Idiopathic condylar resorption: diagnosis, treatment protocol, and outcomes.

Idiopathic condylar resorption is a poorly understood progressive disease that affects the TMJ and that can result in malocclusion, facial disfigurement, TMJ dysfunction, and pain. This article presents the diagnostic criteria for idiopathic condylar resorption and a new treatment protocol for management of this pathologic condition. Idiopathic condylar resorption most often occurs in teenage girls but can occur at any age, although rarely over the age of 40 years. These patients have a common facial morphology including: (1) high occlusal and mandibular plane angles, (2) progressively retrudi

Costochondral grafts to replace mandibular condyles in juvenile chronic arthritis patients: long-term effects on facial growth.

Long-term facial growth and clinical outcome after replacement of arthritic mandibular condyles by costochondral grafts and postoperative orthodontic guidance of the occlusion were studied until completion of facial growth in patients (n = 12) with juvenile chronic arthritis (JCA) affecting their temporo-mandibular joints (TMJs). The patients were between 10.1 and 16.7 years of age at surgery. Clinical records and radiographs for cephalometric measurements were taken preoperatively, 6-8 weeks after surgery and after completion of facial growth. A considerable potential for growth of the costoc

Oropharyngeal airway dimensions and functional-orthopedic treatment in skeletal Class II cases.

Mandibular deficiency may be a factor in reduced oropharyngeal airway (OAW) dimensions and related impaired respiratory function. The purpose of this study was to evaluate the use of functional-orthopedic devices in increasing OAW dimensions in children with Class II skeletal patterns (ANB > 4) and clinically deficient mandibles. Comparisons were made between two groups, one comprising 26 treated patients and the other comprising 15 controls. Student's t-tests, paired t-tests, discriminant analyses, and Pearson's r-correlation coefficients were performed to evaluate group differences and to se

Lengthening of the hypoplastic mandible by gradual distraction in childhood--a preliminary report.

Uni- or bilateral mandibular hypoplasia can be associated with various syndromes or is acquired after early traumatic or inflammatory disease in the temporomandibular joint (TMJ). Early treatment is necessary to avoid consequent impairment of midfacial growth. The standard treatment of these malformations consists of the application of bone grafts which can lead to unpredictable growth. Furthermore, these procedures often require intermaxillary fixation and sometimes blood transfusions. Lengthening of the mandible by gradual distraction, according to the method of Ilizarov, opens new perspecti

Early surgical-orthodontic treatment of mandibular hypoplasia in juvenile chronic arthritis.

This study is an intermediate evaluation of early surgical-orthodontic treatment in 7 patients with uni- or bilateral hypoplastic mandibular growth due to juvenile chronic arthritis. Resection of the condylar heads and reconstruction with costochondral grafts was undertaken between 10 and 14 years of age and followed by orthodontic treatment. At the latest follow-up, 2-4.5 years postoperatively, all patients showed mandibular growth. Some even showed a tendency towards progenia. The scheduling of the surgery in relation to completion of growth seemed to be of importance in relation to the magn

The CHARGE association: report of two cases.

Although many reports of the CHARGE association appear in the literature, the dental findings were never discussed before. In this report of two cases, both patients present with delayed eruption of the permanent teeth and a remarkable similarity of the eruption pattern of the mandibular teeth. One lower permanent central incisor is congenitally missing, while the other is malformed. The lower permanent laterals of JM have erupted lingually and interfere with his speech and function of the tongue. Mandibular retrognathism is present. TM had a V-shaped constricted upper arch, which was expanded

The effect of mandibular osteotomy in three patients with hypersomnia sleep apnea.

Hypersomnia sleep apnea (HSA) is characterized by apneic episodes during sleep and daytime hypersomnolence. Patients afflicted as a result of upper airway obstruction have been treated traditionally with permanent tracheostomy. Three patients with HSA and mandibular retrognathism are presented. Each patient had a retrognathic mandible that stemmed from a different cause. Surgical advancement of their underdeveloped mandibles corrected the symptoms of HSA rapidly. The literature concerning HSA is reviewed and the advantages of mandibular surgery in selected cases are discussed.

The Relationship between Upper Airway Anatomy and Obesity in Patients with Obstructive Sleep Apnea.

Rationale: Obesity is the most important risk factor for obstructive sleep apnea (OSA). However, the complex relationship between obesity and upper airway anatomy (craniofacial structure, soft tissues, and airway caliber) has not been robustly examined in patients with OSA. Objectives: To evaluate the relationship between obesity, on the basis of body mass index (BMI), and upper airway anatomic structures in adult patients with moderate or severe OSA. Methods: In this cross-sectional study, 583 patients with apnea-hypopnea index ≥15 events/h (mean age, 53.7 ± 10.4 yr; 81.0% men) were included

Re-thinking lactation-related nipple pain and damage.

