Nursing home patients with diabetes: prevalence, drug treatment and glycemic control.

AIMS: Determine prevalence of diabetes, and describe use of blood glucose lowering (BGL) drugs and glycemic control in Norwegian nursing homes. METHODS: In this cross-sectional study we collected details of BGL drugs, capillary blood glucose measurements (CBGM) in the last four weeks and HbA1c measurements in the last 12 months from the medical records of patients with diabetes, within a population of 742 long-term care patients from 19 randomly selected nursing homes in Western Norway. Descriptive statistics were applied, and Pearson's chi-squared (P≤0.05) or non-overlapping 95% confidence i

Glycemic control after brain injury: boon and bane for the brain.

Hyperglycemia is a common phenomenon in the early phase of brain injury (BI). The management of blood glucose levels after BI, however, is subject of a growing debate. The occurrence of elevated blood glucose concentrations is linked to increased mortality and worse neurologic outcomes indicating the necessity for therapeutic glucose-lowering. Intensive glucose-lowering therapy, on the other hand, inevitably results in an increased rate of hypoglycemic episodes with detrimental effects on the injured brain. In this review, we give an overview on the current knowledge about causes and pathophys

Influence of glycemic control on tympanic membrane healing in diabetic rats.

OBJECTIVES/HYPOTHESIS: It is generally assumed that glycemic control in diabetic patients is important in optimizing wound healing. The goal of this study was to examine tympanic membrane (TM) wound healing in spontaneously diabetic rats depending on the diabetic metabolic state compared to nondiabetic control animals. STUDY DESIGN: Prospective controlled study in experimental animals. METHODS: Right-sided myringotomy was performed in 20 normoglycemic rats, 17 well-compensated, and 23 poorly compensated diabetic rats. TMs were observed for a total of 3 weeks. Effect of diabetic metabolic sta

Improving glycemic control: the case for self-monitoring blood glucose levels.

Diabetes can be potentially identified, monitored, treated, and prevented more easily now than at any other time in history. Yet, the prevalence of diabetes continues to grow and the complications associated with it continue to afflict millions of Americans. Glycemic control, or the maintenance of appropriate levels of blood sugar, measured through the self-monitoring of blood glucose or a glycated hemoglobin test, represents the best opportunity for persons with diabetes to prevent complications by helping them understand the effect of their lifestyle choices and medications on their health.

Glycemic control and alveolar bone loss progression in type 2 diabetes.

This study tested the hypothesis that the risk for alveolar bone loss is greater, and bone loss progression more severe, for subjects with poorly controlled (PC) type 2 diabetes mellitus (type 2 DM) compared to those without type 2 DM or with better controlled (BC) type 2 DM. The PC group had glycosylated hemoglobin (HbA1) > or = 9%; the BC group had HbA1 < 9%. Data from the longitudinal study of the oral health of residents of the Gila River Indian Community were analyzed. Of the 359 subjects, aged 15 to 57 with less than 25% radiographic bone loss at baseline, 338 did not have type 2 DM, 14

Effect of glycemic control on vitamin B12 metabolism in diabetes mellitus.

To determine the effect of glycemic control on vitamin B12 (B12) metabolism in diabetes mellitus, we studied B12 metabolism in 19 diabetic patients with poor glycemic control and 15 normal individuals. The diabetic patients had significantly higher total B12 binding capacity (3303 +/- 963 pg/ml), higher serum B12 levels (1173 +/- 503 pg/ml) and unsaturated B12 binding capacity (2131 +/- 902 pg/ml) when compared with the normal controls, but there was no difference in R-binder levels and the B12 binding ratio between the two groups. During a 2-week admission to establish glycemic control, the f

Drug interactions in diabetic patients. The risk of losing glycemic control.

Any number of prescription and over-the-counter medications, including diuretics and salicylates, can affect glycemic control in diabetic patients. In addition, patients being treated with either insulin or sulfonylurea risk hypoglycemic coma or death if they ingest large quantities of alcohol. The authors of this article discuss the medications most likely to be a problem and provide lists of agents associated with hypoglycemia and hyperglycemia.

Metformin beyond Glycemic Control: New Mechanistic Insights and Expanding Therapeutic Horizons.

Metformin, while central to diabetes management, functions as a highly pleiotropic agent with mechanisms that extend far beyond simple glycemic control. In age-related degenerative diseases, including neurodegenerative disorders, it may modulate mitochondrial function, reduce oxidative stress, and influence longevity-related pathways, suggesting possible anti-aging effects. Emerging evidence also points to anticancer activity, with studies reporting reduced incidence and improved outcomes across several malignancies, potentially through mammalian target of rapamycin (mTOR) inhibition, metaboli

Beyond glycemic control: clinical cardiovascular effects of tirzepatide-a narrative review.

