Asymptomatic jaundice after fasting: a diagnostic dilemma.

An 18-year-old female with asymptomatic jaundice presented to the emergency department after fasting. She was referred to the regional medical center for evaluation and treatment. The diagnosis of Gilbert's syndrome was made by fractionation of serum unconjugated and conjugated bilirubin fraction by alkaline methanolysis, followed by thin-layer chromatography and analysis of fasting-state levels of cholyl conjugated bile acids. Methods for diagnosing this disorder are discussed.

Fasting-induced reduction of intestinal reperfusion injury.

BACKGROUND: Fasting is associated with significant structural, functional, and metabolic alterations in the intestinal mucosa. Before abdominal surgery, patients are usually fasted the night before surgery or for a longer period of time if chronic illness is present. The splanchnic organs may experience varying degrees of ischemia/reperfusion as blood vessels are occluded during the various manipulations. METHODS: To study whether fasting alters intestinal reperfusion injury, rats were fasted for 0 to 2 days, and the mesenteric artery was occluded for 30 minutes and then reperfused for 1 hour

Effects of bombesin on fasting bile formation.

Adult dogs were previously prepared by cholecystectomy, ligation of the lesser pancreatic duct, and insertion of cannulae into the duodenum and stomach. After a 2-week period of postoperative recovery and an overnight fast, bile ducts were cannulated, gastric cannulae placed to open drainage and sodium taurocholate 500 mg hr-1 was administered to replace bile acids lost from the interrupted enterohepatic circuit. Bombesin was infused IV for 1 hour over the dose range, 0.625-10 ng kg-1 min-1. In control experiments 0.15 N NaCl was infused. Bombesin caused a significant increase in fasting bile

Alterations in brain cholecystokinin receptors after fasting.

Cholecystokinin (CCK) is the parent molecule of a family of polypeptide hormones, some of which are secreted from the small intestine after food ingestion and stimulate both exocrine secretion and gall bladder contraction. CCK-like molecules have also been identified in the central and peripheral nervous systems. The significance of CCK in the brain is unknown, but it could mediate satiety. The satiety produced by introduction of food into the intestine can be mimicked by systemic injections of CCK and its analogues-these hormones are also effective when injected into the hypothalamus and the

20/ (fasting C-peptide × fasting plasma glucose) as a novel marker for insulin resistance in obese children and adolescents.

BACKGROUND: Insulin resistance (IR) constitutes a critical pathophysiological mechanism linking pediatric obesity to type 2 diabetes mellitus (T2DM) and cardiovascular disease. Recent investigations in adult populations have proposed 20/ fasting C-peptide × fasting plasma glucose (20/ (Fcpep X FBG)) as a good biomarker for IR. We aimed to evaluate the diagnostic utility of this novel index in detecting IR among obese pediatric patients. METHODS: This case-control study included 100 children (50 obese and 50 controls), aged 6-16 years. Anthropometric, clinical, and laboratory assessments were

Impact of Prolonged Fasting and Refeeding on Enteroendocrine Hormone Expression: One More Piece of the Fasting/Refeeding Metabolic Puzzle.

Introduction: Prolonged fasting induces histological and ultrastructural changes of the intestinal mucosa that may reduce absorption in malnourished patients with high risk of refeeding syndrome. Endocrine function of the intestinal mucosa may be affected by starvation with potential implications for nutritional support. Objective: The present study aims to evaluate the expression of gastrointestinal hormones in duodenal enteroendocrine cells (EECs) of patients after a long starvation period and to assess the changes in EEC hormonal expression after enteral refeeding in the same individuals. Changes in non-fasting concentrations of blood lipids after a daily Chinese breakfast in overweight subjects without fasting hypertriglyceridemia.

BACKGROUND: Overweight is always accompanied by hypertriglyceridemia (HTG), but the change in non-fasting triglyceride (TG) concentration in overweight subjects without postprandial hypertriglyceridemia was unknown. METHODS: Concentrations of serum lipids were measured at 2 and 4 h in matched overweight (OW group, n = 54) and control subjects (CON group, n = 55) after a daily meal. Concentrations of remnant cholesterol and non-HDL cholesterol were calculated according to the formulas. The diagnostic criteria for non-fasting HTG were based on 2 different consensus statement. Prolonged fasting promotes systemic inflammation and platelet activation in humans: A medically supervised, water-only fasting and refeeding study.

OBJECTIVE: Prolonged fasting (PF), defined as abstaining from energy intake for ≥4 consecutive days, has gained interest as a potential health intervention. However, the biological effects of PF on the plasma proteome are not well understood. METHODS: In this study, we investigated the effects of a medically supervised water-only fast (mean duration: 9.8 ± 3.1 days), followed by 5.3 ± 2.4 days of guided refeeding, in 20 middle-aged volunteers (mean age: 52.2 ± 11.8 years; BMI: 28.8 ± 6.4 kg/m2). RESULTS: Fasting resulted in a 7.7% mean weight loss and significant increases in serum beta-hydr

Increasing fasting glucose and fasting insulin associated with elevated bone mineral density-evidence from cross-sectional and MR studies.

UNLABELLED: We performed a cross-sectional study using the National Health Examination and Nutrition Survey (NHANES) data and a Mendelian randomisation (MR) study using the GWAS summary statistics from European populations. The T2D-related indices (fasting plasma glucose (FPG), fasting insulin (FI), and insulin resistance (IR)) were found to associate with elevated bone mineral density (BMD). INTRODUCTION: The known associations amongst FPG, FI, IR, and BMD remain inconsistent. This study aims to explore the abovementioned associations by using cross-sectional and MR designs. METHODS: Data f

Effects of alternate-day fasting, time-restricted fasting and intermittent energy restriction DSS-induced on colitis and behavioral disorders.

