Safety and effectiveness of the Vibrant Soundbridge in treating conductive and mixed hearing loss: A systematic review.

OBJECTIVES/HYPOTHESIS: For many years, the therapeutic approach for conductive and/or mixed hearing loss has consisted of middle ear surgery with replacement of defect ossicles, and if possible the application of a hearing aid. Advances in technology have led to the introduction of electronmagnetic active implantable devices such as the Vibrant Soundbridge (VSB). With its various coupling techniques for different pathophysiological situations in the middle ear, the VSB offers greater improvement in the hearing performance of affected persons. Performance considerations of prosthetic actuators for round-window stimulation.

Round-window (RW) stimulation has improved speech perception in patients with mixed hearing loss. In cadaveric temporal bones, we recently showed that RW stimulation with an active prosthesis produced differential pressure across the cochlear partition (a measure related to cochlear transduction) similar to normal forward sound stimulation above 1 kHz, when contact area between the prosthesis and RW is secured. However, there is large variability in the hearing improvement in patients implanted with existing modified prosthesis. This is likely because the middle-ear prosthesis used for RW stim

[Long-term results after stapes surgery for otosclerosis].

In a retrospective study, hearing results before and after stapedectomy were compared with those of individuals not undergoing surgery. Subjects were 51 patients who were surgically diagnosed as having otosclerosis and who could be followed up for more than 5 years. Of these, 31 contralateral ears surgically diagnosed as otosclerotic did not undergo surgery (nonsurgery group). Nineteen ears underwent stapedectomy using a wire loop prosthesis (TS group). Thirty-eight ears underwent stapedotomy using a wire piston prosthesis (SFS group). In the nonsurgery ear group, the last air conduction thres

[Materials for reconstruction of the middle ear].

To rehabilitate most cases of conductive hearing loss closure of ear drum perforations and rebuilding of the ossicular chain can be performed. Due to the great number of biocompatible bone substitute materials available it is occasionally difficult for the surgeon to choose the most favorable substitute. Autogenous structures (ossicles, cortical bone, cartilage) and allogenous tissues (ossicles, cortical bone, cartilage, dentin) are possible bone replacement materials. Xenogenic tissue is currently not used in middle ear surgery. Ionomer cement is a hybrid material for replacement of bone but

[Surgical treatment of chronic otitis media. I: Indications, preoperative care and surgical principles].

The two classic types of chronic otitis media--chronic suppurative disease and cholesteatoma--not only differ in their clinical course and prognosis, but they also require different therapeutic strategies. Benign chronic suppurative disease with its chronic irritating otorrhea can often be managed conservatively. Surgery is recommended if improvement of hearing is the goal, but also to heal the chronic inflammatory disease. Diagnosis of a cholesteatoma, however, is a clear indication for surgical treatment; conservative and wait-and-see strategies are the exception. Even in cases of precholest

Hearing rehabilitation with single-stage bilateral vibroplasty in a child with Franceschetti syndrome.

Hearing is of utmost importance for normal speech and social development. Even children who have mild or unilateral permanent hearing loss may experience difficulties with understanding speech, as well as problems with educational and psycho-social development. The increasing advantages of middle-ear implant technologies are opening new perspectives for restoring hearing. Active middle-ear implants can be used in children and adolescents with hearing loss. In addition to the well-documented results for improving speech intelligibility and quality of hearing in sensorineural hearing loss active

[The results of stapedoplasty with the use of an auricular autocartilage as a stapes bone prosthesis in the patients presenting with a mixed form of otosclerosis].

The authors present immediate and long-term results of stapedoplasty with the use of an auricular autocartilage as a stapes bone prosthesis in 52 patients presenting with mixed (types I and II) otosclerosis. The surgical procedure is described in considerable detail. The average bone-conduction auditory threshold was shown to slightly increase depending on the duration of the disease (less than and over 15 years): by 5 dB and 9 dB in mixed otosclerosis of types I and II respectively. Mean values of the bone-conduction auditory threshold before and after surgery were not significantly different
