Pharmacotherapy of bulimia nervosa--experience with fluoxetine.

A number of compounds having an effect on serotonergic transmission have been shown to have a beneficial therapeutic action in patients with bulimia nervosa. Many previous studies, however, have suffered from methodological shortcomings, in particular small population sizes. This article reviews present data which suggest that fluoxetine is effective in the treatment of bulimia nervosa. Two trials of unprecedented size (n = 387 and 398, respectively) have recently been reported. These two studies show that fluoxetine, 60 mg daily, has a clear beneficial effect on behavioural and other (attitud

Fluoxetine: prescribing guidelines for the newest antidepressant.

Fluoxetine is an antidepressant drug with a unique chemical configuration which enhances serotoninergic transmission by inhibiting serotonin uptake. The chronic presence of serotonin in the synaptic cleft reduces postsynaptic receptors, a postulated explanation for its antidepressant efficacy. Comparative studies show that the therapeutic effectiveness of fluoxetine is equal to that of imipramine, amitriptyline, and doxepin. A 20 mg morning dose alleviates most depressions. The long half-life of one to three days for the parent compound and seven to 15 days for the active metabolite, desmethyl

Fluoxetine Protects Retinal Ischemic Damage in Mice.

BACKGROUND: To evaluate the neuroprotective effect of the topical ocular administration of fluoxetine (FLX) in a mouse model of acute retinal damage. METHODS: Ocular ischemia/reperfusion (I/R) injury in C57BL/6J mice was used to elicit retinal damage. Mice were divided into three groups: control group, I/R group, and I/R group treated with topical FLX. A pattern electroretinogram (PERG) was used as a sensitive measure of retinal ganglion cell (RGC) function. Finally, we analyzed the retinal mRNA expression of inflammatory markers (IL-6, TNF-α, Iba-1, IL-1β, and S100β) through Digital Droplet

Drug safety evaluation of olanzapine/fluoxetine combination.

INTRODUCTION: Bipolar disorder and treatment-resistant depression (TRD) are common and recurrent conditions associated with significant disability, morbidity and mortality. Despite the clear need for effective treatments, only a few medications have been approved in the US for these indications. The combined formulation of olanzapine-fluoxetine (OFC) has been available for a decade now, thus a review on its safety profile and comparative efficacy is timely and can help clinicians to determine the benefit/risk profile of OFC within the context of other treatment alternatives. Behavioural outcomes of perinatal maternal fluoxetine treatment.

During and following pregnancy, women are at considerable risk of experiencing depression. For treatment, selective serotonin reuptake inhibitor drugs, such as fluoxetine, are commonly prescribed, yet the potential effects of perinatal exposure to these drugs on the brain and behaviour have not been examined in humans beyond childhood. This is despite abundant evidence from studies using rodents indicating that altered serotonin levels early in life affect neurodevelopment and behavioural outcomes. These reported effects on behaviour are inconsistent, however, and the testing of females has of

Birth outcomes in pregnant women taking fluoxetine.

BACKGROUND: Although fluoxetine is the most frequently prescribed antidepressant drug in the United States, its safety in pregnant women has not been established. METHODS: From 1989 through 1995, we prospectively identified 228 pregnant women taking fluoxetine. We compared the outcomes of their pregnancies with those of 254 women identified in a similar manner who were not taking fluoxetine. RESULTS: The rate of spontaneous pregnancy loss did not differ significantly between the women treated with fluoxetine and the control women (10.5 percent and 9.1 percent, respectively), nor was the rate

Fluoxetine suppresses human cataplexy: a pilot study.

We evaluated fluoxetine in six patients with poorly controlled cataplexy by beginning patients on a single 20 mg dose without any change in their current medications. Once benefit occurred, we discontinued or reduced other anticataplectic medication. Reduction in the number of cataplectic episodes per week after 20 weeks of treatment constituted the basis for efficacy determination. Patients experienced a mean reduction of 92% (a mean drop in frequency of 21.7 to 1.7 episodes per week). Although fluoxetine-induced headache occurred in one patient, the other five reported no side effects at the

Increase of Parkinson disability after fluoxetine medication.

Depression is a major clinical feature of Parkinson's disease. We report the increased amount of motor disability in four patients with idiopathic Parkinson's disease after exposure to the antidepressant fluoxetine. The possibility of a clinically relevant dopamine-antagonistic capacity of fluoxetine in Parkinson's disease patients must be considered.

Fluoxetine: adverse effects and drug-drug interactions.

