To determine whether ciprofloxacin might be efficacious in therapy for chronic bacterial prostatitis, its diffusion into prostatic fluid was studied in ten healthy volunteers. One hour after administering 500 mg ciprofloxacin, the concentration of ciprofloxacin was measured by the agar diffusion method and bioassay. Ciprofloxacin is a fluorinated carboxyquinolone that inhibits Enterobacteriaceae, staphylococci, and Pseudomonas at low concentrations. It has poor activity against Bacteroides fragilis. CASE REPORT: We describe a case of a 74-year-old woman who experienced visual hallucinations after ciprofloxacin administration, when she was also taking theophylline, which resolved on cessation of the ciprofloxacin.
DISCUSSION: Although uncommon, all ophthalmologists should be aware of this potential problem and be familiar with the adverse visual effects which may occur in patients simultaneously administered quinolones and theophylline.

Ciprofloxacin-resistant strains of Neisseria gonorrhoeae have been on the increase over the past few years in Singapore and worldwide. The aim of this study is to correlate treatment failures with in vitro ciprofloxacin resistance of N. gonorrhoeae. Bilateral achilles tendinitis developed in a 40-year-old man following treatment with Ciprofloxacin, 500 mg twice a day, for prostatis. The signs of tendinitis resolved 6 weeks after cessation of the drug. In recent years very few cases of this rare complication have been reported. In some cases it results in a torn achilles tendon. If diagnosed early, complete recovery usually follows about 6 weeks after cessation of the drug.

In general, ciprofloxacin is well tolerated. The most common adverse effects are nausea, vomiting, diarrhea, dizziness, nervousness, and rash. Reversible leukopenia can be associated with the use of oral ciprofloxacin, as shown by our case. Leukopenia should be watched for as the use of this drug increases.

OBJECTIVE: Ciprofloxacin resistance has been reported in 4.5% of patients with otorrhea and is increasing in prevalence. Due to ototoxicity, only fluoroquinolones are Food and Drug Administration approved for topical therapy in the middle ear. Furthermore, there is an assumption that antibiotic resistance is less relevant to topical therapy due to in vivo concentrations much higher than the minimum inhibitory concentration used to determine resistance. This study investigates ciprofloxacin-resistant infections and seeks to develop a better understanding of treatment options and outcomes.
