Extramedullary masses as presenting features of acute monoblastic leukemia.

Acute monoblastic leukemia is an uncommon type of non-lymphocytic leukemia that generally first manifests with signs and symptoms of bone marrow failure. This study describes four patients with acute monoblastic leukemia in whom the initial manifestations of the disorder were related to extramedullary masses. Three of the four patients were children. All patients had bone marrow involvement. The extramedullary mass lesions of acute monoblastic leukemia may be difficult to distinguish from other hematopoietic neoplasms, such as granulocytic sarcoma, malignant lymphoma and malignant histiocytosi

Isolated allogeneic bone marrow-derived mesenchymal cells engraft and stimulate growth in children with osteogenesis imperfecta: Implications for cell therapy of bone.

Treatment with isolated allogeneic mesenchymal cells has the potential to enhance the therapeutic effects of conventional bone marrow transplantation in patients with genetic disorders affecting mesenchymal tissues, including bone, cartilage, and muscle. To demonstrate the feasibility of mesenchymal cell therapy and to gain insight into the transplant biology of these cells, we used gene-marked, donor marrow-derived mesenchymal cells to treat six children who had undergone standard bone marrow transplantation for severe osteogenesis imperfecta. Each child received two infusions of the allogene

How I utilize somatic alterations in the diagnosis, risk stratification, and therapy of hypocellular bone marrow failure.

Hypocellular bone marrow failure (BMF) may be acquired due to immune-mediated disease, the prototype being immune aplastic anemia (IAA), or inherited, due to germ line defects in genes important for hematopoietic stem cell function and maintenance (inherited BMF syndromes [IBMFS]). Proper diagnosis of the underlying etiology of hypocellular BMF, particularly distinguishing IAA, myelodysplastic syndrome (MDS; most relevant in this setting is hypoplastic MDS), and the IBMFS, is important given the differences in clinical management. Clonal hematopoiesis (CH), in this context comprising somatic m

[Jamshidi biopsy in clinical hematology. Method, indications and results of over 1,000 completed biopsies with special reference to chronic myeloproliferative diseases].

One thousand and five bone marrow biopsies were performed in patients with haematologic or oncologic disorders during a ten year period from 1976 to 1985 according to the method of Jamshidi and Swaim. Indications and method of biopsy are discussed in detail. Major side effects were not observed, however minor accidents (0.2%) as well as problems in yielding biopsy-material (1.6%) are reported. The rate of biopsy-failure, including biopsies with insufficient (crushed) material, was 5%. In our hands the predominant value of the Jamshidi-biopsy for diagnosis of hematologic disorders is given by t

Therapeutic discovery for marrow failure with MDS predisposition using pluripotent stem cells.

Monosomy 7 or deletion of 7q (del(7q)) are common clonal cytogenetic abnormalities associated with high grade myelodysplastic syndrome (MDS) arising in inherited and acquired bone marrow failure. Current non-transplant approaches to treat marrow failure may be complicated by stimulation of clonal outgrowth. To study the biological consequences of del(7q) within the context of a failing marrow, we generated induced pluripotent stem cells (iPSCs) derived from patients with Shwachman Diamond Syndrome (SDS), a bone marrow failure disorder with MDS predisposition, and genomically engineered a 7q de

Increased bone marrow mast cells in preleukemic syndromes, acute leukemia, and lymphoproliferative disorders.

Increased numbers of bone marrow mast cells were found in 45 (2.2%) of 2,000 bone marrow specimens obtained from patients who had hematologic disorders. Mast cells were most frequently seen in the marrows of patients who had preleukemic syndromes, lymphoproliferative disorders, and acute leukemia. The 16 patients who had preleukemic syndromes included those with refractory sideroblastic and megaloblastic anemia (with or without an excess of blasts), idiopathic pancytopenia or pure erythrocytic aplasia, paroxysmal nocturnal hemoglobinuria, idiopathic refractory neutropenia, agranulocytosis or t

Do bone marrow fat cells or their precursors have a pathogenic role in idiopathic aplastic anaemia?