This overview summarizes the major and minor side effects and drug interactions of fluoxetine. The adverse reactions include the "serotonin syndrome", cardiovascular complications, extrapyramidal side effects such as akathisia, dyskinesias, and parkinsonian-like syndromes and an apparently increased risk of suicidality. Fluoxetine-induced mania and hypomania, seizures and sexual disorders are evaluated along with minor symptoms of allergic reactions, stuttering, hematological changes, psoriasis, and inappropriate secretion of the antidiuretic hormone. The major fluoxetine-drug interactions inv

Fluoxetine: a serotonin-specific, second-generation antidepressant.

Fluoxetine is a bicyclic antidepressant that is a specific and potent inhibitor of the presynaptic reuptake of serotonin. It has essentially no effect on the reuptake of norepinephrine or other neurotransmitters. Similarly, it has negligible binding affinity for neurotransmitter receptor sites. It is well absorbed after oral administration, with absolute bioavailability in dogs of approximately 72 +/- 27.6%. The mean Tmax is between 4 and 8 hours, and it is approximately 94% protein bound. After a single dose, the elimination half-life is 1-3 days. After long-term administration, the eliminati

Reversible Cerebral Vasoconstriction Syndrome Associated With Fluoxetine.

BACKGROUND: Reversible cerebral vasoconstriction syndrome (RCVS) is an increasingly recognized neurological syndrome that typically presents with a severe headache. The proposed etiology is transient and segmental constriction of cerebral arteries, which in severe cases can lead to cerebral ischemia. Multiple case reports have been identified associating the use of serotonergic medications with this syndrome. OBJECTIVE: A review of the literature describing RCVS in patients taking selective serotonin reuptake inhibitors and other serotonergic medications is summarized. This report also descri

Rapsyn congenital myasthenic syndrome worsened by fluoxetine.

INTRODUCTION: Fluoxetine is a selective serotonin reuptake inhibitor and long-lived open channel blocker of the acetylcholine receptor, often used in the treatment of slow-channel congenital myasthenic syndromes (CMS). METHODS: We report a 42-year-old woman who had a history of episodic limb weakness that worsened after initiation of fluoxetine for treatment of depression. Genetic testing for CMS revealed a homozygous pathogenic mutation in the rapsyn (RAPSN) gene (p.Asn88Lys). Electrodiagnostic testing was performed before and 1 month after discontinuation of fluoxetine. RESULTS: The 2 Hz r

Reversible Fluoxetine-Induced Hyperthyroidism: A Case Report.

BACKGROUND: Selective serotonin reuptake inhibitors (SSRIs) are typically used as antidepressants. Clinically significant SSRI-induced thyroid dysfunction is rare. CASE: We report a case of hyperthyroidism induced by fluoxetine in a female patient with major depressive disorder. Her thyroid profiles indicated hyperthyroidism after a 10-week treatment with fluoxetine and were restored after discontinuation of fluoxetine and administration with venlafaxine. In the first year of follow-up, her thyroid functions as well as her depressive symptoms remained perfectly normal. CONCLUSIONS: This case

Fluoxetine during pregnancy: impact on fetal development.

Women are at greatest risk of suffering from depression during the childbearing years and thus may either become pregnant while taking an antidepressant or may require a prescription for one during pregnancy. The antidepressant fluoxetine (FX) is a selective serotonin reuptake inhibitor (SSRI), which increases serotonin neurotransmission. Serotonin is involved in the regulation of a variety of physiological systems, including the sleep-wake cycle, circadian rhythms and the hypothalamic-pituitary-adrenal axis. Each of these systems also plays an important role in fetal development. Compared wit

Fatal fluoxetine ingestion with postmortem blood concentrations.

A 37-year-old male ingested 12 gm of fluoxetine approximately 2 hours prior to arrival at an emergency department. The patient developed tonic-clonic seizures, which resolved with diazepam and midazolam therapy. The patient then developed profound bradycardia that progressed to ventricular fibrillation and asystole. A postmortem toxicology analysis reported a fluoxetine concentration of 4500 mcg/L and diazepam of 500 mcg/L. No other drugs were detected. We report an unusual case of massive fluoxetine ingestion resulting in neurological and cardiovascular toxicity resulting in death.

BACKGROUND: We assessed the tolerability of and response to citalopram in depressed patients who had discontinued fluoxetine treatment due to adverse events. METHOD: Fifty-five outpatients with DSM-IV major depressive disorder and a confirmed history of intolerance to fluoxetine (mean final dose = 24.6 mg/day) were switched to citalopram (20 mg/day) after a 2- to 4-week single-blind placebo washout period. During a 6-week, open-label treatment protocol, citalopram could be titrated up to 40 mg/day. Safety and tolerability, including reemergence of symptoms that previously had been associated