The risk of cardiovascular disease is significantly increased by both type 2 diabetes mellitus (T2DM) and obesity through overlapping pathways involving insulin resistance, dyslipidemia, systemic inflammation, and endothelial dysfunction. Tirzepatide, the first dual glucose-dependent insulinotropic polypeptide and glucagon-like peptide-1 receptor agonist, has shown significant glycemic and weight-reducing effects and is increasingly recognized as a cardiometabolic therapy that extends beyond glucose control. This narrative review summarizes clinical and translational evidence about the cardiov

Nutritional considerations for athletes with diabetes: optimizing performance and glycemic control.

Athletes with diabetes encounter the intricate physiological challenge of harmonizing optimal physical performance with meticulous glycemic control. Although nutritional management is crucial for success, generic guidelines often do not provide the required differentiation for various diabetes causes and contemporary insulin delivery methods. This narrative review aims to consolidate the latest research on optimizing macronutrient intake, hydration, and micronutrient support specifically for athletes with Type 1 (T1D) and Type 2 (T2D) diabetes. A structured search of the literature was conduct

Calcium Nanoliposome Improves Glycemic Control in a Mouse Diabetes Mellitus Model.

INTRODUCTION: Intracellular calcium in pancreatic beta cells plays a crucial role in insulin synthesis and secretion. Diabetes impairs this calcium-mediated action, necessitating an effective delivery system such as liposomes to facilitate calcium uptake. METHODS: Calcium lactate nanoliposomes (6.25 mg/mL) were prepared via the thin-film hydration method using lecithin and cholesterol as bilayer lipids. Their glucose-lowering efficacy was tested in hyperglycemic mice induced by oral glucose (1 g/kg) and intraperitoneal streptozotocin (45 mg/kg). Pancreatic calcium levels were measured using X

Engineered promoter-free insulin-secreting cells provide closed-loop glycemic control.

Diabetes mellitus is currently a priority health issue worldwide, but existing therapies suffer from insufficient donors, inability to provide glucose-dependent endogenous insulin secretion, transplantation risks, and immune rejection. Especially, reported engineered cells are mostly promoter-induced glucose-independent insulin producing cells. Here we constructed a closed-loop of insulin secretion with glucose-dependent IRES to achieve glucose-sensitive endogenous insulin secretion. Those cells successfully reversed hyperglycemia in diabetic mice for at least 60 days after transplantation wit

Glycemic Control Status After Six Months in Post-COVID-19 Patients.

Background Despite being typically a viral respiratory disease, COVID-19 has harmful effects that go beyond the respiratory system. The endocrine system is particularly susceptible to damage due to the high expression of angiotensin-converting enzyme-2 receptors. This study evaluates glycemic status in survivors of COVID-19. Methodology In this prospective, observational study, 96 individuals were enrolled from the COVID-19 unit of Bangabandhu Sheikh Mujib Medical University (BSMMU). Mild and moderate COVID-19 patients were classified as non-severe, whereas severe and critical cases were class

Effects of SGLT2 Inhibitors beyond Glycemic Control-Focus on Myocardial SGLT1.

Selective sodium-glucose cotransporter 2 (SGLT2) inhibitors reduced the risk of hospitalization for heart failure in patients with or without type 2 diabetes (T2DM) in large-scale clinical trials. The exact mechanism of action is currently unclear. The dual SGLT1/2 inhibitor sotagliflozin not only reduced hospitalization for HF in patients with T2DM, but also lowered the risk of myocardial infarction and stroke, suggesting a possible additional benefit related to SGLT1 inhibition. In fact, several preclinical studies suggest that SGLT1 plays an important role in cardiac pathophysiological proc

Hospitalization as an Opportunity to Optimize Glycemic Control in Oncology Patients.

PURPOSE OF REVIEW: Many patients experience hyperglycemia during cancer treatment, either as a new-onset condition or as an exacerbation of existing diabetes. This can impact treatment and outcomes, increasing the risk of complications and worsening health-related quality of life (HRQoL). These issues may be particularly significant when patients are hospitalized and/or acutely ill. The purpose of this review is to identify common barriers and strategies specific to the inpatient setting to improve glycemic control and minimize complications both while patients are hospitalized and after disch
