Development of a safe dexamethasone-eluting electrode array for cochlear implantation.

OBJECTIVES: Cochlear implantation can result in trauma leading to increased tissue response and loss of residual hearing. A single intratympanic application of the corticosteroid dexamethasone is sometimes used clinically during surgery to combat the potential effect of trauma on residual hearing. This project looked at the safety and efficacy of dexamethasone eluted from an intracochlear array in vivo. METHODS: Three trials were conducted using normal hearing adult guinea pigs implanted with successive iterations of dexamethasone-eluting (DX1, DX2, and DX3) or non-eluting (control) intracoch

Flexible PET/ITO electrode array for implantable biomedical applications.

Flexible PET/ITO (PolyEthylene Terephthalate/ Indium Tin Oxide) implantable electrode array for spinal cord stimulation and retina prosthesis have been developed. The electrode array is fabricated on a thin PET/ITO substrate and encapsulated with insulating material, SU-8. The PET substrate made electrodes flexible so that they could shape to contoured tissues. A layer of gold on the stimulation sites served to reduce the electrode/tissue interface impedance. Prototypes of 1 × 8 and 3 × 8 electrode arrays are fabricated for monophasic and biphasic stimulation of spinal cord respectively. The e

Fracture of the subcutaneous patch electrode in a patient with an implanted cardioverter-defibrillator.

There are more than 20 years of experience with implantation of defibrillator devices in humans and the procedure is an important therapeutic option for patients at high risk of life-threatening ventricular arrhythmias. The incidence of new defibrillator implantation has gradually increased, being used even in children, (1) although pediatric use is associated with several complications, especially with epicardial systems,(2) including fracture of the subcutaneous patch,(3-5) mainly because of growth.(3) We present a case of subcutaneous patch electrode fracture in the left axillary pectoral r

Cochlear implantation without mastoidectomy: the pericanal electrode insertion technique.

OBJECTIVE: A new minimally invasive cochlear implantation method with direct electrode insertion through the external auditory canal (EAC) is presented. MATERIAL AND METHODS: Surgery begins with a retroauricular skin incision. The bony surface of the mastoid plane behind the ear is dissected free and the skin of the EAC is elevated together with the posterior part of the tympanic membrane. Cochleostomy is performed through the EAC with a microdrill anterior to the round window. A rim is drilled into the postero-superior region of the bony EAC immediately above the incus towards the outer bord

Fluoroscopy guided electrode-array insertion for cochlear implantation with straight electrode-arrays: a valuable tool in most cases.

PURPOSE: To highlight the advantages of real time fluoroscopy guided electrode-array (EA) insertion (FGI) during cochlear implants surgery. METHODS: All surgical procedures were performed in a dedicated operating room equipped with a robotic C-arm cone beam device, allowing for intraoperative real time 2D FGI and postoperative 3D imaging. Only straight EAs were used. Patients were sorted out in three groups: ANAT, with anatomical concerns; HP, with residual hearing; NPR: patients with no particular reason for FGI. In all cases the angle of EA-insertion was measured. In the HP group pre and po

Effects of Electrode Position on Vestibular Implant Performance in Rhesus Macaque.

PURPOSE: Vestibular implants that target the three semicircular canal branches of the vestibular nerve can partially restore the 3-dimensional vestibulo-ocular reflex (3D VOR) in individuals disabled by bilateral vestibular hypofunction. A key goal of implant design is optimizing the number, spacing, and placement of stimulating and return electrodes to maximize response strength and selectivity. While computational models provide initial insights, empirical data are essential to validate performance. METHODS: We unilaterally implanted stimulating electrodes in each semicircular canal and pos

Determining ideal electrode insertion depth in paediatric cochlear implantation.

BACKGROUND: Speech and language outcomes following early cochlear implantation vary widely among patients. Identifying modifiable, device-specific surgical parameters contributing to this variability is essential to optimise therapeutic outcomes. OBJECTIVE: This study investigates the association between electrode angular depth of insertion (aDOI) and speech perception outcomes in a homogeneous cohort of children implanted with a single lateral wall electrode design. METHODS: A cohort of 25 prelingually deaf children, aged 4 to 5 years at implantation, received unilateral Oticon Neuro ZTI co

Frequency-to-Place Mismatch and Cochlear Implant Outcomes by Electrode Type.

IMPORTANCE: Speech recognition outcomes with a cochlear implant (CI) are highly variable. One factor suggested to correlate with CI-aided speech recognition is frequency-to-place mismatch, or the discrepancy between the natural tonotopic organization of the cochlea and the electric frequency allocation of the CI electrodes within the patient's cochlea. OBJECTIVE: To evaluate the association between frequency-to-place mismatch and speech recognition outcomes in a large cohort of postlingually deafened adult CI users, while controlling for various clinical factors known to be associated with th

Electrocochleography Latency: Correlation With Electrode Position During Cochlear Implantation.

