The aim of this study was to evaluate the efficacy of the Intubating Laryngeal Mask Airway™ (ILMA) and Laryngeal Mask Airway CTrach™ (LMA CTrach) in facilitating tracheal intubation in morbidly obese patients. Eighty patients (body mass index >40 kg x m(-2)) were randomly allocated to the ILMA or the LMA CTrach. The median (IQR [range]) total time taken for tracheal intubation was shorter with the ILMA than with the LMA CTrach (78 (63-105 [40-265]) s vs 128 (98-221 [60-423]) s, respectively; p<0.001). Significantly more manoeuvres were applied for the satisfactory ventilation and viewing of th

BACKGROUND: Although the cases of upper airway obstruction with ProSeal laryngeal mask airway (PLMA) during spontaneous respiration were reported, the structural comparison with other types of laryngeal mask are not known. We thus examined the relationship between upper airway obstruction and the structure of laryngeal mask.
METHODS: With approval of the human research committee at Kawanishi City Hospital, and written informed consent, we enrolled 27 patients scheduled for elective surgery. Anesthesia was induced with fentanyl 25-50 microg and propofol (2-4 mg x kg(-1)), and PLMA was inserted

BACKGROUND: The low-pressure airway seal of the Classic laryngeal mask airway (CLMA) can be inadequate for positive pressure ventilation (PPV) in children. The ProSeal laryngeal mask airway (PLMA) forms a more effective seal of the airway than the CLMA and facilitates gastric tube placement in adults. The size 3 PLMA can be used in adults and children.
METHODS: The CLMA and PLMA were studied in random order -- crossover -- in 30 anaesthetized, non-paralysed children (average age 10.6 years, average body weight 39 kg). Airway leak pressure, maximum tidal volume, ease of insertion, quality of i

BACKGROUND: In recent years, numerous scientific publications have endorsed the superiority of the ProSeal™ laryngeal mask airway (PLMA) over the Classic™ laryngeal mask airway (cLMA) in adults, children, and infants. The PLMA forms a better seal for both the respiratory and gastrointestinal tracts, provides easier access to the gastrointestinal tract, and exerts lower mucosal pressures for a given seal pressure. This study aims to determine whether this superiority can also be observed for the size 1 PLMA used in anesthetized neonates and infants with positive pressure ventilation.

Many problems with the Classic laryngeal mask airway (CLMA) in infants are believed to be related to its inadequate cuff design. One of the main limitations of the CLMA is that the resulting low-pressure seal can be inadequate for positive pressure ventilation (PPV). The ProSeal LMA (PLMA), a new laryngeal mask airway with a modified cuff, has been shown to form a more effective seal than the CLMA in children. The first infant size PLMA, size 1(1/2), became available recently. We studied 30 anesthetized, nonparalyzed infants aged 15 mo (2-30 mo) and weighing 9 kg (5-12 kg). The CLMA and PLMA w

BACKGROUND: Laryngeal mask airway (LMA) insertion contributes to airway protection in patients with a laryngeal tumor around the glottis. There has been no report of LMA insertion itself exacerbating airway obstruction in such patients.
CASE PRESENTATION: A 62-year-old male underwent elective surgical resection of a large laryngeal polyp. The polyp was attached to the right vocal fold and synchronously swung inward into the trachea and outward to the larynx with inspiration and expiration, respectively. Although manual positive pressure ventilation was easily achieved without any airway obstr

BACKGROUND: The Laryngeal Tube Suction (LTS : Smith Medical, Tokyo) is a new laryngeal tube that has an additional channel for gastric tube placement. Similarly, the Proseal Laryngeal Mask Airway (PLMA) also has a drain tube for gastric tube placement. This study compared the functions of these two devices.
METHODS: Forty patients undergoing general anesthesia were randomly allocated to the group LTS (n = 20) or group PLMA (n = 20). Ease of insertion, quality of airway seal, ease of gastric tube insertion, fiberoptic view of the glottis through each device after placement, and postoperative s

