Charles bonnet syndrome associated with age-related macular degeneration.

PURPOSE: The visual hallucinations in Charles Bonnet syndrome (CBS) are most commonly attributed to release phenomena that arise due to a paucity of sensory input to visual association areas of the cerebral cortex (Cogan's sensory deprivation theory of visual hallucinations). The authors wished to gain further insight into this mechanism behind CBS by examining a patient who experienced reversible visual hallucinations. METHODS: The authors present a 77-year-old man with a history of severe age-related macular degeneration who experienced formed and moving visual hallucinations associated wit

Charles Bonnet syndrome and periodic alternating nystagmus: Moving visual hallucinations.

OBJECTIVE: To describe and discuss potential mechanisms for modulation of visual hallucinations by nystagmus. METHODS: We present 2 patients with coexistent Charles Bonnet syndrome and periodic alternating nystagmus in the context of acquired visual loss. RESULTS: The combination has given rise to a rare phenomenon: visual hallucinations that move in a manner governed by the nystagmus, specifically by the direction and velocity of the slow phase. The perceived modulation of movement is selective for a surface in one case and a landscape in the other but not present for hallucinated individua

Screening for Charles Bonnet syndrome: Should the definition be reconsidered?

PURPOSE: Charles Bonnet syndrome (CBS) is a condition in which individuals with visual impairment (VI) and with no cognitive deficits experience visual hallucinations, typically with no other sensory hallucinations. Although few isolated case reports of CBS from India have been published, the prevalence for CBS in India is largely unknown. The primary aim of this study was to estimate CBS prevalence in patients with vision impairment visiting a tertiary eye care center. METHODS: The study was conducted in two phases. In phase 1, patients with VI, age ≥40 years with presenting visual acuity wo

The Charles Bonnet Syndrome: a Systematic Review of Diagnostic Criteria.

PURPOSE: To perform a systematic review of diagnostic criteria for the Charles Bonnet syndrome (CBS). RECENT FINDINGS: Across 33 studies that specified diagnostic criteria for CBS, hallucinations and vision loss were a common requirement, but there was considerable heterogeneity regarding hallucination properties (i.e., formed vs. unformed) and the severity of vision loss. The exclusion of confounding neuropsychiatric disorders was also common, but specific disorders and their method of ascertainment were variable. There is considerable diagnostic heterogeneity for CBS in the literature. Thes

[Visual hallucinations and giant cell arteritis: the Charles Bonnet syndrome].

In patients with visual hallucinations, diagnostic strategy is unclearly codified. In patients known to have giant cell arteritis, the main diagnostic assumption is disease relapse. Indeed, this should lead to rapid corticosteroid therapy. However, the Charles Bonnet syndrome, that is a poorly known etiology of visual hallucinations usually observed in elderly people, should be part of the differential diagnosis. We report a 87-year-old woman, with a 2-year history of giant cell arteritis who was admitted with an acute onset of visual hallucinations and who met all the criteria for Charles Bon

[Psychiatric, psychological comorbidities of typical and atypical Charles-Bonnet syndrome].

OBJECTIVES: Charles-Bonnet syndrome (CBS) is conventionally defined by the presence of visual hallucinations in patients suffering from lowered visual acuity without having psychosis or dementia. Actually, it is a syndrome that interests many specialties, especially ophthalmology, geriatrics, neurology and psychiatry. "Atypical CBS" or "CBS plus" was introduced to designate any kind of visual hallucinations that could be considered as a CBS but accompanied by a low level of insight, a possible cognitive decline, other hallucinatory modalities etc. Since all patients suffering from CBS have to

Acute reversible Charles Bonnet syndrome precipitated by sudden severe anemia.

PURPOSE: To report the sudden onset of reversible Charles Bonnet syndrome precipitated byacute severe anemia. METHODS: The charts of three patients (Usher syndrome, bilateral macular degeneration, and bilateral retinal vein occlusion) with acute Charles Bonnet syndrome in the setting of severe anemia were reviewed. RESULTS: Anemia resulted from bladder surgery, recto-colitis, and severe urinary tract infection. Hemoglobin ranged from 78 to 86 g/L. Decreased visual acuity and formed visual hallucinations (giants, flowers, animals) were present in all three patients. Rapid reversal of Charles

Charles Bonnet syndrome and vitamin B12 deficiency: a case report.

The Charles Bonnet syndrome (CBS) is a condition associated with complex visual hallucinations occurring in the elderly in patients with visual impairment and normal mental health. Here, we report the case of a 78-year-old woman who has a limited visual acuity with a CBS that we postulated to be in relationship to a vitamin B12 deficiency. This case is the first report of vitamin B12 deficiency-associated CBS.

[Charles Bonnet syndrome: report of one case managed with haloperidol].

Charles Bonnet syndrome is an underrecognized condition characterized by complex visual hallucinations, ocular problems causing visual deterioration and preserved cognitive status. Its prevalence is 5/1000 in ambulatory ophthalmologic patients. Generally occurs in elderly people in whom it may be confused with delirium or dementia. The first management step is to improve vision, if possible. Hallucinations may be managed pharmacologically. We report a 94 year-old woman with the characteristic clinical picture of the syndrome that improved with haloperidol.

[Visual hallucinations in mentally healthy persons--the Charles Bonnet syndrome].

Charles Bonnet syndrome is described as complex visual hallucinations that are known to occur in individuals with impaired vision whose emotional and intellectual functions are within normal range. This paper presents a case of a 82 years old woman who experienced an individual hallucinatory episode during three days: seeing, alert and with her eyes open, different figures and images, static and moving in lovely colors that disappeared with closing her eyes and appeared as new with opening it. The diagnostic procedure comprised EEG and ophthalmologic examination. Vitamins and hemorrheologicall

The Charles Bonnet syndrome: a brief review and case report.

A case of a woman with visual hallucinations in the absence of other organic or psychiatric findings--symptoms consistent with a diagnosis of Charles Bonnet syndrome--is reported. The women was HIV positive, although asymptomatic for conventional complications of HIV infection. After considering possible causes of the hallucinations, including prescription drug effects and conversion or factitious disorder, the authors suggest that the woman's symptoms may have been associated with undetectable effects of HIV on the brain.

Charles Bonnet Syndrome in the Setting of a Traumatic Brain Injury.

Traumatic brain injuries are often associated with a broad range of neuropsychiatric sequelae, for which no straightforward treatment approach is established. Historically, the treatment of traumatic brain injuries has been tailored towards the symptoms presented by the patient. In this paper, we present the case of a 42-year-old male with a past medical history significant for retinitis pigmentosa who suffered a traumatic brain injury and subsequently developed visual hallucinations consistent with Charles Bonnet syndrome.

[Charles Bonnet's syndrome: clinical insights in a fascinating disease].

Even mentally and cognitively healthy people can experience visual hallucinations. These are called Charles Bonnet's syndrome, named after the Swiss scientist, who first described such perceptions in the 18th century. Usually patients possess insight in the unreality of their visual experiences. The definitions of CBS are not consistent. Visual impairment or blindness is not part of every definition, but often is associated with CBS. Ophthalmologists should be aware of this syndrome, since its prevalence is high among blind or visually impaired people: patients often are reluctant to admit the
