Do not forget the brachial plexus-prevalence of distal brachial plexus pathology on routine shoulder MRI.

OBJECTIVES: Most of the shoulder magnetic resonance imaging (MRI) examination focuses on internal joint structures but disregarding other structures like the distal brachial plexus, which may miss important findings. Hereby, we attempt to evaluate the prevalence of distal brachial plexus abnormalities and/or muscular denervation changes seen on routine shoulder MRI examinations and discuss common pathologies affecting the distal brachial plexus. MATERIAL AND METHODS: A total of 701 routine shoulder MRI studies were evaluated. The evaluation of each exam was focused on the visualized brachial

Long-Term Outcomes of Brachial Plexus Reconstruction with Sural Nerve Autograft for Brachial Plexus Birth Injury.

BACKGROUND: Infants with brachial plexus birth injury who do not recover motor function spontaneously in a timely manner are candidates for brachial plexus reconstruction with nerve autograft. Outcomes of this intervention are incompletely understood. The authors present the long-term outcomes of brachial plexus reconstruction with sural nerve autograft in infants with brachial plexus birth injury. METHODS: The authors retrospectively reviewed all infants with brachial plexus birth injury who underwent brachial plexus reconstruction with sural nerve autograft between 1992 and 2014 with a mini

[Results of brachial plexus surgery and replacement operations in traumatic brachial plexus birth injury-induced paralysis].

Since the development of microsurgical methods, the timing and concepts of surgery have significantly changed in obstetrical palsies. Because of the high axon regeneration potential in children, early and successful microsurgical reconstruction of the brachial plexus provides very encouraging results. Such successful nerve regeneration is the basis for secondary surgery which aims at complete restoration of function of the upper limb.

Evaluation of the brachial plexus and spinal cord with diffusion tensor imaging and synthetic MRI in obstetric brachial plexus palsy: DTI of brachial plexus and spinal cord in OBPP.

PURPOSE: Obstetric brachial plexus palsy (OBPP) is conventionally staged as a peripheral nerve injury, yet conventional magnetic resonance imaging (MRI) poorly reflects microstructural neural and spinal cord changes. We evaluate whether magnetic resonance image compilation (MAGIC), a form of synthetic MRI, shows strong agreement with conventional T2-weighted short-tau inversion recovery (T2-STIR) for postganglionic injury detection, compare diffusion tensor imaging (DTI) metrics between injured and adjacent intact segments of the brachial plexus and spinal cord, and explore whether these metri

Lumbosacral plexus injury and brachial plexus injury following prolonged compression.

We report the case of a 36-year-old woman who developed right upper and lower limb paralysis with sensory deficit after sedative drug overdose with prolonged immobilization. Due to the initial motor and sensory deficit pattern, brachial plexus injury or C8/T1 radiculopathy was suspected. Subsequent nerve conduction study/electromyography proved the lesion level to be brachial plexus. Painful swelling of the right buttock was suggestive of gluteal compartment syndrome. Elevation of serum creatine phosphokinase and urinary occult blood indicated rhabdomyolysis. The patient received medical treat

Ultrasound accuracy for brachial plexus pathology.

AIM: To determine (a) the accuracy of ultrasound in detecting brachial plexus pathology and (b) outline the advantages and limitations of ultrasound compared to MRI for imaging the brachial plexus. MATERIAL AND METHODS: cases with clinically suspected brachial plexus pathology were evaluated first by ultrasound, followed by MRI. Patients with prior brachial plexus imaging were excluded. The final diagnosis was based on a combination of ultrasound, MRI, clinical follow-up, and surgical findings. The accuracy of the ultrasound was assessed by comparing the ultrasound and the final diagnoses. Th

Discussions About Obstetric Brachial Plexus Injuries.

Although patients with obstetric brachial plexus injuries (OBPI) have been recognized and treated for greater than 100 years there is much that is not understood or is mis-understood. I address 6 areas for discussion: the cause of OBPI and whether it matters to nerve surgeons; the value of the Narakas grading; whether surgeons should perform primary nerve surgery, especially in patients with incomplete OBPI; the cause and treatment of shoulder tightness; the cause and treatment of elbow contracture; and whether patients with OBPI need surgery in adulthood.

