PURPOSE: We present a 17-year follow-up of an unusual patient with median cleft of the lower lip, mandible, and tongue with a flexion contracture.
CONCLUSION: Timing of the mandibular reconstruction needs to take into account growth of the mandible and masticatory function. In this case, fixation of the mandible performed at the age of 6 years 6 months and reconstruction carried out at 7 years 8 months of age resulted in satisfactory occlusion and masticatory function.

This article describes the case of a 78-year-old patient whose mandibular fracture was treated with miniplate osteosynthesis. After initial treatment, panoramic radiography revealed a fracture of the miniplate, and at follow-up, a loosening of the replacement plate. For the dental practitioner, this clinical case highlights the importance of panoramic radiography and occlusal analysis and stabilization for diagnosis of mandibular fracture, evaluation of miniplate fracture and treatment, especially in the absence of trauma.

The surgical treatment of mandibular condyle fractures currently offers several possibilities for stable internal fixation. In this study, a finite element model evaluation was performed of three different methods for osteosynthesis of low subcondylar fractures: (1) two four-hole straight plates, (2) one seven-hole lambda plate, and (3) one four-hole trapezoidal plate. The finite element model evaluation considered a load applied to the first molar on the contralateral side to the fracture. Results showed that, although the three methods are capable of withstanding functional loading, the lamb

Pediatric craniofacial structures differ from those of adults in many ways. Because of these differences, management of pediatric craniofacial fractures is not the same as those in adults. The most important differences that have clinical relevance are the mechanical properties, craniofacial anatomy, healing capacity, and dental morphology. This article will review these key differences and the management of pediatric maxillary fractures. From the mechanical properties' perspective, pediatric bones are much more resilient than adult bones; as such, they undergo plastic deformation and ductile

OBJECTIVE: In this study we report on finite element calculation to verify the mechanical stability induced by the new geometry of a novel condylar process plate. We report our clinical experiences in the treatment of condylar process fractures using the novel plate. The plate has been designed according to finite element calculations in order to optimize mechanical stability and stiffness. The potential influence of this plate on the postoperative complication rate was assessed.
STUDY DESIGN: Thirty-five mandibular condylar process fractures in 30 patients (20 male, 10 female, age: 34.3 +/-

PURPOSE: Intermaxillary fixation (IMF) is a fundamental principle in the management of mandibular fractures but with recent advent of open reduction and internal fixation (ORIF), use of IMF is almost limited intraoperatively. Therefore, we compared the efficacy of Erich arch bar versus embrasure wires for intraoperative IMF in mandibular fractures.
METHOD: This prospective study was comprised of 50 patients with mandibular fractures who required ORIF with intraoperative IMF. Patients were categorized into two groups of 25 patients each: Erich arch bar technique was used for group A and embras

Poor facial cosmesis resultant from dysgnathias usually accounts for a range of these individuals' psychological and social problems. Disturbances of both esthetics and function are caused by jaw deformity and associated structures' deformity such as malocclusion. The correction of these deformities is one of the most challenging and intriguing aspects of maxillofacial surgery. Despite having become routine only relatively recently, rigid internal fixation has advanced rapidly, its results are consistent and predictable and thus, its knowledge applies in orthognathic surgery besides other area

Distraction osteogenesis (DO) is a surgical process in which two bony segments are gradually separated so new soft tissue and bone will form between them by applying tension through a fixation device. There are three phases to this process: latency phase, distraction phase, and consolidation phase. The technique was originally applied to long bones but in recent years the method has been adapted for use in maxillofacial surgery. Distraction osteogenesis is a new variation of more traditional; orthognathic surgical procedures which can be applied for the correction of dento-facial deformities a

Large defects of the human face often cause esthetic as well as functional disorders. We present a new technique for reconstruction of the mandible with prosthodontic rehabilitation in a single surgery, using the implant-supported, bar-retained overdenture as an external fixator. A 58-year-old patient presented with a near total defect of the mandible after cancer resection. For rehabilitation, the mandibular condyles were virtually positioned in the centre of the fossae, and four dental implants were planned. The position of the fibula segments as well as their angulation and lengths were ada

Horizontal ridge augmentation with symphyseal autograft is a reliable method of treating alveolar ridge deficiencies in the anterior mandible. Autogenous bone grafts from intraoral donor sites have been regarded as the gold standard for reconstructing jaws. The symphysis is an easily available source of corticocancellous bone with good osteogenic, osteoinductive, and osteoconductive characteristics. This case report illustrates the horizontal ridge augmentation with symphyseal block grafts successfully before implant placement. Fixation screws were used to fix harvested blocks to the ridged de

PURPOSE: A comparative study was performed to evaluate the function of the temporomandibular system after open and closed treatment of condylar fractures.
MATERIALS AND METHODS: A total of 37 temporomandibular joints were examined that had undergone either open treatment (n = 24) or closed treatment with maxillomandibular fixation (n = 13) for condylar fracture. The joints were investigated clinically and on the basis of radiographs. In addition, the movements of the condyles of the temporomandibular system were recorded in 3 dimensions with a computed electronic jaw tracking system (stereogn

The intraoral vertical osteotomy is a versatile and simple technique usually applied to correct mandibular prognathism. Its greatest advantages include ease of performance and minimum neurosensory disturbances, whereas maxillomandibular fixation is pointed as its major disadvantage. Another important concern about this technique is condylar displacement, which may lead to temporomandibular dysfunction. Technical notes to avoid condylar luxation are discussed after 15 years of experience using this osteotomy as the technique of choice to correct mandibular prognathism. Three similar cases are p

Animal bites can cause severely contaminated open fractures, especially in the hand, wrist, and lower extremities, requiring expert orthopedic care. This case report emphasizes the unique issues posed by such injuries, particularly in low-income areas with limited access to comprehensive medical services. Pediatric examples, such as open tibiofibular fractures caused by animal encounters, highlight the significance of individualized treatment techniques. Crocodile bites, though infrequent, present an extreme peril and potential fatality. In Sri Lanka, where various crocodile species inhabit th

The aim of this study was to compare the mechanical characteristics of five commonly used fixation methods for sagittal split osteotomy with a major advancement and counterclockwise rotation with a model that mimics the human mandible. Twenty-five virtually osteotomised and 3-dimensionally printed hemimandibles were fixed with an 8mm gap at the upper border and a 12mm gap at the lower border of the osteotomy. The following fixation methods were used: Group 1: a single miniplate, group 2: two miniplates, group 3: three bicortical screws in the upper border, group 4: three bicortical screws in a

Reconstruction of bimaxillary stage 3 medication-related osteonecrosis of the jaw (MRONJ) defects remains a significant challenge due to extensive lesions and patients' complex comorbidities. This study reports a case of a female patient with metastatic breast cancer who had received long-term denosumab therapy. A partial maxillectomy was performed, and the defect was closed using the double-layer soft tissue closure technique based on the buccal fat pad (BFP). A segmental mandibulectomy was performed through an extraoral approach, and the defect was reconstructed using the reconstruction plat
