When First-Line Therapies Fail: Surgical Site Infections and the Declining Antimicrobial Utility in a Resource-Limited Nation.

UNLABELLED: Antimicrobial resistance (AMR) poses a critical threat to surgical care in resource-limited settings. This study describes the epidemiology of AMR in surgical site and wound infections in Belize from 2018 to 2024. METHODS: A retrospective analysis of national laboratory data focused on Escherichia coli, Klebsiella pneumoniae, and Staphylococcus aureus from surgical and wound specimens. Resistance phenotypes (ESBL, MRSA), susceptibility profiles, and temporal-spatial trends were analyzed. RESULTS: Among 850 isolates, ESBL production among Enterobacteriaceae more than doubled from

American cutaneous leishmaniasis.

This review outlines the present knowledge of American cutaneous leishmaniasis, a disease which, owing to the increase in international travel, is being seen with increasing frequency in Europe and North America. A knowledge of this disease is of particular importance to the military medical officer as in recent years approximately one-hundred and fifty cases of leishmaniasis have been seen in British troops who have served in Belize. Leishmania braziliensis sp. have been isolated from a number of these cases. Organisms of this complex had not previously been recorded in Belize and the Army Me

Biochemical systematics and population genetic structure of Anopheles pseudopunctipennis, vector of malaria in Central and South America.

An electrophoretic survey of 42 populations of Anopheles pseudopunctipennis collected throughout its known geographic distribution was performed to clarify the taxonomic status of this important malaria vector species. The results indicated strong differences in the allele frequencies of three enzyme loci (glycerol dehydrogenase, 6-phosphogluconate dehydrogenase, and phosphoglucomutase) of the 33 loci analyzed. No fixed electromorphic differences separate the populations of An. pseudopunctipennis. The populations of An. pseudopunctipennis showed little genetic divergence, with Nei distances ra

Ectopic Thymic Tissue Presenting as an Epiglottic Mass Compromising a Neonatal Airway: A Case Report.

An epiglottic mass (EM) is rarely found in neonates and poses life-threatening airway complications. We present the case of an infant urgently transferred from Belize via the World Pediatric Project with a lingual EM. The EM was misdiagnosed twice. The patient's home country of Belize initially misdiagnosed the EM as an elongated uvula and then again as an esophageal polyp. The true nature of the upper airway mass (UAM) was only discovered intraoperatively on rigid bronchoscopy as a lingual, mobile EM. Pathology of the lingual EM showed fibrovascular tissue with an area of exuberant capillary

Diagnosis of Acute Chagas Disease in a Belizean Child with Evidence of a Multiclonal Trypanosoma cruzi Infection.

In January 2020, we instituted acute febrile illness surveillance in 11 hospitals and clinics across Belize. Within 3 months, we diagnosed an acute case of Chagas disease by polymerase chain reaction in a 7-year-old child in the northern part of the country. Phylogenetic analyses of the parasite from the acute blood specimen revealed a multiclonal Trypanosoma cruzi infection, including parasites from the TcII (25.0% of haplotypes), TcIV (2.5% of haplotypes), and TcV (72.5% of haplotypes) discrete typing units. The family reported no history of travel, and three Triatoma species vectors were fo

Complete genome reconstruction and genetic analysis of Taura syndrome virus of shrimp from archival Davidson's-fixed paraffin embedded tissue.

Davidson's-fixed paraffin-embedded (DFPE) shrimp tissue are a priceless biological resource for pathogen discovery and evolutionary studies for aquaculture disease diagnostic laboratories worldwide. Nucleic acids extracted from DFPE tissues are often not adequate for most downstream molecular analysis due to fragmentation and chemical modifications. In this study, next generation sequencing (NGS) was used to reconstruct the complete genome of three geographical isolates (Belize, Venezuela and Hawaii) of a ~10 kb length RNA virus of shrimp, Taura syndrome virus (TSV), from DFPE tissues that hav

A third case of amelia in Morelet's crocodile from the Yucatan Peninsula.

Congenital defects in crocodilians have received little interest. In the context of global change and increasing threats to biodiversity, data on birth defects occurring in wildlife could be of importance for estimating the health of species populations and their ecosystems. Herein, we report the first case of amelia (i.e. absence of limbs) in Morelet's crocodiles Crocodylus moreletii from Mexico and the third on the southern Yucatan Peninsula. The crocodile in question was a juvenile (41 cm total length) captured in July 2012 in the Río Hondo, the river that forms the border between Mexico an

Misdiagnosis of cutaneous leishmaniasis and recurrence after surgical excision.

