A patient with severe chronic obstructive pulmonary disease was studied during acute respiratory failure. On the day of intubation his respiratory rate was 42, the tidal volume 295 ml, and the maximal inspiratory pressure 8 cm H2O. These parameters improved with rest by mechanical ventilation to 16, 620 ml, and 30 cm H2O, respectively, on the day of successful weaning. Daily tidal volumes correlated significantly with maximal inspiratory muscle pressures (r = 0.936; p less than 0.001). Respiratory system compliances and resistances were measured by the inflation, the end-inspiratory occlusion,

BACKGROUND: Tuberculosis (TB) remains an important public health problem worldwide, as its residual lesions result in functional and quality of life impairments. Few studies have investigated multiple-drug-resistant pulmonary TB (MDR-TB), and the literature regarding the functional parameters of this group of patients is scarce. Functional characterization may point to the need for post-treatment intervention measures that optimize the quality of life in patients with MDR-TB. Thus, this study sought to analyze the respiratory function, functional capacity, and quality of life of patients who w

INTRODUCTION: Breathing pattern disorders (BPD) are more prevalent in females and are characterised by irregular breathing patterns. The Nijmegen Questionnaire (NQ) is commonly used to assess symptoms associated with BPD. Research suggests that females with BPD may have weaker respiratory muscles, as indicated by thinner diaphragms. This study examines the relationship between symptoms associated with BPD (as measured by the NQ) and inspiratory muscle strength, assessed via Maximum Inspiratory Pressure (MIP).
METHODS: A cross-sectional study was conducted with 92 female participants. Symptoms

Asthma and chronic obstructive pulmonary diseases (COPD) lead to functional obstruction of airways, identified by increased inspiratory and expiratory resistances. Increased expiratory resistances cause, in turn, a reduction in expiratory flow. The analysis of flow-volume loops shows that, as the disease progresses, the flow generated during expiration of a tidal volume becomes very close to the flow generated during forced maximal expiration. In such condition, where there is little or no reserve of expiratory flow, higher tidal volumes need to be reached in order to increase the expiratory f

Cervical magnetic stimulation (CMS) produces a greater twitch transdiaphragmatic pressure (TwPdi) than electrical stimulation. This may be because CMS produces rib cage muscle activation, thus producing an inspiratory action independent of the diaphragm. Alternatively, CMS could merely stiffen the rib cage, allowing the diaphragm to act efficiently, by contracting against a stable rib cage. To examine these two hypotheses we studied five patients with isolated bilateral diaphragm paralysis using CMS and bilateral electrical phrenic stimulation (BES). TwPdi, twitch esophageal pressure (TwPes),

We assessed the cardiorespiratory function in young persons with adolescent idiopathic scoliosis as they entered adult life; the condition had been detected and managed through a Montreal area school screening program in the years 1975 to 1979, and all patients had been discharged with stable condition. Thirty-two youths (average age 17.7 years) with Cobb angle from 3 to 32 degrees and 93 girls (average age 17.3 years) with Cobb angle from 3 to 46 degrees did not differ from their unaffected school friends in smoking habit, respiratory symptoms, lung volumes, expiratory flow rates, diffusing c

Congenital muscular dystrophies and congenital myopathies represent a heterogeneous group of disorders of the muscle characterized by an early onset of hypotonia and muscle weakness and consequently, a high respiratory morbidity and mortality. The diagnosis and characterization of the weakness of the respiratory muscles is crucial for clinical management of patients and the evaluation of innovative therapies. Routine respiratory evaluation is based on noninvasive volitional tests, such as the measurement of lung volumes, spirometry, and maximal static pressures, which may be difficult or impos

BACKGROUND: There is evidence of musculoskeletal and respiratory involvement in patients with chronic kidney disease (CKD). This is attributed to protein calorie imbalance that is caused by the disease process, and hemodialysis and is generally referred to as uremic myopathy. This results in calcification of respiratory muscles such as diaphragm and intercostal muscles. There are limited data about respiratory muscle strength in patients with CKD. We intended to evaluate the effect of kidney transplantation on respiratory muscles strength in patients with CKD.