Pathophysiologic Origins of Brachial Plexus Injury.

OBJECTIVE: To analyze the origins and consequences of cases of brachial plexus injuries and their relationship to shoulder dystocia. METHODS: We conducted a retrospective cohort study and identified all neonates with brachial plexus injury delivered at our institution between March 2012 and July 2019. A review was performed of the maternal and neonatal records of each neonate to identify obstetric antecedents, including the occurrence of shoulder dystocia and persistence of the injury. Experience of the delivering clinician was also examined. Statistical analysis was performed with the Fisher

[Acute dyspnoea during brachial plexus blockade].

A 73-year-old woman was scheduled for brachial shunt surgery. She received regional anaesthesia of the right brachial plexus via a supraclavicular approach. During surgery we noticed acute dyspnoea with a saturation reduction. A postoperative chest X-ray showed a high position of the right diaphragm, which was caused by a phrenic nerve blockade due to the anaesthesia. This completely disappeared within 24 hours.

Central Adaptation following Brachial Plexus Injury.

Brachial plexus trauma (BPT) often affects young patients and may result in lasting functional deficits. Standard care following BPT involves monitoring for clinical and electrophysiological evidence of muscle reinnervation, with surgical treatment decisions based on the presence or absence of spontaneous recovery. Data are emerging to suggest that central and peripheral adaptation may play a role in recovery following BPT. The present review highlights adaptive and maladaptive mechanisms of central and peripheral nervous system changes following BPT that may contribute to functional outcomes.

Renaissance of supraclavicular brachial plexus block.

Due to frequent complications, especially pneumothorax, supraclavicular brachial plexus block became less popular. Ultrasonography is a very powerful tool in modern medicine and a real milestone in regional anaesthesia. Ultrasound- guided supraclavicular brachial plexus block reduces the probability of major complications occurrence (like pneumothorax, Horner's syndrome, phrenic nerve palsy). In this review we present the usefulness of ultrasonographic imaging and how to perform efficient ultrasound-guided blockade safely.

Bronchospasm following supraclavicular brachial plexus block.

Supraclavicular brachial plexus block is commonly performed for upper limb surgeries. In patients with compromised respiratory function or in the elderly it may be preferred over general anaesthesia. Bronchospasm, albeit a rare complication of this procedure, may turn the advantages of regional anaesthesia in these patients into a disadvantage. Bronchospasm following interscalene approach has been previously reported. However, the same following the supraclavicular approach has not yet been reported. A 70-year-old woman presented with cellulitis of the left upper limb. A successful left brachi

[Risk management in brachial plexus block].

Peripheral nerve block has many advantages in surgical anesthesia with or without general anesthesia; postoperative analgesia, faster postoperative rehabilitation, and chronic pain management. However, serious adverse complications after peripheral nerve block can happen. Therefore, anesthetists should obtain full informed consent for possible complications, and require scrupulous attention to this procedure. This review focuses on complications of brachial plexus block because it is the most popular peripheral nerve block.

Management of birth brachial plexus palsy.

INTRODUCTION: The indications for surgical repair of congenital brachial plexus palsy are controversial. Our objective was to determine the results of early brachial plexus surgery following obstetric-induced brachial plexus palsy. METHODS: We performed a retrospective analysis of the outcome of 58 cases of brachial plexus surgery. The indication for operation consisted of the presence of less than antigravity strength in the biceps, triceps, and deltoid muscle groups at 6 months of age. Data gathered prospectively, previously, showed the likelihood of improvement with less than antigravity s

Secondary surgery following brachial plexus injuries.

The favourable treatment of post-traumatic brachial plexus lesions based on our experience of 362 cases over a 12 year period is reported. Twenty-five percent of the patients needed secondary operations. The spectrum of the latter consisted of arthrodesis, tenodesis, and musculotendinous transfer, including free neurovascular tissue transfer partially innervated by nerve transposition. Functionally, secondary tendon transfer can help to improve the effect of nerve repair techniques. To restore shoulder function the trapezius transfer (n = 22) has been used mainly; elbow flexion has been regain
