Single cochlear fibre responses in guinea pigs with long-term endolymphatic hydrops.

Some cochlear fibre response properties have been measured in two GPs approximately one year after induction of endolymphatic hydrops (by surgical obliteration of the endolymphatic sac and duct). These animals are considered as models of the effects of hydrops in Menière's disease, and the purpose of the study was to examine any modifications of fibre response properties which may underly auditory symptoms of the disease in man. Neurones towards more apical cochlear regions (with low characteristic frequencies) showed the greatest deterioration in tuning properties; on average, in the 1-6 kHz

Is perilymphatic pressure altered in tinnitus?

Tinnitus is characterized by the continuous or intermittent auditory perception of various sounds (buzzing, whistling, etc.) in the absence of any external stimulus. Perilymphatic hyperpressure is one of the numerous mechanisms which could hypothetically be involved in tinnitus generation. In the present experiment, perilymphatic pressure was measured indirectly using the tympanic membrane displacement technique. Twenty-five tinnitus patients were investigated at 10, 15 and 20 dB above the acoustic reflex threshold with ipsilateral stimulation. The variables Vi (inward tympanic displacement),

Cochleostomy or endolymphatic sac surgery for advanced Meniere's disease.

The operation of cochleostomy is described. The surgical technique is vital for the true assessment of results. If the cochlear duct is punctured, the risk to hearing is unacceptably high. If only the osseous spiral lamina is punctured, the results are acceptable in the short term, though the rationale cannot be explained. In the management of difficult patients with more advanced Meniere's disease, some variant of cochleostomy should probably be included in the therapeutic options. The results of endolymphatic sac surgery are significantly better in a matched group of patients.

Daple deficiency causes hearing loss in adult mice by inducing defects in cochlear stereocilia and apical microtubules.

The V-shaped arrangement of hair bundles on cochlear hair cells is critical for auditory sensing. However, regulation of hair bundle arrangements has not been fully understood. Recently, defects in hair bundle arrangement were reported in postnatal Dishevelled-associating protein (ccdc88c, alias Daple)-deficient mice. In the present study, we found that adult Daple-/- mice exhibited hearing disturbances over a broad frequency range through auditory brainstem response testing. Consistently, distorted patterns of hair bundles were detected in almost all regions, more typically in the basal regio

[High resolution MR imaging of the inner ear apparatus using 3D-Fast spin echo sequence].

To evaluate the efficacy of high resolution MR imaging of the inner ear with 3D-Fast spin echo sequence, one volunteer and 12 patients were imaged with a 1.5 T MR scanner. High resolution T2-weighted images were obtained with a head coil in 13 minutes, and the voxel size was 0.39mm x 0.45mm x 1.2mm in most subjects. Original images and maximum intensity projection (MIP) images of the inner ear were evaluated. On original images, the endolymphatic duct was visualized in 72% of subjects, the cochlear aqueduct in 96%, three branches of the cranial nerves in the internal auditory canal in 100%, an

Electrophysiological measures of cochlear function in guinea pigs with long-term endolymphatic hydrops.

Experimental endolymphatic hydrops was induced in guinea pigs by obliteration of the endolymphatic sac and duct. From 3 to 24 months after this operation, cochlear action potential (AP) audiograms and AP tuning curves were measured. The purpose of this study was to establish parallels, if any, between this supposed animal model of Menière's disease and the auditory symptoms of the disease in man. In some animals, low and middle frequency AP threshold elevations were observed whilst higher frequency regions maintained normal sensitivity. Other animals developed flat or very gradually sloping AP

LDLR expression in the cochlea suggests a role in endolymph homeostasis and cochlear amplification.

There is now growing evidence that hypercholesterolemia and high serum levels of low-density lipoproteins (LDL) predispose to sensorineural hearing loss. Circulating LDL-cholesterol is delivered to peripheral tissues via LDL receptor (LDLR) -mediated endocytosis. Recently, it has been shown that LDLR gene polymorphisms are associated with higher susceptibility to sudden deafness. These findings suggested that we should investigate the expression of LDLR from the postnatal maturation of the mouse cochlea until adulthood. In the cochlea of newborn mice, we observed that LDLR is mostly expressed

Functional and morphological changes in experimental endolymphatic hydrops.

Electrocochleograms (ECochG) in response to tone bursts and electronystagmograms (ENG) during sinusoidal rotational stimulation were monitored in guinea pigs equipped with chronically implanted round-window and periocular electrodes before and for as long as 90 days after uni- or bilateral surgical obliteration of the endolymphatic duct. The presence of cochlear and saccular endolymphatic hydrops was verified during microdissection of the inner ear, and cochlear hair cell counts were performed in some of the animals. The results indicate that in surgically-induced endolymphatic hydrops, cochle

[Waardenburg Syndrome: a review of literature and case reports].

