Sonography of the costal cartilage: normal anatomy and preliminary clinical application.

Costal cartilages of 10 controls and 12 patients with Tietze's syndrome (n = 6), deformed (exaggerated bowing) cartilage (n = 4), focal enlargement (n = 1), and cancer-invaded cartilage (n = 1) were examined with ultrasonography. In controls, ultrasonography detected 237 of 240 costal cartilages (98.8%) and 58 of 105 intercartilage connections (55%) with three variations. Correlation with CT was available in 7 patients (1 Tietze's syndrome, 4 deformities, 1 cartilage enlargement, and 1 cancer) among 8 patients whose CT results were known to the sonographer. Sonography showed normal (n = 4) or

Pseudomonas infection of the sternum and costal cartilages. Report of three cases.

Pseudomonas osteochondritis of the chest wall and sternum has rarely been reported, but when present it has been extremely difficult to eradicate. Multiple operations to debride the involved areas and use of antibiotics have often been inadequate to control this resistant infection. This report describes our experience with three patients who had the late development of Pseudomonas osteochondritis of the chest wall. The infection occurred following crush trauma, sternotomy for congenital heart disease, and mastectomy and amputation of the upper extremity for carcinoma of the breast. Antibiotic

Costal cartilage fracture: A commonly missed thoracic injury in trauma patients.

In this report, we describe a case of a 43-year-old male who sustained a displaced mid chondral costal cartilage fracture along with multiple ipsilateral rib fractures following a jet ski collision. Costal cartilage fractures (CC) are commonly missed on imaging evaluation, and accordingly underreported in the literature. High-energy blunt chest trauma represents the most common mechanism for CC injuries. Computed Tomography (CT) is the modality of choice to diagnose CC fractures along with the associated cardiopulmonary and abdominal injuries in polytrauma patients. There is currently no conse

Against the Overgrowth Hypothesis: Shorter Costal Cartilage Lengths in Pectus Excavatum.

BACKGROUND: Pectus excavatum is a common chest wall deformity with no known cause. A common hypothesis is that in patients with pectus excavatum, there is an overgrowth of costal cartilage relative to healthy individuals. MATERIALS AND METHODS: We obtained radiological curvilinear three-dimensional measurements of the fourth to eighth costal cartilage and associated ribs in 16 patients with pectus excavatum and 16 age- and gender-matched controls between the ages of 6 and 32 y. An analysis of variance was used to compare bone length, cartilage length, and their ratios between patients and con

Reconstruction of saddle nose deformity with three-dimensional costal cartilage graft.

Saddle nose deformity is characterized by depression of the nasal bone and the cartilage dorsum associated with a collapse in the upper lateral and alar cartilages. Etiopathogenesis usually involves trauma or invasive excision of the bone and cartilage. Surgical treatment for functional and aesthetic recovery relies on the use of grafts. Options for grafting include autogenous tissue such as bone or cartilage and alloplastic augmentation. Nine patients with saddle nose deformity underwent surgical reconstruction with autogenous costal cartilage. The deformity was the result of trauma in seven

Biochemical, Mechanical, and Warping Behavior of Fresh Frozen Costal Cartilage Allograft.

BACKGROUND: Fresh frozen costal cartilage allografts are used in rhinoplasty, but their biomechanical and biochemical properties remain under-characterized. OBJECTIVE: To determine whether cut orientation, harvest location, or irradiation status influences the biochemical, mechanical, and warping properties of a commercial fresh frozen costal cartilage allograft. METHODS: Biochemical assays, three-point bending, and warping assessments were performed. Mixed-effects and linear regression models assessed associations with orientation, harvest location, and irradiation status. Intraclass correl

Costal cartilage graft harvesting for auricular reconstruction: donor-site morbidity assessment.

Auricular reconstruction is a challenging procedure, and the use of autologous costal cartilage represents the gold standard for the treatment of congenital microtia. Since adolescents are a significant demographic for congenital reconstruction, age-related psychosocial factors must be considered. The aim of this study was to evaluate postoperative patient perception of donor-site morbidity following costal cartilage graft harvesting and its impact on social and physical activities. We included patients treated at the University Hospital of Parma (Italy) between 2010 and 2020. Data collected i

Correction of Severe Short Nose Using a Costal Cartilage Extension Framework.

BACKGROUND: Various techniques for the aesthetic correction of short noses have been described, but the selection of the adequate graft material remains controversial. Previous reports have mainly focused on the application of septal cartilage or alloplastic materials for short nose elongation, but the lengthening effect is often unsatisfactory for severe short noses. We propose costal cartilage as an alternative treatment for short noses, describe the technique, and discuss outcomes, patient selection, and complications based on our 15-year experience. METHODS: From February 2004 to December

Anthropometric Analysis of Secondary Cleft Lip Rhinoplasty Using Costal Cartilage Graft.

Despite the improvement of primary repair of nasal deformities during the management of cleft lip in infancy, this does not exclude the need for revision rhinoplasty in adulthood for complete patients' rehabilitation. The purpose of this study was to evaluate the aesthetic outcome of secondary rhinoplasty using costal cartilage grafts in patients with unilateral cleft lip nasal deformity. Twenty patients who were operated at earlier ages for correction of cleft lip and had a residual unilateral cleft lip nasal deformity were included in this study. Costal cartilage rib grafts were harvested; c

Decorin expression, straw-like structure, and differentiation of human costal cartilage.

Costal cartilage is much understudied compared with the load-bearing cartilages. Abnormally grown costal cartilages are associated with the inherited chest wall deformities pectus excavatum and pectus carinatum resulting in sunken and pigeon chests, respectively. A lack of understanding of the ultrastructural and molecular biology of costal cartilage is a major confounder in predicting causes and outcomes of these disorders. This study analyzed the structure of marginal human costal cartilage (ribs 6-10) through scanning electron and atomic force microscopes and identified the presence of stra

Atomic force microscopy characterization of collagen 'nanostraws' in human costal cartilage.

Costal cartilage, a type of hyaline cartilage that bridges the bony ribs and sternum, is relatively understudied compared to the load bearing cartilages. Deformities of costal cartilage can result in deformation of the chest wall, where the sternum is largely pushed toward or away from the spine, pectus excavatum and pectus carinatum, respectively, with each condition having significant clinical impact. In the absence of extensive literature describing morphological features of costal cartilage, we characterized a sample from the costal margin immunohistologically and through atomic force micr

Chest wall deformities and thoracic scoliosis after costal cartilage graft harvesting.

Donor-site complications, specifically chest wall deformities and thoracic scoliosis, occurring after harvest of costal cartilage grafts are presented and discussed. The cases of 18 patients (12 male and 6 female), who underwent costal cartilage grafts for microtia reconstruction from 1975 to 1993, were reviewed for donor-site complications using radiography and physical examination. Ribs from which costal cartilage had been harvested showed increased inward bowing on radiographs in 16 of 32 donor sites. The frequency of rib deformity in donor sites was 20.0 percent when cartilages were harves
