OBJECTIVE: To study the relationship between chronic tension-type headache, cranial hemodynamics, and cerebrospinal pressure.
BACKGROUND: Cerebrospinal pressure has been found to be above 200 mm in about 50% of patients with chronic tension-type headache.
METHODS: Heart rate, blood pressure, common carotid artery diameter and blood flow, and craniovascular resistance and pain at regular intervals before, during, and after head-down tilt-a procedure which increases cerebrospinal pressure, were recorded. After head-down tilt, subcutaneous injections of either placebo or 6 mg of sumatriptan wer

Postless hip distraction technique is a novel traction method for hip arthroscopy developed in recent years. Studies have shown that, compared with the traditional post hip distraction technique, it can effectively solve perineal complications, lower the technical threshold of hip arthroscopy, and improve patients' postoperative satisfaction. The standardization of this technique is expected to promote the advancement and popularization of hip arthroscopy. To this end, this Consensus identified 17 questions of the greatest clinical concern that were selected through systematic literature retri

The liver is an essential multifunctional organ and constantly communicates with nearly all the tissues in the body. Spaceflight or simulated microgravity has a significant impact on the livers of rodent models, including lipid accumulation and inflammatory cell infiltration. Whether similar liver lipotoxicity could occur in humans is not known, even though altered circulating cholesterol profile has been reported in astronauts. Using a 42-day head-down bed rest (HDBR) model in rhesus macaques, the present study investigated whether simulated microgravity alters the liver of non-human primates

The laparoscopic operating technique is being applied increasingly to a variety of intra-abdominal operations. Intra-abdominal gas insufflation, i.e. pneumoperitoneum (PP), is then used to allow surgical access. The haemodynamic effects of PP in combination with different body positions have not been fully examined. Eleven patients without signs of cardiopulmonary disease were studied before and during laparoscopic cholecystectomy under propofol-fentanyl anaesthesia with controlled ventilation. Swan-Ganz and radial arterial catheterization were used to determine haemodynamic data in the horizo

BACKGROUND: We tested the hypothesis that selective spinal anesthesia for ambulatory knee arthroscopy can be accomplished with a small dose of bupivacaine at the L3/4 interspace with or without a head-down tilt of 5 degrees when the patients were in the lateral decubitus position.
METHODS: In this double-blind study, 123 patients were randomly allocated to receive spinal anesthesia with 4 mg of hyperbaric bupivacaine inserted at either the L2/3 interspace, while the vertebral column was kept horizontal (L2/3 group), or the L3/4 level, with the vertebral column horizontal (L3/4H) or tilted 5 d

Investigations of volunteered subjects in whom antiorthostatic hypokinesia was modeled by tilting the cranial end of the body by -15 degrees were to answer whether pharmacological countermeasures of the vestibular/autonomous syndrome (motion sickness, sea sickness) alter the orthostatic tolerance and cardiovascular parameters. As was stated, the drugs allow controlled correction of hemodynamic resistance to the head-down tilt. Ephedrine, phencarol, ephedrine combined with scopolamine and pipolphen, and stugeron normalized cerebral circulation. Medicaments were unable to moderate the sympathoad

Patients who underwent lower abdominopelvic surgeries in the lithotomy position (LP) and the Trendelenburg position (TP) with the leg holder are at risk of developing well leg compartment syndrome (WLCS). However, contributing factors related to the LP with TP associated with WLCS are unknown. This study aimed to investigate the associations between external pressure at the calf in the LPs at different angulations of the TP and physiological characteristics. Eighty-four university students (age, 21.7 ± 0.9; 42 men and 42 women) voluntarily participated in the study. The awake participants were

PURPOSE: Radiographic analysis is often used as a screening tool to assess for risk of sleep-related breathing disorders. This study aimed to address 2 questions: (1) Does head posture significantly affect the minimum cross-sectional area (MCA)? and (2) Is the NBC3 (nasion-basion-C3) angle a reliable measurement to control for alteration of head position in cone-beam computed tomography (CBCT) scans?
METHODS: Study design: prospective cohort study.
SETTING: Private practices affiliated with a research institution.
PARTICIPANTS: convenience sample of adult volunteers.
VARIABLES: CBCT scans

Weightlessness induces a cephalad shift of blood and cerebrospinal fluid that may increase intracranial pressure (ICP) during spaceflight, whereas lower body negative pressure (LBNP) may provide an opportunity to caudally redistribute fluids and lower ICP. To investigate the effects of spaceflight and LBNP on noninvasive indicators of ICP (nICP), we studied 13 crewmembers before and after spaceflight in seated, supine, and 15° head-down tilt postures, and at ∼45 and ∼150 days of spaceflight with and without 25 mmHg LBNP. We used four techniques to quantify nICP: cerebral and cochlear fluid pre

INTRODUCTION: The Raised Pelvic Position, also known as Trendelenburg position, consists of the elevation of the pelvis above the horizontal plane in the supine position with the head lowered. The position is named after Friedrich Trendelenburg, a German surgeon, who flourished in Berlin at the end of the nineteenth century. Although modern studies trace the position's principle back to the first century BC, we herein present even earlier descriptions, dating back to the fifth century BC.
MATERIAL AND METHODS: The whole ancient Greek literature was digitally searched. All relevant references

