Frameless neuronavigation based only on 3D digital subtraction angiography using surface-based facial registration.

OBJECT: Cerebrovascular lesions can have complicated abnormal anatomy that is not completely characterized by CT or MR angiography. Although 3D rotational angiography provides superior spatial and temporal resolution, catheter angiograms are not easily registered to the patient, limiting the use of these images as a source for neuronavigation. However, 3D digital subtraction angiography (DSA) contains not only vascular anatomy but also facial surface anatomy data. The authors report a novel technique to register 3D DSA images by using only the surface anatomy contained within the data set with

The microsurgical treatment of cranio-orbital tumors assisted by intraoperative electrophysiologic monitoring and neuronavigation.

OBJECTIVE: Total resection of cranio-orbital tumors without any major complications is still a challenge for neurosurgeons. Intraoperative electrophysiologic monitoring and neuronavigational system have been widely used in microneurosurgery, but their effects during microsurgical treatment of cranio-orbital tumors warrant further investigation. METHODS: Thirty-eight patients with cranio-orbital tumors admitted to our hospital underwent microsurgery assisted by intraoperative electrophysiologic monitoring. We also simultaneously performed intraoperative neuronavigation in 20 patients. A retros

From Functional Mapping to Functional Recovery: The Emerging Role of Neuronavigated rTMS in Neurorehabilitation.

Background/Objectives: Repetitive transcranial magnetic stimulation (rTMS) has been increasingly investigated as an adjunctive intervention in neurorehabilitation, particularly for motor recovery after stroke. However, conventional rTMS protocols remain limited by variability in target localization, inter-individual anatomical differences, lesion-related network reorganization, and limited reproducibility across treatment sessions. Neuronavigated repetitive transcranial magnetic stimulation (nrTMS) integrates structural neuroimaging with real-time coil tracking, enabling more precise and repro

Neuronavigated percutaneous gasserian radiofrequency thermorhizotomy for trigeminal neuralgia: how I do it.

BACKGROUND: Radiofrequency thermorhizotomy (TRZ) is an established treatment for trigeminal neuralgia (TN). TRZ can result risky and painful in a consistent subset of patients, due to the need to perform multiple trajectories, before a successful foramen ovale cannulation. Moreover, intraoperative x-rays are required. METHOD: TRZ has been performed by using a neuronavigated stylet, before trajectory planning on a dedicated workstation. CONCLUSION: Navigated-TRZ (N-TRZ) meets the expectations of a safer and more tolerable procedure due to the use of a single trajectory, avoiding critical stru

Recognition and Processing of Visual Information after Neuronavigated Transcranial Magnetic Stimulation Session.

BACKGROUND: Transcranial magnetic stimulation (TMS) is a method of noninvasive and painless stimulation of the nervous system, which is based on Faraday's law of electromagnetic induction. Over the past twenty years, the TMS technique has been deployed as a tool for the diagnosis and therapy of neurodegenerative diseases, as well as in the treatment of mental disorders (e.g., depression). METHODS: We tested the inhibitory effects of repetitive TMS (rTMS) on reaction times to militarily relevant visual stimuli amidst distractors and on accompanying blood oxygenation level dependent (BOLD) sign

Removal of a Frontal Sinus Osteoma and Reconstruction by a Custom-Made Implant with Neuronavigation Assistance.

The authors report the surgical treatment of an extensive right frontal sinus osteoma assisted by neuronavigation and reconstruction by a hydroxyapatite custom-made implant. The patient presents with ptosis, hypoglobus, and proptosis of the right eye, without any visual impairment. Computed tomographic (CT) scan showed a very large bony mass involving right frontal sinus and displacing the orbital roof. A stereolithographic model-guided planning was carried out to obtain a practical simulation of the surgical operation and it was submitted to a new CT scan to acquire the reference point to rea

A Novel Approach Using Electromagnetic Neuronavigation and a Flexible Neuroendoscope for Placement of Ommaya Reservoirs.

OBJECTIVE: Placement of intraventricular catheters in oncology patients can be associated with morbidity given their small to slit-like ventricles and underlying hematologic disorders. We studied the accuracy of placing Ommaya reservoirs using neuronavigation and a flexible neuroendoscope to verify catheter positioning. METHODS: Ommaya reservoirs placed in 25 oncology patients between 2013 and 2015 were retrospectively reviewed. Twenty-five ventricular catheters were placed using the AxiEM stealth frameless neuronavigation system and a flexible neuroendoscope. Postoperative catheter accuracy,

Hybrid fluoroscopy-neuronavigation technique for percutaneous balloon compression in trigeminal neuralgia: how I do it.

Percutaneous balloon compression is an established treatment for trigeminal neuralgia, especially in patients refractory to medical therapy or unfit for open surgery. Cannulation of the foramen ovale can be challenging due to anatomical variations. This article describes a hybrid technique combining fluoroscopy and electromagnetic neuronavigation to improve accuracy and safety. Preoperative CT planning allows individualized trajectory design, while intraoperative dual guidance minimizes multiple passes. Balloon compression is performed for 1-3 min to balance efficacy and complications. This mi

Course of brain shift during microsurgical resection of supratentorial cerebral lesions: limits of conventional neuronavigation.

BACKGROUND: The authors have conducted a prospective study to evaluate the amount and course of brain shift during microsurgical removal of supratentorial cerebral lesions, and to assess factors which potentially influence these shifts. METHOD: In 61 patients the displacement of 2-3 cortical landmarks on the cerebral surface was dynamically quantified during surgery, i.e. during dissection of the tumour at the estimated half-time of surgery, and at the end of microsurgical removal of the cerebral lesion using the neuronavigation system EasyGuide Neuro. In 14 of these patients the displacement

Neuronavigation based on CT angiography for surgery of intracranial aneurysms: primary experience with unruptured aneurysms.

Several reports have demonstrated the use of three-dimensional (3D) computed tomographic angiography (CTA) for preoperative planning in patients with intracranial aneurysms. Until now, there are no reports on the potential role of navigation systems in combination with CTA in aneurysm surgery. In the present study we report our experience with neuronavigation based on CTA in 16 patients with unruptured anterior circulation aneurysms for 1) planning craniotomy; 2) guided approach to the aneurysm; and 3) 3D presentation of the aneurysm and adjacent arteries in correct orientation.
