Gender-specific reference ranges of the vocal extent measure in young and healthy adults.

Objective: The recently developed vocal extent measure (VEM) quantifies a patient's vocal capacity as documented in the voice range profile (VRP). This study presents the first reference ranges of the VEM for young subjects without voice complaints. Furthermore, this study investigates the influence of gender on the VEM as well as the correlation of the VEM with the dysphonia severity index (DSI).Patients and methods: Reference ranges were captured by combining a retrospective analysis of subjects who received a medical fitness certificate of a healthy voice (n = 135) and a prospective analysi

Effectiveness of prowling with proprioceptive training on knee hyperextension among stroke subjects using videographic observation- a randomised controlled trial.

INTRODUCTION: Knee hyperextension is seen in 40-68% of ambulating hemiparetics. If left untreated it may lead to laxity of posterior structures of knee resulting in early degeneration of knee leading to pain, reduced independence in activities of daily living (ADL), deformities and instability. In this study we hypothesize walking with bent knee attitude (prowling) along with proprioceptive training may help to reduce knee hyperextension during the stance phase of gait cycle. OBJECTIVE: To test the efficacy of prowling along with proprioceptive training on knee hyperextension range, dorsiflex

Repair of Displaced Partial Articular Fracture of the Distal Femur: The Hoffa Fracture.

PURPOSE: The Hoffa fracture is a rare fracture pattern consisting of a unicondylar posterior fracture of the distal femur. This video demonstrates a case of Hoffa fracture repair using headless screw compression. METHODS: Hoffa fractures are intra-articular in nature. Given that isolated Hoffa fractures are rare, there is little information available as to the best management of this injury. However, several small studies have demonstrated good-to-excellent functional results after anatomical reduction and rigid fixation of the Hoffa fracture, followed by early mobilization. RESULTS: This vi

A Central Role for Sympathetic Nerves in Herpes Stromal Keratitis in Mice.

PURPOSE: Herpes simplex virus type 1 (HSV-1) is a neurotrophic virus that can cause herpes stromal keratitis (HSK), a severe corneal inflammation that can lead to corneal scarring and blindness. This study identified neurologic changes that occur in HSV-1-infected corneas and related them to HSV-1-induced immunopathology. METHODS: Corneas of BALB/c and C57BL/6 mice were infected with HSV-1 strains that induce HSK. Changes in sensory nerves were identified by immunofluorescence staining of sensory and sympathetic nerves for substance P (SP) and tyrosine hydroxylase (TH), respectively, and conf

Emphysematous lungs do not affect visibility of virtual-assisted lung mapping.

BACKGROUND: It has recently been suggested that virtual-assisted lung mapping may enable safer and more reliable intraoperative navigation compared to conventional preoperative lung marking strategies. This study aimed to examine the relationship between emphysematous lungs and the intraoperative visibility of virtual-assisted lung mapping markings. METHODS: We retrospectively analyzed the data of 21 patients who underwent virtual-assisted lung mapping followed by lung resection between October 2013 and May 2015. The visibility of the intraoperative lung markings was graded. The degree of pul

Anesthetic Management of a Patient With Takayasu Arteritis.

Takayasu arteritis is a rare chronic progressive panendarteritis involving the aorta and its main branches. Anesthesia in patients with this disease can be complicated by severe uncontrolled hypertension, end-organ dysfunction, and stenosis of major blood vessels. In this case, general anesthesia was induced with sevoflurane and remifentanil without complications. To prevent intraoperative complications, we conducted intubation with a rigid video laryngoscope with careful consideration of the concentrations of analgesics and sedatives used. This case demonstrates the importance of anesthetic t

Towards Objective Voice Assessment: The Diplophonia Diagram.

OBJECTIVES: Diplophonia is an often misinterpreted symptom of disordered voice, and needs objectification. An audio signal processing algorithm for the detection of diplophonia is proposed. Diplophonia is produced by two distinct oscillators, which yield a profound physiological interpretation. The algorithm's performance is compared with the clinical standard parameter degree of subharmonics (DSH). STUDY DESIGN: This is a prospective study. METHODS: A total of 50 dysphonic subjects with (28 with diplophonia and 22 without diplophonia) and 30 subjects with euphonia were included in the study

Smile analysis in different facial patterns and its correlation with underlying hard tissues.

