BACKGROUND: Thoracotomy is still required for major lung resections, though minimally invasive approaches are increasing. Postoperative pain and respiratory issues remain challenges, and current analgesia like epidurals have limitations. Intercostal nerves cryoanalgesia has emerged as a promising alternative, but its clinical and economic impact in thoracotomy remains underexplored.
MATERIALS AND METHODS: We conducted a retrospective, observational, single-center study comparing two groups: a standard of care group (SOC, n = 54) and a cryoanalgesia group (CRYO, n = 29). All patients underwent

Exogenous discharge can positively promote nerve repair. We, therefore, hypothesized that endogenous discharges may have similar effects. The phrenic nerve and intercostal nerve, controlled by the respiratory center, can emit regular nerve impulses; therefore these endogenous automatically discharging nerves might promote nerve regeneration. Action potential discharge patterns were examined in the diaphragm, external intercostal and latissimus dorsi muscles of rats. The phrenic and intercostal nerves showed rhythmic clusters of discharge, which were consistent with breathing frequency. From th

Slipping rib syndrome (SRS) is an underdiagnosed condition causing chronic debilitating thoracic wall pain due to hypermobility of costal cartilage and irritation of intercostal nerves. Conservative management includes analgesics and intercostal nerve blocks. When these measures fail, surgical rib fixation is considered. This case is the first reported successful use of intercostal nerve radiofrequency ablation (RFA) as a preoperative bridge in the management of SRS. We report a case of a 25-year-old woman with hypermobile Ehlers-Danlos Syndrome and slipping rib syndrome who developed recurren

Pectus excavatum is a common chest wall deformity, most often treated during adolescence, that presents a significant postoperative pain control challenge for pediatric surgeons following surgical correction. The purpose of this article is to review the technique and outcomes of intercostal spinal nerve cryoablation for postoperative analgesia following surgical correction of pectus excavatum. Contemporary and historic literature were reviewed. Findings are summarized to provide a concise synopsis of the benefits of intercostal spinal nerve cryoablation relative to alternative analgesic modali

INTRODUCTION: The minimally invasive repair of pectus excavatum (PE) is a painful procedure that can result in long-term hospitalization and opioid use. To mitigate the length of stay and opioid consumption, many different analgesia strategies have been implemented. The aim of this study is to review the use and patient outcomes of intercostal nerve cryoablation (INC) during PE repair reported in the literature.
METHODS: An unfunded literature search using PubMed identifying articles discussing INC during PE repair from 1946 to 1 July 2023 was performed. Articles were included if they discuss

A 37 years-old white man, smoker, was interned for having dry cough and pain in the left hemithorax (HE). The exam has show diminished vesicle mumble in the medium third part of the HE. The thorax computer tomography showed expansive nodular lesion of the 7 degrees coster arch projecting within the HE, with density of the bulk part and discreet enhance by the contrast. Imuno-histochemistry was compatible to benign schwannoma. We consider the intercostal benign schwannoma diagnosis must be taken before all the posterior or lateral intrathoracic mass that follows with small syntomathology and ab

OBJECT: Direct coaptation of intercostal nerves (ICNs) to the musculocutaneous (MC) nerve was performed to restore elbow flexion in 25 patients with brachial plexus root avulsions.
METHODS: Seventy-five ICNs were transected as close as possible to the sternum to obtain sufficient length and then tunneled to the axilla and coapted to the MC nerve. Direct coaptation was achieved in 95% of ICNs, and functional elbow flexion was regained in 64% of the patients. The results were compared with several reported transfer techniques in which either an ICN or other donor nerves were used.
CONCLUSIONS:

OBJECT: The goal of this study was to find which central nervous system (CNS) pathways are involved in volitional control over reinnervated biceps or pectoral muscles.
METHODS: Intercostal nerves (ICNs) were coapted to the musculocutaneous nerve (MCN) or the medial pectoral nerve (MPN) in 23 patients with root avulsions of the brachial plexus to restore biceps or pectoral muscle function. The facilitatory effects of respiration and voluntary contraction on cortical motor-evoked potentials of biceps or pectoral muscles were used to study CNS control over the reinnervated muscles. The time cour

