Peritoneal adhesions after laparoscopic gastrointestinal surgery.

Although laparoscopy has the potential to reduce peritoneal trauma and post-operative peritoneal adhesion formation, only one randomized controlled trial and a few comparative retrospective clinical studies have addressed this issue. Laparoscopy reduces de novo adhesion formation but has no efficacy in reducing adhesion reformation after adhesiolysis. Moreover, several studies have suggested that the reduction of de novo post-operative adhesions does not seem to have a significant clinical impact. Experimental data in animal models have suggested that CO₂ pneumoperitoneum can cause acute perit

Clinical Outcomes of Patients Managed in a Temporary COVID-19 Step-Down Unit.

Background During the coronavirus disease 2019 (COVID-19) pandemic, unprecedented surges of patients strained healthcare resources, especially intensive care units (ICUs), which were quickly saturated by patients with respiratory failure. To inform future responses, we conducted a retrospective analysis of patient outcome data from a temporary COVID-19 transitional care/step-down unit (TCU) we implemented during January-February 2021. Methods Our TCU was embedded on a medical-surgical floor at our academic hospital institution to offload ICU patients with stable or improving respiratory failur

Vayu bubble continuous positive airway pressure is a promising solution with favorable treatment outcomes for respiratory distress syndrome in newborns: a qualitative study in Bangladesh.

BACKGROUND: According to Bangladesh Demographic and Health Survey (2022), neonatal mortality, comprising 67% of under-5 deaths in Bangladesh, is significantly attributed to prematurity and low birth weight (LBW), accounting for 32% of neonatal deaths. Respiratory distress syndrome (RDS) is a prevalent concern among preterm and LBW infants, leading to substantial mortality. The World Health Organization (WHO) recommends bubble continuous positive airway pressure (bCPAP) therapy, but the affordability and accessibility of conventional bCPAP devices for a large number of patients become major hur

Study on prevention of hypercapnia by Nasal High Flow in patients with endoscopic submucosal dissection during intravenous anesthesia.

BACKGROUND: For relatively invasive upper gastrointestinal endoscopy procedures, such as an endoscopic submucosal dissection (ESD), intravenous anesthesia is routinely used to reduce patient anxiety. However, with the use of intravenous sedation, even at mild to moderate depth of anesthesia, there is always a risk of upper airway obstruction due to a relaxation of the upper airway muscles. With the advent of Nasal High Flow (NHF) devices that allow humidified high flow air through the nasal cavity, can be used as a respiratory management method in the context of anesthesia. AIRVO is commonly us

Aerosolized surfactant improves pulmonary function in endotoxin-induced lung injury.

Surfactant dysfunction is a primary pathophysiologic component in patients with adult respiratory distress syndrome (ARDS). In this study we tested the efficacy of aerosolized surfactant (Sf ) replacement in a severe lung injury model of endotoxin-induced ARDS. Twenty-one certified healthy pigs were anesthetized, surgically prepared for measurement of hemodynamic and lung function, then randomized into one of four groups: (1) control (n = 5), surgical instrumentation only; (2) lipopolysaccharide (LPS) (n = 6), infused with Escherichia coli LPS (100 microgram/kg) without positive end- expirator

Continuous intratracheal gas suctioning combined with ventilator leak-compensation may enhance carbon dioxide removal efficiency: a proof-of-concept study using a porcine model.

BACKGROUND: Although tracheal gas insufflation (TGI) and aspiration of expiratory gas in the dead space (ASPIDS) have been shown to enhance CO₂ removal by washing out the anatomical dead space, clinical implementation has been limited due to complications including unintended airway pressure increases and mucosal damage, as well as their complexity. We developed a novel respiratory management system combining continuous intratracheal gas suctioning with pressure control ventilation, utilizing the leak compensation function of modern ventilators. The aim of this study was to document the prelim

The short-term efficacy of high flow nasal oxygen therapy on cardiovascular surgical patients: a randomized crossover trial.

BACKGROUND: Oxygen therapy after extubation in the intensive care unit (ICU) is essential in order to maintain adequate oxygenation, especially in patients who have undertaken cardiovascular surgery. A Venturi mask (VM) has been routinely used as an oxygen therapy in the ICU. Recently, however, the high flow nasal cannula (HFNC) has become available, and this device can deliver up to 60 L/min of humidified oxygen. The aim of this study is to evaluate the short-term efficacy between HFNC and VM in cardiovascular surgical patients. METHODS: Forty patients who underwent cardiovascular surgery we

Pericyte, but not astrocyte, hypoxia inducible factor-1 (HIF-1) drives hypoxia-induced vascular permeability in vivo.

BACKGROUND: Ways to prevent disease-induced vascular modifications that accelerate brain damage remain largely elusive. Improved understanding of perivascular cell signalling could provide unparalleled insight as these cells impact vascular stability and functionality of the neurovascular unit as a whole. Identifying key drivers of astrocyte and pericyte responses that modify cell-cell interactions and crosstalk during injury is key. At the cellular level, injury-induced outcomes are closely entwined with activation of the hypoxia-inducible factor-1 (HIF-1) pathway. Studies clearly suggest tha
