Daily Oscillation of the Excitation-Inhibition Balance in Visual Cortical Circuits.

A balance between synaptic excitation and inhibition (E/I balance) maintained within a narrow window is widely regarded to be crucial for cortical processing. In line with this idea, the E/I balance is reportedly comparable across neighboring neurons, behavioral states, and developmental stages and altered in many neurological disorders. Motivated by these ideas, we examined whether synaptic inhibition changes over the 24-h day to compensate for the well-documented sleep-dependent changes in synaptic excitation. We found that, in pyramidal cells of visual and prefrontal cortices and hippocampa

Blepharospasm and the modulation of cortical excitability in primary and secondary motor areas.

BACKGROUND: Traditionally, benign essential blepharospasm (BEB) is considered a disorder caused by basal ganglia dysfunction. Electrophysiologic and brain imaging studies suggest pathologic changes in excitability in the primary motor cortex (MC), anterior cingulate (AC), and secondary motor areas, such as premotor (PMC) and supplementary motor cortices (SMA). METHODS: In this pilot study of 7 patients with BEB, we experimentally reduced cortical excitability of 4 areas: MC (first dorsal interosseus area), PMC, SMA, and AC, each with 3 noninvasive techniques: low-frequency repetitive transcra

The effect of benzodiazepines and flumazenil on motor cortical excitability in the human brain.

In the present study, the effects of benzodiazepines (diazepam) were evaluated in terms of cortical excitability changes, as tested with transcranial magnetic simulation (TMS). In particular, analyzed were drug-induced changes regarding two selected parameters of TMS: (1) the cortical excitability threshold and (2) the silent period duration (SP). For this purpose, we evaluated the effects of long-term therapy with diazepam in the patients affected by anxiety disorders and the changes induced by single oral doses of diazepam in both healthy controls and patients. In addition, we tested cortica

Acetylcholine facilitation, atropine block of synaptic excitation of cortical neurons.

Sinigle neurons in cat visual cortex were driven by afferent stimuli to determine the effects of drugs liberated from multibarreled micropipettes on synaptic transmission. Acetylcholine increased, and atropine decreased, the number of neuron discharges fired in response to stimulation of the mesencephalic reticular formation without necessarily affecting responses to other stimuli or affecting spontaneous firing.

Nitric oxide and glutamate interaction in the control of cortical and hippocampal excitability.

PURPOSE: We investigated the role of nitric oxide (NO) as a new neurotransmitter in the control of excitability of the hippocampus and the cerebral cortex, as well as the possible functional interaction between NO and the glutamate systems. METHODS: The experiments were performed on anesthetized rats. The bioelectrical activities of the somatosensory cortex and the CA1 region of the hippocampus of these rats were recorded. Pharmacologic inhibition of NO synthase (NOS) through the nonselective and brain-selective inhibitors, N-nitro-L-arginine methyl ester (L-NAME) and 7-nitroindazole (7-NI),

[Analysis of cortical excitation and inhibition using transcranial magnetic stimulation].

The method of magnetic transcranial stimulation of the motor cortex produces both excitatory and inhibitory effect. Definition of excitability and inhibitory threshold represents clinically useful indicator. In order to reduce variability of the thresholds, it is important to notice mental activity, posture, type of magnetic stimulator, involuntary movement, muscle tonus and age. Excitability threshold was elevated in patients with stroke and reduced in patients with epilepsy. Enhancement of motor excitability by subthreshold magnetic stimulation of the motor cortex prior to voluntary movement

Motor inhibition and excitation are independent effects of magnetic cortical stimulation.

We administered magnetic cortical stimulation (MCS) during voluntary contraction of intrinsic hand muscles to 8 patients with motor neuron disease (MND), 5 patients with pure lower motor neuron syndromes (LMN), a patient with severe subacute sensory neuropathy (SSN), and 10 healthy volunteers. Patients with MND had clinical evidence of upper MND and elevated thresholds for (3 patients) or absence of (5 patients) motor evoked potentials (MEPs). MCS during sustained contraction inhibited electromyographic activity in 6 of 8 patients with MND, without preceding MEPs. MCS had no effect on the elec

Spontaneous activation of cortical somatosensory networks depresses their excitability in preterm human neonates.

