Transcranial Motor-Evoked Potentials Provide More Reliable Intraoperative Neuromonitoring Than Somatosensory-Evoked Potentials in Charcot-Marie-Tooth Patients Undergoing Periacetabular Osteotomy for Hip Dysplasia.

BACKGROUND: Charcot-Marie-Tooth (CMT) disease is a common inherited progressive sensorimotor neuropathy which can complicate intraoperative monitoring (IOM). Transcranial motor-evoked potentials (TcMEPs) have been found to be effective in obtaining IOM in CMT patients undergoing spine surgery. The aim of this study was to examine the effectiveness of TcMEPs and somatosensory-evoked potentials (SSEPs) in IOM in CMT patients undergoing periacetabular osteotomies (PAOs) for hip dysplasia. METHODS: A retrospective case series was conducted in all CMT patients aged <19 years at a single institutio

Somatosensory evoked potentials, sensory nerve potentials and sensory nerve conduction in hereditary motor and sensory neuropathy type I.

Thirty-nine patients from six families with hereditary motor and sensory neuropathy type I and control subjects were included in this study. A neurological deficit score (NDS) was derived from a neurological examination and compared with neurophysiological test findings. Further, sensory nerve conduction velocities (SNCV) were compared with the motor nerve conduction velocities (MNCV). Five patients whom peaks of N11/N13 complex and N20 of the median nerve sensory evoked potential (SEP) could be recorded showed normal interpeak latency. The interpeak separation P14-N20 measured in six patients

A novel bone-thinning technique for transcranial stimulation motor-evoked potentials in rats.

Transcranial electrical stimulated motor-evoked potentials (tcMEPs) are widely used to evaluate motor function in humans, and even in animal studies, tcMEPs are used to evaluate neurological dysfunction. However, there is a dearth of reports on extended tcMEP recordings in both animal models and humans. Therefore, this study examined a new technique for stably recording tcMEPs over several weeks in six healthy female Sprague-Dawley rats. We thinned the skull bone using the skull base and spinal surgery technique to reduce electrical resistance for electrical stimulation. tcMEPs were recorded o

Use of motor evoked potentials during lateral lumbar interbody fusion reduces postoperative deficits.

BACKGROUND CONTEXT: Intraoperative neurophysiological monitoring (IONM) has gained rather widespread acceptance as a method to mitigate risk to the lumbar plexus during lateral lumbar interbody fusion (LLIF) surgery. The most common approach to IONM involves using only electromyography (EMG) monitoring, and the rate of postoperative deficit remains unacceptably high. Other test modalities, such as transcranial electric motor-evoked potentials (tcMEPs) and somatosensory-evoked potentials, may be more suitable for monitoring neural integrity, but they have not been widely adopted during LLIF. Intraoperative Loss of Tibialis Anterior Transcranial Electrical Motor Evoked Potentials Predicted Postoperative Footdrop.

BACKGROUND: Footdrop secondary to L5 root injury is a rare complication associated with lumbar surgery. It is unclear whether intraoperative neuromonitoring can detect such an injury. CASE DESCRIPTION: A 54-year-old man who had had bilateral chronic L5 radiculopathy underwent L4-S1 lumbosacral decompression and fusion. During surgery, the patient lost transcranial electrical motor evoked potentials (tceMEPs) from the left tibialis anterior (TA) at the time of L5-S1 intervertebral cage placement. Neither intraoperative free-run electromyography nor somatosensory evoked potentials detected any

Somatosensory and motor evoked potentials in patients with tumours in the spinal canal.

Motor and sensory evoked potentials were recorded in 27 patients with expanding spinal tumour. The patients were divided into 2 groups: I. tumours at the level of the spinal cord and II. at the level of the cauda equina. On the basis of the localization of the tumour, midline and lateral subgroups were distinguished. The latencies of motor evoked potentials were prolonged in most of the patients, even those without paresis, in both groups. The motor evoked potentials detected subclinical motor lesions in 7 patients. All patients but one manifested sensory deficits, which could not be shown wit

Effects of sex, height and age on motor evoked potentials with magnetic stimulation.

Magnetic stimulation of the brain and cervical and lumbar spinal roots was performed on 48 healthy subjects in order to investigate the effects of sex, height and age on motor evoked potentials (MEPs). The compound muscle action potentials were recorded from the abductor pollicis brevis and abductor hallucis muscles. The central motor conduction time (CMCT) was measured between the cerebral cortex and the cervical root and also between the cerebral cortex and the lumbar root. A multiple regression analysis was used to determine which of the physical variables, namely sex, height and age, were

Motor evoked potentials in the preoperative and postoperative assessment of normal pressure hydrocephalus.

Motor evoked potentials and central motor conduction time (CMCT) were examined from both upper and lower limbs in patients with normal pressure hydrocephalus to find a predictor for the success of shunting procedures. The hypotheses that walking disturbances are due to pyramidal tract compression as well as the possibility that the upper limbs are affected subclinically in these patients were also studied. The study suggests that the walking disturbances are not the result of a major pyramidal tract dysfunction but probably involve the sensorimotor integration leading to normal gait. Furthermo

Motor evoked potentials of the human diaphragm elicited through magnetic transcranial brain stimulation.

