Brucellar sternoclavicular arthritis, the forgotten complication.

Of 511 cases of brucellosis studied between December 1983 and February 1986, four (0.8%) had sternoclavicular (STCL) arthritis. Two were male and two female, and only one was younger than 50 years old. All four cases had significantly high specific IgG antibody titres (1 of 1280), measured by the indirect immunofluorescent (IIF) test, and two had Brucella melitensis isolated from their blood. In two cases, STCL arthritis was the presenting problem, and it was associated in one with ankle arthritis, hepatitis, renal impairment, orogenital ulcers and a haematological picture of myelodysplasia; i

From misdiagnosis to definitive diagnosis of SAPHO syndrome during a routine health check-up: a case report and literature review.

BACKGROUND: SAPHO syndrome is a rare chronic autoinflammatory disease with heterogeneous clinical manifestations. Because there are no specific diagnostic biomarkers, the condition is often diagnosed late or misdiagnosed. CASE PRESENTATION: A 51-year-old man presented with unexplained chest pain and low back pain. He had been evaluated at several hospitals and had received different diagnoses related to bone disease. During a routine health check-up, imaging revealed multiple osteolytic lesions in the sternum, together with sclerosis and hyperostosis involving the sternoclavicular and sternoc

[Sterno-costo-clavicular hyperostosis (author's transl)].

A case is described in which bilateral subclavian vein occlusion was caused by fusiform expansion of the clavicle with hyperostosis, ankylosis of the sterno-clavicular joint, widening and thickening of the sternum and ossification of the costal cartilage of the first ribs. Histologically this proved to be due to an hyperostotic spongiosclerosis.

Segmental vertebral artery agenesis with deep cervical reconstitution in a cervicothoracic variant cluster.

PURPOSE: To report a complex cervicothoracic vascular-osseous phenotype detected on CTA and define its surgical and radiological relevance. METHODS: Neck CTA of a 76-year-old woman was reviewed on multiplanar reconstructions for aortic arch branching, supraaortic courses, vertebral artery segments, collateral channels, cervical ribs, styloid processes, and pharyngeal carotid relationships. RESULTS: CTA showed a common origin of the brachiocephalic trunk and left common carotid artery (colloquially termed bovine aortic arch), bilateral rudimentary cervical ribs, high-riding pretracheal brachi

Transoesophageal echocardiographic evaluation of central venous catheter positioning using Peres' formula or a radiological landmark-based approach: a prospective randomized single-centre study.

BACKGROUND: The lower superior vena cava (SVC), near its junction with the right atrium (RA), is considered the ideal location for the central venous catheter tip to ensure proper function and prevent injuries. We determined catheter insertion depth with a new formula using the sternoclavicular joint and the carina as radiological landmarks, with a 1.5 cm safety margin. The accuracy of tip positioning with the radiological landmark-based technique (R) and Peres' formula (P) was compared using transoesophageal echocardiography. METHODS: Real-time ultrasound-guided central venous catheter inser

Native Joint Septic Arthritis: Comparison of Outcomes with Medical and Surgical Management.

OBJECTIVE: To determine whether there are differences in the outcomes of native joint septic arthritis (SA) in adults, based on medical versus surgical management. METHODS: A 10-year retrospective single-center study was conducted of patients admitted to a tertiary care hospital between January 1, 2006 and December 31, 2015 with a diagnosis of SA to compare outcomes based on the management approach taken: medical (bedside closed-needle joint aspiration) versus surgical (arthrotomy/arthroscopy). Evaluated outcomes included joint recovery, time to recovery, length of stay, disposition to home v

Successful treatment of deep sternal infections following open heart surgery by bilateral pectoralis major flaps.

OBJECTIVES: Severe sternum necrosis requiring extended resection necessitates plastic reconstruction of the resulting defect and stabilization of the chest. We analyzed the outcome of patients undergoing bilateral pectoralis major flap repair on functional and cosmetic results, chest stabilization and pulmonary function. METHODS: Twelve patients undergoing cardiac surgery between 1997 and 2001 suffered from a deep mediastinal wound infection and sternum necrosis. After a mean of two attempts of extensive wound debridement, all 12 patients underwent complete sternal resection with plastic reco

18F-fluorodeoxyglucose PET/CT uptake characteristics in immune checkpoint inhibitor-induced polymyalgia rheumatica compared to idiopathic polymyalgia rheumatica.

