Immunological and inflammatory features of Kaposi's sarcoma and other Kaposi's sarcoma-associated herpesvirus/human herpesvirus 8-associated neoplasias.

During the last 15 years, virologic and immunologic studies have provided a series of valuable clues on the modalities of gamma-herpesvirus-induced oncogenesis, which do not only consist of the direct subversion of intracellular signaling pathways, leading to a frank neoplastic molecular network in the infected cell, but also rely on viral manipulations of the cellular and cytokine microenvironment, especially in conditions of immunodeficiency in the host. At the virus-host interface, something iniquitous, strikingly favoring the aggressive expansion of human herpesvirus 8-infected lympho-endo

Pan-European Fragile Populations Cohort for COVID-19: What Worked, What Didn't, and Lessons Learned.

The COVID-19 pandemic exposed the vulnerability of immunocompromised hosts and the scarcity of evidence guiding their management. Within the European Horizon 2020 ORCHESTRA project, a multinational consortium connected existing and new cohorts to harmonize data, laboratory methods, and clinical expertise across fragile populations. The fragile patients' cohort became a model for how collaborative infrastructure can generate actionable evidence during a crisis. Through prospective follow-up and centralized immunologic assessment, ORCHESTRA defined the clinical spectrum of COVID-19 in transplant

Choice of Induction in the Era of Single-Antigen Bead Testing: An Idiosyncratic Case Report.

Use of induction therapy after kidney transplant is based on immunologic risk status, but accurate assessment of risk in the era of advanced immunologic testing can be complex. Here, we describe the case of a young kidney recipient who had a Castleman disease, often regarded as a benign lymphoma. Our patient, a white male patient with Castleman disease, underwent a first kidney transplant with rabbit antithymocyte globulin induction but returned to dialysis after primary nonfunction occurred. A second donor became available who shared 3 class I HLA antigens with the first donor, but only low-l

Gastrointestinal food allergy: new insights into pathophysiology and clinical perspectives.

Adverse reactions to food that result in gastrointestinal symptoms are common in the general population; while only a minority of such individuals will have symptoms due to immunologic reactions to foods, gastrointestinal food allergies do exist in both children and adults. These immune reactions are mediated by immunoglobulin E-dependent and -independent mechanisms involving mast cells, eosinophils, and other immune cells, but the complexity of the underlying mechanisms of pathogenesis have yet to be fully defined. Knowledge of the spectrum of adverse reactions to foods that affect the digest

Immunosurveillance in clinical cancer management.

The progression of cancer involves a critical step in which malignant cells escape from control by the immune system. Antineoplastic agents are particularly efficient when they succeed in restoring such control (immunosurveillance) or at least establish an equilibrium state that slows down disease progression. This is true not only for immunotherapies, such as immune checkpoint inhibitors (ICIs), but also for conventional chemotherapy, targeted anticancer agents, and radiation therapy. Thus, therapeutics that stress and kill cancer cells while provoking a tumor-targeting immune response, refer

The Imbalance Between Tumor Immunosurveillance and Tumor Immune Escape in Glioblastoma.

Cancer immunosurveillance is the immune-mediated identification and elimination of malignant cells and is an essential mechanism to inhibit cancer growth. However, during cancer progression, tumors evolve various strategies to evade effective immune-mediated eradication, a process termed tumor immune escape. Glioblastoma (GBM) is the most common form of primary brain cancer and illustrates the delicate balance between immune escape and effective immunosurveillance, and the subsequent consequences upon acquiring various strategies of evasion. GBM exhibits several strategies that drive tumor imm

Figuring fact from fiction: unbiased polling of memory T cells.

Immunization generates several memory T cell subsets that differ in their migratory properties, anatomic distribution, and, hence, accessibility to investigation. In this issue, Steinert et al. demonstrate that what was believed to be a minor memory cell subset in peripheral tissues has been dramatically underestimated. Thus, current models of protective immunity require revision.

Options for immunologic support of renal transplantation through the HLA and immunology laboratories.

HLA and immunology laboratories are an integral part of clinical kidney transplant programs. They assist transplant clinicians with evaluating the immunological suitability of potential recipients for transplantation and selecting donor-recipient combinations with a low risk of immunological failure. With sophisticated new techniques becoming available for posttransplant immunological monitoring, laboratories play an increasing supporting role during posttransplant follow up. The level of precision at which immunological testing predicts clinical outcome, however, leaves room for improvement.

Lupus nephritis II: Treatment and monitoring.

Renal involvement in systemic lupus erythematosus (SLE), lupus nephritis (LN), is common and can result in significant morbidity, including progressive renal dysfunction, and even ultimately leading to death. LN is heterogeneous complicated by the immunologic component, and it is critical to accurately classify LN to direct optimal therapy. Accordingly, identification of objective markers is paramount in reflecting disease stage and monitoring treatment response. In part two of this series, we comprehensively examine LN disease classification, therapies and potential markers to guide therapeut

Intestinal parasites and pregnancy.