Intermittent fasting (IF) has been reported to have beneficial effects on improving gut function via lowering gut inflammation and altering the gut microbiome diversity. In this study, we aimed to investigate the differential effects of three different common IF treatments, alternate day fasting (ADF), time-restricted fasting (TRF), and intermittent energy restriction (IER), on a dextran sodium sulfate (DSS)-induced colitis mouse model. The results indicated that TRF and IER, but not ADF improved the survival rates of the colitis mice. TRF and IER, but not ADF, reversed the colitis pathologica

Associations of a metabolic syndrome severity score with coronary heart disease and diabetes in fasting vs. non-fasting individuals.

BACKGROUND AND AIMS: Many traditional assessments of risk for coronary heart disease (CHD) and diabetes require laboratory studies performed after an 8-h fast. We assessed whether metabolic-syndrome (MetS) severity would remain linked to future CHD and diabetes even when assessed from non-fasting samples. METHODS AND RESULTS: Participants in the Atherosclerosis Risk in Communities study were assessed at 4 visits and followed for 20-years of adjudicated CHD outcomes. We used Cox proportional-hazard models (for 20-year CHD outcomes) and logistic regression (for 9-year diabetes outcomes) to comp

Associations of 25-hydroxyvitamin D with fasting glucose, fasting insulin, dementia and depression in European elderly: the SENECA study.

PURPOSE: The classical consequence of vitamin D deficiency is osteomalacia, but recent insights into the function of vitamin D suggest that it may play a role in other body systems as well. The objective of this study was to examine the association between 25-hydroxyvitamin D (25(OH)D) and markers of glucose metabolism (n = 593), global cognitive functioning (n = 116) and depression (n = 118) in European elderly participating in the SENECA study. Moreover, we wanted to explore whether the observed associations of 25(OH)D with depression and global cognitive performance were mediated by fasting

Alveolar sampling and fast kinetics of tetrachloroethene in man. II. Fast kinetics.

Fast kinetic phenomena were studied in human subjects exposed to tetrachloroethene (perchloroethylene, PER). The duration of exposure ranged from one to 60 minutes and the concentration of PER in inhaled air ranged from 0.02 to 0.40 mmol/m3. The PER concentration in mixed venous blood (pulmonary artery) was estimated by alveolar concentration (CAlv) measured after a residence time of 10 s. During exposure, stoppage of intake (breath holding up to 50 s) caused a decrease of CAlv down to about 60% of the CAlv at a residence time of 10 s. At the end of exposure, stoppage of intake (breathing fres

Liver X receptors participate in the fasting response by assisting CEBPB binding to enhancers and promoting the fasting transcriptional program.

Upon fasting, a group of transcription factors (TFs) activates an extensive transcriptional program in the liver to alter hepatocyte metabolism and cope with fasting. This is achieved by TF-mediated activation of enhancers, which become more accessible, leading to gene induction. Liver X receptors alpha (NR1H3) and beta (NR1H2), collectively termed here LXRs, are important TFs known to participate in transcriptional programs in the fed state, but their role in fasting is unclear. Here, we examined the effect of LXR deletion on the transcriptional and chromatin programs activated in the fasted

Association of a Liberal Fasting Policy of Clear Fluids Before Surgery With Fasting Duration and Patient Well-being and Safety.

IMPORTANCE: Current fasting guidelines for procedures under anesthesia are poorly implemented, leading to negative metabolic sequelae. Recent studies in children showed support of liberal clear fluid intake; adult physiology can support clear fluid intake, but implementation studies are lacking. OBJECTIVE: To evaluate the successfulness of implementation of a liberal clear fluid policy with regard to fasting duration, well-being, and safety in adults scheduled for anesthesia. DESIGN, SETTING, AND PARTICIPANTS: This was a quality improvement study conducted from January 2016 to July 2021 at a

Lowered fasting chenodeoxycholic acid correlated with the decrease of fibroblast growth factor 19 in Chinese subjects with impaired fasting glucose.

The gut-derived hormone Fibroblast growth factor 19 (FGF19) could regulate glucose metabolism and is induced by bile acids (BAs) through activating Farnesoid X Receptor (FXR). FGF19 was found to decrease in subjects with isolated-impaired fasting glucose (I-IFG) and type 2 diabetes mellitus (T2DM). However, the reason for the change of FGF19 in subjects with different glucometabolic status remained unclear. Here we measured six BAs including chenodeoxycholic acid (CDCA), cholic acid, deoxycholic acid, their glycine conjugates and FGF19 levels during oral glucose tolerance test (OGTT) in normal

FAST-GOAL: Fast and Efficient Global-Local Object Alignment Learning.

Vision-language models such as CLIP have shown impressive capabilities in aligning images and text, but they often struggle with lengthy and detailed text descriptions due to pre-training on short and concise captions. We present FAST-GOAL (Fast and Efficient Global-local Object Alignment Learning), an efficient fine-tuning method that enhances ability of CLIP to handle lengthy text through global-local semantic alignment. Our method consists of two key components. First, Fast Local Image-Sentence Matching (FLISM) efficiently extracts local image regions through object detection and spatial di
