International review of psychiatry (Abingdon, England)2026eng
Mental health policies are increasingly informed by international guidelines and evidence. However, translating these frameworks into sustained system-level change often proves difficult. This paper presents a policy learning case study examining how large-scale, deinstitutionalization-focused mental health reform was initiated and subsequently evolved in Czechia. Building on prior descriptive analyses of ESIF-supported reform, and drawing on administrative data, project documentation, evaluation reports, and qualitative feedback from 23 people with lived experience (PWLE), we analyse mental h

Nursing homes have played a major role in deinstitutionalization, and their increased use for the mentally ill has been questioned. We performed a controlled study of nursing homes as an alternative to continued psychiatric hospitalization. Men (N = 403) referred for nursing home placement from eight Veterans Administration medical centers were randomly assigned to community nursing homes (CNHs), Veterans Administration nursing care units, continued care on the same ward, or transfer to another psychiatric ward. Patients met defined criteria for schizophrenia or organic brain disease. Data wer

This paper attends to the sociality available in the clozapine clinic regimen and suggests that the social dimensions of clozapine treatment may be as important as the biochemical efficacy of clozapine. The clozapine clinic is where people diagnosed with chronic schizophrenia who take the antipsychotic clozapine go for routine monitoring of clozapine side effects, particularly haematological effects. Psychopharmaceutical treatments are often criticized for being reductionistic and dehumanizing, but clozapine clinics offer increased clinical contact in the age of deinstitutionalization. The ina

Reforms of psychiatric care systems in Western countries have been guided by several principles such as de-institutionalization, mental healthcare close to the community, and integration into general healthcare. This paper reflects the achievements, shortfalls, and problems of selected aspects of mental health reforms. Despite major improvements, there have also been some setbacks. We consider such analyses to be essential for future care planning.

The concept of Mental Homelessness is presented and developed. This paper will provide a historical review of the connection between mental illness and housing and the changing approaches toward institutionalization and de-institutionalization over several centuries. Case illustrations from practice in Israel will be presented to highlight the theme of home, or rather the theme of lacking a home as an element which may be inherent to a mental illness. More specifically, the paper argues that homelessness is a state of flection of. If so, even if a mental patient does initially own a home, he o

AIM: This paper analyses, in what way psychiatrists considered housing and work as criteria of social integration of mentally ill people and what models of care were suggested in Germany throughout the 20th century.
METHOD: Publications in 29 German professional and scientific psychiatric journals through the complete period from 1900 to 2000 and monographs were searched for papers on the above issues.
RESULTS: Until the second half of the century, integrative initiatives related to housing and work generated in asylums without the aim of a full social integration of the patients. In the act

Over the past decade self-help groups have become an important way of helping people cope with various life crises. Groups have organized to help individual members deal with a wide range of health-related and other problems. The authors define the meaning of self-help in such groups and describe the range of groups now available, including a number of mental-health-related groups. The part self-help groups play in providing social support, preventing illness and death, and reducing the need for hospitalization is discussed. The authors also examine the role of professionals in initiating and

Advances in the medical treatment of haemophilia have made it possible for the majority of haemophilic children to attend ordinary rather than special schools. A survey carried out in the Glasgow and Edinburgh regions among families with haemophilic children and the schools which these children attend has drawn attention to certain implications of this new policy.

Associations of families of the mentally ill arose in Italy in the early 1980s mainly in the country's northern and central regions. They denounced the inadequacies of an extramural service conceived of as an out-patient clinic filling the void left by the shutting down of asylums. The positions elaborated by these associations focus on four main themes: the distribution of the "burden of care" for the ill between services and families; the ill-conceived interpretation of the families' needs owing to the professional's alleged tendency to "point the finger of guilt" at the families themselves;

OBJECTIVES: Members of Ypsilon, a Dutch family organization for relatives of patients with schizophrenia or chronic psychosis, were surveyed to determine whether patients whose families were involved in the organization were representative of all patients with schizophrenia in the Netherlands and whether Ypsilon was similar to family organizations in other countries, to learn about members' experience of the course of their relatives' illness, to determine the kinds of mental health care received by the patients, and to discern members' opinions of that care.

A component of the 1965 Medicaid Act, the Institutions for Mental Diseases ("IMD") Exclusion was supposed to be a remedy for the brutal, dysfunctional mental healthcare system run through state hospitals. In the years since Medicaid was created, the IMD Exclusion has instead barred thousands of those in need of intensive, inpatient treatment from receiving it. As a result, many severely mentally ill individuals are left without adequate care and without a home. They struggle in the street where they are otherized by those in their community and are susceptible to confrontational episodes with

INTRODUCTION: In dentistry, managing anxious children is a daily challenge. Anxiety, affecting 10-20% of children, poses a barrier to care. This issue is exacerbated by deinstitutionalization and the law for equal rights, exposing orthodontists to a new patient group combining disability-related traits and heightened anxiety. Faced with anxious children, orthodontists have various management strategies. However, conventional behavioral techniques (CBT) have shown limits in children with neurodevelopmental disorders (NDD), contributing to disparities in access to care. Practitioners' reluctance

In January 2023, the fiftieth anniversary passes of David Rosenhan's article On being sane in insane places appearing in the prestigious journal "Science". This publication has become one of the most influential psychiatric papers of the second half of the 20th century, achieving 1,276 citations up to mid-2022. In the article, eight healthy persons are described, who came to psychiatric hospitals in the USA, reporting auditory hallucinations. They were all admitted, mainly with suspected schizophrenia, and ordered pharmacological treatment. Their stay ranged from 7-52 (mean 19) days, even thou

OBJECTIVE: Japan has the largest number of psychiatric beds in the world and has been in the process of deinstitutionalization since 2004. The majority of psychiatric inpatients are elderly long-term patients, who are at risk of homelessness after they are discharged. There is little information about homeless people with mental illnesses in Japan, and the aim of this study was to describe characteristics of people with a mental illness in homeless shelters in Tokyo.
METHODS: A face-to-face survey was conducted from December 2012 to March 2013 by the staff of a nonprofit organization (NPO) th

Changes in psychiatric health care delivery driven by such major shifts as deinstitutionalization, community-based care, and managed care have greatly altered the educational milieu for third-year psychiatry clerkships. Students may be assigned exclusively to alcohol and substance abuse treatment units, consultation-liaison services, or outpatient clinics, and may not have as broad an exposure as is desirable to patients with a variety of psychiatric illnesses. The authors describe a pilot course they developed in 2001, Clinical Psychiatric Assessment and Diagnosis, for third-year medical stud

How things are perceived from a distance may help better understand their nature. Perceptions at home are likely to shape perceptions abroad. The mutual cross-references between local and distant perspectives on the Italian Mental Health Law 180 may help understand the process which preceded and resulted in the reform. This editorial argues that Law 180 came about at a unique - enabling - time in history. It argues that the run-up to and passing of Law 180 constituted a great accomplishment by professionals, the wider public and politicians/administrators. This editorial goes on to argue that

This small, mixed-methods study sought to understand the use of special housing units among formerly incarcerated men. In the present study, 110 participants were placed in solitary confinement, ranging from ≤30 days to 20 years, with a mean of 2 years (SD = 2.55). These men ranged in age from 35 to 67 years, with a mean age of 46 years. Years of incarceration ranged from less than 30 days to 34 years; the mean number of years incarcerated was 4 (SD = 6.39). Of the 110 participants, qualitative interviews were conducted with 30 men to explore their experiences during time spent in solitary. Th