The safety and efficacy of continuous extrapleural intercostal nerve block has been well established in adults. This review of our initial paediatric experience suggests a role for this technique in children and discusses risks and benefits relative to other forms of regional analgesia for thoracotomy. Nine children aged one to twelve years received extrapleural infusions of bupivacaine 0.1-0.2% following lateral thoracotomy for lung resection. An extrapleural catheter was placed by the surgeon prior to thoracotomy closure, and correctly positioned under direct vision external to the parietal

Total spinal anesthesia following intrathoracic intercostal nerve blocks with bupivacaine performed for postoperative pain relief during thoracotomy is described. Possible mechanisms for this complication include: (1) inadvertent placement of the needle through an intervertebral foramen, (2) puncture of a long dural cuff, and (3) intraneural injection with central spread. Recognition of this potential complication is important, and facilities for proper support must be available.

BACKGROUND: Pain management after lung transplantation (LT) is challenging, with most cen-ters using a multimodal analgesia approach that includes opioids and thoracic epidural analge-sia (TEA). Intercostal nerve cryoanalgesia (INCA), which blocks peripheral nerves to provide pain control for about two months, is gaining popularity worldwide but has rarely been stud-ied in the context of LT.
METHODS: Retrospective, before-and-after single-center study conducted between October 2024 and July 2025. A group without INCA (PRE-INCA) was compared to a group with INCA (POST-INCA). All patients recei

Schwannomas are nerve sheath tumors arising from Schwann cells. Schwannomas of the intercostal nerve are extremely rare, with limited published literature. These tumors are generally regarded as benign asymptomatic. When symptomatic, they manifest with radiculopathy or chest pain. Surgery is considered when the pain does not respond to medication. Complete resection has a low risk of recurrence or malignant transformation. Herein, we report a pathologically confirmed case of synchronous intercostal nerve schwannoma diagnosed with chest wall pain. A 38-year-old woman with a breast cancer histor

INTRODUCTION: Intercostal nerve cryoablation is an increasingly adopted technique to decrease postoperative pain in patients undergoing surgical correction of pectus excavatum (SCOPE). Concerns regarding cryo-induced systemic hypothermia have been raised in pediatric patients; however, assessment of a cooled cryoprobe on body temperature has not been performed. We aimed to determine the energy flux from a maximally cooled cryoprobe and model the possible effects on a whole-body system.
METHODS: To directly measure energy flux, a maximally cooled cryoSPHERE probe (AtriCure, Inc, Mason, OH) was

BACKGROUND: Severe pain and pulmonary complications commonly follow rib fractures, both of which may be improved by surgical stabilization of rib fractures (SSRFs). However, significant postoperative pain still persists which may negatively impact in-hospital outcomes. Combining intercostal nerve cryoablation (INCA) with SSRF may improve those outcomes by further decreasing postoperative pain, opioid consumption, and pulmonary complications. The hypothesis is that INCA plus SSRF reduces opioids consumption compared with SSRF alone.
METHODS: The retrospective analysis included trauma patients

This study examined the beneficial effects and potential systemic toxicity from continuous intercostal nerve block by repeated bolus injections of bupivacaine. In this double-blind, randomized study, 20 post-thoracotomy patients were assigned to receive four doses of either: 20 ml 0.5% bupivacaine with epinephrine 5 micrograms.ml-1 (bupivacaine group, n = 10), or 20 ml preservative-free saline (placebo group, n = 10) through two indwelling intercostal catheters every six hours. Patients receiving intercostal bupivacaine injections had greater decreases in visual analogue pain scores (VAS) (P l

Since 1978, 10 intercostal nerve-cauda equina anastomoses and 11 intercostal nerve-lumbar anastomoses were performed for thoracolumbar spinal cord transection. Three to four pairs of intercostal nerves above the traumatic level of the cord were anastomosed with two to four pairs of cauda equina or lumbar nerve roots below the traumatic level, using microsurgical technique. The surgical techniques used for the latter 11 cases were performed through an extraperitoneal approach. The operative technique is described in detail. These 21 cases have been followed-up for six to 48 months. Electromyogr
