Retrograde weight implantation for correction of lagophthalmos.

OBJECTIVES: Gold weight implantation is the most commonly used method for surgical correction of paralytic lagophthalmos. Numerous techniques for placement of the weight have been described, yet complications with these methods continue to occur (implant migration or extrusion, wound infection, failure to correct the lagophthalmos, and excessive postoperative ptosis). We developed a retrograde, postlevator aponeurosis method for implantation to improve the placement and fixation of the weight. This study describes the rationale, technique, and surgical outcome of the retrograde approach. STUD

Anatomical variations of the inguinal nerves and risks of injury in 110 hernia repairs.

The aim of this study was to identify the anatomical variations in the inguinal course of the ilioinguinal nerve (IIN) and the iliohypogastric nerve (IHN) during operative repair of inguinal hernias. A consecutive series of 110 primary inguinal hernias were repaired by the mesh technique. Particular attention was paid to early identification and recording of the course of both the IIN and IHN and preserving them throughout the operative procedure. The course of both nerves was found to be consistent with that described in anatomical texts in only 46 of 110 explorations (41.8%). The course of o

Traction avulsion amputation of the major upper limb: a proposed new classification, guidelines for acute management, and strategies for secondary reconstruction.

Major replantation of a traction avulsion amputation is undertaken with the goal of not only the reestablishment of circulation, but also functional outcome. This type of amputation is characterized by different levels of soft-tissue divisions involving crushing, traction, and avulsion injuries to various structures. Between 1985 and 1998, 27 cases were referred for secondary reconstruction following amputation of the upper extremity involving both arm and forearm. Replantation was performed by at least 12 qualified plastic surgeons using different approaches and management, resulting in diffe

Acute effects of intramuscular aponeurotomy on rat gastrocnemius medialis: force transmission, muscle force and sarcomere length.

Acute effects of intramuscular aponeurotomy on muscle force and geometry as a function to muscle length were studied in rat m. gastrocnemius medialis (GM). Acutely after aponeurotomy, activation of the muscle at increasing lengths (acute trajectory) showed a spontaneous and progressive but patial tearing of the connective tissue interface between the fibres inserting directly proximally and distally to the location of the section. After this the muscle consisted morphologically of a stable proximal and a distal part (post-aponeurotomy). Post-aponeurotomy mean active sarcomere length within fib

[Video-parietoscopic surgery of the abdominal wall. A study of 15 cases].

AIM: The aim of videoparietoscopic surgery is to repair an abdominal wall weakness or defect without affecting the overlying skin, mainly for cosmetic reasons, and especially in young women. PATIENTS AND METHOD: A supra-pubic or umbilical approach was used, depending on the site of the lesion to be repaired. A 10 mm trocar with a sponge mandrin was placed in contact with the aponeurosis. After an initial dissociation of the cellular tissue, progressive insufflation with CO2 produced extensive detachment of the cutaneous plane upwards and laterally. Using one or two 5 mm trocars, the aponeurot

[Subaponeurotic lipoma of the forehead].

On the forehead, the epicranial fascia or aponeurosis (galea) splits to enclose the musculus frontalis. The superficial fascia, thin and transparent, separates the muscle from the fatty layer (panniculus) of the overlying skin. The deep fascia, thick and fibrous, is separated from the periosteum by a loose areolar tissue which allows the whole skin to move over the skeleton. The two bellies of the m. frontalis are anchored jointly on the midline, where their superficial and deep fasciae are tightly joined together. The loose areolar tissue beneath the galea is a cleavage plane facilitating pla

Real-world treatment patterns among newly diagnosed patients with plantar fibromatosis in the United States.

BACKGROUND: Plantar fibromatosis (PFI) is a rare condition that affects the plantar aponeurosis resulting in painful nodules on the foot/feet. PURPOSE: There are no clinical guidelines for treatment in the United States, and real-world evidence regarding treatment patterns is limited. METHODS: This retrospective cohort study used linked data from adjudicated claims and electronic medical records to identify patients with incident PFI. PFI prevalence in 2021 was estimated, and treatment patterns for conservative (nonsurgical) and surgical interventions were assessed during the 24 months after

Modifying Upper Eyelid Crease Using Hyaluronic Acid Fillers: A Clinical Case Series.

BACKGROUND: Upper eyelid crease morphology is a critical determinant of periorbital aesthetics. In Asian patients, anatomic variations in levator aponeurosis insertion and preaponeurotic fat distribution frequently result in crease asymmetry or attenuation, particularly in mild-to-moderate cases without true levator dysfunction. Although surgical blepharoplasty remains the standard corrective approach, it is associated with scarring, downtime, and irreversibility. OBJECTIVE: This study aimed to evaluate the clinical efficacy and safety of hyaluronic acid (HA) filler injections as a nonsurgica

Fatal Outcome of Widespread Cranial Basal Cell Carcinoma Due to Intraparenchymal Brain Extension in a Setting of Local Recurrences.

Basal cell carcinoma (BCC) is one of the most commonly encountered carcinomas; however, progression to invasive or locally advanced disease is extremely rare. The clinical course of locally advanced BCC is typically aggressive and has a poorer prognosis with variable survival. We present the case of a 58-year-old unhoused female with recurrent BCC of the left cranial vertex who underwent excision at an outside hospital. Intraoperative frozen section margins were negative for BCC; however, permanent sections demonstrated widespread residual tumor with morpheaform histology involving the parieta

Can a Peritoneal Conduit Become an Artery?

OBJECTIVE: Current vascular grafts all have limitations. This study examined peritoneum as a potential graft material and the in vivo transfer of peritoneum into a functional artery like conduit after end to end anastomosis into the common carotid artery of sheep. The aim was to investigate whether implantation of a peritoneal tube into the arterial tree results in a structure with function, histological findings, and gene expression like an artery, and whether such arterialisation occurs through a conversion of the phenotype of peritoneal cells or from host cell migration into the implant. M
