Acute lateral ankle ligament ruptures in the athlete: the role of surgery.

High-level athletes have significantly greater load and demand on their ankle joints than the average population. Therefore, treatment of acute lateral ankle ligament ruptures in the high-demanding athlete is a challenge. This article reviews the treatment of acute lateral ankle ruptures in athletes, with special emphasis on the role of surgical treatment.

The jump shot - a biomechanical analysis focused on lateral ankle ligaments.

Handball is one of the top four athletic games with highest injury risks. The jump shot is the most accomplished goal shot technique and the lower extremities are mostly injured. As a basis for ankle sprain simulation, the aim of this study was to extend the ankle region of an existing musculoskeletal full-body model through incorporation of three prominent lateral ankle ligaments: ligamentum fibulotalare anterius (LFTA), ligamentum fibulotalare posterius (LFTP), ligamentum fibulocalcaneare (LFC). The specific objective was to calculate and visualise ligament force scenarios during the jumping

Injuries of the lateral collateral ligaments of the ankle: assessment with MR imaging.

The aim of this study was to evaluate the ability of MRI to display injuries of the lateral collateral ligamentous complex in patients with an acute ankle distorsion trauma. The MR examinations of 36 patients with ankle pain after ankle distorsion were evaluated retrospectively without knowledge of clinical history, outcome and/or operative findings. The examinations were performed on a 1. 5-T whole-body imager using a flexible surface coil. The signs for ligamentous abnormality were as follows: complete or partial discontinuity, increased signal within, and irregularity and waviness of the li

Reconstruction of the lateral ligaments of the ankle using a regional periosteal flap.

We have treated 94 patients with chronic instability of the lateral side of the ankle by reconstruction of the ligaments with local periosteal tissue. We reviewed 90 cases after a mean follow-up of 2.8 years (2 to 9) using a questionnaire, clinical examination and radiography. The results on a 100-point ankle score indicated that 81% had a good or excellent result. The periosteal flap-replacement technique allows anatomical reconstruction and does not sacrifice other ligaments or tendons in the foot.

Acute lesions of the lateral ligaments of the ankle. Clinical and radiographic review.

We reviewed 36 patients who had undergone reconstructive surgery of the ligaments of the lateral compartment of the ankle between 1981 and 1986. The follow-up period averaged 6 years (range, 3-9 years). Ligamentous lesion was diagnosed by clinical examination and contact thermography. Eighty-six percent of the subjects reviewed achieved a good result, resuming sports activity and regaining full joint stability. Reports in the literature are conflicting: good results have been achieved by both conservative treatment and reconstructive surgery. We are convinced that surgery offers young athletes

A prospective study of the treatment of severe tears of the lateral ligament of the ankle.

A prospective randomized clinical study was undertaken to compare bandaging, plaster cast immobilisation and operative treatment for recent tears of the lateral ligament of the ankle. The follow-up period was two years. Subjectively, only the fear of giving-way showed a clear difference in favour of operative repair. Objective evaluation, including stress radiographs, demonstrated no statistical differences between the three methods of treatment. Although bandaging appeared somewhat less satisfactory with respect to the resulting stability of the ankle, the differences were not statistically s

[Stress test and anatomical study of the lateral collateral ligaments of the ankle].

UNLABELLED: Many authors have discussed on the instability of the ankle joint. We have therefore investigated the variety, courses and the functions of the lateral collateral ligament by dissecting 16 freshly amputated limbs. It was composed of 4 ligaments, i.e., the anterior talofibular, the posterior talofibular, the calcaneofibular and the lateral talocalcaneal ligaments. These ligaments have various courses and influence the stability of the ankle joint in various manners. Transection of the anterior talofibular ligament alone caused inversion and forward instability, and transection of ca

Lateral ligament reconstruction of the ankle with a modified Watson-Jones operation.

A modified Watson-Jones technique, using only half of the peroneus brevis tendon, was applied as an operative procedure for lateral ankle instability. Twenty-nine patients with 30 operated ankles were examined with an average follow-up period of 7 years and 6 months. Functional stability was achieved in all but one of the operated ankles. In 15 cases there were periodic pains and swelling on activity, while the remaining 14 ankles were without symptoms. Supination of the foot was slightly decreased in 4 cases.

Evaluation of the Healing Status of Lateral Ankle Ligaments 6 Weeks After an Acute Ankle Sprain.

