Deep transfer learning for breast cancer detection in underserved regions.

BACKGROUND AND OBJECTIVE: In Palestine, breast cancer is the leading cancer in women, constituting more than 34% of all cancer cases in women and 12% of all cancer related deaths. The Palestinian health care system is suffering from shortages, lack of advanced diagnostic equipment, among other issues. This study proposes a new two-step deep learning method for breast cancer detection in mammograms, with a particular focus on the potential of using it in low-resource settings like Palestine. The framework categorizes the tumor and adds a cost-effective and scalable diagnostic support tool that

Long term safety and efficacy of a functionalized nanogel with endothelin-1 and bradykinin receptor antagonist peptides for treatment of osteoarthritis of the metacarpophalangeal and distal interphalangeal joints in horses: a descriptive clinical pilot study.

BACKGROUND: Intra-articular drug delivery systems (DDS) are emerging as promising therapies for osteoarthritis (OA), yet their efficacy in spontaneous clinical cases remains largely untested. This uncontrolled, descriptive pilot study was designed to provide a proof of concept for the feasibility, safety, and preliminary clinical effects of intra-articular administration in sport horses with naturally occurring OA. The study involved a peptide-functionalized nanogel composed of chitosan and hyaluronic acid, delivering endothelin type A (BQ-123) and bradykinin B1 (R-954) receptor antagonists, w

Gamified Virtual Reality Training for Pneumatic Reduction of Intussusception in Children: Formative Evaluation of a Low-Fidelity Prototype.

BACKGROUND: Gamification, defined as the application of game design principles in nongame contexts, has garnered increasing attention for its potential to enhance motivation and engagement across various fields, including medical education and training. However, there is a lack of virtual reality (VR)-based simulations on pneumatic reduction of intussusception (PRI) in children in Brunei, thereby limiting practical training opportunities for medical students and surgical trainees. OBJECTIVE: This study aims to obtain experts' reviews of proposed gamification guidelines by conceptualizing a ga

Pharmacogenetic Evaluation of Hospitalized Patients Requiring Naloxone for Reversal of Acute Opioid Toxicity.

Background: Opioids are utilized for acute pain in hospitalized patients and carry the risk of unintentional toxicity. The relationship between unintentional toxicity within a hospital setting and genetic polymorphisms has not been fully evaluated within the literature to date. Assessment and utilization of pharmacogenetic data may be a way to prevent unintentional toxicity in hospitalized patients and reduce the need for naloxone administration. Objective: This study aimed to provide proof of concept for the comparison of allele frequencies of hospitalized patients who received naloxone for o

Nose-to-Brain Delivery of Donepezil Hydrochloride via Oleic Acid-Conjugated PAMAM G4 Dendrimers for the Treatment of Alzheimer's-Like Dementia.

Alzheimer's disease (AD) is a complex, progressive neurodegenerative disorder characterized by dementia and cognitive impairments. Acetylcholinesterase (AChE) inhibitors are generally prescribed for clinical management of AD symptoms. Donepezil hydrochloride (DPZ) is a reversible, selective, and noncompetitive inhibitor of AChE recommended for the treatment of mild-to-moderate AD. However, a higher dose of this drug must be administered to achieve adequate therapeutic concentrations in the brain, leading to peripheral side effects. In the present research work, oleic acid (OA) conjugated PAMAM

FDG-PET combined with learning vector quantization allows classification of neurodegenerative diseases and reveals the trajectory of idiopathic REM sleep behavior disorder.

BACKGROUND AND OBJECTIVES: 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET) combined with principal component analysis (PCA) has been applied to identify disease-related brain patterns in neurodegenerative disorders such as Parkinson's disease (PD), Dementia with Lewy Bodies (DLB) and Alzheimer's disease (AD). These patterns are used to quantify functional brain changes at the single subject level. This is especially relevant in determining disease progression in idiopathic REM sleep behavior disorder (iRBD), a prodromal stage of PD and DLB. However, the PCA method is limited in

An interpretable machine learning model for detecting vision-threatening diabetic retinopathy among patients with diabetic retinopathy: a web-based cross-sectional study.

BACKGROUND: Vision-threatening diabetic retinopathy (VTDR) is a severe complication of type 2 diabetes mellitus (T2DM), particularly prevalent in patients with prolonged disease duration, poor glycemic control, and systemic comorbidities. This condition frequently progresses asymptomatically toward irreversible blindness without timely intervention. The early identification of VTDR is challenging due to the lack of validated biomarkers and a reliance on subjective clinical assessments. This study aimed to develop and validate an interpretable machine learning (ML) model to detect VTDR among pa

A translational multimodal machine-learning prototype predicting valproate response in epilepsy treatment.

OBJECTIVE: Epilepsy affects ~1% of the global population and often requires lifelong antiseizure medication (ASM) therapy. Valproic acid (VPA) is a commonly prescribed first-line ASM, yet only approximately half of patients achieve sustained seizure freedom. Treatment selection remains largely empirical. We aimed to develop and independently validate a multimodal predictive model to estimate response to VPA and support more individualized treatment strategies. METHODS: This cross-sectional treatment response modeling study used data from a subset of the international Epi25 cohort (Belgium, Fi

Using Vibration for Secure Pairing With Implantable Medical Devices: Development and Usability Study.

