Chordoma with a predominant retrosacral tumor mass.

Chordomas are malignant tumors with lobulated structure which usually consists of vacuolated physaliferous cells in a mucoid stroma (Enzinger et al. 1969). These cells originate from remnants of the chorda dorsalis. Most chordomas are located either at the end of the vertebral column, i.e., in the sacrococcygeal region or at the base of the skull near the sphenooccipital synchondrosis. More than half of all tumors are located in the sacrococcygeal region. In contrast to most cases of sacrococcygeal chordomas which show a predominant presacral tumor infiltration we report on a patient suffering…

Endoscopic Endonasal Resection of Clival Chordomas.

The endoscopic endonasal approach is a major step forward in the treatment of clival chordomas. It has improved the resection rate and in the same time reduced the complications. Chordomas located in the upper and mid-clival regions are well-suited for a purely endoscopic endonasal resection. Although with angulated endoscopes and curved high-speed drills, lateral parts of the tumor can be resected, lower clival lesions often necessitating a combination of both endonasal and transcranial approaches. A major limitation is a caudal tumor extension beyond the dens axis. For more caudally located …

[Chordoma: is there a molecular basis for diagnosis and treatment?].

Chordomas are malignant bone tumours with a reported annual incidence of 0.08 per 100,000 cases. They show a notochordal differentiation and are characterised by their nuclear expression of brachyury (TBXT). Chordomas are localised in the axial skeleton, where they occur from the clivus to the sacrococcygeal region. They are slow growing, locally destructive tumours, and are often not diagnosed until they have reached an advanced stage. Putative precursor-lesions are benign notochordal cell lesions, which are microscopically small and intraosseous. Different histological chordoma subtypes exis…

Oncogenic Viruses in Skull Base Chordomas.

BACKGROUND: Chordomas are rare tumors assumed to derive from notochordal remnants. We believe that a molecular switch is responsible for their malignant behavior. The involvement of oncogenic viruses has not been studied, however. Thus, in the present study, we investigated the presence of oncogenic viruses in chordomas. METHODS: DNA and RNA from snap-frozen chordoma (n = 18) and chondrosarcoma (n = 15) specimens were isolated. Real-time PCR or RT-PCR was performed to assess the presence of multiple oncogenic viruses, including herpesviridea (herpes simplex virus [HSV]-1, HSV-2, Epstein-Barr …

A review of the surgical management of sacral chordoma.

BACKGROUND: Sacral chordomas are rare low-to-intermediate grade malignant tumours, which arise from remnants of the embryonic notochord. This review explores prognostic factors in the management of sacral chordomas and provides guidance on the optimal treatment regimens based on the current literature. PATIENTS AND METHODS: Electronic searches were performed using MEDLINE, Embase and the Cochrane library to identify studies on prognostic factors in the management of sacral chordomas published between January 1970 and December 2013. The literature search and review process identified 100 artic…

[Chordoma: diagnostic considerations and review of the literature].

Chordoma is a rare malignant tumor of the bone; it arises from embryonic remnants of the primitive notochord and occurs along the midline from the skull base to the sacrum. In this article, we reviewed the origin, location, clinical, histopatological and imaging features, treatment, and differential diagnosis of chordoma.

The role of chemotherapy in pediatric clival chordomas.

The purpose of this retrospective study was to review the role of chemotherapy in the treatment, management and outcome of children diagnosed with clival chordomas. The medical records of six pediatric chordoma patients diagnosed at Childrens Hospital Los Angeles between 1995 and 2005 were reviewed. Of the six patients reviewed, all underwent an initial surgical resection. Following resection, three received a combination of chemotherapy and radiation therapy, two received chemotherapy alone and one patient refused both forms of therapy; this patient expired of progressive tumor. One patient d…

[Sacral chordoma: a rare tumor of the buttocks].

INTRODUCTION: Described and recognized for the first time as a pathological entity by Virchow in 1857, chordoma is a tumour of embryonic origin secondary to an attack of the notochord. In most cases it is asymptomatic, resulting in fairly late diagnosis. PATIENTS AND METHODS: We report the case of a 62-year-old patient presenting a bulky tumefaction, nodular in places, not very painful, and extending towards the anal area, scrotum and the posterior aspect of the upper left thigh. Histopathological examination of a macrobiopsy sample of this tumefaction pointed to chordoma. On magnetic resonan…

Chordomas involving multiple neuraxial bones.