Intestinal parasites and pregnancy commonly coexist. Environmental, nutritional, and immunologic factors influence the clinical manifestations and determine the need for treatment of intestinal parasitism during pregnancy. No serious medical or obstetric problems attributable to intestinal parasites developed among 147 parasitized pregnant refugees living and delivering in a refugee camp in Southeast Thailand. These patients received adequate nutrition, careful prenatal monitoring, and no antiparasitic drug therapy. During pregnancy chemotherapy for intestinal parasites should not be used unle

Infectious disease surveillance and management in clinical xenotransplantation: Experience with the first human porcine kidney transplant.

The success and safety of clinical xenotransplantation are determined by technical aspects of surgery, the nature and intensity of immunosuppression, xenograft function, and the ability to avoid immunologic, hematologic, infectious, and malignant complications. In clinical xenotransplantation from swine, infectious challenges relate to the potential spread of pig pathogens to immunosuppressed humans as well as manifestations of infections of the recipient from prior exposures to common human organisms. Robust strategies are required for donor screening, recipient monitoring, and infection prev

[Multiple myeloma: pathophysiology and progress in management].

There have been dramatic recent advances in understanding the basic pathophysiology and treatment strategies for multiple myeloma (MM). Research has shown both a high inter-patient diversity and intra-clonal heterogeneity even in a single patient in terms of cytogenetic/molecular abnormalities, and has also identified marked diversity in the immunological factors involved in tumor surveillance among MM patients. In the presence of a variety of novel agents, including molecular-targeted agents, immunomodulatory agents, and monoclonal antibodies, the optimal translation of current knowledge on b

Importance of immune monitoring approaches and the use of immune checkpoints for the treatment of diffuse intrinsic pontine glioma: From bench to clinic and vice versa (Review).

On the basis of immunological results, it is not in doubt that the immune system is able to recognize and eliminate transformed cells. A plethora of studies have investigated the immune system of patients with cancer and how it is prone to immunosuppression, due in part to the decrease in lymphocyte proliferation and cytotoxic activity. The series of experiments published following the demonstration by Dr Allison's group of the potential effect of anti-cytotoxic T lymphocyte-associated antigen-4 (CTLA-4) paved the way for a new perception in cancer immunotherapy: Immune checkpoints. Calreticulin exposure orchestrates innate immunosurveillance.

Calreticulin (CALR) exposure on the cell surface is known to deliver robust pro-phagocytic signals to myeloid cells. In Nature, Sen Santara et al. demonstrate that surface-exposed CALR also operates as an endogenous activator of natural killer (NK) cells. Collectively, these findings suggest that CALR exposure orchestrates multiple facets of innate immunosurveillance.

Exploring PAH kinetics in wild vs. transplanted triploid and diploid oysters at a contaminated field site using immunological techniques.

Crassostrea virginica is a well-established bivalve species for biomonitoring persistent organic pollutants such as polycyclic aromatic hydrocarbons (PAH) in aquatic environments. Differing biomonitoring methods employing either wild oysters inhabiting sites of interest or naïve cultured oysters deployed to sites for extended periods can be used for site evaluations. However, important differences in total contaminant concentrations accumulated have been observed between the wild and transplanted groups. Furthermore, although rearing cultured triploid oysters is widely popular in commercial fa

Immunodiagnosis of systemic mycoses in animals: a review.

Immunologic methods have been used with varying success in the diagnosis and prognosis of systemic mycoses in animals. Information on the application of the tests and on the interpretation of test results in monitoring therapy was complied. Serotesting provided diagnostic and prognostic data in immunocompetent patients, and predicted relapse in one case after apparent clinical cure. The tests were useful in the management of animals with mycoses that respond slowly and unpredictably to therapeutic agents for which pharmacologics and pharmacokinetics have not been established in domestic animal

Sublingual allergen immunotherapy in immunosuppressed rheumatoid arthritis patients: Safety and clinical outcomes.

BACKGROUND: Allergen immunotherapy (AIT) is an effective disease-modifying treatment for allergic rhinitis. However, autoimmune diseases such as rheumatoid arthritis (RA) are traditionally considered relative contraindications, and evidence on the safety and efficacy of AIT in patients receiving immunosuppressive therapy remains limited. OBJECTIVE: We assessed the clinical efficacy, safety, and immunologic effects of sublingual immunotherapy (SLIT) for Japanese cedar pollen (JCP)-induced allergic rhinitis in patients with RA well controlled by stable immunosuppressive treatment. METHODS: Six

Assessing the performance of longitudinal T-lymphocytes as biomarkers of immune recovery in HIV-infected children with or without TB co-infection.

In this paper, the receiver operating characteristic (ROC) curve was used to investigate the performance of longitudinal CD4+ T cell counts as a biomarker of disease recovery in HIV-TB co-infected children from sub-Saharan Africa. According to the standard of care for monitoring pediatric HIV infection, routine assessment of immunologic/virologic markers is required. However, due to the lack of well-equipped laboratories and trained personnel, monitoring these markers as regularly as recommended is not feasible in sub-Saharan Africa. Thus, in the absence of routine laboratory monitoring, metho
