BACKGROUND Obturator hernias are very rare and mostly affect elderly women. Among these, obturator hernia with ureteral entrapment is exceptionally rare. Diagnosis and surgical treatment can be very difficult and is usually performed by an open surgical technique. Here we present the first published case of a ureteral obturator hernia where a laparoscopic extended transabdominal preperitoneal patch technique (TAPP) was performed for hernia repair. CASE REPORT In this case, a 79-year-old woman was referred to the emergency department because of right-sided flank pain that had been present for w

PURPOSE: To clarify indications for treating incidental obturator hernia (IOH) detected during laparoscopic groin hernia repair by comparing features of IOH with obturator hernia (OH) complicated by incarceration.
METHODS: Twenty patients who underwent laparoscopic OH repair between January 2017 and July 2024 were included in this retrospective study. They were divided into groups with incarceration or a past history of incarceration (the OH group) or without incarceration (the IOH group). Univariate analysis was performed, and ROC curves were constructed with the cut-off values representing

Obturator hernia is rare, contributing to less than 2% of the total incidence of small bowel obstructions. The diagnosis of obturator hernia is still a big challenge today. Abdominopelvic Computed Tomography scans have better diagnostic value preoperatively, showing the bowel loop between the pectineus and obturator externus muscle. We report an unusual case of a 77-year-old woman who presented to the emergency ward with an acute abdominal pain. Plain abdominal radiography showed small bowel obstruction. However, a computed tomography scan only showed signs of closed-loop obstruction and stran

Obturator hernia repair has traditionally been performed via an intra-abdominal approach, although laparoscopy is also emerging as a feasible alternative. On the other hand, the Kugel method is a minimally invasive and effective form of repair of groin hernia, but there have been few reports on its use for an incarcerated obturator hernia. We describe how we used the Kugel method to repair an obturator hernia in two patients. Both patients presented with acute intestinal obstruction, necessitating emergency surgery. Via a preperitoneal approach, the impacted obturator hernia was carefully rele

Obturator hernia is relatively rare and occurs mostly in elderly, thin, multiparous women. Recent reports have highlighted the importance of pelvic computed tomography (CT) for the preoperative diagnosis. Thirty-six patients with an obturator hernia operated in our hospital were divided retrospectively into two groups (group A: 18 operations from 1973 to 1986, before we used CT; group B: 18 CT cases from 1987 to 1995). Preoperative diagnoses, operative procedures, and postoperative course were reviewed. No statistically significant differences were found between groups A and B in terms of pati

Inguinal, femoral, and obturator hernias are a bulge or protrusion of abdominal contents into the inguinal canal, femoral ring, and obturator ring, respectively. Only a few cases of combined inguinal and femoral or obturator hernias have been reported. Laparoscopic and robotic repair seems safe and feasible in these cases. A 74-year-old male presented with significant pain and a bulge protruding in the right groin reaching into the scrotum. He consented to robotic-assisted inguinal hernia repair. Eight mm trocars were placed into the umbilicus and to the left and right of it. On exploration,

INTRODUCTION: Obturator hernias are rare, occur mainly in slender people and predominantly in females. Underlying pathology of the obturator hernia is a weakening of the obturator membrane. The obturator hernia is situated between the pubic and ischial bones and is therefore clinically occult. Patients predominantly present with symptoms of bowel obstruction, but can also present with sensory disturbance, leg pain and hip pain. Due to the usually delayed diagnosis, the obturator hernia is associated with increased morbidity and mortality.

BACKGROUND: Obturator hernia is a rare but serious disease associated with high morbidity and mortality, as it usually develops in the elderly. Herein, we evaluated the results of a less-invasive strategy for incarcerated obturator hernia (IOH) treated by laparoscopic repair after ultrasound sonography reduction. Study design: We retrospectively reviewed the records of 15 consecutive patients with IOH treated between April 2019 and March 2025.
RESULTS: Fifteen patients with IOH were treated at our hospital. Of these, 13 patients underwent ultrasound-guided, non-invasive manual reduction. One

This is the first report on synthetic mesh migration following obturator hernia repair. Obturator hernias have a low incidence--with fewer than 700 published case studies to date--usually occur in elderly women and are difficult to diagnose. Since 1976, there have been no more than 29 reported cases in which synthetic materials were used in obturator hernia repair. Thus, information relevant to the surgical repair of this type of hernia is based on small numbers only. The use of synthetic mesh for the repair of obturator hernias is a recent development, and to date no complications have been r