Waadernburg syndrome (WS) is an autosomal dominant disease clinically and genetically heterogeneous. It is characterized by non-progressive sensorineuronal hearing loss of variable degree and anomalous pigmentation (of the eyes, skin and stria vascularis of cochlear duct) caused by melanocytes absence. The syndrome is caused by mutations in genes that regulate the melanocytes differentiation from the neural crest during embriogenetic development. This study will review the literature on WS and explain more in detail the ocular findings in WS, while reporting new findings on two newly diagnosed

Water permeability of the mammalian cochlea: functional features of an aquaporin-facilitated water shunt at the perilymph-endolymph barrier.

The cochlear duct epithelium (CDE) constitutes a tight barrier that effectively separates the inner ear fluids, endolymph and perilymph, thereby maintaining distinct ionic and osmotic gradients that are essential for auditory function. However, in vivo experiments have demonstrated that the CDE allows for rapid water exchange between fluid compartments. The molecular mechanism governing water permeation across the CDE remains elusive. We computationally determined the diffusional (PD) and osmotic (Pf) water permeability coefficients for the mammalian CDE based on in silico simulations of cochl

The timecourse of apoptotic cell death during postnatal remodeling of the mouse cochlea and its premature onset by triiodothyronine (T3).

Apoptosis underlies various forms of tissue remodeling during development. Prior to the onset of hearing, thyroid hormone (T3) promotes cochlear remodeling, which involves regression of the greater epithelial ridge (GER), a transient structure of columnar cells adjacent to the mechanosensory hair cells. We investigated the timecourse of apoptosis in the GER and the influence of ectopic T3 on apoptosis. In saline-treated mice, activated caspase 3-positive cells were detected in the GER between postnatal days 7 and 13 and appeared progressively along the cochlear duct from base to apex over deve

Dynamics of noise-induced cellular injury and repair in the mouse cochlea.

To assess the dynamics of noise-induced tissue injury and repair, groups of CBA/CaJ mice were exposed to an octave-band noise for 2 hours at levels of 94, 100, 106, 112, or 116 dB SPL and evaluated at survival times of 0, 12, 24 hours or 1, 2, or 8 weeks. Functional change, assessed via auditory brainstem response (ABR), ranged from a reversible threshold shift (at 94 dB) to a profound permanent loss (at 116 dB). Light microscopic histopathology was assessed in serial thick plastic sections and involved quantitative evaluation of most major cell types within the cochlear duct, including hair c

Pathogenesis of experimental endolymphatic hydrops.

The protein content and the d.c. potential of the endolymph differs in the various parts of the endolymphatic space (cochlea, utricle, semicircular canals and endolymphatic sac) as also does the ion composition (chloride, potassium, sodium). 12 months after obliteration of the endolymphatic sac and duct in guinea pigs the d.c. potential falls, whereas, the sodium activity increases. The endolymphatic hydrops is not caused by an increased colloid osmotic pressure. The increased water-binding capacity of the cochlear endolymph is correlated with the increased Na+ activity.

Cochlear fluid balance. A clinical/research overview.

Stria edema, and in some cases atrophy, follows osmotic agents, loop-inhibiting diuretics, acoustic trauma, and rupture of Reissner's membrane. All have in common an imbalance of fluid and electrolytes in the cochlear duct. The glycerol test causes temporary improvement in hearing in Menière's disease. Glycerol causes stria edema and collapse of Reissner's membrane in the chinchilla. Stria edema, as well as stria atrophy, are found in Menière's disease. Metabolic manipulation of the stria might be that the best approach in the search for successful treatment of Menière's disease.

Human cochlear changes in noise induced hearing loss.

The temporal bone histopathological findings in 14 ears with noise induced hearing loss are presented. The morphological changes consist mainly of hair cell loss, which is more severe in the 9 mm to 13 mm region of the cochlear duct. Within the area of maximum hair cell loss, there is a greater loss of outer hair cells than of inner hair cells. There is a good correlation between the permanent auditory threshold shifts and the spatial location of the sensory lesion according to the anatomical frequency scale.

3D-reconstructions of Bast's Valve and Membranous Labyrinth: Insights for Vestibular Implantation and Meniere's Disease.

HYPOTHESIS/BACKGROUND: Bast's valve is a poorly understood inner ear structure located at the junction between pars superior and inferior in the membranous labyrinth. Anatomically precise three-dimensional reconstructions (3D-reconstructions) of Bast's valve can help illuminate the morphology of the valve, and point toward its role in normal physiology and pathological states such as endolymphatic hydrops. This is of particular relevance to the development of a vestibular implant, a device intended to rehabilitate deficits in the vestibular system. METHODS: Six postmortem human temporal bones

Pathology of prelingual profound deafness: magnitude of labyrinthitis fibro-ossificans.

Quantitative histologic studies were performed on 15 temporal bones from eight adult persons who were known to have prelingual bilateral profound hearing loss. The pathologic changes are characterized by severe degeneration of the structures of the cochlear duct, often with degeneration of the vestibular sense organs, causing a reparative host response that features osteoneogenesis and fibrous proliferation followed by retrograde neuronal degeneration. The pathology is consistent with meningogenic bacterial or viral labyrinthitis that occurred subclinically or went undiagnosed. Bone and fibrou