Magnetic transcranial brain stimulation has been used for several years now to study the function and integrity of the corticospinal tracts. In this study magnetic transcranial motor cortex stimulation of the diaphragm was carried out in order to describe the characteristics of the recorded motor evoked potentials (MEPs) of this muscle in humans. The motor cortex was stimulated transcranially in 10 healthy subjects, followed by stimulation of the cervical roots so that central motor conduction time (CMCT) could be calculated. Diaphragm MEPs were recorded at deep inspriration. Normal values wer

Motor and somatosensory evoked potentials in mice infected with Theiler's murine encephalomyelitis virus.

We used an in vivo technique to record spinal motor and somatosensory evoked potentials in SJL/J and B10 mice chronically (4-10 months) infected with Daniel's strain of Theiler's murine encephalomyelitis virus (TMEV). SJL/J mice demonstrated primary spinal cord demyelination with chronic TMEV infection, whereas B10 mice were resistant to TMEV induced demyelination. Analysis based on the velocity of the initial peak of evoked responses demonstrated significantly slower conduction velocities in infected SJL/J mice as compared to age-matched uninfected SJL/J controls (p < 0.01) and infected B10 m

Examination and quantification of motor evoked potentials in the non-target resting leg.

Transcranial magnetic stimulation (TMS) is a research tool that can noninvasively assess efficiency of the nervous system. However, TMS of the lower limb presents considerable challenges. Guidelines to streamline data collection procedures are warranted. As a secondary analysis of previously published data (PMID: 37267320), this study aimed to determine to what extent motor evoked potentials (MEPs) are evident from the resting ipsilateral leg when the vertex is stimulated. A secondary aim was to provide methodological insight into how to identify MEPs in the resting ipsilateral limb when the c

Sensitivity and Negative Predictive Value of Motor Evoked Potentials of the Facial Nerve.

OBJECTIVE: The objective of this study was to determine the performance of the standard alarm criterion of motor evoked potentials (MEPs) of the facial nerve in surgeries performed for resections of vestibular schwannomas or of other lesions of the cerebellopontine angle. METHODS: This retrospective study included 33 patients (16 with vestibular schwannomas and 17 with other lesions) who underwent the resection surgery with transcranial MEPs of the facial nerve. A reproducible 50% decrease in MEP amplitude, resistant to a 10% increase in stimulation intensity, was applied as the alarm criteri

Transoesophageal spinal cord stimulation for motor-evoked potentials monitoring: feasibility, safety and stability.

OBJECTIVES: Specificity of transcranial motor-evoked potentials (MEPs) is low because amplitude fluctuation is common, which seems due to several technical and fundamental reasons including difficulty in electrodes positioning and fixation for transcranial stimulation and susceptibility to anaesthesia. This study aimed to investigate the feasibility, safety and stability of our novel technique of transoesophageal spinal cord stimulation to improve the stability of MEPs. METHODS: Ten anaesthetized adult beagle dogs were used. Transoesophageal stimulation was performed between the oesophageal l

Effect of neck flexion on somatosensory and motor evoked potentials in Hirayama disease.

UNLABELLED: Hirayama disease (HD) is a rare motor disorder mainly affecting young men, characterized by atrophy and weakness of forearm and hand muscles corresponding to a C7-T1 myotome distribution. The weakness is usually unilateral or asymmetric and progression usually stops within several years. The etiology of HD is not well understood. One hypothesis, mainly based on MRI findings, is that the weakness is a consequence of cervical flexion myelopathy. The aim of this study was to explore the function of corticospinal and ascending somatosensory pathways during neck flexion using evoked res

The role of motor-evoked potentials in the management of cervical spondylotic myelopathy.

BACKGROUND CONTEXT: Cervical spondylotic myelopathy (CSM) is a common disorder, but its management and the role of surgery are still a matter of controversy. The assessment of surgical outcome is complicated by the lack of reliable and objective methods to assess the severity of the myelopathy and its evolution. Motor-evoked potentials (MEPs) are a useful and reliable tool to measure noninvasively the involvement of the corticospinal tract in patients with CSM. Recent evidence suggests that MEPs could also have a role in monitoring the effect of surgical therapy. PURPOSE: The aim of the prese

Intra-operative monitoring of lower extremity motor-evoked potentials by direct cortical stimulation.

OBJECTIVE: Motor-evoked potentials (MEPs) are commonly recorded from upper-extremity muscles, whereas lower-extremity MEP (LE-MEP) monitoring has not been adequately established. The goal of the study was to develop a MEP monitoring method using direct cortical stimulation (DCS) for predicting motor deficits of lower extremities. METHODS: Intra-operative LE-MEP monitoring was performed in 22 patients. After craniotomy, a subdural electrode was placed on the cortex so that the optimal contact was positioned 2 cm lateral from the midline on the motor cortex. The electrodes for stimulation consi
