Comparison of permanent left ventricular and biventricular pacing in patients with heart failure and chronic atrial fibrillation: a prospective hemodynamic study.

BACKGROUND: Left ventricular pacing (LVP) and biventricular pacing (BVP) have been proposed as treatments for patients with advanced heart failure complicated by discoordinate contraction due to intraventricular conduction delay. For patients in sinus rhythm, BVP works in part by modulating the electronic atrial-ventricular time delay and thus optimizing contractile synchrony, the contribution of atrial systole, and reducing mitral regurgitation. However, little is known of the mechanisms of BVP in heart failure patients with drug-resistant chronic atrial fibrillation. HYPOTHESIS AND METHODS:

Diagnostic utility of early premature ventricular complexes in differentiating atrioventricular reentrant and atrioventricular nodal reentrant tachycardias.

BACKGROUND: His-refractory premature ventricular complexes perturbing a supraventricular tachycardia (SVT) establish the presence of an accessory pathway (AP). Earlier premature ventricular complexes (ErPVCs) may perturb SVTs but are considered nondiagnostic. OBJECTIVE: The purpose of this study was to test the hypothesis that an ErPVC will always show a difference >35 ms in its advancement of the next atrial activation during atrioventricular nodal reentrant tachycardia (AVNRT). During atrioventricular reentrant tachycardia (AVRT), a PVC delivered close to the circuit can result in greater a

Fetal and Neonatal Arrhythmias.

Cardiac arrhythmias are an important aspect of fetal and neonatal medicine. Premature complexes of atrial or ventricular origin are the main cause of an irregular heart rhythm. The finding is typically unrelated to an identifiable cause and no treatment is required. Tachyarrhythmia most commonly relates to supraventricular reentrant tachycardia, atrial flutter, and sinus tachycardia. Several antiarrhythmic agents are available for the perinatal treatment of tachyarrhythmias. Enduring bradycardia may result from sinus node dysfunction, complete heart block and nonconducted atrial bigeminy as th

Remote monitoring of a high-risk patient with Brugada syndrome: association of different arrhythmic manifestations.

We report on the remote arrhythmia monitoring of a 34-year-old man with highly symptomatic Brugada syndrome, who initially presented with syncope, paroxysmal atrial fibrillation, and spontaneous coved-type electrocardiogram. The patient received a dual-chamber implantable cardioverter-defibrillator (ICD) with Home Monitoring™ facilities and experienced recurrent ICD shocks for spontaneous ventricular fibrillation (VF) episodes during the first year after ICD implantation. Remote monitoring revealed an increased burden of premature ventricular complexes and atrial arrhythmias each time VF spont

The Effects of Different Hormones on Supraventricular and Ventricular Premature Contractions in Healthy Premenopausal Women.

Background and Objectives: The effects of gender differences on cardiac parameters have been well-established. In this study, we aimed to evaluate the possible associations of plasma levels of different sex hormones with premature atrial or ventricular contractions in premenopausal women. Materials and Methods: We conducted a prospective study which included women in late reproductive age who presented with palpitations during an eight-month period. A 12-lead electrocardiography, a transthoracic echocardiogram, blood samples, and 24-hour rhythm Holter were conducted on the third day of the men

[Extrasystole - a widespread phenomenon].

Background: Extrasystole is a common reason for visits to general practitioners. In cardiological literature, supraventricular extrasystole in particular tends to play a minor role. This article discusses the current state of knowledge and common practice for this frequently encountered clinical problem. Question: Due to increasing health awareness among the population and the use of new diagnostic technologies (e.g. smartwatches), there has been an increase in GP consultations among young and healthy patients with symptomatic or asymptomatic extrasystole. Comorbidities play a decisive role in

Mechanism of conversion of atypical right atrial flutter to atrial fibrillation.

The purpose of this study was to explore the mechanisms of conversion from atypical atrial flutter (AFL) to atrial fibrillation (AF), and the long-term results of cavotricuspid isthmus ablation in these patients. We retrospectively reviewed the records of 221 patients with typical AFL referred to our hospital for ablation. A total of 25 patients had atypical AFL, and cavotricuspid isthmus ablation was performed in 23 with isthmus-dependent atypical AFL, as well as in 180 patients with typical counterclockwise and/or clockwise AFL. In all, 13 spontaneous transitions from atypical AFL to AF were

Management of cardiac arrhythmias in acute coronary syndromes.

A variety of atrial and ventricular arrhythmias are encountered in patients with acute coronary syndromes. These include both brady and tachyarrhythmias of supraventricular and ventricular origin. Sinus bradycardia and atrial fibrillation are the most common of supraventricular origin. Ventricular arrhythmias that merit consideration include premature ventricular complexes, accelerated idioventricular rhythm, ventricular tachycardia, and ventricular fibrillation. Intraventricular and atrioventricular conduction blocks associated with acute coronary syndromes include bundle branch blocks, fasci

Supraventricular tachycardia following insertion of a central venous catheter.

