Given the range of disorders that produce headache, a systematic approach to classification and diagnosis is an essential prelude to clinical management. For the last 15 years, the diagnostic criteria of the International Headache Society (IHS) have been the accepted standard. The second edition of The International Classification of Headache Disorders (January 2004) reflects our improved understanding of some disorders and the identification of new disorders. Neurologists who treat headache should become familiar with the revised criteria. Like its predecessor, the second edition of the IHS c

Chronic Daily Headache is a descriptive term that includes disorders with headaches on more days than not and affects 4% of the general population. The condition has a debilitating effect on individuals and society through direct cost to healthcare and indirectly to the economy in general. To successfully manage chronic daily headache syndromes it is important to exclude secondary causes with comprehensive history and relevant investigations; identify risk factors that predict its development and recognise its sub-types to appropriately manage the condition. BACKGROUND: Headache is a common problem in primary care and one of the main reasons general practitioners (GP) consult the neurology service. We developed an online adult headache guideline (Supporting Information Appendix S1) for the greater Wellington Region as a resource for GP to guide identification of concerning headaches, initiation of prophylactic medications for migraine and management of analgesic overuse headache.
AIMS: To examine the effectiveness of this adult headache guideline in reducing demand on the neurology outpatient service for headache patients that could readily be ma

INTRODUCTION: The introduction of monoclonal antibodies (mAbs) targeting the calcitonin gene-related peptide (CGRP) pathway has revolutionized migraine treatment. In this regard, galcanezumab, a humanized immunoglobulin G4 (IgG4) that binds the CGRP molecule, has rapid onset of action, monthly dosing, and favorable pharmacokinetics.
AREAS COVERED: This review, which draws upon electronic databases such as PubMed, Web of Science, FDA, and EMA, provides a comprehensive summary of galcanezumab for migraine prophylaxis. It covers the drug's mechanism of action, its clinical efficacy in episodic m

Pain catastrophizing and cutaneous allodynia are commonly altered in patients with chronic migraine associated with medication overuse headache (CM-MOH) and tend to improve in parallel with clinical improvement. The relation between pain catastrophizing and cutaneous allodynia is poorly understood in patients with CM-MOH receiving OnabotulinumtoxinA therapy. In this single-arm open-label longitudinal observational study, patients with CM-MOH were assigned to structured withdrawal and then administered OnabotulinumtoxinA (5 sessions on a three-month basis, 195 UI per 31 sites). BACKGROUND: Initial evidence have shown the short-term efficacy of sTMS in the acute and preventive treatment of migraine. It is unknown whether this treatment approach in the long-term is effective and well tolerated in difficult-to-treat migraine.
METHODS: This is a prospective, single centre, open-label, real-world analysis conducted in difficult-to-treat patients with high-frequency episodic migraine (HFEM) and chronic migraine (CM) with and without medication overuse headache (MOH), who were exposed to sTMS therapy. Patients responding to a three-month sTMS treatment, continued the treat

BACKGROUND: Behaviors of substance dependence are common among patients with medication-overuse headache (MOH). Whether MOH, like other substance use disorders, is associated with an increased risk for suicide is unknown.
METHODS: In this cross-sectional study, newly diagnosed chronic migraine (CM) patients with or without coexisting MOH were enrolled prospectively. Headache diagnoses were made through face-to-face interviews by headache specialists, and a specifically designed questionnaire was used to collect demographics, headache profiles, Migraine Disability Assessment, Hospital Anxiety

Headaches can be classified into primary and secondary headaches. Typical examples of primary headaches are migraine, tension-type headache and cluster headache. Secondary headaches in internal medicine might be due to hypertensive encephalopathy, lung diseases, metabolic and endocrine diseases and disturbances of electrolytes like increased and decreased serum calcium levels. Ibuprofen, immunoglobulins, and antibiotics might lead to aseptic meningitis. Drug-induced headaches are seen after use of nitroglycerine, calcium channel blockers and immunoglobulins.

Headache diagnosis is based on the criteria of the International Headache Society. Migraine and tension type headache are the most frequent primary headache disorders. Diagnosis can be made based on clinical criteria and further diagnostics are not mandatory. Imaging is primarily needed in suspected case of secondary headache disorders. Distinct imaging methods are used for research purposes especially for research about underlying pathophysiology of headache disorders.

BACKGROUND: A systematic approach to the diagnosis of primary and secondary headache disorders requires the measurement of the frequency and the duration of headache.
OBJECTIVE: This article presents thumbnail sketches of the most important headache subtypes and discusses some of the new revisions and headache types included in the International Classification of Headache Disorders (ICHD 2).
DISCUSSION: The clinical features that distinguish secondary headaches requiring urgent investigation (red flags) from those that can be more safely monitored (blue flags) are discussed.

The 'Other Primary Headaches' include eight recognised benign headache disorders. Primary stabbing headache is a generally benign disorder which often co-exists with other primary headache disorders such as migraine and cluster headache. Primary cough headache is headache precipitated by valsalva; secondary cough has been reported particularly in association with posterior fossa pathology. Primary exertional headache can occur with sudden or gradual onset during, or immediately after, exercise. Similarly headache associated with sexual activity can occur with gradual evolution or sudden onset.

OBJECTIVE: "Pain interference" and "headache impact" refer to negative consequences that pain and headache have on one's life. This study investigated determinants of these negative impacts in a large patient cohort who have chronic migraine with medication overuse.
METHODS: Six hundred and eleven adults were enrolled from 34 headache, neurology, and primary care clinics. Negative consequences of chronic migraine with medication overuse were determined using the Patient-Reported Outcomes Measurement Information System (PROMIS) Pain Interference 6b questionnaire and the Headache Impact Test 6.

BACKGROUND: Acute headache is one of the most frequent neurological symptoms in pregnant women. The early diagnosis of underlying secondary conditions has a major influence on patient outcome, especially in emergency settings. However, at the time being no well-established guideline for diagnostic evaluation of acute headache during pregnancy exists. In this study, we aimed to characterize acute headache in pregnant women concerning demographic, clinical, and diagnostic features, and to determine predictors of secondary headache.
METHODS: We analysed retrospectively the data of 151 pregnant w