BACKGROUND: Growing evidence indicates that Sleep bruxism (SB) is recognized as a multifaceted condition that potentially has systemic implications, including effects on respiratory functions, exercise capacity, and possibly other body systems, which may reduce overall quality of life (QOL). This study aimed to investigate respiratory functions, respiratory muscle performance, functional capacity, and QOL in individuals with SB compared to healthy controls.
MATERIALS AND METHODS: This study included 47 individuals with probable SB and 21 healthy controls. The classification of SB was based on

Behçet's disease (BD) has well-defined pulmonary complications, but cardiopulmonary exercise testing and the strength of the respiratory muscles have not been studied in detail. We aimed to investigate the pulmonary functions tests, inspiratory and expiratory muscle strength and endurance, cardiopulmonary exercise test response, and the relationship of these parameters in patients with BD. Forty BD patients and 20 healthy control subjects were recruited for this study. Mean age was 32.95 +/- 7.66 years for the BD group and 33.85 +/- 6.63 years for the control group with no statistically signif

BACKGROUND: Pleural effusion adversely affects the pressuregenerating capacity of the diaphragm. It uncouples the lung and chest wall, which may result in diaphragmatic dysfunction. Information on the effects of effusion drainage on diaphragmatic function is limited, but several studies report relief of dyspnoea after drainage, which was attributed to improved diaphragmatic mechanics, even if this issue was never formally addressed.
OBJECTIVE: To investigate the effect of drainage of unilateral pleural effusion on diaphragmatic function.
DESIGN, SETTING AND PATIENTS: In a prospective twostep

This study was conducted to ascertain whether resistive or flow type training would better improve both strength and endurance in the respiratory muscles of healthy subjects. Subjects performed a battery of respiratory strength and endurance tests before and after training, which consisted of either control, cycling exercise (E), inspiratory loading (IL), expiratory loading (EL), or hyperventilation (H) training for 4 weeks. Maximal sustained ventilation increased after training in the E and H groups. Both IL and H improved inspiratory fatigue resistance. IL improved maximal inspiratory pressu

BACKGROUND: Many patients with amyotrophic lateral sclerosis (ALS) experience respiratory failure. The use of respiratory muscle training exercises can improve the respiratory function of these patients. This study aimed to evaluate the effect of inspiratory muscle training (IMT) on respiratory muscle function in ALS patients.
METHODS: In the current randomized controlled clinical trial study, 22 patients were randomly divided into intervention (n = 11) and control groups (n = 11). In the control group, patients used only chest-opening training and diaphragm exercises. Patients in the interve

Breath-hold diving poses significant physiological challenges to maintaining O2 homeostasis. During a maximal breath-hold, numerous, subsequently augmenting, involuntary respiratory muscle contractions against a closed glottis (i.e., involuntary breathing movements, IBMs) may elicit respiratory muscle fatigue, especially under conditions of arterial hypoxemia. Thus, the purpose of this study was to determine whether respiratory muscle fatigue occurs following a series of static breath-holds in 14 trained male divers. To do so, we measured transdiaphragmatic twitch pressure (PDI,TW), maximal in

BACKGROUND AND OBJECTIVE: Continuous positive airway pressure (CPAP) is the most common treatment for obstructive sleep apnoea (OSA), but many patients fail long-term therapy. Bilevel positive airway pressure (BPAP) is a potential alternative. We hypothesized that BPAP could improve treatment adherence and outcomes in patients who cannot tolerate CPAP.
METHODS: Patients with OSA who failed CPAP (usage < 4 h/day) and were referred to a tertiary sleep centre between 2014 and 2017 for BPAP were included. Age, gender, body mass index (BMI), co-morbidities, CPAP use and reasons for failure, Epwort

In sixteen patients with myasthenia gravis, the weakness of respiratory muscles, especially of expiratory, caused some disturbances in lung function. Thirty minutes after injection of neostigmine, general improvement of muscle function was accompanied by an increase of strength and endurance of respiratory muscles. The mean value of maximal inspiratory mouth pressure (PImax) increased by 33%, maximal expiratory mouth pressure (PEmax) by 23.5% and maximal voluntary ventilation (MVV) by 21%. As a consequence of these changes amelioration of lung function indices was found, vital capacity increas

Type-1 hereditary motor and sensory neuropathy (HMSN I) is a slowly progressive disease resulting in distal muscle weakness with atrophy, and in sensory disturbance. Restrictive lung disease and respiratory muscle failure, common in many advanced neuromuscular disorders, is not a predominant feature of HMSN-I. Recently, there have been several reports of respiratory dysfunction in patients with HMSN I, complicated by diaphragmatic weakness. In five patients with HMSN I (3 men and 2 women, mean age 55.4 yrs), we measured spirometric variables, maximal inspiratory pressure, and maximal expirator
