Polyaxial locking plates in treating distal humeral fractures: a comparative randomized trial for clinical outcome.

BACKGROUND: Management of distal humeral fractures remains to be one of the most challenging aspects in trauma surgery. Low profile plating systems with variable angle screw fixation represent a crucial advancement to the established angular stable locking plates with considerable attention in current research. The aim of the prospective randomized trial was to review the preliminary results and patients' outcome following treatment with these newly developed implants and to rule out potential differences in fracture treatment of two different plating systems. METHODS: Twenty patients with di

Triceps-sparing extra-articular step-cut olecranon osteotomy for distal humeral fractures: an anatomic study.

BACKGROUND: This anatomic study investigated the distal humeral articular surface exposure achievable through a triceps-sparing oblique extra-articular osteotomy of the olecranon with a step-cut modification compared with the anconeus flap transolecranon apex distal chevron osteotomy. In addition, the bone contact surface areas of the osteotomized surfaces after transolecranon and extra-articular osteotomies were compared. METHODS: Seven pairs of fresh adult cadaveric elbow joints were examined. Each of the right elbows underwent triceps-sparing extra-articular step-cut olecranon osteotomy (S

Treatment of distal humeral fractures using conventional implants. Biomechanical evaluation of a new implant configuration.

BACKGROUND: In the face of costly fixation hardware with varying performance for treatment of distal humeral fractures, a novel technique (U-Frame) is proposed using conventional implants in a 180 degrees plate arrangement. In this in-vitro study the biomechanical stability of this method was compared with the established technique which utilizes angular stable locking compression plates (LCP) in a 90 degrees configuration. METHODS: An unstable distal 3-part fracture (AO 13-C2.3) was created in eight pairs of human cadaveric humeri. All bone pairs were operated with either the "Frame" techniq

Triceps tongue versus olecranon osteotomy for intra-articular distal humeral fractures: a matched-cohort study.

BACKGROUND: There are various approaches to the distal humerus when managing a distal humeral fracture, and controversy exists regarding which approach is optimal. The purpose of this study was to report the outcomes of the triceps tongue (TT) approach when used for osteosynthesis of AO type 13-A, B, and C distal humeral fractures. Secondarily, we aimed to compare the outcomes of the TT approach vs. olecranon osteotomy (OO) when used for osteosynthesis of AO type 13-C distal humeral fractures. METHODS: We performed a retrospective review of patients with distal humeral fractures treated with

Comparison between anterior and posterior plating systems in extra-articular distal-third diaphyseal humeral fractures.

PURPOSE: The ideal surgical treatment of extra-articular distal-third diaphyseal humeral fractures is controversial in terms of the surgical approach and plate position. The aim of this study was to compare the clinical and radiological outcomes between anterior and posterior plating methods for extra-articular distal-third diaphyseal humeral fractures. METHODS: Twenty-four patients were treated with anterior plating with modified placement of the proximal humeral internal locking system (PHILOS) plate, and 22 patients were treated with posterior plating of the extra-articular distal humerus

A combined paratricipital and triceps-splitting approach for distal humeral metaphyseal-diaphyseal fractures in adults.

BACKGROUND: Distal metaphyseal-diaphyseal fractures of the hume-rus can be challenging. The success lies in achieving a stable fixation that could allow early functional recovery. Our aim is to combine dif-ferent approaches already reported, to obtain an ideal surgical strategy for treating these fracture patterns. METHODS: In this retrospective study, we present the clinical outco-me of a 12-patient cohort in which we used a combined paratricipital and triceps-splitting approach to the distal humerus. The mean age of the group was 50 years (range 17 - 88). Clinical and radiographic evaluatio

Plating osteosynthesis of mid-distal humeral shaft fractures: minimally invasive versus conventional open reduction technique.

Results of two methods, conventional open reduction-internal plating and minimally invasive plating osteosynthesis (MIPO), in the treatment of mid-distal humeral shaft fractures were compared. Thirty-three patients were retrospectively analysed and divided into two groups. Group A (n = 17) patients were treated by MIPO and group B (n = 16) by conventional plating. The mean operation time in group A was 92.35 +/- 57.68 minutes and 103.12 +/- 31.08 minutes in group B (P = 0.513). Iatrogenic radial nerve palsy in group A was 0% (0/17) and 31.3% in group B (5/16 (P = 0.012). The mean fracture unio

Complex distal humeral fractures: internal fixation with a principle-based parallel-plate technique. Surgical technique.