BACKGROUND: Implantable medical devices (IMDs), such as pacemakers, increasingly communicate wirelessly with external devices. To secure this wireless communication channel, a pairing process is needed to bootstrap a secret key between the devices. Previous work has proposed pairing approaches that often adopt a "seamless" design and render the pairing process imperceptible to patients. This lack of user perception can significantly compromise security and pose threats to patients. OBJECTIVE: The study aimed to explore the use of highly perceptible vibrations for pairing with IMDs and aim to

Development and characterisation of a thermoreversible sol-gel for intranasal delivery of MRI and PET imaging agents.

Central nervous system disorders remain particularly difficult to manage due to the blood-brain barrier preventing effective uptake and distribution of most therapeutic and diagnostic agents to brain parenchyma. Intranasal delivery using thermoreversible sol-gels is a non-invasive strategy proposed to bypass the blood-brain barrier, enhance mucosal residence, and enable sustained release of an agent. This study investigated poloxamer-based sol-gels for the intranasal delivery of Gd-DTPA for MRI and [64Cu]Cu2+ for PET. Formulations containing a blend of poloxamers and hydroxypropyl methylcellul

Leveraging livestock movements to urban slaughterhouses for wide-spread Rift Valley fever virus surveillance in Western Kenya.

Rift Valley fever virus (RVFV) is an economically devastating, zoonotic arbovirus endemic across Africa with potential to cause severe disease in livestock and humans. Viral spread is primarily driven by movement of domestic ruminants and there is a high potential for transboundary spread. Despite influx of livestock to urban areas in response to the high demand for meat and animal products, RVFV has not been detected in any urban center. The objectives of this study were to determine the feasibility of assessing risk of RVFV introduction to urban Kisumu, Kenya, by testing slaughtered livestoc

An adenovirus-derived protein: A novel candidate for anti-diabetic drug development.

AIMS: Exposure to human adenovirus Ad36 is causatively and correlatively linked with better glycemic control in animals and humans, respectively. Although the anti-hyperglycemic property of Ad36 may offer some therapeutic potential, it is impractical to use an infectious agent for therapeutic benefit. Cell-based studies identified that Ad36 enhances cellular glucose disposal via its E4orf1 protein. Ability to improve glycemic control in vivo is a critical prerequisite for further investigating the therapeutic potential of E4orf1. Therefore, the aim of this study was to determine the ability of

Characterizing co-circulating respiratory virus genomic diversity in Switzerland with hybrid-capture sequencing and phylogenetic reconstructions: Insights into the 2023/24 season.

Respiratory viruses circulate yearly with strain-specific patterns. Although SARS-CoV-2 and Influenza A/B genomic surveillance is well-developed, most respiratory viruses are unevenly monitored, lacking geographical diversity to capture wider population dynamics. Consequently, insights into respiratory virus evolution are limited. We investigated the genetic diversity of these viruses within one country. During the 2023/24 season, we conducted whole-genome sequencing of 1'129 clinical samples using a hybrid-capture protocol. These samples were pre-tested by real-time PCR panels throughout Swit

Instrumented assessment of lower and upper motor neuron signs in amyotrophic lateral sclerosis using robotic manipulation: an explorative study.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a lethal progressive neurodegenerative disease characterized by upper motor neuron (UMN) and lower motor neuron (LMN) involvement. Their varying degree of involvement results in a clinical heterogenous picture, making clinical assessments of UMN signs in patients with ALS often challenging. We therefore explored whether instrumented assessment using robotic manipulation could potentially be a valuable tool to study signs of UMN involvement. METHODS: We examined the dynamics of the wrist joint of 15 patients with ALS and 15 healthy controls us

Chronic Activation of Tubulin Tyrosination Improves Heart Function.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is the most common cardiac genetic disorder caused by sarcomeric gene variants and associated with left ventricular hypertrophy and diastolic dysfunction. The role of the microtubule network has recently gained interest with the findings that microtubule detyrosination (dTyr-MT) is markedly elevated in heart failure. Acute reduction of dTyr-MT by inhibition of the detyrosinase (VASH [vasohibin]/SVBP [small VASH-binding protein] complex) or activation of the tyrosinase (TTL [tubulin tyrosine ligase]) markedly improved contractility and reduced stiff

A Prospective Phase II Study of Safety and Efficacy of Sorafenib Followed by 90Y Glass Microspheres for Patients with Advanced or Metastatic Hepatocellular Carcinoma.

PURPOSE: The most common cause of death in advanced/metastatic hepatocellular carcinoma (HCC) is liver failure due to tumor progression. While retrospective studies and meta-analyses of systemic therapy combined with liver-directed therapy have been performed, prospective studies of safety/efficacy of antiangiogenesis followed by intra-arterial therapies are lacking. We tested our hypothesis that sorafenib followed by yttrium 90 glass microspheres (90Y GMs) is safe and that survival outcomes may improve by controlling hepatic tumors. METHODS: We enrolled 38 Child-Pugh A patients with advanced

Oral sildenafil in infants with persistent pulmonary hypertension of the newborn: a pilot randomized blinded study.

BACKGROUND: Persistent pulmonary hypertension (PPHN) occurs in as many as 6.8 of 1000 live births. Mortality is approximately 10% to 20% with high-frequency ventilation, surfactant, inhaled nitric oxide, and extracorporeal membrane oxygenation but is much higher when these therapies are not available. Sildenafil is a phosphodiesterase inhibitor type 5 that selectively reduces pulmonary vascular resistance. OBJECTIVE: Our goal was to evaluate the feasibility of using oral sildenafil and its effect on oxygenation in PPHN. DESIGN: This study was a proof-of-concept, randomized, masked study in i
