The Relationship Between Volume Overload in End-Stage Renal Disease and Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is common, underdiagnosed, and undertreated among patients with end-stage renal disease (ESRD). As in all cases, pathogenesis of OSA is related to repeated upper airway (UA) occlusion or narrowing, but in ESRD, additional contributory factors likely include uremic destabilization of central respiratory control and anatomic changes in the UA related to fluid status. Pulmonary congestion is common in acute and chronic kidney failure and is a consequence of cardiomyopathy and fluid overload, two potentially reversible risk factors. Emerging evidence suggests that vol

The source of net ultrafiltration during hemodialysis is mostly the extracellular space regardless of hydration status.

Fluid shifts are common in patients undergoing chronic hemodialysis (HD) during the intradialytic periods, as several liters of fluid are removed during ultrafiltration (UF). Some patients have experienced frequent intradialytic hypotension (IDH). However, the characteristics of fluid shifts and which fluid space is affected remain controversial. Therefore, we designed this study to evaluate the fluid spaces most affected by UF and to determine whether hydration status influences the fluid shifts during HD. This was a prospective cohort study of 40 patients undergoing HD. We measured the patie

Low versus high pressure for in vitro determination of hydraulic conductance of human dentine.

The purpose of this study was to evaluate the effect of applied pressure and measurement time on the in vitro measurement of hydraulic conductance of human dentine. Dentine slices were prepared from 50 third molars. Water was forced through the slices under a constant hydrostatic pressure. Five pressures were tested: 1.3 kPa (n = 10), 13.3 kPa (n = 10), 26.6 kPa (n = 10), 40 kPa (n = 10) and 53.3 kPa (n = 10). The volume that went through the slices was recorded every 10 min for 3 h. The volume, the fluid flow rate and the hydraulic conductance under the five pressures were compared. The volum

Fluid Volume Management in Prosthesis Users: Augmenting Panel Release with Pin Release.

BACKGROUND: Management of fluid in the limbs is a challenge faced by people with disabilities. In prosthetics, a means for transtibial prosthesis users to stabilize their residual limb fluid volume during the day may improve socket fit. OBJECTIVE: To determine if releasing the panels and locking pin of a cabled-panel adjustable socket during socket release significantly improved limb fluid volume recovery and retention over releasing the panels alone. DESIGN: Repeated-measures experiment to assess the effects on limb fluid volume retention. SETTING: Participants were tested in a laboratory

Evening sock marks as an adjunct to the clinical prediction of obstructive sleep apnea.

STUDY OBJECTIVES: Fluid overload shifting from the legs to the upper airway during sleep promotes obstructive sleep apnea (OSA) and interventions targeting fluid attenuate OSA. Fluid shift has been previously measured by bioelectrical impedance, a complex and time-consuming technique not applicable in the daily clinical settings. The aim of this study is to evaluate the presence of clinically detectable fluid overload and shift and its association with OSA. METHODS: Patients undergoing sleep study for suspected OSA were asked to report the presence of 11 signs/symptoms associated to excessive

Quantification of brain tissue alterations in Fabry disease using diffusion-tensor imaging.

UNLABELLED: Central nervous system involvement is a major burden in Fabry disease. Conventional cranial magnetic resonance imaging (MRI) shows micro- and macroangiopathic changes such as severe and progressive white matter lesions (WMLs) at an early age on T2- and fluid-attenuated inversion recovery-weighted images, increased signal intensity in the pulvinar on T1-weighted MRI, as well as tortuosity and dilatation of the larger vessels (dolicho-ectasia). Using diffusion tensor imaging (DTI), a new structural MRI-technique that measures water diffusion characteristics, we showed marked brain ti

Volume responsiveness.

PURPOSE OF REVIEW: In the ICU only half of the patients are volume responsive - that is, they respond to fluid administration by increasing their cardiac output. We aim to summarize the methods available for predicting volume responsiveness, focusing on recent findings in patients with spontaneous breathing activity. RECENT FINDINGS: New information mainly comes from studies that have attempted to find accurate predictors of volume responsiveness in cases of spontaneous breathing activity when heart-lung interaction indices cannot be reliably used. Passive leg raising has emerged as a reliabl

Interstitial fluid homeostasis and pressure: news from the black box.

Extracellular volume expansion may lead to elevated blood pressure. This long-term adaptation of the vascular bed to extracellular volume overload is considered a multifactorial and not perfectly understood 'autoregulatory' event, which is difficult to measure. In this issue, Ebah and colleagues demonstrate a direct relationship between fluid overload and pressure in CKD patients. Surprise, instead of intravascular volume, interstitial fluids and pressures were measured. Finally!

The effect of changes in intra-compartmental bioimpedance measurements with early intra-dialytic hypotension during haemodialysis.

INTRODUCTION: Intra-dialytic hypotension (IDH) remains the commonest problem associated with routine haemodialysis treatments. Fluid shifts from intracellular(ICW) and extracellular(ECW) compartments to refill plasma volume during haemodialysis with ultrafiltration. METHODS: We studied the effect of relative changes in ICW and ECW indifferent body segments using multifrequency segmental bioimpedance during haemodialysis and IDH episodes. RESULTS: Of 42 haemodialysis patients,16 patients (38.1%) developed IDH within the first hour of dialysis. Patients with and without early IDH were well-mat

A Potential Mechanism of Cardio-Renal Protection with Sodium-Glucose Cotransporter 2 Inhibitors: Amelioration of Renal Congestion.