We present a patient with multifocal symptomatic osseous chordomas having unusual growth patterns with review of the pertinent literature. The patient was 62-year-old male and had multiple osseous chordomas located in sacral, thoracic, and paraclival jugular foramen areas. There was no metastasis in other organs. All affected sites were osseous. The multicentric chordomas are extremely rare. This case could be considered as a chordoma involving multiple neuraxial bones. But, the possibility of multicentricity could also be thought. In such cases radical resection should be performed for each l…

Radiosurgery for chordomas and chondrosarcomas of the skull base.

OBJECT: Chordomas and chondrosarcomas of the skull base are aggressive and locally destructive tumors with a high tendency for local progression despite treatment. The authors evaluated the effect of stereotactic radiosurgery (SRS) on local tumor control and survival. METHODS: Twenty-eight patients with histologically confirmed chordomas (18) or chondrosarcomas (10) underwent Gamma Knife SRS either as primary or adjuvant treatment. Their ages ranged from 17 to 72 years (median 44 years). The most common presenting symptom was diplopia (26 patients, 93%). In two patients, SRS was the sole trea…

Update on the cytogenetics and molecular genetics of chordoma.

Chordoma is a rare mesenchymal tumour of complex biology for which only histologic and immunohistochemical criteria have been defined, but no biomarkers predicting the clinical outcome and response to treatment have yet been recognised. We herein review the interdisciplinary information achieved by epidemiologists, neurosurgeons and basic scientists on chordoma, usually a sporadic tumour, which also includes a small fraction of familial cases. Main focus is on the current knowledge of the genetic alterations which might pinpoint candidate genes and molecular mechanisms shared by sporadic and f…

[Chordoma of the ethmoidal sinus. A case report].

Chordomas are dysembryogenic tumors originating from the notochordal process. The case of a 9-month-old male infant with involvement of the ethmoidal sinus and anterior fossa is reported. These tumors are rare in ENT practice and in children arise in the clivus. The diagnosis was made by biopsy, CT and MRI. Surgery is the only effective treatment, with chemotherapy and radiotherapy having little value. The complication and relapse rates are high.

[A case of mediastinal chordoma with Horner's syndrome].

Mediastinal chordoma is a very rare disease. A 32-year-old man was admitted for chest abnormal shadow on the left upper mediastinum. He had left Horner's syndrome on admission. T2 weighted MR image showed heterogeneous tumor shadow. Angiographically, the tumor had feeding arteries. Gallium scintigraphy indicated no abnormal accumulation. Operation was performed and the resected tumor was found to be a chordoma pathologically. He lives well without recurrence and metastasis for 2 years.

Chordoma with osseous element in a F344 rat.

Chordoma occurred in the sacrococcygeal region of the vertebral column of a 97-week old male control F344 rat. Macroscopically, it was a mass (20 mm in diameter) protruding into the retroperitoneum and the cut surface was white to dark-reddish and showed a gelatinous appearance. Histologically, the tumor was composed of physaliphorous and stellate cells growing in nests or cords and divided into lobules by a thin fibrous stroma. The tumor cells locally invaded adjacent tissues, but no metastasis to other organs was noted. Centrally, the tumor often had islands of bone that entrapped physalipho…

Chordoma of the spinal cord in a F344 rat.

A chordoma arose in the lumbar region of the vertebral column from the last two vertebrae of a 112 week old female control F344 rat. Macroscopically it was a mass measuring 32 by 22 mm reaching the pelvic region and pressing against the colon and urinary bladder. Histologically the tumor cells were arranged into lobules and the cells grew in cords. Vacuolated large cells infiltrated the bone and surrounding muscles. This is the first reported case of chordoma in a rodent.

Spontaneous Shrinking and Growing Skull Base Chordoma.

BACKGROUND: Chordomas are rare slow-growing tumors occurring in the axial skeleton and can demonstrate local aggressive behavior, typically extending from the median axis, compressing surrounding tissue. Complete surgical resection and adjuvant radiotherapy are the preferred treatments. We present an unusual case of a spontaneously shrinking and growing off-midline petroclival chordoma. CASE DESCRIPTION: A 23-year-old woman presented with right abducens nerve palsy. Computed tomography and magnetic resonance imaging (MRI) revealed an off-midline petroclival lesion compressing the abducens ner…

Chordoma: Site-Wise Predilections and Pathological Tenets.

BACKGROUND: Chordomas are rare tumours of presumptive notochordal origin, mostly occurring in the axial skeleton of the body. These tumours are slow growing but are locally invasive neoplasms. Diagnosis requires qualification of the radiological, histopathological, and immunohistochemical criteria. Management predominantly comprises surgical resection with negative margins. OBJECTIVE: Our study aimed to analyse the clinico-pathological spectrum of these rare chordoma cases visiting our tertiary care institute. MATERIALS AND METHODS: The study was retrospective, comprising chordoma cases repo…
