The effect of hypochlorous acid on urine color among pediatric ulcerative colitis patients treated with mesalamine.

BACKGROUND: The use of mesalamine for the treatment of ulcerative colitis (UC) is common, and may rarely cause nephrotoxicity. One of the clinical signs of nephropathy is hematuria or dark urine. We aimed to evaluate the effect of hypochlorous acid (HA) found in household bleach on the color of urine among pediatric UC subjects treated with mesalamine. METHODS: This was a prospective case-control study. We obtained fresh urine samples from UC subjects in remission and with intact kidney function. A urinalysis was used to rule out hematuria or signs of infection. We noted the color of the urin

Non-Adherence Rate to Oral Mesalamine in Ulcerative Colitis Patients: A Systematic Review with Meta-Analysis.

Background/Objectives: Ulcerative colitis (UC) is a part of inflammatory bowel disease (IBD) and it is characterized by colonic-mucosal chronic inflammation with intermittent clinical activity. Personalized medicine is becoming more and more a relevant method of approach in this field, and the identification of potential concerns in a single patient may contribute to the improvement of the clinical approach. Mesalamine represents the cornerstone of therapy for mild-moderate disease forms, but non-adherence to medical therapy represents a critical health problem, although it is underestimated b

Repurposing minocycline as an added-on treatment for ulcerative colitis patients on mesalamine: a randomized clinical pilot study.

One of the most prevalent forms of chronic inflammatory bowel disease is ulcerative colitis (UC). The key characteristics observed in UC patients involve fecal urgency, abdominal discomfort, and bloody diarrhea, all of which significantly lower their quality of life. Preclinical studies investigated the protective effect of minocycline in animal models of colitis. This study aimed to assess minocycline's potential efficacy and safety in mesalamine-treated UC patients. This randomized, controlled pilot clinical research included 46 individuals with mild to moderate UC who met the inclusion crit

Mesalamine Reduces Intestinal ACE2 Expression Without Modifying SARS-CoV-2 Infection or Disease Severity in Mice.

INTRODUCTION: Coronavirus Disease 2019 (COVID-19) is an ongoing public health crisis that has sickened or precipitated death in millions. The etiologic agent of COVID-19, Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2), infects the intestinal epithelium, and can induce GI symptoms similar to the human inflammatory bowel diseases (IBD). An international surveillance epidemiology study (SECURE-IBD) reported that the standardized mortality ratio trends higher in IBD patients (1.5-1.8) and that mesalamine/sulfasalazine therapy correlates with poor outcome. The goal of our study was to

Xylan from Pineapple Stem Waste: a Potential Biopolymer for Colonic Targeting of Anti-inflammatory Agent Mesalamine.

We have successfully conjugated mesalamine (5-aminosalicylic acid, 5-ASA) with xylan, a biopolymer isolated from pineapple stem waste, to form xylan-5-ASA conjugate. The biopolymer was used to provide colon-targeting properties for 5-ASA, a golden standard anti-inflammatory agent commonly used for ulcerative colitis treatment. A series of data from FTIR spectroscopy, UV-Vis spectrophotometry, and HPLC confirmed the xylan-5-ASA conjugate formation. To ensure successful colon targeting properties, in vitro and in vivo drug release studies after oral administration of xylan-5-ASA conjugate to Wis

Drug repositioning of mesalamine via supramolecular nanoassembly for the treatment of drug-induced acute liver failure.

Rationale: Acute liver failure (ALF) is characterized by rapid hepatic dysfunction, primarily caused by drug-induced hepatotoxicity. Due to the lack of satisfactory treatment options, ALF remains a fatal clinical disease, representing a grand challenge in global health. Methods: For the drug repositioning to ALF of mesalamine, which is clinically approved for the treatment of inflammatory bowel disease (IBD), we propose a supramolecular prodrug nanoassembly (SPNs). Mesalamine is modified with a functional peptide of the FRRG sequence. The resulting mesalamine prodrugs form nanoassemblies solel

Successful treatment for ulcerative proctitis with rectal tacrolimus in an 8-year-old girl with intolerance to mesalamine.