Placement of central venous catheters (CVCs) in patients is associated with several risks including endocardial injury and dysrhythmias. In addition, CVC extending into intracardiac chambers can provoke premature atrial and ventricular complexes, which have been reported to initiate supraventricular tachycardia (SVT). A 15-year-old boy with end-stage renal failure developed SVT after insertion of a CVC.

The cumulative risks of prolapsing mitral valve. 40 years of follow-up.

Prolapsing mitral valve is a common cardiac condition, occurring in approximately 16 million people in the United States alone. Primary prolapsing mitral valve may be familial or nonfamilial and may be associated with myxomatous degeneration of the mitral valve leaflets, such as occurs in Marfan syndrome and other connective tissue disorders. Secondary forms may be associated with such entities as rheumatic fever (especially after commissurotomy) and coronary artery disease (in the presence of ruptured chordae tendineae), and with such congenital conditions as interatrial defect and primary ca

Congestive heart failure in two pet rabbits.

This case report describes congestive heart failure with pleural effusion in two middle-aged, pet house rabbits. Both had a history of acute onset dyspnoea, weakness and weight loss. Bi-atrial enlargement was seen on echocardiography in both rabbits. One rabbit had atrial fibrillation and ventricular premature complexes identified on electrocardiography. There was a radiographically evident pleural effusion in both rabbits and thoracocentesis was undertaken in one rabbit. These findings were confirmed on post-mortem examination. The aetiology for the underlying heart disease was not found, but

Electrocardiographic manifestations of dual atrioventricular node conduction during sinus rhythm.

OBJECTIVES: The objective of this study was to correlate electrocardiographic (ECG) PR interval changes during normal sinus rhythm with recent observations regarding the anatomy and physiology of the dual, slow and fast atrioventricular (AV) pathways. BACKGROUND: The least common manifestation of dual AV conduction is an abrupt PR interval change in the setting of sinus rhythm. Whereas isolated cases of this phenomenon have been reported, the relatively large series we have collected makes it possible to correlate the ECG findings with the anatomy, composition and electrophysiology of the dua

Novel Phenotypic Effects of a Rare SCN5A (c.2482C>T) Mutation.

In a familial cohort with 8 heterozygous carriers of a rare pathogenic SCN5A mutation (c.2482C>T), 4 female mutation carriers manifested with fetal ventricular tachycardia and 2:1 atrioventricular block. One presented with multifocal ectopic premature Purkinje-related complexes-like phenotype and atrial fibrillation later in life. These novel findings inform the need for robust fetal monitoring of mutation carriers.

Delta (Δ) 12-lead electrocardiography and vectorcardiography to identify the origin of focally induced atrial and ventricular premature depolarizations in horses.

BACKGROUND: Cardiac arrhythmias are common in horses, but their clinical and prognostic relevance remains poorly defined. Despite the importance of localization for diagnosis and treatment, noninvasive methods to identify arrhythmogenic foci are limited. HYPOTHESIS/OBJECTIVES: Evaluate the ability of delta (Δ) 3- and 12-lead ECGs and 2-dimensional (2D) and 3-dimensional (3D) vectorcardiography (VCG) to identify the site of origin of focally induced atrial and ventricular premature depolarizations in horses. A very unusual wide QRS tachycardia: VT or SVT?

INTRODUCTION: A 13 old girl presented with recurrent wide QRS tachycardia since she was 4. METHODS: An electrophysiologic study was performed. RESULTS: The electrophysiologic study showed that QRS complexes identical to those of the tachycardia could be elicited with premature atrial extrastimuli but with a shorter atrioventricular (AV) delay when the QRS was wide compared with narrow QRS complexes. The tachycardia was ablated at 9 o'clock on the tricuspid annulus demonstrating the presence of an atriofascicular fiber. CONCLUSION: We believe that this atypical behavior can be explained by A

Heartbeat Classification by Random Forest With a Novel Context Feature: A Segment Label.

OBJECTIVE: Physicians use electrocardiograms (ECG) to diagnose cardiac abnormalities. Sometimes they need to take a deeper look at abnormal heartbeats to diagnose the patients more precisely. The objective of this research is to design a more accurate heartbeat classification algorithm to assist physicians in identifying specific types of the heartbeat. METHODS AND PROCEDURES: In this paper, we propose a novel feature called a segment label, to improve the performance of a heartbeat classifier. This feature, provided by a Convolutional Neural Network, encodes the information surrounding the p

Pacemaker-associated bigeminy.

BACKGROUND: Pacemaker-associated bigeminy is an electrocardiographic pattern characterized by a regular alternation of two distinct cardiac events. It can arise from normal operation of device algorithms or indicate underlying dysfunction, such as sensing abnormalities. Accurate differentiation is crucial to avoid misdiagnosis and inappropriate clinical intervention. OBJECTIVE: To systematically elucidate the mechanisms underlying pacemaker-associated bigeminy and establish a practical diagnostic framework. METHODS: We conducted a retrospective analysis of six patients exhibiting pacemaker-a