Idiopathic aplastic anaemia (AA), aplastic anaemia of unknown aetiology, is usually defined as marrow failure with fatty replacement of hemopoietic tissue and peripheral pancytopenia. The pathophysiology is largely unknown, though many mechanisms have been hypothesized. These include the absence of or defects in hemopoietic stem cells (HSC), abnormalities of the bone marrow (BM) microenvironment, immune system disorders and abnormalities of the regulatory factors that control hemopoiesis. The characteristic feature of AA is the replacement of hematopoietically active marrow by fat cells; howev

Shwachman-Diamond Syndrome: Molecular Mechanisms and Current Perspectives.

Shwachman-Diamond syndrome (SDS) is a rare inherited disease mainly caused by mutations in the Shwachman-Bodian-Diamond Syndrome (SBDS) gene. However, it has recently been reported that other genes, including DnaJ heat shock protein family (Hsp40) member C21 (DNAJC21), elongation factor-like 1 (EFL1) and signal recognition particle 54 (SRP54) are also associated with an SDS-like phenotype. Interestingly, SBDS, DNAJC21, EFL1 and SRP54 are involved in ribosome biogenesis: SBDS, through direct interaction with EFL1, promotes the release of the eukaryotic initiation factor 6 (eIF6) during ribosome

[Clinical significance of comprehensive genomic profiling in hematopoietic failure].

This review outlines the clinical significance of comprehensive genomic profiling (CGP) in patients with hematopoietic failure, including severe unexplained cytopenia. Since March 2025, Japanese National Health Insurance has covered CGP for hematologic malignancies and related disorders, facilitating a genome-based precision medicine approach that complements conventional clinical approaches, including morphologic and cytogenetic analyses. In bone marrow failure phenotypes, CGP supports differentiation of aplastic anemia from myelodysplastic syndromes and related diseases by evaluating genetic

[Clinical significance of clonal hematopoiesis and disease boundaries in bone marrow failure diseases].

Aplastic anemia (AA) is a non-neoplastic bone marrow failure syndrome caused by the destruction of hematopoietic stem and progenitor cells by the immune system. However, in some cases of AA, a small number of specific clones with gene mutations are observed without clinical manifestations. Cases with mutated PIG-A, BCOR/BCORL1, or HLA class I allele clones respond better to immunosuppressive therapies (ISTs). Cases with MDS-related clones, such as DNMT3A or ASXL1 mutations, are at a higher risk for secondary MDS. In this review, I will focus on the clonal hematopoiesis (CH) in AA and discuss i

Myelodysplastic Syndrome, Acute Myeloid Leukemia, and Cancer Surveillance in Fanconi Anemia.

Fanconi anemia (FA) is a DNA repair disorder associated with a high risk of cancer and bone marrow failure. Patients with FA may present with certain dysmorphic features, such as radial ray abnormalities, short stature, typical facies, bone marrow failure, or certain solid malignancies. Some patients may be recognized due to exquisite sensitivity after exposure to cancer therapy. FA is diagnosed by increased chromosomal breakage after exposure to clastogenic agents. It follows autosomal recessive and X-linked inheritance depending on the underlying genomic alterations. Recognizing patients wit

Hematopoietic stem cell transplantation in childhood inherited bone marrow failure syndrome.

Aplastic anemia is a rare disease in children that is most commonly idiopathic and less often a hereditary disorder. Hereditary bone marrow failure (BMF) syndromes, however, should be considered both in children and in adults before any attempt at treatment. Precise diagnosis is important because it will modify prognostic treatment options and the results of bone marrow transplantation. In this review, we will report recent results of treatment of Fanconi anemia and other hereditary BMF syndromes.

Inherited bone marrow failure with ERCC6L2 gene mutation: presentation of aplastic anemia in a 3-year-old child: a case report.

BACKGROUND: Aplastic anemia is a rare hematologic disorder characterized by bone marrow hypoplasia and peripheral pancytopenia. While most pediatric cases are acquired, inherited bone marrow failure syndromes (IBMFS) are important differential diagnoses. ERCC6L2-associated IBMFS is a newly recognized and rare condition with significant implications for diagnosis and long-term care in hematology. CASE PRESENTATION: A case report of a 3-year-old Ethiopian girl presenting with dry cough, low-grade fever, poor appetite, and spontaneous gum bleeding. Examination revealed microcephaly, café-au-lait

[Genetic abnormalities in bone marrow failure].