The cortical activity of preterm human infants is highly discontinuous, comprising transient high-amplitude bursts separated by periods of relative quiescence. While the functional significance of these bursts is well established, the underlying mechanism remains unclear. This burst-quiescence pattern could arise from a transient refractoriness within excitatory recurrent cortical networks following spontaneous activation. To assess this possibility, we tested whether such activation is followed by a transient reduction in excitability by evaluating whether externally evoked tactile responses

Prefrontal transcranial magnetic stimulation changes cortical excitability across local and distributed brain regions.

OBJECTIVE: This study sort to identify the continuous effects of repetitive transcranial magnetic stimulation (rTMS) over the dorsolateral prefrontal cortex (DLPFC-rTMS) on local and distributed brain activities. METHODS: In a double-blinded and sham-controlled design, 24 participants received either a real or sham DLPFC-rTMS session, with concurrent TMS-EEG being recorded at baseline, 5 and 60 min after stimulation. RESULTS: DLPFC stimulation induced immediate cortical excitability in the frontal regions, while a continuous effect was identified in the parietal cortex for up to 60 min. More

Effects of Transcutaneous Auricular Vagus Nerve Stimulation on Cortical Excitability in Healthy Adults.

OBJECTIVE: Vagus nerve stimulation (VNS) has recently been reported to exert additional benefits for functional recovery in patients with brain injury. However, the mechanisms underlying these effects have not yet been elucidated. This study examined the effects of transcutaneous auricular VNS (taVNS) on cortical excitability in healthy adults. MATERIALS AND METHODS: We recorded subthreshold and suprathreshold single- and paired-pulse motor-evoked potentials (MEPs) in the right-hand muscles of 16 healthy adults by stimulating the left primary motor cortex. Interstimulus intervals were set at

Altered Visual Cortical Excitability Is Associated With Psychopathological Symptoms in Major Depressive Disorder.

Previous studies suggest that in people with major depressive disorder (MDD), there exists a perturbation of the normal balance between the excitatory and inhibitory neurotransmitter systems in the visual cortex, indicating the possibility of altered visual cortical excitability. However, investigations into the neural activities of the visual cortex in MDD patients yielded inconsistent findings. The present study aimed to evaluate the visual cortical excitability utilizing a paired-pulse stimulation paradigm in patients with MDD and to access the paired-pulse behavior of recording visual evok

The Influence of Cortico-Cerebellar Structural Connectivity on Cortical Excitability in Chronic Stroke.

Brain imaging has recently evidenced that the structural state of distinct reciprocal cortico-cerebellar fiber tracts, the dentato-thalamo-cortical tract (DTCT), and the cortico-ponto-cerebellar tract (CPCeT), significantly influences residual motor output in chronic stroke patients, independent from the level of damage to the corticospinal tract (CST). Whether such structural information might also directly relate to measures of cortical excitability is an open question. Eighteen chronic stroke patients with supratentorial ischemic lesions and 17 healthy controls underwent transcranial magnet

Cortical Somatosensory Excitability Is Modulated in Response to Several Days of Muscle Soreness.

Changes in excitability of the sensorimotor cortex have been demonstrated in clinical musculoskeletal pain, although the timing is unknown. Eccentric exercise provokes delayed-onset muscle soreness providing a model to study the temporal profile of sensorimotor cortical plasticity during progressively developing muscle soreness. Twelve healthy participants performed eccentric exercise of the wrist extensors. Likert pain scores, pressure pain thresholds at the extensor carpi radialis (ECR) muscle, somatosensory evoked potentials from electrical stimulation of the radial nerve, maximal wrist ext

Cortical excitability in very mild Alzheimer's disease: a long-term follow-up study.

