Treatment of PTSD in people with severe intellectual disabilities: a case series.

OBJECTIVE: There is a dearth of information regarding the treatment of PTSD in people with severe intellectual disabilities (ID). The purpose of the present case studies was to assess the applicability and effects of an evidence-based treatment method for psychological trauma with this population. METHODS: The treatment of four single cases with Eye Movement Desensitization and Reprocessing (EMDR) was evaluated. Participants included adults and children with a variety of symptoms, as well as different histories of negative life events. RESULTS: In all cases PTSD symptoms decreased. In all bu

Attention fixation training: training people to form cognitive maps help to control symptoms of panic disorder with agoraphobia.

Nine individuals diagnosed with panic with agoraphobia received three elements of Attentional Fixation Training (AFT): Directed attention to the external environment, directed topographical synthesis, and directed orientation in space-time to control characteristics of panic. They then walked a standard 2.5 km route and practiced these elements upon entering one of the five panic-inducing situations: (a) walking alone near a busy street with the examiner following at 20 m, (b) walking alone near a busy street with the examiner out of client's visual field, (c) shopping with the examiner presen

Agoraphobics' understanding of the development and maintenance of their symptoms.

A 24-item test was devised to study the degree of understanding a group of agoraphobics had of the etiology and behavioral treatment of their symptoms before and after treatment, whether understanding was related to mode of symptom acquisition and, finally, if increase in understanding was related to treatment outcome. Clients were found to be ill-informed in areas most directly related to realistic exposure strategies, but there was no difference between those who had conditioning as opposed to cognitive acquisition histories. Changes in understanding were found to be related to outcome measu

The current status of behavioral psychotherapy: theory and practice.

Behavioral psychotherapy has become one of the definitive treatments available for the relief of psychiatric suffering. Although hardly a panacea for all ills, the behavioral approach is the treatment of choice for certain selected problems. Behavioral methods make their contribution in the context of general psychiatric management and often must be used in conjunction with other treatment. The theoretical and practical aspects of behavioral psychotherapy have changed a great deal over the past few years and continue to evolve rapidly. Although useful molecular theories are emerging to guide t

Improvement of mood and sleep alterations in posttraumatic stress disorder patients by eye movement desensitization and reprocessing.

Posttraumatic stress disorder (PTSD) patients exhibit depressive and anxiety symptoms, in addition to nightmares, which interfere with sleep continuity. Pharmacologic treatment of these sleep problems improves PTSD symptoms, but very few studies have used psychotherapeutic interventions to treat PTSD and examined their effects on sleep quality. Therefore, in the present study, we sought to investigate the effects of Eye Movement Desensitization Reprocessing therapy on indices of mood, anxiety, subjective, and objective sleep. The sample was composed of 11 healthy controls and 13 PTSD patients

Treatment of traumatic birth experience with postpartum early eye movement desensitization and reprocessing therapy: a randomized clinical trial.

BACKGROUND: A traumatic birth experience can have negative consequences for both mother and infant, including developing (symptoms of) posttraumatic stress disorder, postpartum depression, difficulties in mother-infant bonding, fear of childbirth, or reduced quality of life. OBJECTIVE: This study aimed to investigate the effectiveness and safety of early trauma-focused therapy, specifically eye movement desensitization and reprocessing therapy, in reducing symptoms and incidence of posttraumatic stress disorder at 9 weeks postpartum in women with a traumatic birth experience, compared with ca

Post-traumatic stress disorder: evolving conceptualization and evidence, and future research directions.

The understanding of responses to traumatic events has been greatly influenced by the introduction of the diagnosis of post-traumatic stress disorder (PTSD). In this paper we review the initial versions of the diagnostic criteria for this condition and the associated epidemiological findings, including sociocultural differences. We consider evidence for post-traumatic reactions occurring in multiple contexts not previously defined as traumatic, and the implications that these observations have for the diagnosis. More recent developments such as the DSM-5 dissociative subtype and the ICD-11 dia

Perceptual discrepancies in the experience and reporting of violence against children are more pronounced among social workers compared to laypeople.

When violence against a child is reported to authorities, it is crucial that its severity is communicated effectively and evaluated accurately by professionals. This study, conducted in two phases, investigated perceptual differences in calibration and accuracy regarding psychological and physical violence against children among parents, social workers, and laypeople. In Phase 1, parents who had witnessed violence against their children, committed by a current or former partner, provided first-person narratives describing these events and rated the severity of the violence. In Phase 2, social

Exploratory design and evaluation of a user interface for virtual reality exposure therapy.

Virtual reality exposure therapy is slowly becoming a viable option for therapists. For virtual reality systems to be used in the daily practice of therapists, their usability needs to be taken into consideration. This paper describes the current state-of-the-art in interfaces for these systems, and describes several proposals for improving the design of these systems. An exploratory evaluation is performed to assess the merits of aspects of the proposed user interface.

The beneficial effects of one treatment session and recording of nightmares on chronic nightmare sufferers.

Twenty subjects with chronic nightmares for 17.2 years mean duration were randomly divided into two groups: Rehearsal and Recording. At inception, subjects in both groups were instructed to write down their nightmares for one month. The Recording group received no other intervention. Rehearsal subjects received a single treatment group session teaching an imagery rehearsal technique to reduce nightmare frequency. At inception and three months follow-up, both groups were compared for nightmare frequency and for self-rated distress with scales (Symptom Checklist and Symptom Questionnaire) measur

An update on the efficacy of psychological therapies in the treatment of obsessive-compulsive disorder in adults.

We conducted a review to provide an update on the efficacy of psychological treatments for OCD in general and with regard to specific symptom presentations. The PubMed and PsycINFO databases were searched for randomized controlled trials (RCTs) published up to mid February 2012. Forty-five such studies were identified. Exposure and response prevention (ERP) and cognitive-behavioral therapy (CBT) were found to be efficacious and specific for OCD. More purely cognitive interventions that did not include ERP or behavioral experiments were found to be possibly efficacious, as were Acceptance and C

Blood-injury phobia.

Blood-injury phobia is a unique and peculiar phobia. It is different from other phobias in that it evokes a diphasic cardiovascular response; it has a propensity to induce fainting and nausea more than fear and anxiety; and often there is a family history of a similar phobia. Although it may cause no great difficulty or social handicap in everyday life compared to social phobia, agoraphobia or other simple phobias, it can have grave implications and may even become life-threatening when it prevents essential medical procedures. Unfortunately, not many of its sufferers come for treatment until

Opioids: cellular mechanisms of tolerance and physical dependence.

Morphine and other opioids are used and abused for their analgesic and rewarding properties. Tolerance to these effects develops over hours/days to weeks, as can physical and psychological dependence. Despite much investigation, the precise cellular mechanisms underlying opioid tolerance and dependence remain elusive. Recent studies examining mu-opioid receptor desensitization and trafficking have revealed several potential mechanisms for acute receptor regulation. Other studies have reported changes in many other proteins that develop during chronic opioid treatment or withdrawal and such cha

Effectiveness of psychological interventions in reducing post-traumatic stress among post-myocardial infarction patients: a systematic review and meta-analysis.

AIMS: Myocardial infarction (MI) can lead to post-traumatic stress disorder (PTSD) which frequently occurs with anxiety and depression, impairing daily functioning and increasing the risk of recurrent cardiovascular events. While psychological interventions have shown promise in reducing anxiety and depression, their effectiveness for PTSD in post-MI patients remains unexplored. This systematic review and meta-analysis aim to evaluate the effectiveness of psychological interventions on PTSD, anxiety, and depression in post-MI patients. METHODS AND RESULTS: A comprehensive search of databases
