Emerging infections, many zoonotic, are caused by a variety of pathogens with global distribution. Previously rare pathogens have emerged; global travel facilitates their rapid spread. Human encroachment on remote areas has brought contact with zoonotic diseases never before characterized. Although systematic study of rare outbreaks can be challenging, knowledge of emerging pathogens and their effects on women is accumulating. This article discusses effects of lymphocytic choriomeningitis virus, West Nile virus, severe acute respiratory syndrome coronavirus, avian influenza A virus, viral hemo

Until recently, most reported cases of bacteraemia caused by multidrug-resistant strains of Enterobacteriacae producing an extended-spectrum beta-lactamase (ESBL) in Europe have been nosocomial in origin. However, increasing numbers of reports of community-acquired bacteraemia and urinary tract infection caused by ESBL-producing microorganisms suggest that the geographical origin of patients should be taken into account as a risk-factor for possible ESBL production. Early identification of patients at high-risk of infection with ESBL-producing microorganisms, based on their geographical origin

We report a case of acute hepatitis E occurring in a 51-year-old French woman, revealed by an abrupt onset of polyarthritis involving the ankles and knees followed by the wrists and fingers. Polyarthritis lasted up to 3 months without recurrence. Our case was characterised by a 9-month prolonged viraemia with persistence of specific IgM in the serum. The hepatitis E virus belonged to genotype 3 and may have been contracted in France or during travel to hyperendemic areas. Our case shows that acute polyarthritis could be another systemic manifestation of hepatitis E virus infection.

We describe a case of interstitial pneumonia that was confirmed as scrub typhus by immunohistochemical (IHC) staining of an eschar. When a patient presents with interstitial pneumonia accompanied by generalized lymphadenopathy on the thoracic CT scan, clinicians should suspect scrub typhus, especially when the patient has a history of travel to a scrub typhus-endemic area. IHC staining on an eschar revealed invasion by Orientia tsutsugamushi coccobacilli of the patient's sweat ducts and glands as well as vascular endothelium. IHC staining of an eschar could be used as an early-confirmation dia

In the period between 1974 and 2003, the incidence of TBE increased appreciably in most European countries. Numerous factors, including climatic changes, biological (ecological) and non-biological factors, have an influence on the epidemiology of TBE. In addition, a greater awareness and better understanding (of the problem), improvements in diagnosis, an increase in travel, political and associated major social changes have had an impact on epidemiological data banks. We shall succeed in turning back the rising tide only by achieving further progress in the field of risk analysis, and redoubl

International travel and increasingly exotic diets have resulted in an increase in cases of cutaneous larva migrans in industrialized countries. A broader spectrum of clinical presentation and complications of cutaneous larva migrans is recognized by clinicians. A new syndrome, eosinophilic enteritis, has been described in Australia and may be more widespread as new diagnostic tests are used more widely. Other causes of cutaneous migration, such as gnathostomiasis and sparganosis, should be considered, and a recent outbreak of gnathostomiasis in Mexico suggests that clinicians must be alert to

West Nile virus infection has quickly become a feared cause of neurologic disability and death, particularly when it presents with encephalitis. Recent epidemics in endemic regions of Eurasia and Africa, as well as its recent spread to North America, have highlighted the need for all physicians to be aware of its clinical presentation and course. In particular, because of the increased susceptibility of West Nile virus infection during outdoor activities, as well as during travel to the Middle East and Southeastern Europe, military physicians should be informed about case recognition, manageme

There are a whole series of emerging and re-emerging zoonotic diseases present in the Northern Hemisphere and the author describes four of them, namely, bartonellosis, leptospirosis, Lyme borreliosis and plague. Reasons for the emergence or re-emergence of such diseases are not clear, but factors such as human demographics, economic development and land use, international travel and commerce, and microbial adaptation, are thought to be involved. Control of emerging and re-emerging diseases has become a major challenge for the international community and it is important to disseminate informati

Biliary parasitosis is one of the common causes of biliary obstruction in developing countries and can often be confused with stone disease. With increased worldwide travel and immigration, these conditions are not limited to the developing countries alone. Ascariasis, hydatid liver disease, clonorchiasis, and fascioliasis are the commonly encountered parasitic infestations of the biliary tract usually presenting with biliary colic or cholangitis. Endoscopy has an important role in the diagnosis and emergent management of these conditions and in elective endoscopic therapy of associated compli

Histoplasmosis is an illness which occurs very rarely in Europe and it is especially rare in Germany. A generalised infection with Histoplasma capulatum, a systemic mycosis of the mononuclear phagocyte system (MPS), occurs only in individuals with weakened immune systems. Within the framework of diagnostics, a pathologist can be confronted with histoplasmosis since there has been an increase in travel to and from endemic regions, as well as an increase in the number of diseases of the immune system. The presented case reports the histological intravital and post-mortem diagnostics of dissemina

