Randomised controlled single-blind study of conventional versus depot mydriatic drug delivery prior to cataract surgery.

BACKGROUND: A prerequisite for safe cataract surgery is an adequately dilated pupil. The authors conducted a trial to assess the efficacy (in terms of pupil diameter) of a depot method of pre-operative pupil dilatation, as compared with repeated instillations of drops (which is time-consuming for the nursing staff and uncomfortable for the patient). METHODS: A prospective randomised masked trial was conducted comprising 130 patients with no significant ocular history undergoing elective clear corneal phacoemulsification. 65 patients had mydriatic drops (Tropicamide 1%, Phenylephrine 2.5%, Dic

Clinical validation of artificial intelligence algorithms for the detection of different central-involved retinal pathologies and glaucoma from non-mydriatic images.

UNLABELLED: The use of Artificial intelligence (AI) algorithms for detecting different ophthalmic diseases, especially diabetic retinopathy (DR), has become increasingly popular. In this paper, we evaluate the screening performance of different AI algorithms based on convolutional neural networks (CNNs) in a real-world scenario. To that aim, we conducted an observational and cross-sectional study on patients aged ≥18 years with type-2 diabetes mellitus, who had undergone fundus examination for DR screening using a teleophthalmology program. We used the UPRETINA diagnostic system, which consist

Comparing the effectiveness of retina funduscopy using direct ophthalmoscope and handheld non-mydriatic digital retina fundus camera in medical practice.

INTRODUCTION: Retinal examination plays an essential role in ocular assessment and serves as a key tool for diagnosing various eye conditions. The traditional direct ophthalmoscope (DO) remains widely used due to its affordability, accessibility, and rapid application. However, it presents several limitations, including a steep learning curve, narrow field of view, and strong dependence on user expertise. In contrast, the handheld fundus camera (HFC) is a newer innovation that provides high-resolution digital imaging and is more user-friendly. This study compared the usability, diagnostic conf

Use of a smartphone-based, non-mydriatic fundus camera for patients with red flag ophthalmic presentations in a rural general practice.

Introduction Fundus examination by direct ophthalmoscopy is widely used in general practice; however, it offers limited field of view, requires close approximation to the patient, has a steep learning curve and is a difficult skill to master and maintain. Non-mydriatic fundus photography (NMFP) offers an alternative with a wider field of view, ability for image analysis and transmission, and is able to be conducted by allied healthcare staff. Aim This study aimed to compare the use of direct ophthalmoscopy with smart-phone NMFP in a large rural general practice. It also aimed to analyse the nu

Simple non-mydriatic retinal photography is feasible and demonstrates retinal microvascular dilation in Chronic Obstructive Pulmonary Disease (COPD).

BACKGROUND: Chronic Obstructive Pulmonary Disease (COPD) is associated with an increased risk of myocardial infarction and stroke but it remains unclear how to identify microvascular changes in this population. OBJECTIVES: We hypothesized that simple non-mydriatic retinal photography is feasible and can be used to assess microvascular damage in COPD. METHODS: Novel Vascular Manifestations of COPD was a prospective study comparing smokers with and without COPD, matched for age. Non-mydriatic, retinal fundus photographs were assessed using semi-automated software. RESULTS: Retinal images from

Effects of three mydriatic drug regimens on pupil size in rhesus (Macaca mulatta) and African green monkeys (Chlorocebus aethiops).

BACKGROUND: Human mydriatric drug use in non-human primates (NHPs) has been documented but not quantified and compared for effectiveness. The objective of this study was to determine which drug regimen provided the most effective and consistent mydriatic effect for ophthalmologic examinations and other procedures. Secondary objectives were to determine average time to maximum dilation and whether species differences existed. METHODS: Twelve rhesus and 12 African green monkeys were randomly assigned to one of six treatment combinations to test the mydriatic effects of three drug regimens: (1)

Development and validation of an artificial intelligence-based tool to detect subclinical atheroesclerosis using non-mydriatic retinal funduscopic images: the Aitheroscope project.

AIMS: Current cardiovascular risk scores may underestimate the risk of future cardiovascular events, particularly in younger individuals with prolonged exposure to risk factors. In contrast, imaging-based detection of subclinical atherosclerosis provides a more accurate assessment of cardiovascular risk by identifying established vascular disease. We aimed to develop and prospectively validate an artificial intelligence-based tool using retinal fundus images to detect ultrasound-confirmed subclinical atherosclerosis. METHODS AND RESULTS: In this prospective observational study, 931 participan

The use of non-mydriatic ocular fundus imaging in the emergency department for detection of ocular syphilis presenting as presumed "papilledema": a case series.

BACKGROUND: Ocular syphilis and neurosyphilis can present as seemingly "isolated" bilateral optic disc edema mimicking papilledema from intracranial hypertension. As a result, patients are often sent to the emergency department (ED) for "papilledema work-up," which can lead to delayed or missed ocular syphilis diagnosis since serologic testing for syphilis is not routinely performed in patients with presumed "papilledema." Our study describes eight patients initially referred to our ED for "papilledema" workup who were ultimately diagnosed with ocular syphilis based on characteristic findings

High dynamic range widefield fundus photography with transcranial illumination: a contrast agent-free method for non-mydriatic choroidal imaging.

The choroid, a critical vascular layer beneath the retina, is essential for maintaining retinal function and monitoring chorioretinal disorders. Existing imaging methods, such as indocyanine green angiography (ICGA) and optical coherence tomography (OCT), face significant limitations, including contrast agent requirements, restricted field of view (FOV), and high costs, limiting accessibility. To address these challenges, we developed a nonmydriatic, contrast agent-free fundus camera utilizing transcranial near-infrared (NIR) illumination. This system achieves a wide snapshot FOV of up to 185°

Efficacy and safety of Mydriatic Microdrops for Retinopathy Of Prematurity Screening (MyMiROPS): study protocol for a non-inferiority crossover randomized controlled trial.

BACKGROUND: Retinopathy of prematurity (ROP) eye examination screening presupposes adequate mydriasis for an informative fundoscopy of preterm infants at risk, on a weekly basis. Systemic absorption of the instilled mydriatic regimens has been associated with various adverse events in this fragile population. This report aims to present the fully developed protocol of a full-scale trial for testing the hypothesis that the reduced mydriatic drop volume achieves adequate mydriasis while minimizing systemic adverse events. METHODS: A non-inferiority crossover randomized controlled trial will be

Utility of handheld non-mydriatic fundoscopy in a case of bilateral, reversible vision loss in an advanced HIV patient with cryptococcal meningitis in Sub-Saharan Africa.

Cryptococcal meningitis is a fungal infection that is typically caused by Cryptococcus neoformans and most commonly seen in severely immunosuppressed patient. This disease causes severe neurologic disease due to elevated intracranial pressures. In this case report, we describe a patient with newly diagnosed HIV presenting to the hospital with cryptococcal meningitis complicated by sudden vision loss. We highlight the role of prompt diagnosis and treatment in the reversal of vision loss, with subsequent monitoring using a handheld, non-mydriatic fundus imaging device.