Measurement of motor cortex excitability using paired-pulse transcranial magnetic stimulation (pTMS) has been proposed for the early diagnosis of Alzheimer's disease (AD) and could also be useful for monitoring treatment response and disease progression. However, studies conducted at the pre-dementia stage of AD are scarce, very few long-term data are available, and correlations between cortical excitability and cognitive performance have not been addressed. Eleven patients with mild cognitive impairment (MCI) that converted to AD-related dementia and 12 elderly control subjects were selected

Change of excitability in brainstem and cortical visual processing in migraine exhibiting allodynia.

BACKGROUND: Clinical and neurophysiological manifestations of information processing associated with central sensitization are little known. Allodynic migraine (AM) can be caused by the sensitization of trigeminal neuron, but no study has reported on AM between attacks using blink reflex (BR) and pattern-reversal visual evoked potentials (PVEPs). OBJECTIVE: We explored the characteristics of AM between attacks associated with central sensitization using BR and PVEP. METHODS: We recruited 13 patients with interictal AM and 15 patients with nonallodynic migraine (NA), and 30 healthy subjects (

Cortical excitability in cocaine-dependent patients: a replication and extension of TMS findings.

Cortical excitability can be assessed by transcranial magnetic stimulation (TMS). Previously we observed that TMS motor threshold (MT) was elevated in abstinent cocaine-dependent subjects. In the current study we aimed at replicating our initial finding, exploring other TMS-based measures of excitability, and examining association with personality characteristics. Nineteen cocaine-dependent and 12 healthy control subjects were examined. Resting and activated motor thresholds (RMT and AMT) and duration of the cortical silent period (CSP) were examined. The Cocaine Experience Questionnaire (CEQ)

High-definition transcranial random noise stimulation enhances fluid intelligence with increasing cortical excitability.

Objective.Fluid intelligence is a core component of higher-order cognition, yet whether high-definition high-frequency transcranial random noise stimulation (HD/HF-tRNS) can enhance demanding reasoning performance and modulate its neural correlates remains unclear.Approach.We investigated the after-effects of offline HD/HF-tRNS targeting the right dorsolateral prefrontal cortex (DLPFC). In a double-blind, sham-controlled, between-groups design, 26 healthy adults completed Raven's progressive matrices (RPM) before and after receiving active or sham stimulation. A taVNS-TMS Study on Cortical Excitability for Post-Stroke Upper Limb Rehabilitation.

Transcutaneous auricular vagus nerve stimulation (taVNS) has been shown to significantly enhance motor function in stroke rehabilitation. This study investigates the synergistic effects of combined taVNS and transcranial magnetic stimulation (TMS) on corticospinal excitability. A controlled experiment was conducted with five healthy individuals and ten stroke patients. Under optimized stimulation parameters, participants underwent TMS followed by five minutes of continuous taVNS. Resting-state electroencephalography(EEG) was recorded for three minutes before and after stimulation, while motor-

Impaired AIS plasticity in ankyrin-G mutant mice alters cortical excitability and behavior.

The developing brain undergoes neuroplasticity driven by learning, experience, and memory formation. The axon initial segment (AIS) is a specialized membrane domain within the proximal axon that initiates action potential. Studies have demonstrated that the AIS exhibits plasticity by altering its length and/or localization to adjust the excitability in response to neural stimuli. However, how AIS plasticity may affect brain function is unclear. The 480-kDa giant ankyrin-G protein (gAnkG) is the master organizer of AISs and nodes of Ranvier. Previously, we reported that a neurodevelopmental dis

Cortical Excitability as a Prognostic and Phenotypic Stratification Biomarker in Amyotrophic Lateral Sclerosis.

OBJECTIVE: Despite its clinical heterogeneity, amyotrophic lateral sclerosis is unified by early and prominent alterations in cortical excitability, increasingly recognized as contributors to disease progression. This study assessed whether the ratio between motor evoked potential (MEP) amplitude, reflecting upper motor neuron integrity, and compound muscle action potential (CMAP) amplitude, indexing lower motor neuron function, could provide an accessible marker of corticospinal excitability to stratify patients by phenotype, stage, and survival. METHODS: In this multicenter retrospective st
