Nurses' perspectives of COVID-19 medical emergency team responses: A mixed-method study.

BACKGROUND: The COVID-19 pandemic had a sustained and profound impact on the nature and frequency of medical emergency team (MET) activation. Infection control measures in combination with heightened patient acuity increased the risk of delayed recognition of and intervention for deterioration. Evidence describing nurses' experience of triggering MET activation during the COVID-19 pandemic is scant. OBJECTIVES: The objective of this study was to explore ward and intensive care unit liaison nurses' experiences of caring for deteriorating COVID-19-infected patients requiring MET activation in t

Validating the Epic Deterioration Index (DI) for First Episode Rapid Response Team Activation in Sepsis Patients.

INTRODUCTION: The epic electronic medical records software's deterioration index (DI) is an artificial intelligence-driven algorithm, which quantifies the probability of patient deterioration. Our study aimed to assess the relationship between DI score and activation of a rapid response team (RRT) and to identify a cutoff score to screen for deterioration events. MATERIALS AND METHODS: A retrospective case control study was performed on adult sepsis patients across a nonprofit regional health network composed of 15 hospitals between August 2021 and April 2022. Linear mixed regression modeling

Feasibility of a Comprehensive Targeted Cholera Intervention in The Kathmandu Valley, Nepal.

A comprehensive targeted intervention (CTI) was designed and deployed in the neighborhoods of cholera cases in the Kathmandu Valley with the intent of reducing rates among the neighbors of the case. This was a feasibility study to determine whether clinical centers, laboratories, and field teams were able to mount a rapid, community-based response to a case within 2 days of hospital admission. Daily line listings were requested from 15 participating hospitals during the monsoon season, and a single case initiated the CTI. A standard case definition was used: acute watery diarrhea, with or with

Implementation of a multidisciplinary perinatal emergency response team improves time to definitive obstetrical evaluation and fetal assessment.

BACKGROUND: Trauma is the leading cause of nonobstetric death during pregnancy and is associated with an increased risk of maternal and fetal mortality. In an effort to improve the delivery of care to pregnant trauma patients, we developed an institutional multidisciplinary quality initiative designed to improve response times of nontrauma specialists and ensure immediate availability of resources. We hypothesized that implementation of a perinatal emergency response team (PERT) would improve time to patient and fetal evaluation and monitoring by the obstetrics (OB) team and improve both mater

The paradigm shift in treatment of severe venous thromboembolism.

Pulmonary embolism (PE) is the third leading cause of cardiovascular death and the main cause of preventable in-hospital death in the world. The PERT® (Pulmonary Embolism Response Team) concept involves multidisciplinary diagnosis and immediate treatment. Deep venous thrombosis (DVT) is the initial cause of most cases of PE and is responsible for complications such as chronic thromboembolic recurrence, postthrombotic syndrome, and chronic thromboembolic pulmonary hypertension. An aggressive approach to severe cases of iliofemoral DVT similar to the PERT® system can not only reduce the immediat

Challenges of Treating Adenovirus Infection: Application of a Deployable Rapid-Assembly Shelter Hospital.

This article outlines the evolution of a rescue team in responding to adenovirus prevention with a deployable field hospital. The local governments mobilized a shelter hospital and a rescue team consisting of 59 members to assist with rescue and response efforts after an epidemic outbreak of adenovirus. We describe and evaluate the challenges of preparing for deployment, field hospital maintenance, treatment mode, and primary treatment methods. The field hospital established at the rescue scene consisted of a medical command vehicle, a computed tomography shelter, an X-ray shelter, a special l

Does every code need a "reader?" improvement of rare event management with a cognitive aid "reader" during a simulated emergency: a pilot study.

INTRODUCTION: Prompt treatment is necessary to assure patient survival during crisis. Obstetric cardiac arrest (OCA) and malignant hyperthermia (MH) are rarely occurring crises. Cognitive aids (CAs) consolidate management and assist treatment decisions. We investigated a novel method to encourage resident physician CA use during simulated crises. METHODS: Resident physicians were examined during 31 simulated crises of OCA and MH. CAs reviewed in a prior lecture were placed on resuscitation carts. The confederate emergency management team consisted of two anesthesiologists, two critical care n

Development of Rapid Response Capabilities in a Large COVID-19 Alternate Care Site Using Failure Modes and Effect Analysis with In Situ Simulation.

