Individuals with congenital heart disease (CHD) have an increased risk of neurodevelopmental impairments. Given the hypothesized complexity linking genomics, atypical brain structure, cardiac diagnoses and their management, and neurodevelopmental outcomes, unsupervised methods may provide unique insight into neurodevelopmental variability in CHD. Using data from the Pediatric Cardiac Genomics Consortium Brain and Genes study, we identified data-driven subgroups of individuals with CHD from measures of brain structure. Using structural magnetic resonance imaging (MRI; N = 93; cortical thickness

As the costs of genetic sequencing continue to drop and human genomic biobanks grow in scale, the challenge in genomics has shifted increasingly towards disentangling whether and how associated genetic variants cause disease. Clinical imaging in health-system-based biobanks provides quantitative physiological measures that may help bridge this gap. Using genomic data linked to computed tomography (CT) scans from the Penn Medicine Biobank, we performed GWAS on image-derived phenotypes representing organ volume and attenuation. We identified dozens of genetic associations with CT imaging derived

During the past decade, medical imaging has made the transition from anatomical imaging to functional and even molecular imaging. Such transition provides a great opportunity to begin the integration of imaging data and various levels of biological data. In particular, the integration of imaging data and multiomics data such as genomics, metabolomics, proteomics, and pharmacogenomics may open new avenues for predictive, preventive, and personalized medicine. However, to promote imaging-omics integration, the practical challenge of imaging techniques should be addressed. In this paper, we descr

Imaging genomics offers powerful links between genetics and neuroimaging phenotypes, but conventional methods often lack spatial correspondence. We introduce spatial imaging-transcriptomics, a paradigm that directly integrates magnetic resonance imaging (MRI) and spatially resolved transcriptomics (ST) with matched resolution from the same brain through spatially co-registered maps. We apply this framework to wild-type and Alzheimer's disease (AD) mouse models to uncover the molecular underpinnings of diffusion MRI. In the normal brain, fractional anisotropy (FA) correlates with myelination pr

Imaging genetics provides an opportunity to discern associations between genetic variants and brain imaging phenotypes. Historically, the field has focused on adults and adolescents; very few imaging genetics studies have focused on brain development in infancy and early childhood (from birth to age 6 years). This is an important knowledge gap because developmental changes in the brain during the prenatal and early postnatal period are regulated by dynamic gene expression patterns that likely play an important role in establishing an individual's risk for later psychiatric illness and neurodev

Attention-deficit hyperactivity disorder (ADHD) is a neurological and neurodevelopmental childhood-onset disorder characterized by a persistent pattern of inattentiveness, impulsiveness, restlessness, and hyperactivity. These symptoms may continue in 55-66% of cases from childhood into adulthood. Even though the precise etiology of ADHD is not fully understood, it is considered as a multifactorial and heterogeneous disorder with several contributing factors such as heritability, auxiliary to neurodevelopmental issues, severe brain injuries, neuroinflammation, consanguineous marriages, prematur

Imaging genomics for Parkinson's disease (PD) research aims to integrate genetic and imaging biomarkers to explore how genetic alterations influence brain morphology and function. However, traditional methods have been largely correlative, limiting their utility. Recent advances in machine learning offer potential for exploring causal relationships, although these have not yet been applied to investigate genetic variants and brain phenotypes in PD. Thus, we employ a causal deep learning approach for genotype-phenotype analysis in PD using a novel method to assess the causal impact of genetic r

Sjögren's disease (SjD), a chronic autoimmune disorder affecting exocrine glands, faces significant diagnostic challenges due to its highly heterogeneous symptoms, subjective interpretation of imaging findings, and reliance on invasive biopsies, often resulting in delayed diagnosis by 3-7 years. This review posits that the integration of diverse data streams-from medical imaging to molecular omics-is the pivotal key to overcoming diagnostic heterogeneity, and that AI serves as the indispensable engine to power this integration. We systematically synthesize evidence showing that deep learning m

Cerebral small vessel diseases (CSVDs) are among the most common age-related pathologies of the brain. Arteriolosclerosis and cerebral amyloid angiopathy (CAA) are the most common CSVDs. In addition to causing stroke and dementia, CSVDs can have diverse covert radiological manifestations on computed tomography and magnetic resonance imaging including lacunes, T2-weighted white matter hyperintensities, increased density of visible perivascular spaces, microbleeds and cortical superficial siderosis. Because they cannot be visualized directly, research on the pathophysiology of CSVD has been diff