OBJECTIVES: Polymyalgia rheumatica (PMR)-like manifestations are among the most common rheumatological immune checkpoint inhibitor (ICI)-related adverse events. 18F-fluorodeoxyglucose (FDG) PET/CT is useful for the follow-up of various solid organ cancers and for PMR assessment. The aim of this study was to describe PET/CT aspects of ICI-related PMR (ICI-PMR) in comparison with idiopathic PMR. METHODS: This was an observational, comparative, retrospective, single-centre study. Inclusion criteria were a diagnosis of ICI-PMR or idiopathic PMR and availability of an 18F-FDG PET/CT within 3 month

Major Vessels of the Interclavicular Suprasternal Space: A Suprasternal Notch-Referenced Topographic Classification with Potential Procedural Relevance.

Background/Objectives: The suprasternal notch (SN) is the conventional landmark for front-of-neck airway access and anterior cervical surgery, yet the great vessels may extend to or above it. To our knowledge, no previous study has jointly classified the aortic arch (AA), brachiocephalic trunk (BCT), left brachiocephalic vein (LBCV), and sternoclavicular joint (SCJ) configuration relative to the SN in a single adult cohort. Methods: In this single-centre retrospective study, 100 consecutive adult computed tomography angiography (CTA) studies were reviewed. Using a horizontal SN reference plane

Beyond the Skin: Nail Clues to a Bone Disease.

Chronic recurrent multifocal osteomyelitis (CRMO) is a rare autoinflammatory bone disorder that predominantly affects children, and approximately 25% patients also present with an associated inflammatory condition. Most patients respond to nonsteroidal anti-inflammatory drugs, but refractory cases may need corticosteroids, bisphosphonate therapy, or disease-modifying antirheumatic drugs (DMARDs) like methotrexate, sulfasalazine, tumor necrosis factor-alpha (TNF-α) inhibitors, or even anti-interleukin-1 (anti-IL-1) blockers. We report the case of a previously healthy nine-year-old girl presenti

Shoulder girdle injuries involving the medial clavicle differ from lateral clavicle injuries with a focus on concomitant injuries and management strategies: A retrospective study based on nationwide routine data.

INTRODUCTION: Although shoulder girdle injuries are frequent, those of the medial part are widely unexplored. Our aim is to improve the knowledge of this rare injury and its management in Germany by big data analysis. METHODS: The data are based on ICD-10 codes of all German hospitals as provided by the German Federal Statistical Office. Based on the ICD-10 codes S42.01 (medial clavicle fracture, MCF) and S43.2 (sternoclavicular joint dislocation, SCJD), anonymized patient data from 2012 to 2014 were evaluated retrospectively for epidemiologic issues. We analyzed especially the concomitant in

Multifocal septic osteomyelitis mimicking skeletal metastatic disease in a patient with prostate cancer.

We present an unusual case of a 59 years old patient with prostate cancer, who was referred to our hospital with pleurodenia, low back and other sites of bone ostalgia, for bone scintiscan. The patient underwent a whole body bone scanning after the intravenous administration of 740MBq (99m)Tc-methylene diphosphonate (MDP). The main findings of the study were: increased radiotracer uptake at the T5, T9-T10 vertebrae, the head of the 11th rib and the area of the left sternoclavicular joint (SCJ), which were initially attributed to skeletal metastatic lesions. Another "hot" area in the left knee,

A Case Report of Hematogenous Osteomyelitis of the Manubrium Caused by Seeding from a Colovesicular Fistula.

INTRODUCTION: Osteomyelitis can occur at various osseous locations and commonly presents in the emergency department (ED). The incidence of osteomyelitis is 21.8 cases per 100,000 persons annually.1 Hematogenous osteomyelitis typically occurs in the vertebrae; however, it may seldomly occur in the manubrium. Hematogenous osteomyelitis can be seen in patients with complicated thoracic surgery, radiation, fracture, diabetes, immunosuppression, steroid therapy, and malnutrition.2 Because signs and symptoms of osteomyelitis may be nonspecific, clinicians must have high suspicion based on history a