Bone marrow (BM) failure is a condition characterized by peripheral pancytopenia resulting from decreased hematopoiesis in the BM. It includes congenital disorders such as Fanconi anemia (FA), as well as acquired conditions such as acquired aplastic anemia (AA), myelodysplastic syndrome (MDS), and paroxysmal nocturnal hemoglobinuria (PNH). AA presents with pancytopenia and BM hypoplasia, primarily triggered by an autoimmune mechanism involving T cells that damage hematopoietic stem cells (HSCs). Genomic investigations utilizing next-generation sequencing or SNP arrays have revealed that clonal

Sca-1 / c-Kit receptor expression and apoptosis pattern in ENU induced MDS mice.

The bone marrow is the major site of haemopoiesis in adult human. It contains cells that represent the stages in the development of different types of blood cells e.g. myelocytes, metamyelocytes, erythroblasts, reticulocytes, and other lymphoid progenies etc. Bone marrow failure is primarily the result of a specific failure of bone marrow precursor cells to produce mature cells. N-ethyl N-nitroso urea (ENU) is one of the most potent mutagens that can create an abnormal bone marrow microenvironment by causing defect in haematopoietic stem cell maturation cascade. ENU is easy to administer in mo

Bone Marrow Failure and Immunodeficiency Associated with Human RAD50 Variants.

PURPOSE: The MRE11-RAD50-NBN (MRN) complex plays a key role in recognizing and signaling DNA double-strand breaks. Pathogenic variants in NBN and MRE11 give rise to the autosomal-recessive diseases, Nijmegen breakage syndrome (NBS) and ataxia telangiectasia-like disorder, respectively. The clinical consequences of pathogenic variants in RAD50 are incompletely understood. We aimed to characterize a newly identified RAD50 deficiency/NBS-like disorder (NBSLD) patient with bone marrow failure and immunodeficiency. METHODS: We report on a girl with microcephaly, mental retardation, bird-like face,

MPIG6B-related thrombocytopenia and myelofibrosis: A case report.

Congenital myelofibrosis (cMF) is a rare inherited bone marrow failure disorder. We report a 26-year-old Chinese male with a 24-year history of recurrent epistaxis and thrombocytopenia. Laboratory findings revealed severe thrombocytopenia (18 × 10⁹/L) and bone marrow biopsy showed MF-3 fibrosis with impaired megakaryopoiesis. Genetic analysis identified a novel homozygous nonsense mutation in MPIG6B (c.420T > A, p.Y140X), leading to a diagnosis of cMF. This case expands the clinical spectrum of MPIG6B-related disorders and illustrates the long-term natural history in a non-consanguineous popul

A case with multiple myeloma complicated with hemophagocytic syndrome (HS) is presented. Because pancytopenia, liver dysfunction and increase of mature histiocytes in the bone marrow appeared rapidly a diagnosis of HS was made. The patient died of multiple organ failure, despite steroid therapy. Autopsy revealed marked invasion of hemophagocytic histiocytes not only into the bone marrow but also into many other organs such as the liver, lymph nodes and kidneys. HS is a histiocyte proliferative disorders, which is likely to be seen in immunocompromised hosts, but there is no previous report abo

Bone-marrow transplantation in Hurler's syndrome. Effect on skeletal development.

Between 1980 and 1988, displacement bone-marrow transplantation was performed on 25 children with Hurler's syndrome (type-1 mucopolysaccharidosis). We describe the musculoskeletal development of 11 of the 12 surviving children and the orthopaedic procedures undertaken to treat progressive thoracolumbar kyphosis, hip subluxation and carpal tunnel syndrome. We found abnormal bone modelling, focal failures of ossification and an avascular disorder of the femoral head in every patient and offer an explanation for these phenomena. Pregnancy in bone marrow failure syndromes: Diamond-Blackfan anaemia and Shwachman-Diamond syndrome.

Pregnancy in bone marrow failure syndromes has risk to mother and fetus. There are fewer than 30 reports of cases with Diamond-Blackfan anaemia (DBA), and none with Shwachman-Diamond syndrome (SD). We report two DBA and one SD cases. One DBA mother received transfusions intra-partum, and the other only post-partum. Both required caesarean sections (C-sections) for failure of labour to progress and severe pre-eclampsia respectively. Both subsequently resumed pre-pregnancy steroid-induced control of anaemia. approximately 40% of DBA pregnancies required maternal transfusions; 25% delivered by C-
