Activation of P2X7 signaling, due to high glucose levels, leads to blood retinal barrier (BRB) breakdown, which is a hallmark of diabetic retinopathy (DR). Furthermore, several studies report that high glucose (HG) conditions and the related activation of the P2X7 receptor (P2X7R) lead to the over-expression of pro-inflammatory markers. In order to identify novel P2X7R antagonists, we carried out virtual screening on a focused compound dataset, including indole derivatives and natural compounds such as caffeic acid phenethyl ester derivatives, flavonoids, and diterpenoids. BACKGROUND: It remains unknown whether a low diastolic blood pressure (DBP) increases the risks of cardiovascular events and death in patients with heart failure with preserved ejection fraction (HFpEF).
METHODS: We used data from the TOPCAT trial. The primary outcome was a composite of all-cause death, non-fatal myocardial infarction, non-fatal stroke, or hospitalization for heart failure. Hazard ratios (HRs) were analyzed for DBPs of <60, 60-69, 70-79, and ≥90 mm Hg in comparison with a DBP of 80-89 mm Hg using multivariable Cox proportional hazard models.
RESULTS: This study included 3417

BACKGROUND: In our model of a congenital heart defect (CHD) with increased pulmonary blood flow (PBF; shunt), we have recently shown a disruption in carnitine homeostasis, associated with mitochondrial dysfunction and decreased endothelial nitric oxide synthase (eNOS)/heat shock protein (Hsp)90 interactions that contribute to eNOS uncoupling, increased superoxide levels, and decreased bioavailable nitric oxide (NO). Therefore, we undertook this study to test the hypothesis that L-carnitine therapy would maintain mitochondrial function and NO signaling.
METHODS: Thirteen fetal lambs underwent

A study was designed to evaluate the efficacy of the CO2 laser in performing conservative uterine surgery, metroplasty and myomectomy; to determine blood loss using this technique; and to ascertain how much normal uterine tissue was preserved. To assess the effectiveness of this new surgical technique, 22 patients underwent metroplasty, myomectomy or both utilizing the CO2 laser. The blood loss via microlaser surgery was compared to that in a group of 14 patients who underwent conventional surgery. The initial findings indicated that the CO2 laser is a viable surgical instrument for myomectomy

Background/Objectives: The rapid progression of stroke often results in irreversible brain damage and poor outcomes when treatment is delayed. Prophylactic administration of FAD012 (3,5-dimethyl-4-hydroxycinnamic acid), a synthetic derivative of ferulic acid (FA), has demonstrated cerebroprotective effects in ischemic models through antioxidant and endothelial protective mechanisms. This study investigated the effects of FAD012 on cerebral infarction and blood-brain barrier (BBB) integrity using a photothrombotic stroke model in rats. Methods: Male Sprague Dawley rats received a single intrape

Cortical spreading depression (CSD) is a pathophysiological mechanism underlying headache disorders, including migraine. Blood-brain barrier (BBB) permeability is increased during CSD. Recent papers have suggested that heat shock proteins (HSP) contribute to the integrity of the blood-brain barrier. In this study, the possible role of HSP90 in CSD-associated blood-brain barrier leak at the endothelial cell was investigated using an in vitro model, for the blood-endothelial barrier (BEB), and an in vivo model with an intact BBB. We measured barrier integrity using trans endothelial electric res

Although systolic blood pressure (SBP) is routinely considered when treating acute heart failure (HF), diastolic blood pressure (DBP) is hardly been assessed in the situation. There are no previous studies regarding the predictive value of DBP in elderly patients with HF with preserved ejection fraction (HFpEF) in Japan. This study aimed to investigate the prognostic significance of DBP in patients with acute decompensated HFpEF. We analyzed data of all HFpEF patients admitted to Shinonoi General Hospital for HF treatment between July 2016 and December 2018. We excluded patients with acute cor

BACKGROUND: Guidelines recommend targeting systolic blood pressure (SBP) <130 mm Hg in heart failure with preserved ejection fraction (HFpEF) with limited data.
OBJECTIVES: This study sought to determine the optimal achieved SBP and whether the treatment effects of sacubitril/valsartan on outcomes are related to BP lowering, particularly among women who derive greater benefit from sacubitril/valsartan.
METHODS: Using 4,795 trial participants, this study related baseline and time-updated mean achieved SBP quartiles (<120, 120 to 129, 130 to 139, ≥140 mm Hg) to the primary outcome (cardiovascu

BACKGROUND: This study investigated the blood pressure (BP) values over the day-night period in 11 noninstitutionalized patients affected by probable Alzheimer's disease (AD) in its early stage. The scientific aim was to detect whether the BP circadian rhythm (CR) was preserved, given the fact that CR disruption was observed in advanced or institutionalized AD patients.
METHODS: The BP within-day values were gathered via noninvasive ambulatory monitoring. The BP time series were analyzed according to the chronobiological procedure, called Cosinor method with three harmonic components.