BACKGROUND: In this study we compared intraoperative bleeding and regional cerebral oxygenation in patients with different degrees of the reverse Trendelenburg position (RTP) during endoscopic sinus surgery (ESS).
METHODS: In total, 120 patients with chronic rhinosinusitis treated by ESS were randomly assigned to the following 4 groups: a horizontal position (HP) group, and 5°, 10°, and 15° RTP (5-RTP, 10-RTP, and 15-RTP, respectively) groups. The primary outcome was the Boezaart grading scale (BS). The cerebral oxygen saturation (ScO2 ), total blood loss, numerical rating scale (NRS) scores,

Twenty-seven patients with chronic tension-type headache were studied as to end-tidal PCO2, heart rate, mean blood pressure, diameter and blood flow of the common carotid arteries, cranial vascular resistance, and headache intensity at supine rest, after administration of nitroglycerin, and at head down tilt. The results were compared to the results of nitroglycerin and head down tilt provocations in age- and sex-matched controls. During supine rest, no change in chronic tension-type headache occurred. Nitroglycerin and tilting induced significant increase of the headache intensity compared to

OBJECTIVES: The aim of this study was to determine the contribution of pre-load and heart rate to pump flow in patients implanted with continuous-flow left ventricular assist devices (cfLVADs).
BACKGROUND: Although it is known that cfLVAD pump flow increases with exercise, it is unclear if this increment is driven by increased heart rate, augmented intrinsic ventricular contraction, or enhanced venous return.
METHODS: Two studies were performed in patients implanted with the HeartWare HVAD. In 11 patients, paced heart rate was increased to approximately 40 beats/min above baseline and then d

The authors conducted rheographic and opthalmoretinal studies of 85 patients with cerebral venous dystonias and of 20 control patients using functional loads (-30 degrees head-down tilt). In the control subjects the diastolic pressure in the central retinal vein (CRV) in head-down tilt test was 10.1 +/- 0.3 mm Hg, showing no significant difference from the initial level (9.0 +/- 0.4 mm Hg), p greater than 0.05. The patients with cerebral venous dystonias presented a significant increase in the diastolic pressure in the CRV by 4.9 +/- 0.2 mm Hg, p less than 0.001, which is indicative of a compr

This paper addresses the "fluid shift theory" of space motion sickness. The primary purpose of our research was the development of procedures to assess individual differences in response to rostral body fluid shifts on earth. Experiment I examined inner ear fluid pressure changes during head-down tilt in intact human beings. Tilt produced reliable changes. Differences among subjects and between ears within the same subject were observed. Experiment II examined auditory threshold changes during tilt. Tilt elicited increased auditory thresholds, suggesting that sensory depression may result from

The functional state of the nervous system and its autonomic compartment was investigated in 15 men, aged 40-52 years, during their 30-day exposure to head-down tilt (-8 degrees). Group 1 consisted of 9 subjects with early manifestations of cerebral atherosclerosis and Group 2 included 6 subjects with cerebral atherosclerosis and arterial hypertension. The control group incorporated 9 healthy volunteers, aged 25-39 years. During adaptation to head-down tilt, middle-aged and young subjects showed consistent changes in the nervous system. In response to perturbation effects, autonomic reactions

This case report describes a rare incident of fluid exhibiting retrograde flow from the nasopharynx through the nasolacrimal duct and accumulating within the eye guard while under general anesthesia. The patient was in a steep Trendelenburg position for several hours and received multiple liters of intravenous fluid. The patient did not have a history of sinusitis, sinus surgery, cocaine use, dacryostenosis and dacryocystitis, or nasolacrimal duct irrigation and probing. This case provides insight into the potential ophthalmic implications of surgical and anesthetic management. While the patie

The fast and increasing advance in surgical technology during the last decades has led to a remarkable development in laparoscopic surgery. "Minimally invasive" surgery has become very popular in the last few years, not only in gynecological procedures but also in general surgery, orthopedics, thoracic and urological procedures. Gas inflation into the abdominal cavity and patient position provokes physiological changes, as well as complications that are not seen in open surgery. Pneumoperitoneum and the Trendelenburg position beyond 35° cause hemodynamic changes, resulting in an increase in ar

The aim of the present study was to quantitate and compare the density of nerve terminals (NTD), as well as of their synaptic vesicle population (SyVD) in saphenous and brachial vein and artery, obtained from rats maintained in the horizontal or head-down tilted (HDT) position for two weeks. The same technique was applied as that for the head-up tilt study.

Space flight-associated neuroocular syndrome (SANS) is a consequence of long-duration space flight and is detected in two-thirds of astronauts. In-flight, this can cause a change in the refraction of the eyes, requiring graded hypermetropic 'superfocus adjustable' glasses, optic nerve head oedema and choroidal folds. While the optic disc oedema resolves on returning to gravitational force, the choroidal folds and axial length shortening remain. Controversy remains over the role of intracranial pressure in the development of these changes. A recent case report has re-energised the debate as to
