Imatinib mesylate for targeting the platelet-derived growth factor beta receptor in combination with fluorouracil and leucovorin in patients with refractory pancreatic, bile duct, colorectal, or gastric cancer--a dose-escalation Phase I trial.

BACKGROUND: In previous experimental models, because of its ability to inhibit the activity of platelet-derived growth factor beta receptor, imatinib decreased the interstitial fluid pressure and improved the delivery and efficacy of anticancer drugs, including fluorouracil. The objective of this Phase I study was to define the dose-limiting toxicity (DLT) and maximum tolerated dose (MTD) of imatinib in combination with fluorouracil and leucovorin in patients with chemotherapy-refractory gastrointestinal cancer. METHODS: A 3-patient cohort dose-escalating study design was used. Patients recei

Plasminogen activator inhibitor-1 and vitronectin promote vascular thrombosis in mice.

Occlusive thrombosis depends on the net balance between platelets, coagulation, and fibrinolytic factors. Epidemiologic information suggests that plasminogen activator inhibitor-1 (PAI-1), a central regulator of the fibrinolytic system, plays an important role in determining the overall risk for clinically significant vascular thrombosis. Vitronectin (VN), an abundant plasma and matrix glycoprotein, binds PAI-1 and stabilizes its active conformation. This study assessed the role of PAI-1 and VN expression in the formation of occlusive vascular thrombosis following arterial or venous injury. Th

Inducible nitric oxide synthase in the liver: regulation and function.

The inducible nitric oxide synthase (iNOS) gene is expressed by hepatocytes in a number of physiologic and pathophysiologic conditions affecting the liver including septic and hemorrhagic shock. The molecular regulation of iNOS expression is complex and occurs at multiple levels in the gene expression pathway. The cytokines TNF-alpha, IL-1beta, and INF-gamma synergistically activate iNOS expression in the liver, and the human iNOS gene was first cloned from cytokine-stimulated hepatocytes. iNOS expression requires the transcription factor NF-kappaB and is down-regulated by steroids, TGF-beta,

The Taipan snake venom time: a new test for lupus anticoagulant.

AIMS: To develop a specific test for lupus anticoagulant activity with reduced sensitivity to coagulation factor deficiency that would be suitable for analysis of plasmas from patients receiving oral anticoagulants. METHODS: A coagulation test based on the Taipan snake venom time (TSVT) with a platelet neutralisation procedure (PNP) was developed and compared with dilute Russell's viper venom time (DRVVT). The TSVT was used to test plasmas from patients receiving oral anticoagulant or heparin with mild liver dysfunction and with documented lupus anticoagulant. RESULTS: The optimised conditio

Intravenous administration of recombinant human macrophage colony-stimulating factor to patients with metastatic cancer: a phase I study.

PURPOSE: Recombinant human macrophage colony-stimulating factor (M-CSF) has been shown to stimulate specifically macrophage lineage differentiation in vitro and to induce cells capable of antitumor activity alone or in combination with an antibody. The administration of M-CSF to mice has demonstrated antitumor therapeutic effects in vivo. Therefore, a phase I trial of M-CSF administration to patients with metastatic cancer was undertaken. PATIENTS AND METHODS: M-CSF was given by intermittent intravenous bolus infusion every 8 hours for 7 days; the treatment cycle was repeated once after a wee

Hematological variations at rest and during maximal and submaximal exercise in a cold (0 degree C) environment.

The affect of negative thermal stress on hematological variables at rest, and during submaximal (sub ex) and maximal exercise (max ex) were observed for young males who volunteered in two experimental sessions, performed in cold (0 degree C) and in normal room temperature (20 degrees C). At rest, hematological variables such as RBC and derivates Hb and Hct were significantly increased (P less than 0.05) during cold stress exposure, while plasma volume decreased. The findings of this study suggest that the major factor inducing hypovolemia during low thermal stress can be imputed to local plasm

Inflammatory Biomarkers Following Orthopedic Surgery: Current Evidence, Clinical Applications, and Future Perspectives-A Narrative Review.

Orthopedic procedures trigger a complex inflammatory response that plays a central role in tissue repair and postoperative recovery. However, excessive or dysregulated inflammation may contribute to complications such as periprosthetic joint infection, thromboembolic events, delayed healing, or systemic organ dysfunction. Therefore, accurate perioperative monitoring of inflammatory activity has become increasingly important in orthopedic surgery. Evidence discussed in this review was identified through a literature search of PubMed, Scopus, and Web of Science databases covering publications fr

Plasma von Willebrand factor and endothelial-lineage flow-cytometry signals in acute pediatric IgA vasculitis: a cross-sectional pilot study.