BACKGROUND: The ideal surgery for a blood blister-like aneurysm (BBA) in the internal carotid artery (ICA) involves complete termination of blood flow into the BBA by trapping of the ICA at sites both proximal and distal to the BBA. In the present report, we describe a clipping method with ICA trapping for prevention of anterior choroidal artery ischemia, a major problem in ICA trapping with reconstruction surgery using external carotid artery-middle cerebral artery high-flow bypass (HFB).
METHODS: The data from patients with a ruptured BBA treated by the combination of ICA trapping and blood

BACKGROUND: Pulmonary arteriovenous malformations (PAVMs) are a recognised cause of progressive cyanosis following bidirectional superior cavopulmonary anastomosis (BDG). The role of antegrade pulmonary blood flow (APBF) in preventing PAVM formation remains controversial. While preserved APBF may improve oxygen saturation and pulmonary artery growth, its protective effect against PAVMs is uncertain. Increasing evidence suggests that exclusion of hepatic venous blood from the pulmonary circulation, rather than absence of pulsatile flow, may be the primary mechanism underlying PAVM development.

Pyruvate was placed in experimental CPD-adenine (0.25 mM) blood preservative mixtures in four concentrations ranging from 40 to 320 mM. In the 320 mM pyruvate preservative, 2,3-DPG levels were elevated above normal for six weeks of whole blood storage at 4 C. The lower pyruvate concentrations maintained elevated or normal 2,3-DPG levels for less time: four weeks with 160 mM, two weeks with 80 mM, and one week or less with 40 mM or the control. ATP values were best maintained in the control. The higher pyruvate concentrations resulted in the most rapid decreases at ATP. However, even the 320 mM

BACKGROUND: Red blood cell (RBC) preservation is essential to transfusion medicine. Many blood group reference laboratories need a method to preserve rare blood samples for serologic testing at a later date. This study offers a comparison of three common cryoprotective agents and protocols used today: bulk preservation with glycerol and droplet freezing with sucrose-dextrose (S+D) or polyvinylpyrrolidone (PVP).
STUDY DESIGN AND METHODS: Human blood from 14 volunteers was collected and frozen at set intervals over 2 weeks with PVP, S+D, or glycerol. The frozen RBCs were later thawed and the pe

IMPORTANCE: Blood pressure (BP) lowering is considered neuroprotective in patients with cerebral small vessel disease; however, more intensive regimens may increase cerebral hypoperfusion. This study examined the effect of standard vs intensive BP treatment on cerebral perfusion in patients with severe small vessel disease.
OBJECTIVE: To investigate whether standard vs intensive BP lowering over 3 months causes decreased cerebral perfusion in small vessel disease.
DESIGN, SETTING, AND PARTICIPANTS: This randomized clinical trial took place at 2 English university medical centers. Patients we

Traumatic brain injury (TBI) is a leading cause of mortality and long-term disability, yet effective pharmacological therapies remain unavailable. Early blood-brain barrier (BBB) disruption accelerates neuroinflammation and secondary brain injury. It has been well recognized that brain microvascular endothelial cells (BMECs) depend on mitochondrial oxidative metabolism to maintain BBB integrity. Here, we investigated the role of the mitochondria-derived immunometabolite itaconate in regulating cerebrovascular bioenergetics and BBB integrity after TBI. Using a controlled cortical impact model,

Blood supply shortages remain a persistent challenge for transfusion services, yet effective donor recall is hampered by fragmented data across institutions, limited profiling depth, and increasing privacy regulations. This paper proposes a federated multimodal learning framework that enables distributed blood centers to collaboratively train a shared donor profiling model without exchanging raw records. The framework integrates three modality-specific encoders - for demographic attributes, behavioral donation sequences, and textual feedback - unified through a cross-modal attention mechanism

The time interval between the collection of biological material and the performance of toxicological analyses is particularly important when the appropriate storage temperature cannot be maintained. This issue is especially relevant in the determination of ethanol concentration in specimens obtained from drivers suspected of intoxication or being under the influence of ethanol, as well as in samples collected during post-mortem examinations. The present study evaluated the effect of room-temperature storage on ethanol concentrations in post-mortem blood, urine, costal cartilage, and interverte

BACKGROUND: There is conflicting evidence on whether normovolemic hemodilution is efficacious in reducing intraoperative blood loss and transfusions, and existing pediatric literature does not evaluate hemodilution in combination with antifibrinolytics. The purpose of our study was to compare patient outcomes with combined use of hemodilution and antifibrinolytics against antifibrinolytics alone in patients undergoing posterior spinal fusion (PSF) for adolescent idiopathic scoliosis (AIS).
METHODS: Inclusion criteria were diagnosis of AIS and operative management with PSF. Patients with a his

Acute pancreatitis is a common inflammatory condition of the pancreas that can lead to severe complications such as chronic pancreatitis and pancreatic ductal adenocarcinoma. While vascular remodeling is a hallmark of many inflammatory conditions, the molecular mechanisms underlying vascular changes during pancreatitis remain largely unexplored. This study aimed to investigate the vascular changes associated with acute pancreatitis and to identify the molecular mechanisms underlying these changes. Acute pancreatitis was induced in wild-type mice through caerulein injections and resulted in pro

BACKGROUND: Traditionally, platelets were thought to originate solely in the bone marrow, but emerging evidence now indicates that the lung is a major site of platelet generation. Our previous work, along with studies by others, has revealed decreased platelet counts and enhanced platelet activation in BPD patients, linking platelets to the disease's pathogenesis.
METHODS: We investigated the protective effects and underlying mechanisms of umbilical cord blood (UCB) platelets in mitigating hyperoxia-induced lung injury in neonatal rats and in vitro.
RESULTS: Compared with platelet-poor plasm
