PURPOSE: Percutaneous nephrostomy has proved to be an important modality in the nonoperative treatment of bacterial pyonephrosis. The role of this technique in the management of fungal pyonephrosis continues to evolve. The authors retrospectively reviewed their experience with percutaneous nephrostomy in the management of fungal pyonephrosis.
PATIENTS AND METHODS: Seven patients, two neonates and five adults, were identified with proved fungal infections. Eleven percutaneous nephrostomy tubes were placed, all with use of the Seldinger technique.
RESULTS: Percutaneous nephrostomy allowed (a)

A 72-year-old woman with a single kidney and renal calculi was admitted as an emergency with sepsis and anuria. Renal drainage was established by percutaneous nephrostomy (PTN) as an emergency procedure. A nephrostogram showed multiple lucent stones and a single opaque calculus in the renal pelvis. The definitive treatment of the mixed calculi was carried out through the PTN. The lucent stones were dissolved by alkaline irrigation and the opaque stone removed by a biliary basket via the dilated tract. The safety and simplicity of PTN as an emergency procedure and its use for definitive treatme

Pyonephrosis is gross accumulation of pus within an obstructed renal collecting system that, if left untreated, can lead to potentially fatal septic shock. Treatment requires urgent decompression coupled with systemic antibiotics. Percutaneous nephrostomy (PCN) placement, first described in 1976 for the treatment of pyonephrosis, is now widely utilized for emergent decompression in these patients. When performed by an experienced interventional radiologist, PCN is a safe procedure with technical success rates of over 96 to 99%. This article will address the clinical presentation of pyonephrosi

Ureteral obstruction represents a heterogeneous disease pattern and is treated by ureteral stenting or percutaneous nephrostomy (PCN) depending on the necessity. The benefits of urinary diversion with ureteral stenting or PCN in malignant ureteral obstruction (MUO) for patient survival are only moderate. No differences have been found between ureteral stenting and PCN in MUO with regard to median patient survival and complication rates. In cases of MUO there is currently no evidence that urinary diversion improves the quality of life. Alternative concepts of ureteral stenting, such as tandem u

Percutaneous nephropyelostomy is commonly performed prior to endoscopic procedures, such as percutaneous nephrolithotomy or endopyelotomy. We report a case of colonic perforation, complicating percutaneous nephrostomy, which was managed conservatively. A 10-year-old girl was admitted to our hospital for the diagnosis of ureteropelvic junction obstruction. The first percutaneous nephrostomy with sonographic guidance was performed for evaluating renal function. About three weeks later, the second percutaneous nephrostomy with fluoroscopic guidance was performed for endopyelotomy. The tract was d

Until early in the eighties, the initial treatment of choice for pyonephrosis was by emergency surgical excision, which carried a high intraoperative and early postoperative morbidity and mortality. The introduction of percutaneous nephrostomy, a technique with a low complication rate that permits easy access in dilated excretory tracts and is highly effective, as shown by the clinical and analytical course of the patients with pyonephrotic kidney and by the number of nephrectomies performed, represented a substantial change in the initial therapeutic approach. The present article reports our

Adjuvant to cancer therapy, 35 percutaneous ultrasound-guided nephrostomies have been applied in 21 patients with ureteral occlusions or lesions secondary to advanced gynecologic malignancies. Application of the catheters was uncomplicated. Dislodgement of the catheters was the major problem with this technique, occurring in 9 cases. There were no serious complications. In 14 patients, normal ureteral function was restored after the cancer therapy, 2 had to be nephrectomized, and in 5 patients the nephrostomies had to remain permanently. It is concluded that nephrostomy is a useful tool in the

We compared single- and double-stick techniques of percutaneous nephrostomy insertion by retrospectively reviewing 140 percutaneous nephrostomy procedures in 101 patients. All procedures were performed by residents or fellows with direct attending supervision. Either the single-stick or double-stick technique was used based solely on personal attending preference. There were no significant differences in groups in terms of age, sex, or degree of hydronephrosis. In the single-stick technique, the kidney was punctured with sonographic guidance and the tract was serially dilated to accept an 8.5

Percutaneous nephrostomy (NP) is a routinely applied procedure in adults, but sparsely reported in the pediatric population. We report the treatment of acute obstructive renal diseases in 15 children aged one day to 6 years. The causes of obstruction included pelvi-ureteral junction stenosis (10 cases) by calculus or complicated by pyonephrosis, vesico-ureteral junction stenosis (2 cases), urethral valves and stenosis (2 cases) and one complex urogenital malformation. Catheter placement was performed under ultrasonographic guidance without immediate complication. NP should be considered as a p

We report a case of nephrostomy tract tumor seeding following percutaneous nephrostomy tube placement and endoscopic manipulation of ureteral carcinoma. While never previously reported to our knowledge, tumor seeding is a potential risk of percutaneous endoscopic management of upper tract urothelial carcinomas.

We retrospectively reviewed the outcome of 37 prostate cancer patients with ureteral obstruction treated by percutaneous nephrostomy. The over-all survival was 57% at 1 year and 29% at 2 years (median survival time 21 months). The 1 and 2-year survival rates of 15 patients with no prior hormonal therapy were 73 and 47%, respectively, while those of patients who had previously received hormonal therapy were 48 and 19%, respectively. Median survival times of these groups were 24 months and 12 months, respectively. Of 12 patients who had severe renal failure before percutaneous nephrostomy (serum

BACKGROUND: Surgical management of kidney stones in pregnancy can be a serious problem for both mother and fetus.
CASE PRESENTATION: A 20-year-old woman with a second trimester pregnancy presented to the emergency room with renal calculi. She required surgical intervention and a left nephrostomy tube after consultation with urology. Five weeks later, her tube was solidified in place unbeknownst to her care team. She required several surgeries, dealt with encrustation remnants, infections, radiation exposure, persistent infection requiring antibiotic management, and, ultimately, an early deliv

The percutaneous nephrostomy catheter (PCNC) has evolved since its inception. Over more than half a century, it has gone from a temporary maneuver to a permanent fixture in a large proportion of patients who have incurable illnesses with obstructed renal drainage systems. Unfortunately, the research looking specifically at infectious complications associated with PCNCs suffers from oversimplification as studies predominantly assess sepsis alone. There are no standardized definitions or criteria to define the various infectious complications described in this paper. Although the PCNC has a rela