BACKGROUND: The subject's inherent growth pattern can be an effective factor in characteristics of smile. More vertical growth in the posterior maxilla than in the anterior maxilla could result in a changed relationship between the occlusal plane and the curvature of the lower lip upon smile. In order to broaden the understanding of how smile gets affected by growth pattern and the underlying hard tissues, the present study was undertaken to compare smile in various growth patterns, to determine sexual dimorphism, if any; as well as to correlate smile with underlying hard tissues. METHODS: On

A new quantitative method for evaluating freezing of gait and dual-attention task deficits in Parkinson's disease.

People with Parkinson's disease (PD) can exhibit disabling gait symptoms such as freezing of gait especially when distracted by a secondary task. Quantitative measurement method of this type of cognitive-motor abnormality, however, remains poorly developed. Here we examined whether stepping-in-place (SIP) with a concurrent mental task (e.g., subtraction) can be used as a simple method for evaluating cognitive-motor deficits in PD. We used a 4th generation iPod Touch sensor system to capture hip flexion data and obtain step height (SH) measurements (z axis). The accuracy of the method was compa

High definition bronchoscopy: a randomized exploratory study of diagnostic value compared to standard white light bronchoscopy and autofluorescence bronchoscopy.

BACKGROUND: Videobronchoscopy is an essential diagnostic procedure for evaluation of the central airways and pivotal for the diagnosis and staging of lung cancer. Technological improvements have resulted in high definition (HD) images with advanced real time image enhancement techniques (i-scan). OBJECTIVES: In this study we aimed to explore the sensitivity of HD+ i-scan bronchoscopy for detection of epithelial changes like vascular abnormalities and suspicious preinvasive lesions, and tumors. METHODS: In patients scheduled for a therapeutic or diagnostic procedure under general anesthesia v

Posterior parietal cortex activity reflects the significance of others' actions during natural viewing.

The posterior parietal cortex (PPC) has been associated with multiple stimulus-driven (e.g., processing stimulus movements, providing visual signals for the motor system), goal-directed (e.g., directing visual attention to a target, processing behavioral priority of intentions), and action-related functions in previous studies with non-naturalistic paradigms. Here, we examined how these functions reflect PPC activity during natural viewing. Fourteen healthy volunteers watched a re-edited movie during functional magnetic resonance imaging (fMRI). Participants separately annotated behavioral pri

Caloric test and video-head-impulse: a study of vertigo/dizziness patients in a community hospital.

The head-impulse test (HIT) is an important test for examining unilateral vestibular hypofunction. The new video-head-impulse test (vHIT) is more sensitive and specific than the clinical bedside-head-impulse test (bHIT). Alternatively, one can test for vestibular hypofunction with the caloric irrigation test. Various studies showed that both tests may not always identify vestibular hypofunction; instead, the results of the tests might be contradictory. To evaluate the diagnostic value of these tests, we routinely measured patients exhibiting vertigo or dizziness at our community hospital with

Hostile intent in public crowded spaces: a field study.

Hostile reconnaissance is vital to successful terrorist activity. Individuals carrying out this activity are likely to experience raised levels of stress and this will manifest itself at biological, physiological, psychological and behavioural levels, providing an opportunity for detection. A field trial was undertaken in an ecologically valid environment measuring variables considered likely to be salient during hostile intent. The parameters examined in the field trial varied in a predictable manner and suggest that stressed individuals secrete a volatile steroid based marker that could form

What's shaking in the ICU? The differential diagnosis of seizures in the intensive care setting.

To analyze what conditions mimic seizures in the intensive care unit (ICU) setting, we reviewed all bedside electroencephalography (EEG)-videos obtained in the adult ICU setting over an 18-month period. Only those studies performed for "possible seizures" due to motor phenomena and whose clinical events were captured on video were analyzed. A total of 52 studies were performed. Fourteen patients (27%) had epileptic seizures. Thirty-eight (73%) had nonepileptic events. These consisted of 12 (23%) with tremor-like movements, 7 (13.5%) with multifocal myoclonic jerks without electrographic change

Irradiation exacerbates cortical cytopathology in the Eker rat model of tuberous sclerosis complex, but does not induce hyperexcitability.

Tuberous sclerosis complex (TSC) is an autosomal dominant disorder characterized by multi-organ pathologies. Most TSC patients exhibit seizures, usually starting in early childhood. The neuropathological hallmarks of the disease - cortical tubers, containing cytopathological neuronal and glial cell types - appear to be the source of seizure initiation. However, the contribution of these aberrant cell populations to TSC-associated epilepsies is not fully understood. To gain further insight, investigators have attempted to generate animal models with TSC-like brain abnormalities. In the current
