Multiple avascular necrosis in a patient with systemic lupus erythematosus/systemic sclerosis overlap syndrome.

A white female patient developed overlapping features of systemic lupus erythematosus (SLE) and systemic sclerosis (SSc) with severe pulmonary compromise. She was treated with steroids and azathioprine, which improved her clinical condition and spirometric status. In May 2002 she presented with continuous pain in her left ankle that continued even during rest and under treatment with nonsteroidal anti-inflammatory drugs (NSAIDs). Magnetic resonance imaging (MRI) showed multiple avascular necrosis (AVN). Rest and kinesitherapy were indicated for 1 year, and gradually an orthosis was introduced

A matter arising: When should inflammatory and autoimmune rheumatic diseases be considered 'early'?

BACKGROUND: Early diagnosis is pivotal for guiding the intensity of clinical monitoring, optimizing therapeutic strategies and preventing organ damage in inflammatory and autoimmune rheumatic diseases (IARDs). This review summarizes current evidence on early diagnostic and therapeutic approaches of some IARDs, including rheumatoid arthritis (RA), systemic sclerosis (SSc) and detection of large-vessel vasculitis (LVV) in polymyalgia rheumatica (PMR), representing distinct pathophysiological mechanisms of joint synovitis, tissue fibrosis and vasculitis, respectively. METHODS: A comprehensive na

Local delivery of antithrombotic drug prevents restenosis after balloon angioplasty in atherosclerotic rabbit artery.

We investigated the ability of various antithrombotic drugs, delivered locally, to prevent restenosis after angioplasty in hypercholesterolemic rabbits. After dilating atherosclerotic iliac stenoses by balloon angioplasty, a low dose of heparin or a new antithrombotic drug, such as low molecular weight heparin (fragmin), argatroban, or batroxobin, was delivered locally using the balloon double-occlusion technique. In 1 group, high-dose heparin was administered intravenously. Animals that received no drugs served as a control group. After angioplasty, the stenotic segment was dilated and the me

Carbon dioxide vs. warm-water hand baths in systemic sclerosis with secondary Raynaud's syndrome - A capillaroscopy centered randomized controlled trial.

BACKGROUND: Raynaud's phenomenon (RP) affects nearly all patients with systemic sclerosis (SSc) and causes substantial morbidity and functional impairment. We evaluated the immediate, segment-specific microvascular effects of a single carbon dioxide (CO₂) versus warm water hand baths in SSc with secondary RP, including healthy controls as physiological reference. METHODS: In this single-centre, randomized controlled trial, 24 SSc patients were allocated (1:1) to a 15-min CO₂ bath (2 g/L, 35 °C) or warm water bath (40-42 °C). Twelve healthy controls received a CO2 bath. Nailfold capillaroscopy

Association of circulating miR-29b with microvascular dysfunction and digital ulcers in patients with systemic sclerosis: a single-centre pilot study.

OBJECTIVES: Systemic sclerosis is a rare autoimmune disorder marked by vasculopathy, immune dysregulation, and progressive fibrosis. Digital ulcers (DUs) reflect severe microvascular dysfunction. This pilot, single-centre, observational study investigated whether circulating miR-29b is associated with DU occurrence, recurrence, and microvascular impairment. METHODS: Twenty-nine patients with systemic sclerosis and 17 healthy controls (HC) were evaluated. Clinical data, including DU history and recurrence, were collected. Cutaneous blood perfusion was assessed by laser speckle contrast analysi

Thymic stromal lymphopoietin and digital microvascular damage in systemic sclerosis patients: A pilot study.

BACKGROUND: Systemic sclerosis (SSc) is a complex autoimmune connective-tissue disease, characterised by vasculopathy and fibrosis of the skin and internal organs. Activation of microvascular endothelial cells (ECs) causes the intimal hyperplasia that characterises the vascular remodelling in SSc. The most frequent complication of SSc is the development of digital ulcers (DUs). Thymic stromal lymphopoietin (TSLP) may trigger fibrosis and sustain vascular damage. Aim of this study was to evaluate the correlation between serum level of TSLP and DUs. METHODS: 75 consecutive SSc patients were enr

Image enhancement effect on inter and intra-observer reliability of nailfold capillary assessment.

BACKGROUND: Nailfold capillaroscopy (NC) is a diagnostic imaging technique that is used to assess the blood capillary network in the nailfold area. NC is routinely used for patients with microcirculation problems, such as systemic sclerosis and other connective tissue diseases. Experts commonly use subjective evaluation as a reference point in images of nailfold video capillaroscopy, so it is important to reduce the inherent ambiguities in human judgment and diagnosis. Image quality is an important factor that affects measurement error and assessment time of NC images. OBJECTIVE: In this stud

Normal sublingual microcirculation during painful crisis in sickle cell disease.

Obstruction of the microcirculation is the most important cause of painful crisis in sickle cell disease (SCD). Extensive microvascular obstruction has been observed in mouse models of SCD. A technique to determine the extent of the microcirculatory obstructions in humans may be helpful in the clinical setting and for research purposes. Therefore, we measured sublingual microcirculation longitudinally in patients with SCD admitted with painful crisis. Sublingual microcirculation was recorded with side-stream darkfield (SDF) imaging and semi-quantified with a microvascular flow index (MFI) on a

Sublingual Sidestream Dark Field (SDF) Microscopy With GlycoCheck Analysis in Individuals With and Without Cardiovascular Risk Factors and Disease.

BACKGROUND: Microvascular density and endothelial glycocalyx function may provide insights into early atherosclerosis and cardiovascular disease (CVD) risk. Sublingual sidestream darkfield (SDF) microscopy permits imaging of red blood cells (RBC) to assess the microcirculation. We sought to define reference ranges for SDF microscopy parameters in healthy populations and individuals with coronary artery disease (CAD) and to assess factors correlated with microcirculatory abnormalities. METHODS: Adults with and without CVD risk factors and epicardial CAD underwent SDF microscopy using a CapiSco

Predicting taxane-induced peripheral neuropathy by nailfold capillaroscopy: The CAPNEU pilot study.

BACKGROUND: Taxane-induced peripheral neuropathy (TIPN) remains a frequent, dose-limiting toxicity in breast cancer, and reliable pre-treatment predictors are lacking. Nailfold capillaroscopy (NFC) offers a non-invasive window into systemic microvascular morphology, which may reflect vascular susceptibility relevant to neuropathy risk. The objective of the study was to evaluate whether baseline NFC morphology is associated with taxane-related neuropathy severity and symptom burden, and to explore ROC-derived thresholds of NFC parameters for identifying clinically significant neuropathy.