BACKGROUND: This review considers anew the etiology of the cardio-renal protective effect of sodium-glucose cotransporter 2 (SGLT2) inhibitors by extending the discussion to renal congestion, inherent in diabetic kidney disease (DKD) even at an early stage of nephropathy in which heart failure (HF) or salt and water accumulation is asymptomatic. SUMMARY: The interstitial fluid (IF) space of the kidney space plays a crucial role for tubulointerstitial inflammation, renal hypoxia, and ischemic injury, which often leads to renal progression. In DKD, as a result of hyperglycemic milieu, excessive

A teaspoon pump for pumping blood with high hydraulic efficiency and low hemolysis potential.

Virtually all blood pumps contain some kind of rubbing, sliding, closely moving machinery surfaces that are exposed to the blood being pumped. These valves, internal bearings, magnetic bearing position sensors, and shaft seals cause most of the problems with blood pumps. The original teaspoon pump design prevented the rubbing, sliding machinery surfaces from contacting the blood. However, the hydraulic efficiency was low because the blood was able to "slip around" the rotating impeller so that the blood itself never rotated fast enough to develop adequate pressure. An improved teaspoon blood p

The effects of hemodialysis on total intrauterine volume.

Our objective was to assess the effects of maternal fluid shifts during hemodialysis on the amniotic fluid volume. Ultrasound estimation of the total intrauterine volume before and after dialysis was performed between 23 and 26 weeks in a 30-year-old woman with progressive IgA glomerulonephropathy. The total intrauterine volume was reduced after each episode of dialysis. The mean decrease in the amniotic fluid volume was 116.6 ml (range 17.8-287.1 ml), which represented a 1.5-31.1% change in the total intrauterine volume (p = 0.004). The reduction was linearly related to the amount of maternal

Labyrinth and cerebral-spinal fluid pressure changes in guinea pigs and monkeys during simulated zero G.

This study was undertaken to explore the hypothesis that shifts of body fluids from the legs and torso toward the head contribute to the motion sickness experienced by astronauts and cosmonauts. The shifts in body fluids observed during zero-G exposure were simulated by elevating guinea pigs' and monkeys' torsos and hindquarters. Cerebral-spinal fluid pressure was recorded from a transducer located in a brain ventricle; labyrinth fluid pressure was recorded from a pipette cemented in a hole in a semicircular canal. An anticipated divergence in cerebral-spinal fluid pressure and labyrinth fluid

Is Perioperative Fluid and Salt Balance a Contributing Factor in Postoperative Worsening of Obstructive Sleep Apnea?

An understanding of the potential mechanisms underlying recurrent upper airway collapse may help anesthesiologists better manage patients in the postoperative period. There is convincing evidence in the sleep medicine literature to suggest that a positive fluid and salt balance can worsen upper airway collapse in patients with obstructive sleep apnea through the redistribution of fluid from the legs into the neck and upper airway while supine, in a process known as "rostral fluid shift." According to this theory, during the day the volume from a fluid bolus or from fluid overload states (i.e.,

Acute hemodynamic responses to weightlessness during parabolic flight.

Pilots and astronauts experience fluid shifts in variable gravity. Acute effects of fluid shifts on the cardiovascular system were monitored on NASA's KC-135 aircraft during parabolic flight. The variability of R-R intervals in the electrocardiogram was measured as an indication of vagal cardiac neural activity. R-R intervals were measured during the gravity transition from 2-G to 0-G produced by parabolic flight to assess the involvement of the autonomic nervous system in regulating the acute effects of fluid shifts. In seven subjects, a BoMed noninvasive continuous cardiac output monitor (NC

Pathophysiology of fluid imbalance.

Fluid imbalance can arise due to hypovolemia, normovolemia with maldistribution of fluid, and hypervolemia. Trauma is among the most frequent causes of hypovolemia, with its often profuse attendant blood loss. Another common cause is dehydration, which primarily entails loss of plasma rather than whole blood. The consequences of hypovolemia include reduction in circulating blood volume, lower venous return and, in profound cases, arterial hypotension. Myocardial failure may result from increased myocardial oxygen demand in conjunction with reduced tissue perfusion. Finally, anaerobic metabolis

Spacelab Life Sciences flight experiments: an integrated approach to the study of cardiovascular deconditioning and orthostatic hypotension.

The microgravity environment of spaceflight produces rapid cardiovascular changes which are adaptive and appropriate in that setting, but are associated with significant deconditioning and orthostatic hypotension on return to Earth's gravity. The rapidity with which these space flight induced changes appear and disappear provides an ideal model for studying the underlying pathophysiological mechanisms of deconditioning and orthostatic hypotension, regardless of etiology. Since significant deconditioning is seen after flights of very short duration, muscle atrophy due to inactivity plays, at mo