An obturator hernia (OH) is a rare form of pelvic hernia in which the abdominal contents protrude through the obturator canal. Malnourished, frail, and multiparous elderly females are at risk of an OH. Preoperative diagnosis of obturator hernia is challenging because of non-specific symptoms. Most of the cases of OH reported in the literature are diagnosed during a laparotomy for acute intestinal obstruction. However, a contrast-enhanced computed tomography (CECT) scan of the abdomen is the best diagnostic investigation for OH. The morbidity and mortality are high because of the associated com

BACKGROUND: An obturator hernia accompanied with a femoral abscess is rare, and leads to severe infection. Repeated draining is often required due to remnant abscess.
CASE PRESENTATION: We herein reported a case of a perforated obturator hernia with a femoral abscess that was successfully treated via repair using the pectineus muscle. An 84-year-old Japanese woman was referred to our hospital with appetite loss and right femoral pain. Abdominal computed tomography (CT) revealed a right obturator hernia and abscess spreading to the right thigh. Emergency surgery was performed. Intraoperative f

INTRODUCTION AND IMPORTANCE: Obturator hernia (OH) is a rare type of abdominal wall hernia that constitutes <1% of all abdominal hernias and is responsible for 0.2-1.6 % of the cases of mechanical small bowel obstruction.
CASE REPORT: We report three cases of OH in elderly frail patients who presented to the ER with small bowel obstruction. Emergency laparotomy and suturing of the hernial defects with resection anastomosis (two cases) and ileostomy (one case) were done.
CLINICAL DISCUSSION: Early diagnosis is often challenging due to its nonspecific clinical features and inconclusive clinica

A 99-year-old frail woman presented with hip pain, vomiting and abdominal pain. Initial CT was interpreted as normal, and symptoms were attributed to pneumonia. Re-evaluation due to clinical deterioration revealed an obturator hernia with small bowel ileus. The patient was treated laparoscopically with a good outcome. Obturator hernia most often affects elderly, thin women and is difficult to diagnose due to nonspecific symptoms. It is a rare cause of small bowel obstruction, and the case highlights the need for increased awareness and systematic CT review in this patient group to reduce morta

PURPOSE: This retrospective cohort study aims to compare perioperative outcomes between laparoscopic and open surgical repair of obturator hernia, a rare but clinically significant abdominal wall hernia.
METHODS: We analyzed 13 consecutive patients with CT-confirmed obturator hernia causing small bowel obstruction (2017-2023). Six underwent open repair and seven received laparoscopic transabdominal preperitoneal (TAPP) repair with mesh. Outcomes included operative time, blood loss, hospitalization duration, and CRP levels.
RESULTS: Laparoscopic repair significantly reduced hospitalization du

Obturator hernias (OH) are rare, accounting for less than 0.05% of all hernias. The aetiology is usually attributed to chronically raised intra-abdominal pressure or cachexia with poor muscle mass. This case report describes a traumatic obturator hernia, an exceptionally rare aetiology of an already rare surgical condition. There are no previous reports of a traumatic obturator hernia in the current literature. This case reports upon the presentation, operative findings and management of a 48 year old male with abdominal and pelvic trauma following a motor bike collision. Laparotomy findings i

Obturator hernia is a rare form of abdominal hernia and a diagnostic challenge. It is commonly seen in elderly thin females. Its diagnosis is often delayed with resultant increased morbidity and mortality due to bowel ischemia/gangrene. It is mistakenly diagnosed as femoral or inguinal hernia on USG. Computed tomography is diagnostic and is a valuable tool for preoperative diagnosis. This report presents a case of 70-year-old thin female presenting with intestinal obstruction due to left sided obstructed obturator hernia. USG showed small bowel obstruction and an obstructed left sided femoral

The Authors report on tpo cases of strangulated obturator hernia observed in their department during the period 1977-1987 and successfully operated on by mid-line laparotomy, both pith a diagnosis of occlusion of the small bowel. The Authors review the clinical signs and instrumental investigations which may be of use in reaching an early pre-operative diagnosis, which constitutes the only possibility of reducing the mortality associated with this condition. Particular attention is devoted to the somatic characteristics of the patients: elderly, thin, chronic bronchitic, hypertensive cardiopat

Hernia through the obturator canal is usually unsuspected and hence undiagnosed. Patients with obturator hernias present as acute cases of intestinal obstruction secondary to strangulation or incarceration, with high rate of morbidity and mortality due to delayed diagnosis and treatment. The know incidence of obturator hernia is low, representing 0.073% (11 of 15,098) of all hernias repaired at the Mayo Clinic in a retrospective study of 15 years. In this study, we conducted a retrospective analysis of laparoscopic extraperitoneal hernia repairs that were performed between the years 2003 and 2