INTRODUCTION: Endothelial activation is implicated in childhood IgA vasculitis, but candidate endothelial markers may reflect systemic inflammation rather than endothelial perturbation. METHODS: In this single-centre cross-sectional pilot study, we enrolled 13 children with acute IgA vasculitis and 20 age- and sex-comparable healthy controls screened to exclude acute infection, allergic disease and other inflammatory conditions. Plasma soluble fms-like tyrosine kinase 1, soluble vascular endothelial cadherin, von Willebrand factor (vWF) and vascular endothelial growth factor were measured by

Saccharomyces cerevisiae β-Glucan Training Induces a Nonclassical PGE2-High/NO-Low Macrophage Phenotype in Response to Pseudomonas aeruginosa Exopolysaccharide.

Exopolysaccharide (EPS), a major constituent of Pseudomonas aeruginosa biofilms, protects bacteria from M1-macrophage-mediated clearance while promoting chronic inflammation. This study investigated how Saccharomyces cerevisiae β-glucan (BG)-induced macrophage training reshapes subsequent inflammatory and antimicrobial responses to EPS. Peritoneal macrophages from C57BL/6 mice were trained in vitro with BG and subsequently stimulated with EPS purified from a clinical P. aeruginosa isolate obtained from a patient with severe cystic fibrosis. Cytokines, prostaglandin E2 (PGE2), and nitric oxide

Effects of injectable platelet-rich fibrin and Hank's balanced salt solution as rejuvenating media for the periodontal ligament cells of avulsed teeth.

BACKGROUND: The problem of avulsed teeth often being brought to the clinic in a dry state remains unresolved in the literature. Before an avulsed tooth is replanted, the periodontal ligament (PDL) cells require an appropriate storage medium to preserve their viability and biological properties. Hank's balanced salt solution (HBSS) is the reference medium recommended for this purpose in the literature. OBJECTIVES: The aim of the present study was to compare the effects of the reference solution - HBSS - and injectable platelet-rich fibrin (I-PRF) on the differentiation of PDL fibroblasts by ex

The Relationship Between FIB-4 Score and Dynapenia in Older Adults.

Background/Objectives: We evaluated whether fibrosis-4 (FIB-4) is associated with dynapenia and functional status in adults ≥65 years, and its value as a geriatric screening tool. Methods: In this cross-sectional study (July 2023-July 2025), 537 outpatients aged ≥65 years were evaluated. FIB-4 was calculated by the standard formula; a high-risk threshold of ≥2.0 was applied for age ≥65. Participants were compared as low- and high-FIB-4. Functional status was assessed by Basic Activities of Daily Living (ADLs), instrumental ADLs, and gait and balance tests. Dynapenia was defined as low muscle s

Blood pro-thrombotic analytes and platelet activation are associated with post-acute sequelae of COVID-19.

Post-Acute Sequelae of COVID-19 (PASC), or "long COVID," describes persistent symptoms following recovery from SARS-CoV-2 infection. Early identification of circulating biomarkers predictive of PASC is critical for prognosis and therapeutic development yet remains poorly defined. To address this gap, we conducted a longitudinal, multi-omics analysis of blood samples from COVID-19 patients (n = 75), stratified by acute disease severity and PASC status. We integrated targeted multiplex assays, untargeted proteomics (LC-MS), and whole blood flow cytometry to define immune and vascular signatures

Cellular and molecular characterisation of the peripheral immune environment in migraine.

INTRODUCTION: Peripheral immune dysfunction may be critically involved in the pathophysiology of migraine. Some evidence supports a role for peripheral T cells, monocytes, and humoral factors including kynurenine metabolites and cytokines, however a comprehensive picture has yet to emerge. OBJECTIVE: This study sought to undertake a systematic assessment of the immune changes in episodic and chronic migraine across phases of the migraine cycle. METHODS: Migraine patients in different phases of the migraine cycle with a confirmed diagnosis of episodic or chronic migraine and age- and sex-matc

Coagulation abnormalities following brown recluse spider (Loxosceles reclusa) envenomation: A description of 2 cases and review of the literature.

OBJECTIVE: Hemostatic abnormalities, including disseminated intravascular coagulation (DIC), are often cited as a common finding in patients following Loxosceles spider envenomation (ie, loxoscelism). The prevalence and severity of coagulopathy, however, particularly following L reclusa (ie, brown recluse) envenomation, is not well described. This study aimed to characterize coagulation laboratory parameters and coagulopathy in patients following L reclusa envenomation. METHODS: We evaluated the coagulation laboratory parameters (eg, prothrombin time, partial thromboplastin time, coagulation

Surgical intervention with cardiopulmonary bypass in a patient with von Willebrand disease.

BACKGROUND: This article presents a clinical case of surgical intervention with cardiopulmonary bypass (CPB) in a patient with von Willebrand disease (VWD). VWD is the most common inherited bleeding disorder, significantly complicating surgical procedures due to impaired hemostasis. Surgical treatment of patients with VWD, particularly in the context of cardiovascular surgery involving CPB, requires an individualized approach. This includes the optimization of von Willebrand factor (vWF) and factor VIII (FVIII) levels, which are critical to preventing perioperative bleeding. The article descri