In this multicenter, randomized study, we compared ease of insertion, postinsertion hemodynamic repercussion, quality of ventilation, and the capacity to achieve a "hands-free" anesthesia delivery between two new devices: the ProSeal laryngeal mask airway (PLMA) and the Laryngeal Tube (LT). The incidence of postoperative laryngopharyngeal discomfort was examined after short surgical interventions in spontaneously breathing patients. After induction with fentanyl and propofol, the respective airways were inserted into 70 adult ASA physical status I and II patients (35 patients in each group). F

BACKGROUND: Fiberoptical assisted intubation via a placed laryngeal mask airway (LMA) has been described as save and easy procedure to manage a difficult airway. The laryngeal tube (LT) is a promising alternative to the LMA as supraglottic airway device. Fiberoptical assisted intubation via LT is possible, however considered more difficult. The aim of this study was to compare the fiberoptical assisted intubation via LT and LMA.
MATERIALS AND METHODS: A total of 22 anesthesiologists with different levels of experience participated in the study performed on an adult airway model. Primarily the

Choice of induction agent is the decisive factor in utilization of a laryngeal mask (LM) under spontaneous respiration, as insufficient relaxation and unwanted laryngopharyngeal reflexes deteriorate the efficiency of LM functioning during operation and anesthesia. 132 anesthesias with LM were performed, 86 of these for laparoscopic cholecystectomies (LCE) (group 1) and 42 for saphenectomies (group 2). Control group consisted of 60 patients anesthetized for LCE by means of an endotracheal tube (ETT). Combined induction intravenous anesthesia with ketamine, barbiturates, and diprivan was used in

BACKGROUND: Providing effective positive pressure ventilation is considered to be the single most important component of successful neonatal resuscitation. Ventilation is frequently initiated manually with bag and face mask (BMV) followed by endotracheal intubation if respiratory depression continues. These techniques may be difficult to perform successfully resulting in prolonged resuscitation or neonatal asphyxia. The laryngeal mask airway (LMA) may achieve initial ventilation and successful resuscitation faster than a bag-mask device or endotracheal intubation.

We tested the hypothesis that mucosal pressures are higher for the laryngeal tube airway trade mark than the ProSeal laryngeal mask airway. Fifteen fresh cadavers were studied. Microchip pressure sensors were attached to the laryngeal tube airway and ProSeal laryngeal mask airway at four similar anatomical locations (base of tongue, lateral pharynx, posterior pharynx and posterior hypopharynx) and three dissimilar locations (laryngeal tube airway trade mark, anterior and lateral hypopharynx; ProSeal laryngeal mask airway, pyriform fossa). The cuff volume (laryngeal tube airway, < or = 140 ml;

BACKGROUND: We have compared the laryngeal tube and laryngeal mask in 22 patients for the success rate of insertion, gas leak pressure and the incidence of gastric insufflation.
METHOD: In a randomized, crossover design, the laryngeal tube and laryngeal mask were inserted in turn after induction of anaesthesia and neuromuscular block. The cuffs were inflated until the intracuff pressure reached 60 cm H(2)O. We measured adequacy of ventilation and the minimum airway pressure at which gas leaked around the cuff. The presence or absence of gastric insufflation was studied at an inflation pressur

In this study, we compared haemodynamic changes, laryngeal mask airway (LMA) insertion time, and complications in patients anesthetized by inhalation of sevoflurane with those of intravenous induction with propofol. One hundred patients, aged between 20-40 years were enrolled in this study. Group 1 received propofol (2.5 mg/kg i.v.) and group 2 received sevoflurane (6%+50% N2O+50% O2) by inhalation using the tidal volume technique. LMA insertion time was found to be significantly longer in sevoflurane group than in propofol group. Mean arterial blood pressure was significantly lower within eac

Acquired subglottic stenosis in infants is a difficult iatrogenic problem with notable morbidity, primarily caused by prolonged endotracheal intubation. The laryngeal mask airway (LMA) is a recently developed, alternative airway device that does not contact the subglottis. To explore the possibility of preventing subglottic stenosis, we compared the endotracheal tube (ETT) and the LMA in terms of the incidence and severity of glottic and subglottic injury resulting from prolonged intubation in the adult ferret model of the infant airway. Ten adult ferrets were randomly intubated under inhalati