Cutaneous Leishmaniasis (CL) occurs in British troops deployed to Belize, Afghanistan, Iraq and elsewhere. From 1998 to 2009, 156 (45%) of 343 confirmed cases seen in the UK were in military personnel. CL is a rare disease and requires specialist clinical management because numerous pitfalls exist during diagnosis and treatment. A 19-year-old soldier developed CL on his neck 6 weeks after taking part in jungle warfare training in Belize. However, this was not suspected and the diagnosis was not made from either a skin biopsy or following surgical excision. The travel history and the patient's

Identification of major routes of HIV transmission throughout Mesoamerica.

BACKGROUND: Migration and travel are major drivers of the spread of infectious diseases. Geographic proximity and a common language facilitate travel and migration in Mesoamerica, which in turn could affect the spread of HIV in the region. METHODS: 6092 HIV-1 subtype B partial pol sequences sampled from unique antiretroviral treatment-naïve individuals from Mexico (40.7%), Guatemala (24.4%), Honduras (19%), Panama (8.2%), Nicaragua (5.5%), Belize (1.4%), and El Salvador (0.7%) between 2011 and 2016 were included. Phylogenetic and genetic network analyses were performed to infer putative relat

Cutaneous myiasis due to Dermatobia hominis: a report of six cases.

We report six cases of Dermatobia hominis myiasis imported into the U.K. from Belize. With increasing international travel, myiasis may be encountered more frequently in countries in which the parasites are not indigenous. The life-cycle of D. hominis is described, and scanning electron micrographs show the detailed appearance of the larva.

Human depredation by vampire bats (Desmodus rotundus) following a hog cholera campaign.

Hog cholera control efforts in Belize in 1975 included the slaughter of village pigs, a primary food source for the vampire bat (Desmodus rotundus). The bats then fed on secondary food sources, including humans. In 1 village, 22% of the families interviewed were exposed to attacks: 17 children and 2 adults were bit. Human depredation was not continuous as Desmodus located other hosts.

Cutaneous leishmaniasis: a report of two cases seen at a tertiary dermatological centre in Singapore.

Cutaneous leishmaniasis (CL) is not common in South-East Asia and often presents as a granulomatous plaque on the exposed areas, with a high index of suspicion required for diagnosis. Two such cases were seen at the National Skin Centre recently, and both were Gurkha men with a history of travel to Belize. They were treated with intravenous sodium stibogluconate with success. A discussion on CL and its management follows.

Simple and effective field extraction of human botfly, Dermatobia hominis, using a venom extractor.

After a trip to Belize, a 25-year-old man noticed an erythematous papule on his upper right chest that enlarged over a 6-week period and formed a central aperture. The patient reported feeling movement and intermittent lancinating pains under the skin. The history and examination were consistent with cutaneous myiasis, likely secondary to the human botfly, Dermatobia hominis. The objective of reporting this case is to present a simple method of extraction of a botfly larva using a commercial venom extractor. The patient's upper chest was prepared, and an occlusive dressing was placed over the

Hybridization between Crocodylus acutus and Crocodylus moreletii in the Yucatan Peninsula: I. Evidence from mitochondrial DNA and morphology.

The American crocodile (Crocodylus acutus) and the Morelet's crocodile (C. moreletii) are broadly sympatric in Belize and Mexico. The presence of morphologically anomalous individuals in the overlapping range area suggests possible hybridization between these species. Analysis of 477 base pairs of the mitochondrial tRNA(Pro)-tRNA(Phe)-Dloop region revealed the presence of pure C. acutus (N=43) and C. moreletii (N=56), as well as a high proportion of interspecific hybrids (N=17, 14.6%) in the Yucatan Peninsula, Mexico. Although all individuals could be assigned to one species or other based on

The successful removal of a subretinal cysticercus by pars plana vitrectomy.

A 39-year-old woman from Belize presented with recent vision loss in her left eye due to a subretinal cysticercus located near the macula. We are reporting the use of standard vitrectomy instrumentation and techniques to successfully remove the subretinal cysticercus by a transretinal approach. Pars plana vitrectomy with transretinal removal of the cysticercus is a useful alternative to the traditional external sclerotomy.

Unusual presentations of cutaneous larva migrans in British military personnel.

Cutaneous larva migrans (CLM) is one of numerous skin diseases that occur in British military personnel on deployments to the tropics and sub-tropics. It is typically managed by military primary healthcare services, but diagnostic uncertainty or unavailability of anti-helminthic medication may prompt referral to UK Role 4 healthcare services. Cases of CLM seen at the UK Role 4 Military Infectious Diseases & Tropical Medicine Service from 2005 to 2020 were identified and their case notes were reviewed to identify learning and discussion points. There were 12 cases identified, of which five came