Orbital hydatid cyst is rare. Hydatid cysts in other organ systems are not mentioned in the previous papers emphasizing an orbital hydatid. Four cases of inferiorly located retrobulbar hydatid cysts have been reported previously. We present a rare inferiorly located retrobulbar hydatid cyst case in a Turkish child that also had multiple hydatid lesions in the lungs and the liver. We present the orbital CT and MRI findings. Preoperative diagnosis is important to avoid cyst rupture. Although hydatid disease is prevalent in South America, Australasia, the Middle East and Mediterranean countries,

After many years the incidence of infective syphilis (infection with Treponema pallidum) is increasing in the United Kingdom. This may reflect changes in sexual attitudes and behaviour, altering trends in HIV disease, and increased foreign travel. Oral disease as a consequence of primary syphilis is rare. The present report details two patients presenting to an oral medicine clinic in London, within a 6-month period in 1999, with oral ulceration as their only clinical manifestation of undiagnosed primary syphilis. The oral aspects of early syphilis and the need for dentists to be aware of chan

Phage typing of Shigella sonnei has been used to examine isolates from the 1991-2 sonnei dysentery outbreak in England and Wales and compare them with strains isolated during and following a widespread foodborne outbreak in 1994 which was associated with consumption of imported lettuce. The distribution of phage types was different in the three periods studied with PT 3 predominating during 1991-2, PT 2 during the 'lettuce' outbreak in the summer months and PT 6 during the subsequent months. PT 6 was frequently associated with travel outside the UK. Variation was also seen in the distribution

Amoebic appendicitis is very rare, occurring in about 0.5 to 1% of acute appendicitis in tropical countries. The most severe complication is transmural amoebic colitis with perforation, described in 1.6 to 3.2% of cases. The mortality of such cases can be very high (up to 80%). We present a Swiss patient with amoebic appendicitis followed by severe perforated colitis; a total colectomy was necessary and the patient survived. Because of the good results of amoebicidal therapy and because of the severity of the complication after colitis we suggest that patients with signs of acute appendicitis

Pulmonary coccidioidomycosis is a fungal disease seen primarily in the southwestern United States and Mexico. However, with the advent of mass travel it is being seen increasingly in countries outside the zone where it is endemic. Pulmonary coccidioidomycosis, which has a high infectivity, is acquired by inhalation of fungal arthrospores. Its infectivity is increased in immunosuppressed patients, particularly those with the acquired immunodeficiency syndrome. Because of these factors, it is important for radiologists (particularly those practising outside the area of endemicity) to recognize t

A 66-year-old man, immunosuppressed because of a lymphoma and with severe ischaemic heart disease and proteinuria, presented with a swelling on the tongue due to leishmaniasis. His travel history suggested that he had acquired the infection in the Mediterranean area some years earlier. He was treated with liposomal amphotericin B for 21 days and made a good recovery despite a temporary deterioration in renal function. Oral lesions are a rare manifestation of Old World leishmaniasis. Liposomal amphotericin B is a novel treatment that may be well tolerated when there is concern about the toxicit

A 54 year old Bangladeshi man presented with a history and chest X-ray appearances suggestive of pulmonary tuberculosis. Following deterioration 4 weeks later, he required ventilation. Although a blood culture isolate was subsequently found to be Pseudomonas pseudomallei, it was initially misidentified and dismissed as a contaminant. Further cultures demonstrated the organism, but the patient died, despite treatment with ceftazidime. The case illustrates the importance of taking a detailed travel history and having a high index of suspicion in patients from South East Asia and the Indian sub-c

As the fields of astronomy, cosmology, and space travel move rapidly forward, so must space medicine. The manned space program and medical knowledge and support have developed in tandem. Dermatology will play a fundamental role in survival during space flight. This paper reviews past, present, and future accomplishments of the space program as they relate to medicine and characterizes some of dermatology's multiple roles in the future. It further explores the immunologic alterations noted during space flight and the attendant implications for health and well-being both in flight and on return

By its very nature, running allows for a freedom that many athletic endeavors lack. The ability to run anytime and anywhere may be detrimental in itself, in that it may allow runners to place themselves in environmental circumstances that are unhealthy. Acclimatization to heat, altitude, and air pollution conditions can occur. Rapid changes in environment brought about by travel may lead to lack of recognition of appropriate clothing or running routes. Usual workout intensities and durations may not be appropriate for the specific environment at hand. Runners are advised to recognize the poten

Cerebral cysticercosis is no longer an uncommon disease in areas such as Southern California to which there is extensive travel and immigration from adjoining areas in which the disease is endemic. The diagnosis of cerebral cysticercosis should be entertained in patients from such an endemic area when they present with neurological symptoms and characteristic CT findings. There is no specific or definitive therapy for the disease; treatment should be directed at alleviating its manifestations. We recommend surgical consideration in those patients with hydrocephalus, intraventricular cysts, sol