BACKGROUND: Severe comminution, bone loss, and osteopenia at the site of a distal humeral fracture increase the risk of an unsatisfactory result, often secondary to inadequate fixation. The purpose of this study was to determine the outcome of treating these fractures with a principle-based technique that maximizes fixation in the articular fragments and stability at the supracondylar level. METHODS: Thirty-four consecutive complex distal humeral fractures were fixed with two parallel plates applied (medially and laterally) in approximately the sagittal plane. The technique was specifically d

Functional outcome following surgical treatment of intra-articular distal humeral fractures through a posterior approach.

BACKGROUND: While surgical repair is considered the standard of care of displaced intra-articular distal humeral fractures, most investigators have assessed its results with use of surgeon-based and/or radiograph-based outcome measures. The purpose of our study was to determine the functional outcome of fixation of displaced intra-articular distal humeral fractures with use of a standardized evaluation methodology consisting of objective testing of muscle strength and use of patient-based questionnaires (both limb-specific and general health-status questionnaires). METHODS: We identified twen

The new, minimally invasive anteromedial-distal approach for extraarticular distal-third humeral shaft fractures. Its evolution and first clinical results.

UNLABELLED: Introduction In 2020, we published a new minimally invasive anteromedial distal approach for plate fixation of the humerus (MIAMDAH) to address extra-articular distal shaft fractures in a cadaveric study. After operating on our first patients, it was noted that the distal MIPO window was too small to comfortably fix the plate distally. So, a wider MIPO window was developed to make the surgical procedure more comfortable. This study aimed to evaluate clinical outcomes in patients who underwent surgery using either the original approach or its modified version and to determine whethe

The effect of sagittal and rotational malalignment of distal humeral fractures on elbow mobility: a cadaveric study.

This study aims to investigate the correlation between axial and sagittal malrotation of distal humerus fractures and elbow mobility. A transverse distal humerus fracture was generated in 5 cadaveric specimens. Rotation of the distal humeral fragment was performed on the medial column with a stable lateral column, as well as rotation of the lateral column with a stable medial column. Elbow flexion and extension range of motion were measured and repeated with an additional 5° and 10° of sagittal flexion and extension fracture deformity. All 4 fracture types suffered extension loss with increasi

Modified Tension Band Wiring in Adult Distal Humeral Fracture Types A2 and C1.

BACKGROUND: Distal humeral fractures accounts for approximately 2% of all fractures and nearly one-third of humeral fractures in adults. In this regard, Modified Tension Bind Wiring (MTBW) technique was used for the fixation of the distal humeral fractures type A2 and C1 (AO) to evaluate the early movement and complications of the patients. METHODS: This study was conducted on 25 patients, who were subjected to open reduction and internal fixation using MTBW techniques, to evaluate the incidence of complications. RESULTS: The mean age of the participants was 53.7 years. Out of 25 patients, 1

Use of a hinged external fixator for elbow instability after severe distal humeral fracture.

The authors report the case of a forty-year-old man who developed acute elbow instability after fixation of an open, comminuted distal humeral fracture. Treatment with a hinged, external elbow fixator was successful in reestablishing elbow stability and a functional range of elbow motion. To the best of the authors' knowledge, the use of this device for acute elbow instability after distal humeral fracture fixation has not been previously reported.

Fracture-separation of the distal humeral epiphysis. Long-term follow-up of five cases.

Fracture-separation of the distal humeral epiphysis is a rare injury described in young children. The diagnosis is often difficult because most of the ossification centre are not visible. We report a long-term follow-up study of five cases, evaluated after an average of 38 years. At the time of fracture, all the patients were more than 3 years old. Three patients had been treated with overhead traction, closed reduction and immobilisation in a long-arm plaster cast, one patient with closed reduction and immobilisation and another one, initially not diagnosed, had been treated after 2 weeks wit

Fracture separation of the distal humeral epiphysis in children younger than three years old.

This study evaluated the change of carrying angle and the causes of cubitus varus after the fracture separation of the distal humeral epiphysis in young children. Twelve cases of fracture separation of distal humeral epiphysis were treated from January 1995 to July 1997. The age of all patients was younger than 3 years old. A metaphyseal fragment was seen in all cases (Salter-Harris type II), but the size of the metaphyseal fragment was either a very small flake or a large Thurston-Holland fragment. Posteromedial displacement was seen in all cases. There were three treatment groups: closed red
