Interpositional bone grafting and Le Fort I osteotomy for reconstruction of the atrophic edentulous maxilla. A two-stage technique.

This study presents the results from ten consecutive patients who, because of insufficient bone volume for conventional implant placement in the maxilla, were treated with an interpositional bone graft and Le Fort I osteotomy. The endosteal implants were placed six months after the osteotomy. A total of 60 screw-shaped titanium implants (Brånemark) were placed, of which three failed to integrate during the six-month healing period. No further implants were lost during the follow-up period, ranging from 15 to 39 months after placement of the implants. All patients received fixed bridges and all

The use of bovine porous bone mineral in combination with collagen membrane or autologous fibrinogen/fibronectin system for ridge preservation following tooth extraction.

Preservation of the alveolar ridge following tooth extraction is desirable since it facilitates placement of endosseous implants and may improve the adverse esthetics often associated with fixed partial dentures. The purpose of this study was to compare the clinical effectiveness of bovine porous bone mineral (BPBM) used as a graft material combined with either guided tissue regeneration (GTR) or with the autologous fibrinogen/fibronectin system (AFFS) in preserving alveolar ridges following tooth extraction. Twenty-six patients who required extraction of two or more anterior or bicuspid teeth

Zirconia-based versus metal-based single crowns veneered with overpressing ceramic for restoration of posterior endodontically treated teeth: 5-year results of a randomized controlled clinical study.

OBJECTIVES: The aim of this 5-year randomized controlled trial was to compare the longevity and clinical behavior of single posterior crowns made with pressable ceramic on zirconia and on metal frameworks, and if failures occur, to delineate the contributing factors. METHODS: 72 patients, who needed the covering of at least a molar and/or premolar, were included in the study. All teeth were endodontically treated, with absence of periapical lesion or active periodontitis. Ninety single crowns were made with zirconia or metal framework and covered with pressable veneering ceramics. Two indepen

Restoration of the Anterior Segment in a Cleft Palate in Conjunction with Surgically Facilitated Orthodontic Therapy: An Interdisciplinary Approach.

A 26-year-old woman was referred to a periodontal surgical practice for concerns related to gingival recession. After several consultations among the orthodontist, periodontist, and cosmetic/restorative dentist, she decided to have surgically facilitated orthodontic therapy as part of a collaborative, interdisciplinary treatment planning process to correct her constricted maxillary arch form, augment thin dentoalveolar facial bone, simultaneously with gaining root coverage as well as improving attached gingiva width and mucogingival thickness. As a consequence of changing the arch form, an imp

Bone regeneration using a hollow hydroxyapatite space-maintaining device for maxillary sinus floor augmentation--a clinical pilot study.

BACKGROUND: The mere lifting of the maxillary sinus membrane by implants protruding into the sinus cavity allows the establishment of a void space for blood clot and new bone formation. PURPOSE: To evaluate bone formation by using a spherical, hollow, and perforated hydroxyapatite space-maintaining device (HSMD) in a two-stage sinus lift procedure where residual alveolar bone height was ≤2 mm. MATERIAL AND METHODS: Spherical, hollow, and perforated HSMDs with a diameter of 12 mm were manufactured for this pilot study. Three patients with a residual bone height of 1-2 mm, as verified clinical

Osseous Maxillary Reconstruction with Immediate Dental Implant Placement: An Optimized Workflow for the Oncologic Patient.

BACKGROUND: Maxillary reconstruction is a complex undertaking characterized by a 3-dimensional surgical site with deficiencies in multiple tissue types. Before virtual surgical planning (VSP), bony reconstruction was inaccurate and inefficient; thus, reconstructions defaulted to soft-tissue flaps or obturators. The current study describes an efficient and accurate approach to bony maxillary reconstruction with immediate dental implant placement (IDIP). METHODS: A reconstructive workflow was developed for osseous reconstruction to improve functional and aesthetic outcomes. Critical aspects inc

Post-treatment Stability of Maxillary Arch Dimensions in Unilateral Cleft Lip and Palate: A Systematic Review.

Cleft lip and palate (CLP) are structural anomalies resulting from the failure of craniofacial prominences to fuse during embryonic development. Orthodontic treatment seeks to address these conditions by improving dentofacial balance and restoring oral function, but long-term post-treatment stability continues to pose a challenge. This systematic review included a literature search of four electronic databases (PubMed, Scopus, Web of Science, and Cochrane CENTRAL) to identify observational evidence on the stability of orthodontic treatment in CLP. Six studies, comprising three cohort studies a

Clinical outcomes of implant-assisted removable partial dentures in patients with disabilities: a retrospective cohort study.

BACKGROUND: Fixed implant prostheses are often impractical for patients with disabilities due to financial and medical limitations. Implant-assisted removable partial dentures using surveyed crowns (ICRPDs) may provide a cost-effective alternative; however, clinical outcome data in this population remain limited. METHODS: Patients with disabilities treated with ICRPDs between May 2019 and June 2024 were retrospectively identified. Clinical and radiographic data were reviewed to assess implant survival, marginal bone loss (MBL), oral hygiene status, and prosthetic complications. The mean durat

Aesthetic recovery of alveolar atrophy following autogenous onlay bone grafting using interconnected porous hydroxyapatite ceramics (IP-CHA) and resorbable poly-L-lactic/polyglycolic acid screws: case report.

BACKGROUND: Onlay bone grafting techniques have some problems related to the limited volume of autogenous grafted bone and need for surgery to remove bone fixing screws. Here, we report a case of horizontal alveolar ridge atrophy following resection of a maxillary bone cyst, in which autogenous onlay bone grafting with interconnected porous hydroxyapatite ceramics (IP-CHA) and bioresorbable poly-L-lactic/polyglycolic acid (PLLA-PGA) screws was utilized. CASE PRESENTATION: A 51-year-old man had aesthetic complications related to alveolar atrophy following maxillary bone cyst extraction. We per

Incorporating severity and risk as factors to the Fardal cost-effectiveness model to create a cost-benefit model for periodontal treatment.

BACKGROUND: A previously described economic model was based on average values for patients diagnosed with chronic periodontitis (CP). However, tooth loss varies among treated patients and factors for tooth loss include CP severity and risk. The model was refined to incorporate CP severity and risk to determine the cost of treating a specific level of CP severity and risk that is associated with the benefit of tooth preservation. METHODS: A population that received and another that did not receive periodontal treatment were used to determine treatment costs and tooth loss. The number of teeth