We aimed to investigate whether there is clinical and MRI evidence of healing of lateral ligaments 6 weeks after acute lateral ankle sprain (LAS). We prospectively enrolled 18 participants (age 32.7 ± 7.5 years) who sustained an acute LAS and underwent conservative treatment. An ankle MRI was acquired up to 48 hours and 6 weeks following the LAS. A partial tear of the anterior talofibular ligament (ATFL) was observed in 10/18 and a complete tear in 8/18 of the patients. The calcaneofibular ligament (CFL) was partially torn in 11/18 and completely torn in 1/18 of the patients. The healing statu

The improvement of postural control in patients with mechanical ankle instability after lateral ankle ligaments reconstruction.

PURPOSE: Lateral ankle sprain is the most common injury. A previous study demonstrated that patients with mechanical ankle instability suffered deficits in postural control, indicating that structural damage of the lateral ankle ligaments may produce a balance deficit. The purpose of this study was to confirm that lateral ligaments reconstruction could improve postural control in patients with mechanical ankle instability. METHODS: A total of 15 patients were included in the study. Each patient had a history of an ankle sprain with persistent symptoms of ankle instability and a positive anter

Surgical strategies: lateral ligament reconstruction as part of the management of varus ankle deformity with ankle replacement.

Improved designs and surgical technique has led to much better mid-term and longer term outcomes than the first generation ankle replacements of the 1970's. However, there remains multiple recent papers discussing the many potential complications with total ankle replacement surgery. As we proceed into the future, one should be cognizant of the pitfalls and know how to deal with the difficult ankles, especially varus and valgus deformities. There should also be a clear understanding that the greater the varus or valgus, the more challenging the procedure, and the less predictable the outcome o

Lateral ligament reconstruction with allograft in patients with severe chronic ankle instability.

INTRODUCTION: Chronic ankle instability is a condition of perception of giving way and persistent pain usually following multiple ankle sprains. In case of severe joint laxity, surgical treatment with reconstruction of the lateral ligamental complex is recommended. The aim of this study was to evaluate the functional outcome of patients treated with lateral ligament reconstruction with allograft for severe chronic lateral ankle instability. MATERIALS AND METHODS: We performed a retrospective cohort study of ten patients who underwent allograft external non-anatomic ligamentoplasty for severe

Autograft Choice for Anatomic Reconstruction of Lateral Ankle Ligaments in Chronic Ankle Instability: A Systematic Review.

Anatomic reconstruction of the lateral ligamentous ankle complex with an autograft is indicated as a surgical approach in chronic ankle instability management. This approach benefits ankles deficient in ligamentous support and those with greater occupational or sporting demands. Autograft choice varies in clinical practice and is driven by surgeon's choice and patient-specific ankle demands. Current literature does not favor any particular autograft with respect to functional outcomes or postoperative complications. This review systematically compares common autografts through patient-reported

Arthroscopic Anterior Talofibular Ligament Repair Without Anchors for Chronic Lateral Ankle Instability.

In recent years, arthroscopic anterior talofibular ligament (ATFL) repair techniques have been increasingly used in the treatment of chronic ankle instability. Arthroscopic approaches not only allow for the management of concomitant intra-articular pathologies but also help minimize wound-related complications associated with open surgery and improve the assessment of anatomical structures. Ankle sprains are common and tend to recur, often leading to pain and chronic lateral ankle instability. The lateral ankle ligament complex consists of the ATFL, the posterior talofibular ligament, and the

Clinical significance of posterior talofibular ligament injury in chronic lateral ankle instability.

PURPOSE: Although arthroscopic repair of the anterior talofibular ligament (ATFL) is widely performed, the effect of posterior talofibular ligament (PTFL) injury on clinical outcomes remains unclear. This study aimed to evaluate the magnetic resonance imaging (MRI) findings of the PTFL in chronic lateral ankle instability (CLAI) and determine whether the presence or absence of PTFL injury affected the postoperative outcomes of arthroscopic ATFL repair. MATERIALS AND METHODS: Forty ankles of 35 patients who underwent arthroscopic repair for CLAI were included in this study as the experimental

Risk Factors for the Recurrence of Instability After Lateral Ankle Ligament Repair.

BACKGROUND: Arthroscopic anterior talofibular ligament (ATFL) repair is widely performed for chronic lateral ankle instability (CLAI). Although many studies have reported excellent outcomes with this procedure, the recurrence of instability remains a common concern. Therefore, this study aimed to analyze the risk factors for the recurrence of instability after arthroscopic repair for CLAI. METHODS: Fifty-six ankles of 53 patients with a mean age of 31.8 ± 14.7 years were retrospectively reviewed. All patients underwent arthroscopic ATFL repair. If instability remained immediately after ATFL r