OBJECTIVES: Cochlear implant (CI) candidates increasingly exhibit some degree of residual hearing, which should be preserved despite the implantation. Today, cochlear health is monitored during CI surgery by tracking the cochlear microphonic (CM) amplitude from intracochlear electrocochleography (ECochG) measurements. However, recent studies indicate that the insertion depth of the measuring electrode must be considered to accurately interpret these signals. The acoustic path from the cochlear base to the apex induces excitation delays in deeper regions, which should be reflected in the CM mea

Development and Characterization of a Micromagnetic Alternative to Cochlear Implant Electrode Arrays.

To stimulate the auditory nerve, cochlear implants directly inject electrical current into surrounding tissue via an implanted electrode array. While many cochlear implant users achieve strong speech perception scores, there remains significant variability. Since cochlear implant electrode arrays are surrounded by a conductive fluid, perilymph, a spread of excitation occurs. The functionality of the cochlea is spatially dependent, and a wider area of excitation negatively affects the hearing of the user. Importantly, magnetic fields are unaffected by the material properties of biological compo

Systematic Review of Postcochlear Implant Electrode Migration: What Is Known?

BACKGROUND AND OBJECTIVES: Electrode migration after cochlear implantation (CI) is a rare complication that accounts for 1to 15% of all revision surgery. This study is a systematic review of the literature for investigating the knowledge and approaches to the incidence of electrode migration after CI. METHODS: A systematic electronic search of the literature was carried out using PubMed, Cochrane, Virtual Health Library, Scopus and Web of Science (ISI). All original articles that reported electrode migration after CI surgery were included. The Newcastle-Ottawa Scale and CARE checklist were ut

Shift in electrocorticography electrode locations after surgical implantation in children.

Interpreting electrocorticography (ECoG) in the context of neuroimaging requires that multimodal information be integrated accurately. However, the implantation of ECoG electrodes can shift the brain impacting the spatial interpretation of electrode locations in the context of pre-implant imaging. We characterized the amount of shift in ECoG electrode locations immediately after implant in a pediatric population. Electrode-shift was quantified as the difference in the electrode locations immediately after surgery (via post-operation CT) compared to the brain surface before the operation (pre-i

Diaphragmatic myoclonus owing to electrode dislocation of implantable cardioverter defibrillator.

Myoclonus is a sudden and brief involuntary muscle contraction presenting with jerk-like movements that can occasionally involve the trunk muscles or the diaphragm as in the case of spinal myoclonus1. We here present an unusual case with unilateral diaphragmatic myoclonus owing to electrode dislocation of an implantable cardioverter defibrillator.

Percutaneously injectable fetal pacemaker: electrodes, mechanical design and implantation.

We are developing a self-contained cardiac pacemaker with a small, cylindrical shape (~3 × 20 mm) that permits it to be implanted percutaneously into a fetus to treat complete heart block and consequent hydrops fetalis, which is otherwise fatal. The device uses off-the-shelf components including a rechargeable lithium cell and a highly efficient relaxation oscillator encapsulated in epoxy and glass. A corkscrew electrode made from activated iridium can be screwed into the myocardium, followed by release of the pacemaker and a short, flexible lead entirely within the chest of the fetus to avoid

Cochlear implant electrode array misplacement: a cautionary case report.

OBJECTIVE: To report a series of pitfalls and complications in a case of cochlear implantation. METHOD: Case report. RESULTS: An 11-year-old boy affected by auditory neuropathy underwent cochlear implantation. Intra-operative assessment was apparently consistent with correct insertion of the electrode array into the cochlea. However, subsequent high resolution computed tomography revealed that the entire electrode array was curled up within the vestibule. Revision surgery was complicated by cerebrospinal fluid leakage. A straight probe was repeatedly inserted into the internal auditory canal

Effects of electrode implantation angle on thalamic stimulation for treatment of tremor.

Introduction. Chronic thalamic stimulation has been confirmed as an effective treatment for tremor. The optimal target has been commonly accepted to be situated within the ventral thalamus, but a standard trajectory of the deep brain stimulation (DBS) electrode has not yet been established. Materials and Methods. A 53-year-old man with an 11-year history of essential tremor was treated by DBS of the thalamus. In this patient, we had a chance to compare the effects of different trajectory angles of the DBS electrode on tremor. Results. Intraoperative stimulation with the DBS electrode temporari

Averaged electrode voltages in users of the Clarion cochlear implant device.

Averaged electrode voltages (AEVs) are of secondary importance for integrity testing of cochlear implant devices featuring back-telemetry. However, AEVs are device-independent and may show intermittent failures and deviant stimulation patterns unnoticed by telemetry. We collected AEVs from 18 users of the Clarion 1.2 system and 6 users of the HiFocus system in order to establish norms for evaluating AEVs in difficult cases. The stimuli were presented with the standard clinical software. Monopolar stimulation at about 16 microA showed large AEVs (mean, 173 microV) suitable for integrity testing