Preparedness measures for the anticipated surge of coronavirus disease 2019 (COVID-19) cases within eastern Massachusetts included the establishment of alternate care sites (field hospitals). Boston Hope hospital was set up within the Boston Convention and Exhibition Center to provide low-acuity care for COVID-19 patients and to support local healthcare systems. However, early recognition of the need to provide higher levels of care, or critical care for the potential deterioration of patients recovering from COVID-19, prompted the development of a hybrid acute care-intensive care unit. We des

Patterns of unexpected in-hospital deaths: a root cause analysis.

BACKGROUND: Respiratory alarm monitoring and rapid response team alerts on hospital general floors are based on detection of simple numeric threshold breaches. Although some uncontrolled observation trials in select patient populations have been encouraging, randomized controlled trials suggest that this simplistic approach may not reduce the unexpected death rate in this complex environment. The purpose of this review is to examine the history and scientific basis for threshold alarms and to compare thresholds with the actual pathophysiologic patterns of evolving death which must be timely de

The impact of wearable continuous vital sign monitoring on deterioration detection and clinical outcomes in hospitalised patients: a systematic review and meta-analysis.

BACKGROUND: Timely recognition of the deteriorating inpatient remains challenging. Wearable monitoring systems (WMS) may augment current monitoring practices. However, there are many barriers to implementation in the hospital environment, and evidence describing the clinical impact of WMS on deterioration detection and patient outcome remains unclear. OBJECTIVE: To assess the impact of vital-sign monitoring on detection of deterioration and related clinical outcomes in hospitalised patients using WMS, in comparison with standard care. METHODS: A systematic search was conducted in August 2020

The importance of a comprehensive geriatric assessment for older people admitted onto a virtual ward.

A comprehensive geriatric assessment (CGA) provides a holistic assessment for the frail and older person. The CGA considers physical and mental conditions as well as function, environmental and a person's social circumstances. Virtual wards are a new concept within the NHS, and use of virtual wards during the COVID-19 pandemic reduced hospital admissions by 50%. The British Geriatrics Society has set clear guidelines on how virtual wards should be developed within integrated care services via multidisciplinary community rapid response teams to improve patient outcomes. This article considers a

Systems for rapidly detecting and treating persons with ebola virus disease--United States.

The U.S. Department of Health and Human Services (HHS), CDC, other U.S. government agencies, the World Health Organization (WHO), and international partners are taking multiple steps to respond to the current Ebola virus disease (Ebola) outbreak in West Africa to reduce its toll there and to reduce the chances of international spread. At the same time, CDC and HHS are working to ensure that persons who have a risk factor for exposure to Ebola and who develop symptoms while in the United States are rapidly identified and isolated, and safely receive treatment. HHS and CDC have actively worked w

Massachusetts General Hospital participation in Operation Unified Assistance for tsunami relief in Banda Aceh, Indonesia.

When Project HOPE invited Massachusetts General Hospital (MGH) to participate in a tsunami relief mission aboard the USNS Mercy hospital ship in January 2005, MGH responded swiftly and enthusiastically. Overcoming differences in medical styles and standards, as well as a variety of cultural and institutional differences, MGH volunteers collaborated successfully with Navy personnel and volunteers from other hospitals to deliver excellent care to a large number of tsunami victims. Lessons were learned regarding the need for better initial team orientation, more-open communication, and more-trans

Understanding Pediatric Clinical Deterioration Through Rapid Response System Trigger Clusters.

OBJECTIVES: Medical emergency teams (METs) are activated in response to signs and symptoms, or triggers, of clinical deterioration in acute care settings. However, the patterns in which triggers manifest and impact outcomes are poorly understood. We identified and described the patterns in which multiple triggers cluster to activate pediatric METs and examined the associations between these clusters and outcomes. METHODS: Pediatric MET events from January 2015 to December 2019 in the Get With The Guidelines®-Resuscitation national registry focused on METs (N = 4289) were grouped into MET trig