Nipple pain is a common reason for premature cessation of breastfeeding. Despite the benefits of breastfeeding for both infant and mother, clinical support for problems such as maternal nipple pain remains a research frontier. Maternal pharmaceutical treatments, and infant surgery and bodywork interventions are commonly recommended for lactation-related nipple pain without evidence of benefit. The pain is frequently attributed to mammary dysbiosis, candidiasis, or infant anatomic anomaly (including to diagnoses of posterior or upper lip-tie, high palate, retrognathia, or subtle cranial nerve a

Cytogenomic characterization of a de novo 4q34.1 deletion in a girl with mild dysmorphic features and a coagulation disorder.

BACKGROUND: 4q deletion syndrome is a rare chromosomal disorder that mostly arises de novo. The syndrome is characterized by craniofacial dysmorphism, digital abnormalities, skeletal alterations, heart malformations, developmental delay, growth retardation, Pierre Robin sequence, autistic spectrum and attention deficit-hyperactivity disorder, although not every patient shows the same features. Array comparative genomic hybridization (aCGH) use improves the detection of tiny chromosomal deletions and allows for a better understanding of genotype-phenotype correlations in affected patients. Calvarial periosteal graft for second-stage cleft palate surgery: a preliminary report.

OBJECTIVE: The objectives of cleft palate surgery are to achieve optimal outcomes regarding speech development, hearing, maxillary arch development and facial skull growth. Early two-stage cleft palate repair has been the most recent protocol of choice to achieve good maxillary arch growth without compromising speech development. Hard palate closure occurs within one year of soft palate surgery. However, in some cases the residual hard palate cleft width is larger than 15 mm at the age of two. As previously reported, integrated speech development starts around that age and it is a challenge si

[Investigation of a nosocomial outbreak caused by ESBL positive Klebsiella pneumoniae in neonatal intensive care unit by AP-PCR].

In the present study, we describe an outbreak caused by extended-spectrum beta-lactamase (ESBL) producing Klebsiella pneumoniae in the neonatal intensive care unit (NICU) of a tertiary-care hospital. Nosocomial blood-stream infections were detected in three patients hospitalized in NICU. Two of the cases were transferred to NICU due to premature birth and the other due to the presence of cleft palate and retrognathia. K.pneumoniae was isolated on the 5th day of hospitalization of the first patient from umbilical swab and blood culture; on the 15th day of hospitalization of the second patient f

Sliding genioplasty for correction of chin abnormalities.

BACKGROUND: Correction of chin underprojection has a significant effect on facial symmetry. Alloplastic chin implants and sliding genioplasty represent the accepted methods of chin augmentation. While both procedures may be used for retrognathia or microgenia, the sliding genioplasty may also be used in chin asymmetry, prognathia, and vertical height discrepancies. We report our finding from a 5-year review of our experience with sliding genioplasty. OBJECTIVES: To evaluate the results of sliding genioplasties performed by residents and private practitioners, to illustrate the versatility and

Intronic Single Nucleotide Polymorphisms in FGFR2 Gene Association With Non-Syndromic Mandibular Retrognathism.

OBJECTIVE: Mandibular retrognathism (MR) is a skeletal malocclusion in which patients have a deficient mandibular length, resulting in a more posterior position of the mandible. We aimed to investigate the association between Single nucleotide polymorphisms (SNPs) in Fibroblast Growth Factor Receptor 2 (FGFR2) gene and MR in germans. MATERIALS AND METHODS: Genomic DNA and lateral cephalometric radiographs were obtained from orthodontic patients. Patients were allocated into the 'Retruded' group (SNB angle < 78°) and into the 'Well-positioned' group (SNB 78°-82°). The rs4752566, rs10736303, rs

Role of ACTN3 Gene rs1815739 Polymorphism and Its Effects on Mandibular Morphogenesis.

OBJECTIVES: Genetic polymorphism of the α-actinin-3 (ACTN3) gene has an influence on the facial skeletal profile, with the activity of the α-actinins protein influencing the contractile properties of the masseteric muscles. This study examines the association between the ACTN3 rs1815739 polymorphism through a Stop Codon (changing 577RR to 577XX) resulting in variations in mandibular morphogenesis. MATERIALS AND METHODS: Two hundred and fifty subjects were categorised into three groups. The control group (Group 1) comprised 100 patients with skeletal Class I malocclusion. The experimental grou

Nonsurgical treatment of Brodie bite assisted by 3-dimensional planning and assessment.

This case report describes the nonsurgical treatment of an adolescent patient with a severe transverse discrepancy presented as a Brodie bite and retrognathic mandible. Distraction osteogenesis has been often used for similar cases in the literature. However, in this patient, a fixed appliance with 1 maxillary extraction combined with a functional appliance was used to resolve the transverse discrepancy with natural growth. After the orthodontic treatment, the impinging teeth and Brodie bite were corrected with a favorable occlusion and profile. Retention at the 3-year follow-up showed improve

Timing of Surgical Removal of Odontoma in a Growing Orthodontic Patient: A Case Report.

In patients with skeletal malocclusion and odontoma, the prioritization of treatment is ofgreat importance. Ifsurgical removal of odontoma is postponed, the probability of adjacent teeth impaction increases. In this case, skeletal treatment was performed before odontoma rgery due to concerns about facial appearance and the patient's fear of surgery, and therefore adjacent tooth failed to erupt.
