months after surgery, the patients, respectively their parents were explicitly asked for sensory deficits. The majority of patients had no sensory disturbanc

Primary stent placement for late complete occlusion after aortoiliac reconstructive surgery: report of a case.

We report a case that was successfully treated by primary stent placement without thrombolysis or thrombectomy for graft thrombosis after aortoiliac reconstructive surgery. A 79-year-old man presented with a 2-month history of severe intermittent claudication of the right leg. He had undergone a surgical repair of abdominal aortic aneurysm with a bifurcated polyester graft 3 years before presentation. Digital subtraction angiography revealed total occlusion of the right limb of the graft. He underwent primary stent placement on the lesion, and completion angiography showed revascularization of

Biomechanics of open Bankart and coracoid abutment procedures in a human cadaveric shoulder model.

The specific aims of this experiment were (1) to develop a clinically relevant model of anteroinferior shoulder dislocation in the apprehension position to compare the biomechanics of the intact anterior capsuloligamentous structures, and (2) to evaluate the initial strength of an open Bankart and of a coracoid abutment procedure. Fifteen shoulders from deceased donors were used. For the intact shoulders, mean peak load was 486 N, and stiffness was 26,7 N/mm. For the Bankart repair, the mean peak load was 264 N, and mean stiffness was 14.1 N/mm. Transosseous repairs failed by suture pullout th

[Applied anatomy of medial plantar artery combined flaps and repairing heel and adjacent vast soft tissue defects].

OBJECTIVE: To explore an anatomical basis and incising scope for the medial plantar artery combined flaps and its feasibility of repairing heel and adjacent vast soft tissue defects. METHODS: The origin, branches, course and distribution of the medial plantar artery were studied in 20 legs of adult cadavers. The largest area of healthy adult's medial plantar and medialis pedis scope that could be incised for the combined flap was measured. Ten cases of heel and adjacent vast soft tissue defects were repaired by the combined medial plantar and medialis pedis flaps. Age range 28-60 years old. T

[The use of Integra "artificial skin, dermal regeneration template" and the reverse radial forearm fasciocutaneous flap in the primary reconstruction of a septic hand injury].

UNLABELLED: Reconstruction was performed on the back of a hand following a poorly managed septic hand injury leading to skin and extensor apparatus necrosis using a reverse radial forearm fasciocutaneous flap and a bilayer artificial skin substitute. CASE DEMONSTRATION: Authors report on the first Hungarian case using Integra dermal regeneration template. Integra was placed on the back of the hand on an area left uncovered by the reverse radial forearm fasciocutaneous flap as well as on the flap harvest area. After biointegration of the regeneration template, the outer silicon layer was remov

Aesthetic surgery to augment orthognathic surgery.

The purpose of this article is to discuss some of the more commonly performed aesthetic surgical procedures to augment the final outcome of orthognathic surgery. These procedures are divided into two categories: those that can safely and predictably be performed simultaneously with orthognathic surgery and those that ideally should be performed independently of orthognathic surgery. This latter category also includes camouflage surgery, which includes aesthetic surgical procedures that are performed to camouflage or mask underlying dentofacial deformities in patients who, for various reasons,

Osteosynthetic thoracic stabilization after complete resection of the sternum.

We present a case of a 66-year-old diabetic patient with chronic non-healing sternum after CABG operation. After four attempts of sternum refixation within 2 years, the need of bone debridement resulted in a nearly complete loss of sternum. Finally, a mesh graft and a pectoralis muscle flap were used to bridge the sternal space. Despite good wound healing, the thoracic instability led to intolerable chest pain persisting over the next years. In this case report, we describe the successful thoracic stabilization by using transverse plate fixation, which resulted in perfect thoracic stabilizatio

Severe aortic regurgitation after repair of ruptured sinus of Valsalva aneurysm: the role of transesophageal echocardiography.

We report a 27-year-old woman who presented with worsening exertional dyspnea and palpitation. Transthoracic echocardiography showed ventricular septal defect, with left-to-right shunting and mild aortic regurgitation detected. Cardiac catheterization was performed that revealed sinus of Valsalva aneurysm with rupture into the right ventricle. Transesophageal echocardiography (TEE) confirmed a rupture of sinus of Valsalva aneurysm and ventricular septal defect. The patient underwent elective surgical repair of the ruptured sinus and intraoperative TEE demonstrated severe aortic regurgitation a

Double opposing rectangular advancement elongates tension line as much as Z-plasty: an experimental study in the rat inguinal skin.

PURPOSE: Z-plasty is a well-known and commonly used local flap technique in the treatment of contracture bands. Double opposing rectangular advancement is a new and alternate local flap design which can be used in the treatment of wide and linear postburn scar contractures. The technique consists of opposition of two adjacent rectangular flaps by advancement. This experimental study compares lengthening provided by Z-plasty and double opposing rectangular advancement. MATERIALS AND METHOD: Lower extremities of 10 male Sprague-Dawley rats were strained by hanging 400 g of weight to provide ten