Ulcerative colitis (UC) is defined as a chronic inflammatory condition causing continuous mucosal inflammation of the colon without granulomas on biopsy. It affects the rectum, and, to a variable extent, the colon in continuity and is characterized by a relapsing and remitting course. Oral 5-aminosalicylic acid (5-ASA) regimens are recommended as first-line induction therapy for mild to moderately active pediatric UC and for maintenance of remission regardless of other initial treatments. In large clinical trials in adults, mesalamine intolerance was found in 2-5 % of the patients. We present

A random urine test can identify patients at risk of mesalamine non-adherence: a prospective study.

OBJECTIVES: Mesalamine non-adherence is common among patients with ulcerative colitis (UC), and can be difficult to identify in practice. We sought to determine whether a random urine test for salicylates could be used as a marker of 5-aminosalicylic acid (5-ASA) ingestion and identify patients at risk of non-adherence. Our aim is to determine whether measurement of salicylates in a random urine sample correlates with 5-ASA levels, and predicts an individual's risk of mesalamine non-adherence. METHODS: Prospective observational study. Urinary salicylates (by colorimetry) and 5-ASA (by liquid

Double-blind placebo-controlled study of mesalamine in post-infective irritable bowel syndrome--a pilot study.

OBJECTIVE: Post-infective irritable bowel syndrome (PI-IBS) is characterized by continuing symptoms of irritable bowel syndrome, typically diarrhea-predominant, following an episode of acute gastroenteritis. There is often an increase in sub-epithelial inflammatory and neuroendocrine cells on colonic mucosal biopsy. Mesalamine is an anti-inflammatory agent, effective in the treatment of inflammatory bowel disease. The goal of this study was to compare mesalamine to placebo on symptoms and quality-of-life (QOL) in PI-IBS. MATERIAL AND METHODS: Twenty patients who developed diarrhea-predominant

Recurrent Inflammatory Myositis as an Extra-Intestinal Manifestation of Dormant Ulcerative Colitis in a Patient on Long-Term Mesalamine.

Ulcerative colitis (UC) is a chronic systemic inflammatory condition primarily involving the large bowel mucosa with relapsing and remitting episodes. It is also associated with multiple extra-intestinal manifestations [EIM], including skeletal muscle involvement which is rare. Review of the literature reported only a few cases of inflammatory myositis in association with UC. We report an unusual presentation of recurrent inflammatory myositis of lower extremities in a 28-year-old male with quiescent UC and on long-term mesalamine therapy.

Repurposing mesalamine via peptide-functionalized zeolitic imidazolate framework-8 (ZIF-8) nanoparticles for selective breast cancer targeting.

Breast cancer treatment continues to be limited by insufficient tumor selectivity and adverse systemic effects, highlighting the need for delivery systems that improve drug accumulation in malignant cells while minimizing exposure to healthy tissue. In this study, we designed an integrin-targeting nanosystem for drug repurposing by encapsulating mesalamine (MES) within a zeolitic imidazolate framework-8 nanocarrier (nZIF-8) and functionalizing the surface with an Arg-Gly-Asp (RGD) peptide via one-pot synthesis. The MES-RGD@nZIF-8 nanocarriers showed high encapsulation efficiency (99.69%) and p

Mesalamine in Treatment of Intestinal Inflammatory Diseases-Formulation-Driven Strategies for Targeted Colonic Therapy and Remission Maintenance.

Mesalamine (5‑aminosalicylic acid, mesalazine, or 5‑ASA) remains a cornerstone therapy for mild‑to‑moderate ulcerative colitis because of its local anti‑inflammatory and antioxidant actions on the intestinal mucosa, and its clinical performance has increasingly been refined through advances in synthetic medicinal chemistry and drug‑delivery science. Beyond conventional formulations, medicinal chemistry efforts have explored mesalamine analogues and hybrid molecules, including prodrug approaches and conjugates with anti‑inflammatory or redox‑modulating moieties, to extend target engagement beyo