Prostate cancer (PCa) is one of the most common malignancies threatening men's health globally. A comprehensive and integrated approach is essential for its early screening, diagnosis, risk stratification, treatment guidance, and efficacy assessment. Radiomics, leveraging multi-parametric magnetic resonance imaging (mpMRI) and positron emission tomography/computed tomography (PET/CT), has demonstrated significant clinical value in the non-invasive diagnosis, aggressiveness assessment, and prognosis prediction of PCa, with substantial potential when combined with artificial intelligence. Imaging and serological approaches play an important role in the diagnosis and treatment of alveolar echinococcosis; however, they also suffer from some problems during their applications in clinical practices, which urges the identification of potential diagnostic markers. Novel serological, genomics and proteomics diagnostic markers alone or in combination may increase the sensitivity and specificity in early diagnosis of alveolar echinococcosis, which play vital roles in monitoring of disease courses and prognostic evaluation. This review mainly presents the advances in the studies on novel

From surviving fraction to tumour curability, definitions of tumour radioresistance may vary depending on the view angle. Yet, mechanisms of radioresistance have been identified and involve tumour-specific oncogenic signalling pathways, tumour metabolism and proliferation, tumour microenvironment/hypoxia, genomics. Correlations between tumour biology (histology) and imaging allow theragnostic approaches that use non-invasive biological imaging using tracer functionalization of tumour pathway biomarkers, imaging of hypoxia, etc. Modelling dose prescription function based on their tumour radio-r

The UK Biobank study has produced thousands of brain imaging-derived phenotypes (IDPs) collected from more than 40,000 genotyped individuals so far, facilitating the investigation of genetic and imaging biomarkers for brain disorders. Motivated by efforts in genetics to integrate gene expression levels with genome-wide association studies (GWASs), recent methods in imaging genetics adopted an instrumental variable (IV) approach to identify causal IDPs for brain disorders. However, several methodological challenges arise with existing methods in achieving causality in imaging genetics, includin

Despite major advances in high-throughput genomics, proteomics, and multimodal imaging, a substantial gap persists between molecular tumor characterization and clinically actionable therapeutic decision-making, partly due to the limitations of conventional preclinical models in capturing tumor heterogeneity and predicting patient-specific drug response. Patient-derived organoids (PDO) have emerged as a promising platform to bridge this gap by enabling functional interrogation of individual tumors in a physiologically relevant three-dimensional context. PDO retain the genomic, transcriptomic, a

Poretti-Boltshauser syndrome (PBS) is a rare autosomal recessive disorder characterized by cerebellar dysplasia with cysts, developmental delay, and ocular abnormalities. We present a one-year-old male with developmental delay, hypotonia, and abnormal eye movements. Brain magnetic resonance imaging revealed extensive cerebellar dysplasia with multiple cortical and subcortical cysts, an enlarged rhomboid-shaped fourth ventricle, and splayed superior cerebellar peduncles. Genetic testing confirmed compound heterozygous pathogenic variants in the LAMA1 gene, both classified as pathogenic accordin

PURPOSE: Meningiomas are the most common primary central nervous system tumor. Emerging data supports that higher mutational burden portends worse clinical outcomes in meningiomas. However, there is a lack of imaging biomarkers that are associated with tumor genomics in meningiomas.
METHODS: We performed next-generation targeted sequencing in a cohort of 75 primary meningiomas and assessed preoperative imaging for tumor volume and peritumoral brain edema (PTBE). An Edema Index was calculated.
RESULTS: Meningiomas that were high grade (WHO grade II or grade III) had significantly larger tumor

A spontaneous pneumothorax may be the heralding manifestation of diffuse cystic lung disease (DCLD). Historically, these diagnoses were differentiated by unique clinical, radiographic and tissue pathology characteristics. With recent advancements in genomics, several forms of DCLD can now be diagnosed through genetic testing and patients can thereby avoid undergoing an invasive lung biopsy. We present a case of a young patient with recurrent spontaneous pneumothoraces associated with a rare DCLD, Birt-Hogg-Dubé syndrome, that exemplifies the manifestations of this disease through classic histo

Endometriosis is a disease that affects millions of women worldwide and its diagnosis is still challenging. Medical history, symptoms together with imaging data may address the correct diagnosis, but the gold standard remains laparoscopic assessment with histological confirmation. The development of serum markers as diagnostic tools for endometriosis may allow a prompt and noninvasive diagnosis. Several serum biomarkers have been investigated over the years, but none of these have shown a clinical utility and nowadays the more realistic diagnostic biomarker consists in a panel of biomarkers.
